The expression of P-selectin in rats with hepatic ischemia reperfusion injury

Objective To explore the express of P-selectin in liver-tissue,on the model of HIRI in rats,we explored the pathogenesy of HIRI. Methods 32 female Sprague-Dawley rats were divided into four groups randomly ( n = 8 in each group): sham operation group ( group SO ), ischemia 30 min without repe- rfusion group ( group Ⅰ),30 min reperfusion following ischemia 30 min group ( group IR30 min), one hour reperfusion following ischemia 30min group( group IR1h). The model of HIRI was maded by Pringle's hepatic vascular occlusion. The contents of P-selectin in hepatic tissue were measured by kit of SP. Results After ischemia 30 min,the content of P-selectin in hepatic tissue increased less obviously than group SO; After 1 h reperfusion, the content of P-selectin in hepatic tissue vascular endothelial and hepatocyte express more broadly than group SO. Conclusion The content of P-selectin in hepatic tissue was increased gradually in the process of hepatic ischemia reperfusion injury(HIRI) in rats.

Clinical Significance of blood lactic acid dynamic monitoring in the prognosis evaluation of critically ill pediatric

Objective To study the correlation between blood lactic acid level and PCIS scroing system.To study the relationship between dynamic lactic acid monitoring indexes and prognosis of critical patients.Methods Differences of mortality,incidence of MODS and shock,PCIS score between two groups were compared.The patients were divided into 3 groups by PCIS score to compare pathogenetic conditions prognosis, then to study the were divided into death group and survival group by the hospital prognosis. Differences of their lactic indexes and other clinical indicators which reflected organ/system status were compared, prognostic-significant lactic indexes were selected by mu fti-variable logistic analysis. Results Patients with elevated lactic acid level had more incidence of shock and MODS, higher PCIS scores and higher mortality, the differences had statistical significance compared with the normal lactic acid group. There were significant linear correlation between ICU lactic acid level, peak lactic acid level and PCIS scores respectively. For the patients with elevated lactic acid level, peak lactic acid level, 12h lactic clearance had significant correlation with prognosis. Conclusion PCIS scoring system had a significant linear correlation with lactic acid level. For the patients with elevated lactic acid level,peak lactic acid level,12h lactic clearance were good indicators to evaluate patients' prognosis.

Analysis of 42 cases of total mesorectal excision for rectal cancer

Objective To investigate the clinical effect of the radial rectectomy with total mesorectal excision ih rectal cancer patients. Methods 42 cases with rectal cancer received total mesorectal excision were analyzed retrospectively. Results All 42 cases were performed with radial excision successfully,36 cases by stapling device. 3 cases of leak of anastomosis was happened after operation, one case treated with ostomy, the other two cases through drainage and better nutrition were cured. Assistant chemotherapy and radiotherapy were given to the patients. All patientswere followed up for 6 ～ 36 months, 2 cases with local tumor recurrence, liver metastasis 1 case.Conclusion The mesorectectomy excision could prevent the local recurrence after the operation of rectal carcinoma.It was effective to improve the patients'life quality.

Study on the way of blocking the father-child 、mother-child transmission of hepatitis B virus by combination therapy

Objective To study the way of blocking the father-child、mother-child transmission of HBV.Methods 387 fathers or mothers who were positive for HBV-DNA were divided into three groups according to the father and mother's HBV-DNA and anti-HBsAg. Then they were randomly divided into three groups,one group were treated by HBIG,the second group were treated by lamivudine,the third group were treated by HBIG and Lamivudine.90 of their children who were positive for HBV-DNA were randimly dinded into three groups,one group were treated by hepatitis B Vaccine,the second group were treated by hepatitis B Vaccine and HBIG,the third group were treated by hepatitis B Vaccine and Gm-CSF. Their serum HBV-DNA were tested by fluorescence quantitative - PCR method.Results After treatment on the fathers or mothers,the degree of HBV-DNA was decreased obviously. There were significant difference between the groups who were treated by HBIG and lamivudine and the group who were treated only by HBIG or lamivudine( P ＜0.05 ). The rate of their newborn with HBV-DNA( + ) were significantly lower then other groups. After treatment on the newborn with HBV-DNA( + ). The degree of HBV-DNA had significant difference between the groups who were treated by both drug and vaccine and the vaccine group(P ＜0.05). Conclusion The fathers or mothers with HBV-DNA( + ) were treated by HBIG、lamivudine before they want a child, and fheir newborn with HBV-DNA( + ) were treated by vaccine、HBIG、Gm-CSF. This was an important way to block the father-child、mother-child transmission of Hepatitis B Virus.

