Application effects of PRECEDE model in improving self-care behavior among patients with chronic heart failure

Objective To establish health promotion plan based on Predisposing, Reinforcing, and Enabling Causesin Educational Diagnosis and Evaluation (PRECEDE) model for patients with chronic heart failure, and to observe the effects of PRECEDE on the self-care ability, so as to explore the feasibility of the health education based on PRECEDE model. Methods From March 2014 to February 2015, a total of 62 patients with chronic heart failure from the geriatrics department and cardiovascular department of a Class Ⅲ Grade A hospital were selected and divided into intervention group and control group, with 31 cases in each. The patients in the control group received routine heart failure treatment and nursing intervention. On the basis of the control group, the patients in the intervention group received health promotion plan based on PRECEDE model for 3 months. European Heart Failure Self-care Behavior Scale 9 (EHFScBS-9) was applied to compare the self-care scores between the two groups. Results There was no statistical difference before intervention in the total score of EHFScBS-9 between the two groups (t=0.000, P=1.000). The total score of EHFScBS-9 of the intervention group was lower than that of the control group after intervention and 3 months after (t=4.971, 9.156; P<0.001). The results of repeated measurement variance analysis showed that there were time,intergroup, time and group interaction effects between EHFSCBS-9 groups (F=34.489, 40.685, 16.922; P<0.001). Conclusions The health promotion plan based on PRECEDE model is effective and feasible, which can improve the self-care ability of patients with chronic heart failure.

Psychological experience of nurses in rescue of traffic accident: a qualitative study

Objective To explore the cognition, emotion and will experience of nurses in rescue of major traffic accidents. Methods The study was carried out with the phenomenological approach within qualitative research. From January 2014 to June 2017, a total of 10 nurses of China-Japan Union Hospital of Jilin University taking part in traffic accident rescue were selected as interviewees. Colaizzi method was used to sort out and analyze data. Results According to data analysis, 7 themes were induced including management capacity, operation skills of first aid, shock and heartache, sense of responsibility and mission, fear, being conscious of negative emotions as well as finding ways to control negative emotions. Conclusions The controlling ways of psychological pressure problem for first aid nurses are put forward by analyzing psychological experience of nurses in rescue of traffic accident so as to provide a basis for managers to formulate training system.

Effects of nursing based on psychological nursing procedure in patients with chronic obstructive pulmonary disease

Objective To explore the effects of nursing based on psychological nursing procedure in patients with chronic obstructive pulmonary disease (COPD). Methods A total of 280 COPD patients in the Respiratory Department of the First Affiliated Hospital of Xi'an Jiaotong University from January 2016 to January 2017 were selected by simple random sampling method and divided into two groups, with 140 cases in each group. The control group was treated with routine nursing. The observation group was treated according to the psychological nursing procedure on the basis of the control group. The intervention effects were compared using Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), General Self-Efficiency Scale (GSES) and Drug Attitude Inventory (DAI). The recovery of patients' lung function were evaluated and compared with 1 s forced expiratory volume (FEV1), forced vital capacity (FVC) and ratio (FEV1/FVC). Results After 3 months' intervention, the scores of HAMA and HAMD of the observation group were (8.74±3.21) and (7.94±2.17) respectively, which were lower than those of the control group, with statistical significance (t=5.980, 9.920; P<0.01). The total score of GSES and DAI of the observation group were (102.43±26.42) and (17.72±4.53), which were higher than those of the control group, with statistical significance (t=4.162, 5.969; P< 0.01). The FEV1/FVC of the observation group was (69.73±6.78)%, which was higher than that of the control group (57.74±5.16)%, and the difference was statistically significant (t=2.764, P< 0.01). Conclusions The nursing intervention based on psychological care procedure can alleviate COPD patients with anxiety and depression, improve self-efficacy and treatment compliance, which is worthy of clinical promotion.

