Diagnostic criteria for disseminated intravascular coagulation

Since the concept of disseminated intravascular coagulation (DIC) was put forward by American scholars in 1951, there is no gold standard for DIC diagnosis. There are more than ten DIC diagnostic standards which are frequently revised, but there are still many shortcomings. The aim of this paper is to introduce the development of definition , diagnostic criteria of DIC and its assessment. , and the prospect of the future research.

Preparation of biotin-grafted pullulan acetate nanoparticles conjugated with CD3 antibodies and their uptake of T cells

Objective Cytotoxic T lymphocytes are the main effector cells of anti-tumor immunity. Active targeting of nanoparticles to T cells and activation of T cells can be achieved by conjugation with specific antibodies. We prepared the biotin-grafted pullulan acetate nanoparticles conjugated with CD3 (Bio-PA-CD3 NPs), and explored their effects on the proliferation, cytokine secretion and uptake of CD8+T cells.Methods We prepared Bio-PA NPs by the dialysis method, conjugated CD3 antibodies to the surface of NPs to make Bio-PA-CD3 NPs, and measured the diameter and Zeta potential of the NPs. We evaluated the effects of the NPs on the proliferation of CD8+T cells and the secretion of cytokines by CCK-8 assay and ELISA, respectively, and quantitatively analyzed the cellular uptakes of the Bio-PA-CD3 NPs by the flow cytometry.Results The Bio-PA-CD3 NPs exhibited regular spherical shapes of even size and with no adhesion. The content of CD3 antibodies on the surface of the NPs decreased with the increased degree of biotin substitution. The CD3 contents of the Bio-PA-CD3 NPs with biotin substitution degrees of 1.6%, 5.4% and 6.3% were (36.1±4.4), (21.4±4.3) and (10.3±4.7) μg/mg, respectively. Compared with Bio-PA NPs, Bio-PA-CD3 NPs at a certain concentration significantly enhanced the proliferation of CD8+T cells in vitro and promoted the secretion of IFN-γ, TNF-β and IL-2 cytokines. The Bio-PA-CD3 NPs manifested a higher cellular uptake with the increased content of CD3 antibodies.Conclusion The Bio-PA-CD3 NPs we prepared could be a promising agent to enhance the immune effect of T cells.

Expression of Spns2 in gastric cancer tissue and its effect on the proliferation and migration of gastric cancer cells

Objective Few studies are reported on the correlation of Spinster homolog2(Spns2) with cancer.This study aims to investigate the expression of Spns2 in gastric cancer and the adjacent tissue and explore its effects on the proliferation, clone formation and migration of human gastric cancer cell line SGC-7901.Methods Samples of gastric cancer and the adjacent tissue were collected from 20 patients (12 males and 8 females) after surgery between February 2016 and August 2016. The expression of Spns2 in the gastric cancer and adjacent tissues was determined by immunohistochemistry. Eukaryotic expression vector pEX-1 (pGCMV/MCS/EGFP/Neo)-Spns2 and siRNA were constructed and transfected into the human gastric cancer cell line SGC-7901. The cells were divided into a pEX-1- Spns2 (siRNA-Spns2) group and a negative control pEX-1 (siRNA-NC) group. The mRNA and protein expressions of Spns2 were detected by RT-PCR and Western blot, respectively. And the effects of Spns2 on the migration, clone formation and proliferation of the SGC-7901 cells after Spns2 over-expressed and down-regulated were evaluated by cell counting kit-8 (CCK-8) assay, clone formation assay, transwell experiment and wound healing scratch assay.Results The expression of Spns2 was significantly lower in the gastric cancer than in the adjacent tissue (0.39±0.04 vs 0.45±0.02, P<0.05). After transfection, both Spns2 mRNA and protein levels were remarkably higher in the pEX-1-Spns2 (21.96±2.08 and 10.71±2.78) than in the pEX-1 group (0.88±0.12 and 0.83±0.16) (P<0.05), but markedly lower in the siRNA-Spns2 (0.35±0.07 and 0.53±0.07) than in the siRNA-NC group (0.91±0.09 and 0.92±0.08) (P<0.05). At 48, 72 and 96 hours, the proliferation of the SGC-7901 cells was significantly decreased in the pEX-1-Spns2 group as compared with the pEX-1 group (P<0.05) but increased in the siRNA-Spns2 group in comparison with the siRNA-NC group (P<0.05); the clone formation rate was reduced in the pEX-1-Spns2 group as compared with the pEX-1 group (\[33.30±3.81\]% vs \[48.00±4.60\]%, P<0.05) but elevated in the siRNA-Spns2 group in comparison with the siRNA-NC group (\[48.38±4.22\]% vs \[26.25±4.88\]%, P<0.05).Conclusion Spns2 is lowly expressed in gastric cancer, and its over-expression in SGC-7901 cells can reduce its proliferation, clone formation and migration. Spns2 may play a role in inhibiting the development and progression of gastric cancer.