The Analysis of Hearing of Tinnitus Patients

Objective To investigate the necessity to test the hearing of patients whose initial symptom is tinnitus with pure tone audiometer. Methods To analyze the results of pure tone audiometer of 218 tinnitus patients.Results Among 218 cases of tinnitus patients,a total of 123 patients with abnormal hearing,among 95 cases of patients with normal hearing, except for 50 cases of tinnitus, no other cause of disease. Conclusion Among the normal hearing tinnitus patients, we should do further hearing test for them if they are not identified a clear cause of disease,such as auditory brainstem response,otoacoustic emission and other necessary checks to clear the reasons for tinnitus or the position of the unusual auditory system. It is important for early diagnosis and treatment of tinnitus. But the pure-tone audiometer should be a routine examination when tinnitus patients come for the first visit.

The study on the influence of different operation types to the serum immunoglobulin and peripheral blood T lymphocyte in the cholecystectomy

Objective To study and compare the influence of different operation types to the serum immunoglobulin and peripheral blood T lymphocyte in the cholecystectomy. Methods 68 patients treared with cholecystectomy were selected as research object,and they were randomly divided into control group( underwent open cholecystectomy group) 34 cases and observation group( laparoscopic cholecystectomy group) 34 cases, and the operation time,exhaust time, blood loss, VAS score and incidence of complications, serum immunoglobulin before and after the treatment at 12h and 24h,and peripheral blood T lymphocyte before and after the treatment at 24h and 48h of the two groups were analyzed and studied. Results The operation time of the observation group was shorter than that of control group,exhaust time was shorter than that of control group, VAS score was lower than that of control group, serum immunoglobulin and peripheral blood T lymphocyte after the treatment were all higher than those of control group,there were significant differences( all P ＜ 0.05). Conclusion The advantage of laparoscopic cholecystectomy was bigger than that of underwent open cholecystectomy, and the influence was smaller for immune system, and it was worthy of popularization and application.

Influence of Jiangzhipailuan decoction on the related factors of lipid metabolism in patients with polycystic ovary syndrome

Objective To explore the effect of Jiangzhipailuan decoction in regulating PPARα( belong to the nuclear receptor family of ligand-activated transcription factors ), PGC- 1α ( peroxime proliferator activated releptour)and SREBP-1c( belong to the baichelix-loop-helix-leucine zipper class of transcription factors), SCAP( SREBP cleavage activating protein) related to lipid metabolism in the treatment of polycystic ovarian syndrome. Methods 84 patients suffered polycystic ovary syndrome were randomly divided into 4 groups: one received traditional Chinese medicine treatment, one western medicine treatment, one combination therapy and one as the control group. Traditional Chinese medicine group was treated with Jiangzhipailuan decoction treatment for 3 months, western medicine group was treated with up to Diane-35 ( ethinyl cyproterone tablets) for three cycles, while the combination therapy group was treated with traditional Chinese medicine ,western medicine as well as combined treatment for 3 courses. Results In the combination therapy group the PPARα, PGC-1α and decreased SREBP-1c, SCAP copy number was significantly improved ,and body mass index was significantly lowered. The total improvement rate of menstrual in three groups were 71.42% ,75.00% ,92.86% respectively. Conclusion Jiangzhipailuan decoction played a prominent role in regulating PPARα,PGC-1α and SREBP-1c,SCAP related to lipid metabolism in the treatment of polycystic ovarian syndrome.

