Periodontitis and Periodontal treatment of Hansen's Patients in National Sorokdo Hospital (Case Report).

The purposes of this case report(8 case) were to investigate the oral health periodontal conditions (CPITN 1 : 3, CPITN 2 : 182, CPITN 3 : 528) and prevent natural teeth through periodontal treatment(non-surgical periodontal treatment and surgical periodontal treatment) of patients who have Hansen's disease, which is an infectious periodontitis. Operative method of periodontal treatment are scaling, root planning, gingivectomy, partial flap, full flap.

Reappraisal of the lateral tarsal strip in the treatment of paralytic ectropion.

The imbalance between paralyzed orbicularis oculi and normally functioning its antagonists such as the levator of the upper lid and the capsulopalpebral fascia of the lower lid in patients with leprosy causes ectropion, eyelid retraction and lagophthalmos. There are a lot of surgical methods to manage the ectropion and retraction of the lower lid including lateral canthoplasty, lateral canthopexy, spacer graft, canthal sling, medial tarsorraphy as well as temporal fascia transfer. In this study, we applied the lateral tarsal strip combined with Kuhnt- Szymanowski procedure to thirty-two patients with paralytic ectropion and this method was proven to be very effective in correcting the abnormal conditions.

6 Cases of Hansen's Disease(New case & Relapse).

In 2007, we find 3 new cases and 3 relapse cases, diagnosis by the clinical finding, skin smear, skin biopsy, lepromin test, ELISA for PGL-I antibody, and DNA-PCR. The results obtained were summarized as follows: 1. In 3 new cases, 2 Koreans are 70 and 79 years old, one foreigner (Filipino) is 38 years old, mean BI is 2.8, and mean O.D. of PGL-I antibody is 0.22, and numbers of TTC repeat are 10(Korean), 11(Korean), and 12(Filipino), and of GACATC repeats are 4(2 Koreans) and 3(1 foreigner/ Filipino). All new cases have no mutation of folP1 & rpoB gene. 2. In 3 relapse cases, the mean age is 56.8, mean BI is 5.6+, and mean O.D. of PGL-I antibody is 0.70, and numbers of TTC repeat are 11(2 cases), and 12(1 case), and of GACATC repeats are 4(all 3 Koreans). Among 3 relapse cases, 2 cases have the mutation of folP1 gene and 2cases have the mutationo frpoB gene, especially one case have the mutation of both folP1 & rpoB gene.

The summary of recent relapse leprosy cases in Korea(2003~2007).

Relapse may occur the poor killing of M.leprae due to inadequate treatment in early onset relapse, and persisting M.leprae and drug resistance mutants in late onset relapse. The relapse of leprosy after the release from treatment(RFT) has profound implications for the public Hansen program including case management, compliance, disease transmission and control. For the understanding of recent characteristics of the leprosy relapse in Korea, we are summarized and reported the epidemiological factors. 23 relapse cases were reported in Korea from 2003 to 2007. Among 23 cases, 18 cases were the lepromatous type, and 3 cases were the tuberculoid type. The mean duration of past treatment was 16.4 years, and the mean interval to relapse was 28.5 years.

The Role of Suppressor T Cells in Mycobacterial Infection.

Suppressor T cells (Ts cells) once became probably the most controversial topic in the field of immunology. However, recently the picture has changed dramatically. Suppressor T cells, now less provocatively renamed regulatory T cells (Treg cells) are isolated and can be expanded in vitro and in vivo and their role is the subject of intensive investigation. It is now well recognized that Treg cell is central components of fundamental immune functions such as self-tolerance, anti-tumor response, T cell homeostasis, allergic and autoimmune diseases, allograft transplantation and control of infection. Although regulatory T cells play a crucial role in the control of immune responses to bacteria, fungus, virus and parasites, little is known about the role of Treg cells in mycobacterial infections. Here, I briefly describe 1)the biology of Treg cells, 2)induction and expansion of pathogen-specific Treg cells, 3)beneficial and detrimental roles of Treg cells in infection and 4)"Old Friends" mechanism of hygiene hypothesis. This article also explores observations on Treg or Ts cells in mycobacterial infectious diseases such as leprosy and tuberculosis. I finally summarize the potential for Treg-targeted immunotherapy in infectious diseases allergic and autoimmune diseases as well as transplantation and anti-tumor immunity. The correct balance of effector/pathogenic and regulatory T cells for successful immunotherapeutic approach is also emphasized.