Study on promoter methylation of PCDHA13 gene and breast cancer development

Objective The mechanisms of PCDHA13 promoter methylation in breast cancer have not yet been elucidated at present. This study was to investigate the role of PCDHA13 gene promoter methylation in the development of breast cancer.Methods The methylation state of PCDHA13 gene promoter in human breast cancer tissues was detected by MassARRAY mass spectrum methylation sequencing. 100μmol/L 5-Aza was prepared with culture medium. The ZR-75-1 cells with 60% cell confluence were added to the final concentration of 5 μmol/L(low concentration group) and 10 μmol/L(high concentration group) 5-Aza, and the control group was only added culture medium. Detection of methylation status of PCDHA13 gene promoter in human breast cancer cells by bisulfite sequencing and methylation-specific PCR, and analysis of methylation status and mRNA expression of PCDHA13 gene by semi-quantitative RT-PCR. Western blot, MTT and DAPI staining were used to detect the effect of 5-Aza treatment on proliferation and apoptosis of breast cancer ZR-75-1 cells.Results The methylation degree of PCDHA13 gene promoter in the 1, 4-6, 9, 10 and 11 CpG loci in breast cancer tissues was significantly higher than that in normal breast group \[(0.2639±0.1575) vs (0.1612±0.1706), (0.2509±0.1377) vs (0.1688±0.0992), (0.4204±0.2087) vs (0.2621±0.1731), (0.3761±0.1407) vs (0.2824±0.1486), (0.3922±0.1294) vs (0.3072±0.1496)\], and the difference was statistically significant (P<0.05). The methylation rates of PCDHA13 gene promoter were reached 40%~100%in MDA-MB-231 cells and Bcap-37 cells.The methylation rates of PCDHA13 gene promoter were reached 10%~50% in MCF-7, rendered hypomethylation. The PCDHA13 gene promoter was hypermethylated in ZR-75-1 cells, and themethylation rate were 60% in forth CG site, 90% in first, eighth and twelfth CG site, 100% in the other CG sites. PCDHA13 gene was low expressed in MDA-MB-231 cells and Bcap-37 cells, high expressed in MCF-7 cells, but absent in ZR-75-1. There were only amplified methylated PCR products from ZR-75-1 cells in control group.While there were amplified methylated and non-methylated PCR products from ZR-75-1 cells treated with 5-Aza, and the high concentration group was significantly higher than the low concentration group (P>0.05). The expression of PCDHA13 mRNA of ZR-75-1 cells was loss in control group, but the expression of PCDHA13 mRNA was reversed after treated with 5-Aza, and the expression of PCDHA13 mRNA was significantly higher in high concentration group than that in low concentration group(P>0.05). After treated with 5-Aza for 24, 48 and 72 hours, the growth inhibition rates were lower in low concentration group than that in high concentration group (P>0.05). The morphology of the nuclei was basically normal and there was no apoptosis occurred in ZR-75-1 cells. But after treated with 5-Aza, some ZR-75-1 cells showed nuclear condensation, chromatin agglutination and heavy coloration.Conclusion This study showed that the low expression or loss of mRNA is associated with hypermethylation of the PCDHA13 gene promoter in breast carcinoma. The PCHDA13 gene expression can be reversed by 5-Aza in ZR-75-1 cells. The re-expression of PCHAD13 not only inhibit the proliferation of cells, but also promote apoptosis. Abnormal methylation of PCDHA13 may become a potential tumor marker for breast cancer.

