The designing of will tendency scale for inpatients′ articipation in patients for patient safety and the research in its structure of the scale

Objective To design patient paticipatin in patient safety willness and behavior scale (PSWBS) and check its validity and explore structure model. Methods Based on the related literature at home and abroad , the theoretical model was developed on the basis of preliminary survey questionnaire and the formal scale. Preliminary survey by questionnaire was carried out on sample 1, through item analysis, exploratory factor analysis and confirmatory factor analysis , reliability analysis , correlation analysis , and binary content validity test scale evaluation. Results The scale was composed of four dimensions of 14 items , qualified for the theoraticl research of a scale in terms of its internal consistency reliability , content validity , structure validity associated with the school. Conclusion The scale is good at the reliability and validity , suitable for measuring the hospitalized patients′ participation in willingness to maintain their own medical treatment was safe , and mainly adopts propensity to maintain their own medical safety behavior.

Diagnostic efficacy of MRI for preoperative assessment of staging and myometrial invasion in endometrial carcinoma

Objective To assess the effect of magnetic resonance imaging (MRI) on preoperative staging of endometrial carcinoma and depth of myometrial invasion. Methods Fifty-six patients with endometrial carcinoma were underwent conventional and contrast-enhanced MR scan. The results of pathology , MRI examinations of preoperative staging and depth of myometrial invasion were assessed and compared. Results The accurate rate of MRI diagnosis in preoperative staging diagnosis was 82.1% (46/56) and the consistency check as Kappa was 0.771 (P < 0.001). Conclusions MRI may be a good diagnostic tool with high sensitivity and specificity for assessing myometrial invasion and detecting tumour extension and should be considered as a routine part of the preoperative assessment in all patients with endometrial carcinoma.

Influence of acute non-isovolemic hemodilution plus tranexamic acid on coagulation factors and hemorrhage

Objective To study the effect of acute non-isovolemic hemodilution (ANIH) plus tranexamic acid on bleeding and coagulation factors. Methods Forty-two patients with brain tumor under general anesthesia were randomly divided into group N and group T with 21 patients in each group. Group N was given ANIH , while group T was given tranexamic acid and ANIH. Bleeding, transfusion, urine volume were recorded. Coagulation factors and D-dimer were detected one day before and after surgery. Hemoglobin was recorded before and after ANIH and after auto-blood was transfused. Results There was less bleeding in group T. Hemoglobin in group T was higher after transfusion. No significant difference was found in Group T and group N in terms of urine volume and transfusion rate. Both the two groups had no difference on variation of coagulation factors. Conclusion ANIH with tranexamic acid has no significant effect on coagulation but produces synergetic effect on decreasing bleeding. They can be applied in surgery of brain tumor safely.

The etiopathogenisis of fungal infections induced by systemic lupus erythematosus

Objective To explore the etiopathogenisis of fungal infections induced by systemic lupus erythematosus (SLE). Methods One hundred and forty-seven SLE patients with fungal infections during 2004-2013 were assigned as experimental group and the same number of SLE patients without infections were randomly selected as control group. Clinical and laboratory documents of these patients were comparatively analyzed. Results The fungal infections in SLE patients affected oral mucosa , lower respiratory tract and skin , and the pathogenic bacteria were mainly candida albicans. The wide application of glucocorticoid, immunosuppressants and antibiotics pushed the rise of incidence of fungal infections in SLE patients significantly. Conclusions The patients′ use of glucocorticoid , immunosuppressants and antibiotics attributes to a higher risk of fungal infections , especially infections by candida albicans. Fungus infection may raise the level of C4.

The change of large platelets and mean platelet volume in acute cerebral infarction patients with type 2 diabetes

Objective To investigate the clinical value of large platelets (P-LCR) and mean platelet volume (MPV) in acute cerebral infarction patients with type 2 diabetes. Methods 627 patients with acute cerebral infarction from January 2012 to May 2015 were enrolled and divided into type 2 diabetes and non-type 2 diabetes groups. The hemocytometer was used to detect the changes of P-LCR and MPV. Results The levels of P-LCR and MPV in the type 2 diabetes group were significantly higher than those of non-type 2 group (P <0.05). Logistic regression analysis showed that there was a statistically significant association between P-LCR, hypertension and hyperlipemia and the cute cerebral infarction patients with type 2 diabetes. Conclusion The levels of P-LCR and MPV could be used as important indexes for prognosis and early diagnosis of acute cerebral infarction patients with type 2 diabetes.

Autologous transplantation of adipose-derived mesenchymal stem cells improves nenal function in diabetic rats

Objective To investigate the renoprotective effects of autologous transplantation of adipose-derived mesenchymal stem cells (ADMSCs) in diabetic rats. Methods Sprague-Dawley (SD) rats were injected intraperitoneally with 40 mg/kg streptozotocin (STZ) for 5 consecutive days to induce type 1 diabetes. Four weeks following STZ injection, eighteen SD rats were randomized into two groups: the diabetic group (n = 9) and the ADMSCs group (n = 9). Normal nondiaetic rats were set as the normal control (n = 9). Autologous ADMSCs were cultured and identified in vitro , which were intravenously injection to the ADMSCs group rats via the tail vein. At 8 weeks after transplantation, levels of blood glucose, insulin, serum urea nitrogen, serumcreatinine and urine protein were measured. Meanwhile the body weight and kidney weight were examined. Results Mesenchymal cell surface markers were expressed in the cultured ADMSCs. The ADMSCs could differentiate into the adipogenic and osteoblastic lineages. Both the diabetic group and the ADMSCs group rats had higher levels of blood glucose , urea nitrogen , serum creatinine , urine protein and higher ratio of the kidney weight/body weight than those in the normal control group (P < 0.05, respectively). Blood glucose, urea nitrogen and the ratio of kidney weight/body weight in the ADMSCs group rats were significantly decreased compared with the diabetic group (P < 0.05, respectively). The decreased insulin level was attenuated after transplantation of ADMSCs (P < 0.05). Besides, levels of serum creatinine and urine protein in the ADMSCs group were lower than those in the diabetic group with no significant difference. Conclusion Autologous transplantation of ADMSCs can improve metabolic disorder and relieves diabetic renal damage.

