Operation mode and effect of the Sweet Home for diabetic patients

Objective To set up the new models of diabetic education which is suitable for our national conditions in order to improve the teaching effect.Methods Many of methods were used such as training and improving diabetes nurse educators' quality,expanding the forms of diabetes education to attract patients'participate,implementing individual education case management,setting up the intersection of patient and educational group,establishing a long-term management system to offer continuous support.Results Integrated development of diabetes nurse educators' quality was achieved.Thirteen diversified large scale educational activities as well as others different forms were performed.The member of Sweet Home for the diabetic patients was increased from 15 ~20 to 2 586 at the end of 2011.The average glycosylated hemoglobin of the members was 7.16% ± 1.67%,among which,the percent of patients with HbA1c < 6.5% was 42.9% and patients with HbA1c < 7% was 60.5%.The annual rate of outpatient was increased from 7.9% of 2009 to 36% of 2010 and 47% of 2011.Conclusions The diabetes education performed in our Sweet Home which including innovative activities,individual case management,educational group,behavioral change,continuous support and selfcheer,can promote patients to participate in the diabetes education with enthusiasm and improve the result of diabetes education effectively.

Survey and analysis of disaster cognition of community nurses in Chongqing

Objective To realize the community nurses' cognitive condition of disasters nursing,so as to provide the basis for disaster nursing training.Methods Convenience sampling was used to investigate the disaster cognition of 120 nurses in 15 community health service centers (stations) in Chongqing.Results Totalof hundred and twenty questioniare were distributed and one hundred and sixteen questionnaires were retured and the effective rate of retrieve was 97%.97.4% of community nurses showed concerns for disasters and took a positive attitude on dealing with disasters in the future.Their acquaintances with disasters nursing knowledge and nursing technology was 79.3% and 76.7% respectively,but they have a relatively lower cognition in disaster rescue principle with 14.7% and disaster psychological crisis intervention with 9.5 %.Conclusions To strengthen the education and publicity of disaster nursing,and to enhance the training of disaster rescue knowledge and the research of method is important.

Influence of individualized nursing intervention on rehabilitation of patients with chronic heart failure

Objective To observe the influence of individualized nursing intervention (INI) on rehabilitation of patients with chronic heart failure.Methods Totals of 92 patients were randomly divided into two groups,conventional group were given routine nursing,while intervention group were given psychological,diet,exercise,medication and health education and comprehensive nursing intervention,with the insistence of 6 months.And the heart rate,average hospitalization days,improvement in posture with rest and heart function were monitored before and after the intervention.Results Patient number of heart rate less than 100 beat per minute in intervention group was significantly more than that in conventional group (39 vs 30,x2 =4.696,P <0.05).Patient numbers of the intervention group that needing seat,half decubitus,lie low rest at the night respectively were 3,8,35 cases,and those of conventional group were 6,17,23 cases,and the difference was statistically significant (x2 =6.723,P < 0.05).And the difference in the patient numbers with New York Heart Association (NYHA) Ⅱ,Ⅲ,Ⅳ level of cardiac function between intervention group and control group (8 vs 25,2 vs 15,36 vs 6)was statistically significant (x2 =6.114,P < 0.05).The average hospitalization days were shorten in intervention group than those in control group [(13.12 ±7.30) vs (19.62-± 12.46) d,t =-3.010,P < 0.05].Conclusions Individualized nursing intervention can be effectively improve the symptoms of heart failure,heart function,quality of life,and shorten the average hospitalization days for the patients.

Qualitative research on real experience of hematopoietic stem cell transplantation patients during pre-processing period

Objective To explore and describe real experience ot hematopoietic stem cell transplantation(HSCT) patients during pre-processing period,in order to provide basis for psychological nursing and health education.Methods Semi-structured in-depth interviews were conducted on 7 cases of HSCT patients by phenomenological research method in qualitative research.Results Real experience of HSCT patients during pre-processing period included:pessimism,over-sensitive,excessive anxiety,health information needs,economic burden,pressure of the disease and experience of self-value.Conclusions Clinical care should pay attention to the experience and psychological needs of HSCT patients during pre-processing period,deepen the content of holistic nursing,provide deeper and more comprehensive care and emotional support for the patients,so that patients can better cope with the pre-processing period.