Effect of baicalin Combined with phillyrin on the gene expression of influenza A virus nucleoprotein

Objective Influenza A (InfA) is an acute respiratory infectious disease persistently threatening human health and social stability. The high mutation of the InfA virus makes very significant the development of anti-influenza drugs. Traditional Chinese herbal drugs have shown distinct advantages in the prevention and management of InfA. This study aimed to investigate the effect of baicalin combined with phillyrin on the gene expression of InfA virus nucleoprotein (NP).Methods The nucleoprotein eukaryotic expression vector of InfA virus was constructed by transient transfection of InfA virus NP eukaryotic recombinant plasmid pcDNA3.1 (+)/NP into hela cells. Six groups were designed for the experiment: hela cells, liposome, recombinant plasmid (transfected pcDNA3.1(+)/NP), baicalin, phillyrin, baicalin (15.625 μg/mL-1) + phillyrin (12.5 μg/mL-1), each treated with respective agents after transfection. After 48 hours of culture, the number of the copied InfA virus NPs in the hela cells was measured by RT-qPCR and the effect of baicalin combined with phillyrin on the gene expression of InfA virus NP in the target cells evaluated by the statistical method.Results RT-qPCR showed that the gene expression of InfA virus NP was significantly lower in the baicalin + phillyrin than in the recombinant plasmid group (t=6.966, P<0.01). Dose-effect analysis revealed an antagonistic effect of low-dose baicalin + low-dose phillyrin (combination index \[CI\]>1) and a synergistic effect of medium- and high-dose baicalin + medium- and high-dose phillyrin (CI<1).Conclusion Baicalin combined with phillyrin can decrease the gene expression of InfA virus NP and has a synergistic effect within a certain dose range.

Curcumin influence the expression of matrix metalloproteinases in pterygium epithelial via inhibiting NF-κB signaling pathway

Objective Ultraviolet radiation can induce the expression of matrix metalloproteinases (MMPs) in pterygium epithelial cells. To investigate the effects of curcumin on the expression of MMP 2 and MMP 9 in human pterygium fibroblasts (HPFs) on UVA-irradiation and its possible mechanism.Methods After optimizing the dose of UVA irradiation and the concentration of curcumin, HPFs in the second generation were divided into control group (no exposure to UVA, no medication), UVA group (exposure to UVA), and UVA + curcumin group (exposure to UVA + curcumin). MMP-2 and MMP-9 levels were tested by zymography. The mRNA expression of MMP-2 and MMP-9 was detected by RT-PCR. The NF-κB-DNA binding activity was detected by electrophoretic mobility shift assay (EMSA).Results Compared with the control group, the secretion of MMP-2 and MMP-9 in the UVA group was increased significantly (P<0.05), which could be suppressed by curcumin (P<0.05). The MMP-9 mRNA levels were also significantly increased in UVA group \[(100±0)% vs (247.0±10.8)%, P<0.05\], and could be inhibited by curcumin \[(88.7±5.1)% vs (247.0±10.8)%, P<0.05\]. The NF-κB-DNA binding was remarkably increased in UVA group (P<0.05), and significantly decreased after treatment with curcumin (P<0.05).Conclusion Curcumin can inhibit the expression of MMP-2 and MMP-9 in UVA-irradiated HPFs, and the mechanism involved in this protective effect might be its inhibitory effect on NF-κB activation.