Effects of link nurses guided working mode on improving the nursing quality of diabetic care in hospital

Objective To explore the effect of link nurses guided working mode on the improvement of diabetic knowledge and skills in nurses and the nursing quality of clinical diabetic care. Methods The 433 N1 nurses and 67 patients who received training in 2015 were selected as control group; the 433 N1 nurses and 67 patients who were trained in 2016 were enrolled as observation group. The control group was trained with the diabetes specialist team core members guided mode, while the observation group used the mode guided by the diabetes link nurses. The results of the theoretical and skill assessment, the qualified rate of reaching nursing quality, and the qualified rate of patients health education were compared between the two groups. Results In nursing theoretical knowledge, the observation group (qualified rate of 95.2%) was better than the control group (78.5%), and the difference was statistically significant (P<0.01). In practice skills, the qualified rates of the use of insulin and blood glucose meter of the observation group were 93.3% and 91.5% respectively, which were higher than those of the control group (61.7% and 67.2%), the differences were statistically significant (P<0.01). The qualified rates of nursing quality and patients health knowledge of the observation group were all higher than those of the control group, with statistical significance (P<0.01). Conclusions The working mode guided by diabetes link nurses can effectively increase the diabetes knowledge and skills and nursing quality in nurses who are not from endocrinology department.

Effect of different levels positive end-expiratory pressure on patients with severe craniocerebral injury complicated with acute respiratory distress syndrome and nursing measures

Objective To explore the effects of different levels positive end-expiratory pressure (PEEP) on patients with severe craniocerebral injury complicated with acute respiratory distress syndrome and its nursing measures. Methods A total of 46 cases of severe craniocerebral injury combined with acute respiratory distress syndrome patients in Xuzhou Central Hospital from August 2015 to August 2016 were selected. Changes of their arterial partial pressure of oxygen (PaO2), partial pressure of carbon dioxide in artery (PaCO2), oxygenation index (PaO2/FiO2), cerebral perfusion pressure (CPP), mean artery pressure (MAP) and intracranial pressure (ICP) were analyzed when PEEP changes from 3 cmH2O to 15 cmH2O. Appropriate nursing measures were taken during PEEP treatment. Results The value of PaO2was (73.4±9.8), (92.5±14.3), (105.0±16.9) and (114.6±16.4) mmHg when PEEP was 3, 7, 11 and 15 cmH2O respectively, and the difference was statistically significant (F=36.875,P<0.01). With the increase of PEEP, the value of PaO2, PaCO2and PaO2/FiO2showed tendency to ascend. There was a downward trend of MAP and CPP when PEEP was increased. With the increase of PEEP (3-11 cmH2O) ICP increased, and ICP was stable when PEEP was more than 11 cmH2O. Conclusions The safe range of PEEP is 3 to 15 cmH2O. Corresponding adjustment of PEEP according to patient individual condition, with proper nursing measures can improve therapeutic effects and reduce complications.

Effects of nursing human resources training and performance appraisal information management in a grassroots hospital

Objective To explore the effects of nursing human resources training and performance appraisal information management in a grassroots hospital. Methods From January 2017, Beilun branch hospital, the First Affiliated Hospital of Zhejiang University, carried out self-developed nursing human resources training and performance appraisal information management. We selected 50 nurses before information management (between July to December 2016) and 50 nurses after information management (between January to June 2017) as subjects. And then, we compared the theoretical examination achievements, operation examination achievements, the score of nursing quality and satisfaction with training of nurses before and after implementation. Results After information management the scores of theoretical examination achievements, operation examination achievements, the score of nursing quality of nurses were (93.48±1.95), (94.98±1.87) and (95.43±1.05) respectively higher than those before information management with significant differences (t=17.088, 18.035, 20.392;P<0.01). The satisfaction rate of nurses increased from 64.00% to 96.00% with a significant difference (χ2=16.000,P<0.01). Conclusions The application of information management in nursing human resources training and performance appraisal in a grassroots hospital can improve theoretical and operation examination achievements, increase satisfaction with training of nurses and improve nursing quality. It is worth to be popularized and applied in grassroots hospitals.