Investigation on the anxiety and depression of children' s primary caretakers in class "AAA" hospital and influcing factors

Objective To look into the anxiety and dcpression of children' s primary caretakers and analyze the influencing factors,so as to provide scientific references for the implementation of nursing intervention.Methods Convenience sampling was used to enlist 90 primary caretakers of children in a A level hospital in Wuhan from December 2011 to Apria 2012 to the research.And SAS and SDS were used to investigate these caretakers.Results 95 questionnaires were handed out and 90 were recovered with the rate of 94.73%.The average SAS and SDS scores of primary caretakers were respectively (49.57 ± 8.67) and (54.49 ± 8.16),both were higher than those of national norm group (37.23 ± 12.58) and (41.88 ± 10.57),respectively,and the differences were statistically significant (t =13.504,14.650,respectively ; P =0.000).SAS and SDS scores of caretakers with different patients' ages,caretakers' genders,caretakers' educational degrees and family monthly income per person were significantly different (F =15.917,9.257,3.558,3.069,respectively; P < 0.05).The anxiety and depression of caretakers were negatively related to patients' age,caretakers' educational degrees and family monthly income per person (P < 0.05),and positively related to caretakers' genders (P < 0.05).Conclusions The average SAS and SDS scores of caretakers are obviously higher than those of the norm group,and the anxiety and depression were influenced by patients' age,caretakers' gender,caretakers' educational degree and family monthly income per person,which should draw nursing attention.

Effect of motivational interview on the rehabilitative exercise for patients after percutaneous coronary intervention

Objective To evaluate the effect of motivational interviewing(MI) on the rehabilitative exercise for patients after percutaneous coronary intervention (PCI).Methods 132 patients after percutaneous coronary intervention were chosen by the method of convenience sampling from a hospital in Suzhou,and were randomly divided into two groups.The experimental group (n =68) received MI based intervention while the control group (n =64) received routine health education.Results One month after discharge,there were 0,1,0,56 and 11 cases of patients in the experimental group and 0,10,7,35 and 12 cases in the control group who were respectively in the pre-contemplation stage,contemplation stage,preparation stage,action stage and maintenance stage of physical exercise,and the difference was statistically significant (Z =-2.23,P < 0.05).Three months after discharge,there were 0,1,0,56 and 11 cases of patients in the experimental group and 13,12,2,25 and 12 cases in the control group who were respectively in the pre-contemplation stage,contemplation stage,preparation stage,action stage and maintenance stage of physical exercise,and the difference was statistically significant (Z =-3.62,P <0.05).Six months after discharge,there were 0,0,0,3 and 65 cases of patients in the experimental group and 20,8,1,1 and 34 cases in the control group who were respectively in the pre-contemplation stage,contemplation stage,preparation stage,action stage and maintenance stage of physical exercise,and the difference was statistically significant (Z =-5.70,P < 0.05).Patients' exercising frequency were all no less than 5 times per week one month,three months and six months after discharge in the experimental group,and were significantly different from those in the control group (x2 =-4.22,-6.10,-7.12,respectively;P <0.01).There was also significant difference of duration of physical exercises between two groups (x2 =-4.96,-5.93,-7.24,respectively; P < 0.01).Conclusions Motivational interview can effectively promote patients' willingness to enhance physical exercise and improve their exercise compliance after percutaneous coronary intervention.

