Yanhe JU ; Limin LIAO

Chinese Journal of Rehabilitation Theory and Practice.2017;23(5):612-615. doi:10.3969/j.issn.1006-9771.2017.05.027

Objective To investigate the video-urodynamics indication of upper urinary tract dilation secondary to neurogenic bladder that may be resolved only by intermittent catheterization. Methods From January, 2008 to August, 2016, twelve cases of upper urinary dila-tion secondary to neurogenic bladder were treated by intermittent catheterization only. Their clinical data was reviewed. Results The mor-phology and function index of upper urinary tract were improved gradually during the regular follow-ups. The common video-urodynamics characteristics include no detrusor overactivity, no vesicoureteral reflux, cysctometry volume larger than 300 ml and detrusor presser at the capacity lower than 40 cmH2O, poor voiding efficiency with residual volume larger than 150 ml. Conclusion For upper urinary tract dilation secondary to neurogenic bladder characterized as passable storage and poor voiding, intermittent catheterization may be enough to resolve the dilation.

Chinese Journal of Rehabilitation Theory and Practice.2017;23(5):607-611. doi:10.3969/j.issn.1006-9771.2017.05.026

Objective To investigate the risk factors of proximal junctional kyphosis (PJK) after posterior long segmental lumbar fusion for degenerative lumbar disease. Methods From October, 2012 to July, 2014, 118 degenerative lumbar disease patients who accepted posteri-or long segmental fusion and followed up at least 1.5 years were reviewed. All the PJK were evaluated with X-ray. Results Sixty-six pa-tients completed the follow-up (mean of 3.2 years), in which 12 patients with PJK (PJK group), and the others without (non-PJK group, n=54). There was significant difference in body mass index, bone density and the last Oswestry Disability Index scores (t>2.194, P<0.05) be-tween both groups, as well as the rate of the upper instrumented vertebra (UIV) located in the thoracolumbar segment (T11-L1) (χ2=4.63, P<0.05). The PJK angle was more in PJK group than non-PJK group pre-surgery, post-surgery and finally (t>3.862, P<0.001). Binary Logistic regression showed that osteoporosis (OR=5.328, 95% CI: 1.110~25.581), UIV located in the thoracolumbar segment (T11-L1) (OR=6.239, 95%CI:1.157~33.648) and the pre-surgery PJK angle>10° (OR=5.789, 95%CI:1.075~31.183) were the independent risk factors of PJK. Conclusion Osteoporosis, UIV located in the thoracolumbar segment (T11-L1) and the pre-surgery PJK angle>10° are risk factors of PJK af-ter posterior long segmental lumbar fusion.

Chinese Journal of Rehabilitation Theory and Practice.2017;23(5):584-586. doi:10.3969/j.issn.1006-9771.2017.05.020

Objective To evaluate the value of ultrasonic elastography for diagnosis of chronic nonspecific low back pain. Methods From March to September, 2016, 32 patients diagnosed as chronic nonspecific low back pain and other 32 healthy people (controls) were measured lumbar erector spinae with ultrasonic elastography, and calculated Yang's modulus. The correlation between Yang's modulus and Visual Analogue Scale (VAS) or Japanese Orthopaedic Association (JOA) score in the patients were investigated with Pearson's analysis. Re-sults There was no significant difference in Yang's modulus between bilateral lumbar erector spinae in both the patients and the controls (t<1.849, P>0.05), but it was more in the patients than in the controls (t=9.931, P<0.001). The Yang's modulus was positively correlated with VAS score in the patients (r=0.614, P<0.001), but not with the JOA score (r=-0.243, P=0.180). Conclusion Ultrasonic elastography can be applied to differentiate low back pain from the healthy, and measure the intensity of pain.

Chinese Journal of Rehabilitation Theory and Practice.2017;23(5):580-583. doi:10.3969/j.issn.1006-9771.2017.05.019

Objective To explore the construct validity of the Chinese version of Kinesthetic and Visual Imagery Questionnaire (KVIQ)-20 and KVIQ-10 in stroke patients and normal people. Methods From October, 2012, to March, 2014, 60 stroke patients and 60 nor-mal people with matched gender and age participated in this study. They were assessed with the Chinese version of KVIQ-20 and KVIQ-10. The construct validity was investigated with factor analysis. Results Two factors were extracted from KVIQ-20 for stroke patients after vari-max rotation, that accounted for 62.4%of the variance, as well as KVIQ-10 with 67.6%of the variance. Two factors were extracted from KVIQ-20 for normal people with 76.1%of the variance, as well as the KVIQ-10 with 69.6%of the variance. The factors could be named as visual imagery and kinesthetic imagery. Conclusion The Chinese version of KVIQ-20 and KVIQ-10 has good construct validity for assess-ing motor imagery from the dimensions of visual imagery and kinesthetic imagery, both in patients with stroke and normal people.

Chinese Journal of Rehabilitation Theory and Practice.2017;23(5):575-579. doi:10.3969/j.issn.1006-9771.2017.05.018

Objective To apply the MRI T2 signal intensity of hippocampal subfield in patients with amnestic mild cognitive impairment (aMCI) as early imageology. Methods From October, 2014 to August, 2015, 20 aMCI patients accepted cognitive training (training group), 20 aMCI patients accepted speech communication (speech group), and 20 age-and gender-matched healthy old people (control group) were scanned with MRI using FSE-T2 sequence. The margin of hippocampal subfields were outlined manually for each side to measure the T2 sig-nal intensity. The correlation between hippocampal T2 signal intensity and the scores of Mini-Mental State Examinatlon (MMSE) was ana-lyzed in the training group. Results Before treatment, T2 signal intensity in the bilateral hippocampal head was significantly high in the aM-CI patients compared with that in the control group (P<0.05). After treatment, T2 signal intensity in left hippocampal head decreased in the training group compared with that in the speech group (P<0.05), similar to the control group (P>0.05). There was negative correlation be-tween left hippocampal head's T2 signal intensity and the scores of MMSE in the training group before and after treatment (r=-0.61, r=-0.54, P<0.05). Conclusion The T2 signal intensity in left hippocampal head may respond to the cognitive function in patients with aMCI in the early stage, that could be used for diagnosis and evaluation in clinic.