Protective effects of Zhenwu decoction on cardio-renal syndrome rats

Objective To analyze the mechanism of interaction between cardiac and renal failure and observe the protective effects of Zhenwu decoction on cardio-renal syndrome rats. Methods The rats were divided into five groups: Sham group, MI (myocardial infarction) group, MI merger STNx group, MI intervention group, MI merger STNx intervention group. The first three groups were fed with saline (NS) for two weeks after surgery. The latter two groups of rats were fed with an equal volume of Zhenwu decoction. The cardiac function of rats in each group were evaluated , including the mass of heart , left ventricle and left kidney weights as a ratio of body weight, fractional shortening and left ventricular ejection fraction. And serum levels of indoxyl sulfate and serum creatinine were also achieved. Results MI merger STNx could further aggravate the heart function, kidney function and left ventricular remodeling in rats. Zhenwu decoction could obviously improve the heart function, kidney function and left ventricular remodeling in rats. Conclusions Based on the MI models, STNx can worsen heart function and kidney function. Zhenwu decoction can significantly improve heart function and kidney function.

Role of P2X7 receptor in learning and memory dysfunction induced by gp120 in rats

Objective To investigate the role of P2X7 receptor in learning and memory dysfunction induced by HIV-1 enveloped protein gp120 in rats. Methods The imitating HIV-1 associated dementia (HAD) animal models were established by intracerebroventricular (ICV) infusion of gp120 in rats. The effect of gp120 on the learning and memory dysfunction in rats was evaluated by Morris water maze (MWM) test. The role of P2X7 receptor (P2X7R) was studied by Western blot and PCR assay. Results The ICV infusion of gp120 for 3 days in rats could imitated the HAD animal model. Results of MWM test showed that the rats in the model group had longer escape latencies and errors compared with those in the control group (P < 0.01); Results of Western blot and PCR assay showed that the expressions of P2X7R and P2X7 mRNA in hippocampus of rats in the model group were significantly increased (P < 0.01). Conclusions The ICV infusion of gp120 in rats could imitate the HIV-1 associated dementia (HAD) animal models, and P2X7R may be involved in the pathophysiological process of learning and memory dysfunction caused by gp120.

Effects of shenfu injection on the awakening quality of patients with hepatitis B cirrhosis undergoing splenectomy after general anesthesia

Objective To observe the effects of shenfu injection (SFI) on the awakening quality of the patients with hepatitis B cirrhosis undergoing splenectomy under general anethesia. Methods Forty patients with hepatitis B cirrhosis and hepatic insufficiency (ASA classⅡ~Ⅲ) underwent splenectomy by general anesthesia. Patients were all sent into the post-anesthesia care unit (PACU) shortly after the operation with unconscious and no spontaneously breathing. They were randomly divided into two groups: SFI treatment group (Group SFI, n =20) and normal saline controlled group (Group NSC, n = 20). SFI group were treated with SFI (1 mL/kg, i. v.) in 10 minutes, and NSC group were treated with normal saline (1 mL/kg,i.v.). The time of eyes opening, extubation of tracheal catheter and the detention time of PACU were recorded. The heart rate (HR) and the average artery presses (MAP) were monitored at 4 time points: before SFI and normal saline administration, 5 min, 15 min, and 45 min after administration. The incidence of restlessness during the patients awakening period was also recorded. Results The time of eyes opening, extubation and the detention time of PACU of SFI group show no significant difference compared with the NSC group (P > 0.05). SFI and normal saline intravenous injection did not cause significant changes on HR and MAP at the time of 5 min , 15 min and 45 min compared to the time of before administration (P > 0.05). The incidence of restlessness during the patients resuscitation period in SFI group were lower than in NSC group (P < 0.05). Conclusion Shenfu injection can effectively improve the awakening quality and decrease the incidence of restlessness of the patients with hepatitis B cirrhosis undergoing splenectomy under general anesthesia during the awakening period in PACU.

Effects about TMP to expression of proteins relating with JNK signal transduction pathways on cultured rat hippocampal neurons after anoxia-reoxygenation

Objective To investigate the effects about Tetramethyl -pyrazine (TMP)to Bad, Caspases-1 protein relating with JNK signal transduction pathways on cultured rat hippocampal neurons after anoxia-reoxygenation. Methods Rat hippocampal neurons were cultured in vitro and observed respectively on 7 ~ 9 days. Neurons were exposed to TMP in three different concentration (60,200,800 μg/mL) and JNK inhibitors (10 μmol/L). Control/normal groups were set in each experiment, except for the normal group. After 1 hour of treatment, the rat hippocampal neurons were placed in an incubator with 90%N2 + 10% CO2 for 2 hours to induce anoxia. Then, the rat hippocampal neurons were placed in an incubator with 5% CO2+95%air to establish reoxygenation. Bad, Caspases-1 protein were examined by Western Blot. Results After neurons with 60,200, 800 μg/mL TMP and 10 μmol/L JNK inhibitors on damage induced by anoxia-reoxygenation , the Bad , Caspases-1 protein are lower than the control group. But the 200 μg/mL group is better than 60 800 μg/mL groups. Conclusions Through the JNK signal transduction pathways, the Caspases-1, Bad protein expressions are lower than the control group. So TMP has obvious inhibitory action to rat hippocampal neuronal damage induced by anoxia-reoxygenation.