Case of Hansen's Disease with vitiligo and xanthelasma.

Hansens' disease is well known chronic inflammatory granulomatous disease by Macobacterium leprae., and occur rarely in these days. The wide range of clinical manifestations develop by status of the host resistant. These are from mild sensory change, erythematous macular patches, diffuse infiltrating plaque, and nodules to severe destruction of peripheral nerve and internal organ involvements. Vitilgo and xanthelasma rarely may occur in the lepromatous leprosy. The patient was a 60-year-old a farmer who complained erythematous diffuse ill defined infiltrative plaques or nodules on the face, trunk. He was diagnosed as lepromatous leprosy by skin biopsy and fite staining and had taken the standard 3 multidrug (dapsone, rifampicin, lamprene)therapy. About 3 months during the therapy, the existing skin lesions became erythematous and mild edematous, some of which show vitiligo like change, and severe general aching and neuralgia developed. Type 1 lepra reaction with upgrading was diagnosed by clinical symptoms and skin biopsy feature. The vitiligo lesions also appeared on the normal looking skin without previous lepromatous lesion. At that time, yellowish plaque appeared on both eyelid and diagnosed as xanthelasma without hypolipoproteinemia. The mutidrug therapy for lepsory continued and oral predinsolone was given for the general aching of neuralgia. The lepra skin lesions had been improved gradually and the vitiligo lesions also disappeared. Presenting case is very interesting in point of view that he had vitiligo related to type 1 lepra reaction, and simultaneously developed xanthelasma palpebrum.

A Study on the fabrication request for the removable partial denture framework.

The purpose of this study was to investgate and evaluate whether the recording information contained in the work authorization was performed properly and master casts were designed accurately according to the rationale and principles of the removable partial denture when the work authorization and the master casts were sent to the dental framework. the accurate recording of the work authorization sent to dental laboratory in Hwasun for removable partial denture framework, the normal form of the occlusal rest guiding plan of the surveyed crown using the surveyor, the location analysis of the occlusal rest seat, the design patten of the maxillary and mandibulary major connectors, the use of the surveyor on the master cast, the status of the secondary impression taken in the distal extension edentulous areas were investigated for this study. The following results were obtained : 1. In the status of the occlusal rest seat prepared in the abutment, the occlusal rest seat with normal form was 81.25%, 95.45%, that without form was 6.25%, 0.00%, respectively in Class I and Class II on the maxillary cast and the occlusal rest seat with normal form was 76.92%, 95.45%, that without form was 7.69%, 0.00% respectively in Class I and Class II on the mandibular cast. 2. In the status of the guiding plan, the guiding plan with the normal form was 79.17%, 86.36% that with abnormal form was 20.84%, 13.64% respectively in Class I and Class II on the maxillary cast and the guiding plan with normal form was 79.49%, 77.27%, that with abnormal form was 20.51%, 22.73% respectively in Class I and Class II on the mandibular cast. 3. The abutment splinting in the distal extension removable partial denture was 20.78%, 27.27%, respectively in Class I and Class II on the maxillary cast and was 25.64%, 13.64% respectively in Class I and Class II on the mandibular cast. 4. The location and form of the major connector delineated on the maxillary and mandibular master cast was 95.83%, 98.41% respectively. 5. The delineation and recording on the work authorization was incomplete as group II(73.33%), group I(14.07%), group III(12.59%) and group IV (0.00%) in investigating the contents of the work authorization. 6. Aker's clasp of the types of the direct retainer was nots used 72.22%, 74.60%, in the maxillary and mandibular casts respectively. 7. The location of the indirect retainer delineated on the maxillary and mandibular master cast was 51.43%, 50.82% respectively. 8. The types of metal used were mostly Cr-Co non-precious metal alloy and the cases using color coding on the drawing paper of work authorization were not observed. 9. The cases taking secondary impression in the distal extension removable partial denture were 45.71%, 52.46% respectively in the maxillary and mandibular master cast. 10. The delineation of tripoding or reference line on the master cast was not observed in surveying procedure.

