Ependymal Cyst of the Sylvian Fissure.

Cerebral ependymal cysts are very rare. About 30 cases have been reported in the literature. They mimic arachnoid cysts clinically and on imaging studies but are different from arachnoid cysts in pathological findings. They are believed to arise by the sequestration of a small segment of the primitive ependymal lining into either the cortical mantle or the perimedullary mesh. They occupy the central white matter of the frontal or temporoparietal lobes. The authors describe a case of septate ependymal cyst involving left sylvian fissure. The clinicopathological features, treatment, and results of previously reported cases are reviewed, and the etiology and pathogenesis of these cysts are discussed.

Cerebral Parenchymal Racemose Cysticercosis.

Cysticercosis is a relatively common disease in Korea. Racemose cysticercosis, a peculiar form of the neurocysticercosis usually occurs as meningeal forms and rarely involves the cerebral parenchyme. The authors present a case of cerebral parenchymal racemose cysticercosis in a 55 year-old man with generalized seizure. Brain CT and MRI showed a large multilobulated cyst in the right frontal lobe. At surgery, we removed cyst covered by milkish white, multiple septated membranes completely. Histological dignosis of surgical specimen was racemose cysticercosis without scolex.

Ependymal Cysts.

The authors report four cases of ependymal cysts. Among the benign cystic lesions of neuroepithelial origin, ependymal cysts have been reportedly located in the cerebral parenchyme. This usually become symptomatic after 40 years of age, although it is generally thought to be congenital in origin. Symptomatic cases have been treated effectively with resection of the cyst or shunting procedures. Stereotactic removal of the cyst was also a good alternative as management in the authors' cases. Differential diagnosis is mandatory with other benign intraparenchymal cystic diseases.

Subphrenic Pseudocyst as a Complication of Ventriculoperitoneal Shunt: Case Report.

Abdominal pseudocysts have been recorded infrequently as a complication of intraperitoneal shunting. Authors have experienced two cases of subphrenic pseudocyst as a complication of ventriculoperitoneal shunt, which were treated with revision of shunt. First patient had medulloblastoma with hydrocephalus. The other one was a patient who had subarachnoid hemorrhage and hydrocephalus from Struge Weber syndrome. It's diagnosis and treatment were discussed with review of literatures.

A Case of Carotid Cavernous Fistula Treated by Muscle and Polyurethane Foam Embolization: Case Report.

There have been various surgical methods to treat the carotid cavernous fistula such as the carotid ligation, trapping procedure, embolization with muscle, gelfoam and polyurethane foam, balloon technique and direct transcavernous approach. Each method has its own merits and disadvantages. The ideal treatment of the illness would be the selective closure of the fistula with preservation of carotid patency. The authors are reporting a case of C-C fistula in which selective muscle embolism preserving carotid flow was attempted at first, but the closure was incomplete that polyurethane foam embolization was done in addition with success.

A Case of Intramedullary Spinal Neurilemmoma.

A rare case of intramedullary neurilemmoma of the spinal cord has been reported. A 23-year-old woman was admitted to our hospital, on 4th July, 1982, because of weakness of legs and difficulty of urination. Neurological examination revealed motor weakness of the both lower extremities with muscle atrophy. Sensory examination showed hypesthesia below the level of L3 dermatome of the right side and of L4 dermatome of the left. All deep tendon reflexes of the lower extremities were abolishes and Lasehue's sign was not appeared. Manometric Queckenstedt's test showed a complete block, with CSF protein of 460 mg/dl. Pantopaque myelogram demonstrated a complete block at the midportion of L1 spine. Total removal of the intramedullary tumor which was located in the dorsal column of the thoracolumbar segment was performed. The tumor was 4.0x1.5x1.0 cm in size and it proved to be an Antoni-A type neurilemmoma histologically. Only 20 similar cases have been published so far and summarized on Table 1, including our case. The relevant literature was reviewed.

Oligodendroglioma in the Fourth Ventricle: Case Report.

The authors present a case of oligodendroglioma in the 4th ventricle which is extremely rare in occurrence. The patient is 11 year-old male whose complaints were severe headache and vomiting. In the brain computes tomogram, hyperdense mass seated in the 4th ventricle with marked hydrocephalus. The patient had operation for removal of the tumor. In the operating field, there are no relation of the choroid plexus and specific vesselas but the tumor attached firmly on the floor of the 4th ventricle. The tumor was removed totally. The pathologic specimens were verified oligodendroglioma.

A Case of Pleomorphic Xanthoastrocytoma.

A case of pleomorphic xanthoastrocytoma of the temporal lobe in a 27 year-old man who had severe frontal headache and diplopia is presented. Brain computed tomographic scan revealed a relatively wel demarcated, slightly hyperdense mass with cyst surrounding low density in left temporoparietal area. After contrast enhancement, the mass was totally removed. Despite of their highly pleomorphic and bizzarre cytology, which suggests a malignant behavior, these tumors appear to have a relatively favorable prognosis.

A Case of Intrathoracic Meningocele.

An intrathoracic meningocele is a saccular protrusion of the meninges through a normal or enlarged intervertebral foramen or bony defect of the thoracic vertebrae. It extends anteriorly between the ribs and into the thoracic cavity, displacing the pleura and lung and lying in the paravertebral gutter if of sufficient size. Meningocele, single or multiple, are prone to occur in cases of von Recklinghausen's neurofibromatosis, often in association with kyphoscoliosis. In this case, we found a case of intrathoracic meningocele associated with multiple lipoma of variable sizes in the back and hyperpigmentation in the anterior chest and upper back region but no evidence of neurofibromatosis. Because of rarity of this disease entity, we report a case of intrathoracic meninggocele with review of literature.

Clinical Analysis of Functiononing Pituitary Adenoma.

Forty-four patients were operated on functioning pituitary adenomas during 5 years since Jun. 1977 at the Department of Neurosurgery, Seoul National University Hospital. Clinical and laboratory findings were analyzed and the following results were obtained. 1) The peak incidence was in the 3rd and 4th decade 21 cases were male and 23 cases were female. 2) Endocrinologic study revealed prolactinoma in 28 cases(37%), somatotropic adenoma in 13 cases(17%), corticotropic adenoma in 3 cases(4%), endocrine-inactive adenoma in 32 cases(42%). 3) MAin symptoms were endocrine symptoms(86%), headache(75%) and visual field defect(59%). 4) Seven(16%) were in the stage I(micro-adenoma), 27(61%) in the stage II, 6(14%) in the stage III, 4(9%) in the stage IV. 5) The rate of endocrinologic cure or improvement was 65% in prolactinoma and 75% in somatotropic adenoma. This result was less favorable less favorable than the result of other recent studies, but it seemed to be due to large proportion of large or invasive adenoma instead of micro-adenoma in this series. 6) In prolactinoma, 7 out of 12 patients with amenorrhea resumed menstrual cycle and 2 patients had pregnancies postoperatively. 7) The visual field defect was improved in 16 of 21 cases(76%) postoperatively.
