Effects of Captopril on the Patients with Essential Hypertension and Hypertension Complicated with Type 2 Diabetes Mellitus

Objective To investigate if there are differences of antihypertensive effect of captopril on the patients with essential hypertension(EH)alone and EH complicated with type 2 diabetes mellitus(DM).Methods Sixty three EH patients alone and 112 EH patients complicated with DM were studied. Among the patients complicated with DM, 56 cases who were treated with metformin and received captopril for 4 weeks. The changes of blood pressure,blood glucose,insulin,lipid, norepinephrine(NE),epinephrine(E)and uric acid were observed before and after treatment.Results After treatment by captopril the blood pressure was reduced and insulin sensitivity was increased in EH patients without complications,while no significant changes in EH patients with DM. After captopril in combination with metformine for 4 weeks in EH with DM,the blood pressure was significantly reduced and all examinations cited above were improved.Conclusion Captopril is suitable for EH patients without complications but not for EH patients with DM. After captopril combined with metformine resulted in the improvement of hyperglycemia and hyperinsulinemia, the anti-hypertensive effect of captopril was shown.

Role of Oxidative Stress in the Pathogenesis of Myocardial Stunning in Dogs and Effects of Aminoguanidine

Back ground Superoxide anion produced by oxidative stress combined with nitric oxide(NO)released by cellular NO synthase(NOS)generates peroxynitrite(ONOO-)which was one of reactive oxygen species.Objective To study the role of peroxynitrite in the pathogenesis of myocardial stunning(MS)and the effects of aminoguanidine(AMD).Methods Twenty-four dogs were randomly divided into four groups:(1)Short-time myocardical stunning(MS)group:the animals underwent 15 min occlusion of left anterior descending coronary artery(LAD),followed by 120 min of reperfusion.(2)Long-time MS group:60 min of LAD occlusion,followed by 120 min of reperfusion.(3)AMD group:Dogs were subjected to the same ischemia/reperfusion management as Long-MS group,but AMD was administrated(total 100 mg/kg).(4)Sham-operation group.Left ventricular function was evaluated by echocardiography and blood samples were taken from coronary venous sinus for the examination of NO at different time points.The stunned myocardium was examined immunohistochemically and electronic microscope.Results(1)Systolic thickening of stunned myocardium region and left ventricular ejection fraction were markedly declined during LAD occlusion in the 3 experimental groups,but progressively improved with the time of reperfusion.The improvement was faster in short-MS and AMD groups than in long-MS group.(2)Plasma NO concentrations in coronary venous sinus were greatly increased during reperfusion in short-and long-MS group,while no significant elevation was found in AMD group.(3)A marked increase in the immunoreactivity to nitrotyrosine,a specific "footprint" of peroxynitrite,was shown in the stunned myocardium both in short-and long-MS group,with larger and stronger positive foci in long-MS group.The positive staining was located in myocardial cytoplasm,especially at myofibrils and contractile bands.Treatment with AMD significantly reduced the nitrotyrosine staining in the stunned myocardium.(4)No ultrastructural changes of myocardial cells were noted in stunned myocardium in short-MS group except a slight loss of mitochondrial granules.On the contrary,myocyte edema,myofilament fractures,granule loss and swelling of mitochondrias were significant in long-MS group.AMD significantly ameliorated the ultrastructural abnormalities.Conclusion Myocardial stunning was associated peroxynitrite with production which mainly deteriorate the contractile proteins of myofibrils.AMD significantly inhibit the peroxynitrite formation leading to attenuation of the ultrastructural and functional injuries of stunned myocardium.

Therapy of Heart Failure Rats with Mesenchymal Stem Cell Transplantation

Objective Explore the effect of mesenchymal stem cell transplantation on cardiac function in rats with adriamycin(Adr)-induced heart failure.Methods MSCs were isolated from 8-week old F344 rats and cultured in vitro.Cells at the third passage were identified with a flow cytometer and labeled with(5-Bromo-2'-Dexuuridine,BrdU)before transplantation.BrdU-labeled MSCs(3?106 cells)was transplanted into rats(the transplantation group and the sham transplantation group,respectively)via femoral vein.Four weeks after the transplantation,cardiac function was evaluated using physiological recorder.Hearts were harvested and analyzed by immunohistochemistry to identify the transplanted BrdU-labeled cells.Results Four weeks after transplantation:the mortality of transplantion group was 6.2%,significantly lower than 12.5% in sham transplantation group(P

Comparison of Efficacy of Valsartan,Benazepril and Felodipine on Left Ventricular Hypertrophy in Essential Hypertension and Aldosterone Escape During Therapy

Objective To compare the efficacy of valsartan,benazepril and felodipine on reversal of left ventricular hypertrophy(LVH)in patients with hypertension and the relevant "aldosterone escape phenomena".Methods One hundred eleven patients with hypertension-related LVH were randomly to receive valsartan(80-160 mg/d,n=36),benazepril(10-20 mg/d,n=39)and felodipine(5 mg/d,n=36).Plasma angiotensin Ⅱ(Ang Ⅱ)and aldosterone(Ald)were determined before and 10-14 weeks 20-26 weeks after treatment.Echocardiographic examinations and blood routine,urine routine,blood glucose,blood lipid,liver function and renal function were conducted in all subjects before and after treatment.Results Blood pressure was significantly decreased in all three groups(P0.05).Valsartan increased plasma Ang Ⅱ at 10-14 weeks and furthermore at 20-26 weeks;benazepril decreased plasma Ang Ⅱ initiatively with trend of rebound at 20-26 weeks;however,plasma Ang Ⅱ was kept constantly in felodipine group.Valsartan decreased Ald and sustained during the all treatment period.On contrary,benazepril initiatively decreased Ald which was rebound to baseline level at 20-26 weeks.No change in Ald by felodipine was found during the treatment.After treatment plasma Ald level was significantly related to the reduction of LVH in both valsartan and benazepril groups.36% patients in benazepril group was found to have "aldosterone escape".Conclusion The antihypertensive effect was similar between valsartan,benazepril and felodipine.Although three medications all reversed LVH,but valsartan,benazepril was more effective than felodipine.There was no evidence of aldosterone escape in the long-term treatment with valsartan.Valsartan might have more advantages in reversal of LVH than benazepril.Felodipine had no effect on the plasma level of aldosterone.

Risk Factors in Rural Residents with Essential Hypertension of Different Social Economic Levels

Objective To explore the difference of the risk factors among the rural residents with hypertension with different social economic levels.Methods Risk factors survey were conducted in 469 new cases with hypertension(aged over 35) with different economic-social levels in Jizhou city,Hebei province using self-made questionnaire,2799 people(aged over 35) as the controls.The results were analyzed with Logistic regression analysis.Results OR of the risk factors in higher economic level group were:family history of hypertension(2.863),body mass index(BMI,1.286),preserved foods(1.263),the amount of eggs taken daily(1.200) and the ages of the patients(1.052).The OR of risk factors in lower economic levels' group were:family history of hypertension(3.990),smoking(1.767),the amount of eggs taken daily(1.753),drinking(1.728),education background(1.532),sex(1.448),daily amount of meat intake(1.276),BMI(1.205) and the age of the patients(1.068).The higher annual income of the family and the amount of fresh fruits intake daily were the protective factors of hypertension with the OR were 0.708 and 0.788 respectively.Conclusion The results show that the patients at different economic levels have different risk factors of hypertension.

p38 mitogen-activated Protein Kinase in Vascular Smooth Muscle Cells Proliferation in SHR