Treatment of lubar intervertebral disc protrusion with CT guided ozone Injection and radiofrequency Thermocoagulation

Objective To explore the clinical effect of lubar intervertebral disc protrusion treatment with ozone injection combined radiofrequency thermocoagulation. Methods 36 patients with lubar intervertebral disc protrusion confirmed by clinical and imaging examination were underwent radiofrequency thermocoagulation followed ozone injection guided with CT. Results All the patients were followed up for 3 ～6 months,and statistic the improvement rate was evaluated according to Macnab standard. The total effective rate was 94.4%, and the improvement rate was 86.1%. Conclusion CT guided treatment could greatly elevate the accuracy of puncture. Radiofrequency target point thermocoagulation combined with ozone injection was a minimally invasive、safet and markedly effective method for lubar intervertebral disc protrusion treatment.

Effects of Xuebijing injection on inflammatory response in patients with acute respiratory distress syndrome

Objective To evaluate the effects of Xuebijing injection on inflammatory response in patients with acute respiratory distress syndrome(ARDS) by the changes of the level of PCT in bronchoalveolar lavage fluid (BALF) and plasma so as well as serum tumor necrosis factor-α(TNF-α) 、interleukin-6(IL-6) and interleukin-8( IL-8). Methods Sixty patients treated at the intensive care unit(ICU) were recruited in this study;these subjects satisfied the diagnostic criteria for ARDS. They were randomly classified into two groups: study group ( 30 patients) in which patients were given Xuebijing injection 100ml and 5% Glucose injection 150 ml daily,and control group(30 patients) patients were given 5% Glucose injection 250ml daily. The changes of the level of PCT in BALF and plasma so as well as serum TNF-α,IL-6 and IL-8 were monitored before intervention and seven days later. Results The level of PCT in BALF and plasma before intervention were statistically higher than normal( t= 2.882、2.657 all P ＜0.01 )while decreased significantly seven days later ( all P ＜ 0.01 ) in study group but fewer change in control group ( all P ＞0.05 ). Levels of TNF-α,IL-6 and IL-8 were statistically higher than normal before intervention( all P ＜ 0.05 ) while decreased significantly seven days later( all P ＜0.01 )in study group but fewer change in control group( all P ＞0.05 ). The mortality of patients was 26.7% ( study group) and 33.3% ( control group) ( all P ＜ 0.05 ) respectively.Conclusion Xuebijing injection could lower the level of PCT in BALP and plasma of patients with ARDS,played the role of antiinflammatory and improving the oxygenation, and could to reduce the mortality of patients.

Biological evaluation of recombinant human bone morphogenetic protein-2 artificial bone in lumbar intertransverse process fusion

Objective To evaluate rhBMP-2/20%β-TCP/PDLLA complex biological properties.Methods Preparation of casting salting was carried out using three-dimensional porous 20% rhBMP-2/β-TCP/PDLLA. 24 4-month-old healthy male New Zealand rabbits were randomly divided into A,B groups of 12,according to the method described in Boden production L4,5 intertransverse process fusion model. A group was embedding rhBMP-2/20%β-TCP/PDLLA complex,and B group was implanted autogenous iliac bone block. Animals were killed according to postoperative time(3 weeks,6 weeks, 12 weeks) ,for gross observation, X-ray observation of the formation of new bone, artificial bone degradation, and early histological manifestations. Results Composite artificial bone and autologous iliac bone graft were able to achieve good spinal fusion,artificial bone graft where there is no inflammatory response,but also in new bone formation,while reshaping the gradual degradation of carrier material. Conclusions 20%β-TCP/PDLLA showed good biocompatibility. Its structure and micro-environment created by its degradation were conducive to play a role for the rhBMP-2. β-TCP/PDLLA had strong activity in inducing bone and good bone fusion ability,and it was a promising bone graft substitutes, which fusion power was stronger than that in autologous iliac bone graft.