The situation and influencing factors of the breastfeeding self-efficacy among primiparas

Objective To investigate the situation and influencing factors of the breastfeeding selfefficacy among primiparas in order to provide evidence for intervention of breastfeeding promotion.Methods Totals of 382 primiparas were recruited and investigated by convenience sampling method between November 2010 and November 2011 with questionnaires such as breastfeeding confidence scale and social support scale.Results The average scores of breastfeeding self-efficacy among primiparas was (102.86 ±15.29).The primiparas with different ages (20 ~ 24,25 ~ 29,≥ 30 years),period of maternity leave (< 2,2 ~3,> 3 months),breastfeeding education before ante partum had significantly different score of breastfeeding self-efficacy (P < 0.05).The influencing factors of self-efficacy included age,period of maternity leave,breastfeeding education before ante partum.The dimensions of emotion support,information support and evaluation support of social support were positively correlated with the total score of breastfeeding self-efficacy (P < 0.05).Conclusions Breastfeeding education for the primiparas should be strengthened to promote breastfeeding behaviors.Further more,the primiparas' individual and social backgrounds should be considered during the education.

Effect of nursing intervention on the sleep quality of patients with hepatocellular carcinoma after interventional therapy

Objective To investigate the sleep quality situation of patients with hepatocellular carcinoma after interventional therapy,so as to provide evidence for the intervention.Methods Randomly divided 140 patients into intervention group that received new nursing intervention on the basis of routine nursing,and control group received routine nursing,with 70 cases each group.Assessed patients' sleep quality with the Pittsburgh sleep quality index (PSQI) before and the third day after the interventional therapy.Results Before interventional therapy,the total scores of the control group and intervention group were (9.89 ±1.54) and (10.68 ± 1.47),respectively,the difference hasn' t statistical significance (P > 0.05).After interventional therapy,the total score of the control group was significantly higher than that of the intervention group [(9.45±2.05) vs (6.21 ±1.36);t=-3.98,P<0.01].Conclusions Patients with hepatocellular carcinoma after interventional therapy tend to suffer insomnia and poor quality sleep which impact the recovery and cause cancer relapse and metastasis diffusion.Specific nursing intervention can improve the patients' sleep quality effectively in the post-operative period.

Effect of nursing intervention on the anxiety and depression of patients with coronary stenting implantation

Objective To investigate the effect of nursing intervention on the anxiety and depressinn of patients with coronary stenting implantation.Methods From August 2009 to June 2011,110 patients with coronary stenting implantation were randomly divided into control group and observation group,each group was 55 cases.The control group received routine nursing,while the observation group were given nursing intervention measures on the basis of routine nursing,and all patients were examined and analyzed with Zung Self-rating Anxiety Scale(SAS) and Self-rating Depression scale (SDS) after 6 months,and the anxiety and depression changes were observed and evaluated.Results After the intervention,the scores of SAS and SDS of observation group respectively were (36.21 ± 9.07) and (42.24 ± 9.06),and those of control group were (48.33 ± 9.02)and (52.36 ± 7.53),and the difference were statistically significant (x2 =9.354,7.872 ; P < 0.05).The depression and anxiety rate of observation group (21.82% and 32.73%) were significantly lower than those of control group after nursing intervention (21.82% vs 41.82%,32.72% vs 47.27% ;x2 =9.354,7.872;P <0.05).Conclusions It can effectively reduce the incidence of anxiety and depression in patients with coronary stenting implantation through the nursing intervention,and improve the patients' quality life.

Investigation of nursing professional environmental conditions in Grade A hospitals

Objective To investigate the nursing professional environmental conditions in Grade A hospitals,so as to provide evidence for countermeasures.Methods Totals of 1 078 clinic nurses were investigated with Gallup Q12 questionnaire.Results Totals of 1 045 nurses were adopted into the study.Among 1 045 clinic nurses,59.71% nurses showed they "pleasant work",29.95% nurses showed they "feel good",10.33% nurses showed they "feel bad or not pleasant".Nurses with different age,department,positions had significantly different recognition degree on professional environmental (x2 =25.229,31.170,16.250,respectively;P < 0.01).Conclusions Nursing professional environmental conditions in Grade A hospitals is good,while some nurses have low satisfaction degree on it.So manager should pay attention to the implementation of incentive measures,and self growth of nurses in order to improve nursing professional environmental conditions.