Epidemiological study of depression among patients with Hansen Disease at the national sorokdo hospital.

OBJECTIVE: Geriatric depression is a disease, that possibly can cause serious problems, in case it is not detected and treated. As the mean age of patients on Sorok Island increases, possibility of depression along with dementia rises, but up to date prevalence of this disease in this population has not been studied. This study identifies dementia and depression in patients on Sorok Island via a questionnaire survey, to incorporate the results in future treatment. METHOD: Two hundred thirty-six Sorok Island residents (142 male, 94 female) were enrolled in this survey including Geriatric depression scale (GDS), Korean modified Mini Mental Status Exam (k-mMMSE), and Global Deterioration Scale (GDS). RESULTS: Suspicious group of depression was found in 25% and Certain group of depression in 17%, which adds up to a high overall prevalence of 42%. There was no gender difference in Suspicious group of depression, but Certain group of depression was significantly more prevalent in women compared to men. Suspicious group of depression detected with k-mMMSE and GDS showed a prevalence of 30% and 35%, respectively, and prevalence was higher in women. Comparing depression group to non-depression group, a significantly higher prevalence of depression group was detected in patients with symptoms of dementia. CONCLUSION: The results, compared to prevalence studies in other populations, showed a higher prevalence of depression and dementia in patients on Sorok Island. This may be due to the relatively higher mean age or due to a realtively lower functional level of the patients. Possiblity of dementia accompanied by depression is high, and in geriatric patients, the denial of depression or misinterpretation of it as somatic disorders is common. Thus, implication of these results in treatment may yield an improvement of future outcome.

The Study of Anemia in Persons affected leprosy.

The anemia still remains the most common hematologic disorder in the the world despite improvements in general health and nutrition. Recently, the prevalence of anemia was reported 7.7% in 60-69 years and 16.0% in 70 years or older in male and was reported 11.9% in 60-69 years and 19.5% in 70 years or older in female. This study was aimed at assessing the prevalence of anemia in the persons affected leprosy aged over 60 years. For evaluation of anemia, including prevalence, typing, and cause, hemoglobin, MCV(mean corpucular volume), RDW (red blood cell distribution width), ferritin, iron, TIBC, reticulocyte count, and etc were checked. The prevalence of anemia was 42.8%(male 43.6%, female 42.2%), the proportion of the anemia classified by MCV was 11.8%(microcytic), 50.9%(normocytic), and 37.3%(macrocytic). Our results was higher than other past reports. So we will consider about the evaluation of higher prevalence of anemia in persons affected leprosy, and management plan for anemia in them by the in-depth studies.

Clinical observation of the ocular complications in Leprosy patients.

PURPOSE: To determine the prevalence of ocular complications in leprosy patients. METHODS: Screening ocular examinations were performed in 572 leprosy patients, who resided in National sorokdo hospital from May to October, 2011. RESULTS: Overall prevalence of ocular complications was 87.1%. Ocular complication was significantly associated with increasing age. There was no statistically significant difference in the incidence of the ocular complication among males and females. Lepromatous leprosy patients had the greatest prevalence of ocular lesion. CONCLUSIONS: There is a significant rate of ocular complications in leprosy patients in the National sorokdo hospital, and had a potentially sight-threatening ocular complication. Health professionals need to be new eye symptoms and signs require prompt ophthalmology review to prevent avoidable blindness, due to the life-long risk of sight-threatening ocular complications.