Application of MOOC in mother-infant nursing education

Objective To explore the application effects of massive open online courses (MOOC) in mother-infant nursing education. Methods The students from 2 class in Grade 2 of the 3-year midwifery major of Quanzhou Medical College were selected by purposive sampling method. All the students were randomly divided into experimental group and control groups, with 47 students in each. MOOC was applied in the experimental group, which the traditional teaching method was adopted in the control group. The theoretical assessment and satisfaction of the students were compared between the two groups. Results After the course, the theoretical assessment of the experimental group was (86.64±5.44), and that of the control group was (80.51±6.50). The difference in theoretical assessment was statistically significant (t=1.59, P<0.01). The total result of the students' satisfaction in the experimental group was (17.50±2.13), and that in the control group was (14.57±1.73). The difference in satisfaction between the two groups was statistically significant (t=7.320, P<0.01). Conclusions The application of MOOC can improve students' academic achievement and satisfaction. It has positive significance to improve the teaching quality of mother-infant nursing education.

Influence of field tracing defect situational simulation teaching on new nurses' critical thinking

Objective To explore the influence of field tracing defect situational simulation teaching on new nurses' critical thinking. Methods From 2015 to 2016, the new nurses from a general Class Ⅲ hospital were selected by cluster sampling method. The 2015 new nurses were assigned as control group (n=75), and the 2016 new nurses were assigned as observation group (n=83). Traditional situational simulation teaching was adopted in the control group, while the field tracing defect situational simulation teaching was applied in the observation group. The effects of training was evaluated with Chinese Version of Critical Thingking Disposition Inventory. Results The results of critical thinking ability of both the groups increased after training (P<0.05). The result of the observation group was (265.87±44.19), which was higher than that of the control group (243.39±37.25), and the difference was statistical significant (t=3.438, P=0.001). Conclusions The field tracing defect situational simulation teaching can effectively improve new nurses' critical thinking ability.

Effects of micro-teaching model based on guidebooks in operation training among nursing students

Objective To explore the effects of micro-teaching model based on guidebooks in practical training among nursing students so as to provide a reference for training and improving overall quality of nursing students. Methods From June 2015 to May 2016, a total of 63 nursing students of our hospital were selected in the control group by purposive sampling. From June 2016 to May 2017, 65 nursing students were chosen in the observation group. Nursing students of the control group accepted traditional teaching. Nursing students of the observation group were taught with the micro-teaching model based on guidebooks. The theory and operation achievements as well as nursing students' satisfaction with the teaching mode were compared. Results The theory and operation achievements of the observation group were higher than those of the control group with significant differences (P<0.01). The nursing students' satisfaction with the teaching mode of the observation was higher than that of the control group with a significant difference (P< 0.01). Conclusions The micro-teaching model based on guidebooks can improve the theory and operation achievements as well as nursing students' satisfaction with the teaching mode. It is worthy of spreading.

Motivation and cognition of choosing Medical Humanistic Care in non-medical profession college students: a qualitative research

Objective To investigate the motivation of non-medical students choosing Medical Humanistic Care curriculum and their cognition of medical humanistic care, so as to provide reference for the popularization of medical humanistic care knowledge. Methods A total of 10 non-medical students were selected by purposive sampling method for semi-structured interviews. The research data was analyze and refine the theme by phenomenological research method. Results Motivation of taking the course and medical humanities cognition were refined, which consisted of seven aspects, including learning to care others, interest in medical science, attention on the doctor-patient relationship ,the influence of family, doctor benevolence, mutual understanding between doctors and patients, and hard working with regret. Conclusions From the perspective of non-medical majors, teachers should tap their thirst for medical humanistic care, expand their medical knowledge and enhance their humanistic care ability.