Impact of intertrochanteric cortical thickness on hip fracture types under dynamic load: A finite element analysis

Objective The influence of intertrochanteric cortical thickness on hip fracture can be investigated by the finite element method (FEM), but few dynamic FEMs have been established to manifest the extension of the crack. This study aimed to investigate the influence of intertrochanteric cortical thickness on the proximal femoral fracture types by FEM.Methods We recruited a healthy male volunteer from the Department of Traumatic Orthopedics, the First Affiliated Hospital of Guangzhou University of Chinese Medicine in October 2017, established a 3-dimention model, and reconstructed thick, middle and thin intertrochanteric cortex proximal femur models by meshing, defining material properties and setting boundary conditions. We measured the crack moment and von Mises stress, distribution of the fracture line, and the time-stress curves of the earliest damage element.Results Fracture occurred the earliest in the thin-cortex femur model but the latest in the thick one. The von Mises stress contour plot showed that, when damage started, the stress was concentrated on the posterior femoral neck and intertrochanteric region, extending to the posterior inferior part of the lesser trochanter in all the three models and, with the decline of the intertrochanteric cortex thickness, the concentration of the von Mises stress gradually narrowed down and shifted to the intertrochanteric region. The narrowing of the von Mises stress was the most obvious in the thin-cortex model, mainly distributed on the junction of the basal femoral neck and the intertrochanteric region. With the thinning of the cortex, the lesser trochanter became the region of stress concentration. The time-stress curves showed that fracture occurred the earliest in the thin-cortex model, with the maximum stress of 51.6 Mpa, but the latest in the thick-cortex model, with the maximum stress of 96.4 Mpa, and the maximum stress was 89.7 Mpa in the middle cortex model.Conclusion The thickness of the intertrochanteric cortex may be a determinant in the types of hip fracture.

A clinical analysis of the female urethral diverticulum

Objective Surgical treatment of the urethral diverticulum is difficult and may be followed by serious postoperative complications such as urethro-vaginal fistula. The purpose of this study is to investigate the treatment of female urethral diverticulum and its clinical effect.Methods We retrospectively analyzed the clinical data on 20 cases of female urethral diverticulum confirmed and treated in our hospital from January 2010 to December 2017. We obtained the Overactive Bladder Symptom Score (OABSS) from the patients after admission, treated them by transvaginal urethral diverticulectomy under continuous epidural anesthesia, and compared the signs and symptoms before and after operation.Results All the patients showed significantly decreased incidences of perineal discomfort, frequent micturition, urgent urination, and dysuria after operation (P<0.05). The average OABSS was remarkably lower postoperatively than the baseline (4.9±1.8 vs 8.1±2.3, P<0.05). No such postoperative complications as hematuria, urinary fistula, urethral stricture and urinary incontinence were observed in any of the patients.Conclusion Transvaginal urethral diverticulectomy is safe and effective for the treatment of female urethral diverticulum.

Percutaneous transforaminal endoscopic discectomy combined with dynamically assisted visualized intervertebral foraminoplasty for lumbar disc herniation

Objective How to improve the operational safety of foraminoplasty has become a hot spot in present clinical research. This study was to observe the clinical effect of minimally invasive treatment of lumbar disc herniation by percutaneous transforaminal endoscopic discectomy (PTED) combined with dynamically assisted visualized intervertebral foraminoplasty (VIVF).Methods Totally, 61 patients with lumbar disc herniation underwent PTED combined with dynamically assisted VIVF in Jiangsu Provincial Hospital of Integrated Traditional Chinese and Western Medicine from January to November 2017. We evaluated the clinical effects using the Visual Analogue Scale (VAS), Oswestry Disability Index (ODI) and modified Macnab Criteria.Results The VAS scores of the patients were significantly lower at 3 days, 3 months and 6 months after surgery than the baseline (1.10±0.60, 1.03±0.26 and 1.07±0.31 vs 7.64±1.11, P<0.05), and so were the ODIs (2.10±0.54, 1.30±0.49 and 1.23±0.46 vs 34.46±3.57, P<0.05). The excellence rate of treatment 96.72% (59/61). None of the patients experienced such postoperative complications as nerve root injury, spinal injury, and dural matter, and no recurrence was observed.Conclusion PTED combined with dynamically assisted VIVF is safe and effective for the treatment of lumbar disc herniation.

Clinical study on endoscopic closure of gastrointestinal fistula using the over-the-scope-clip system

Objective Gastrointestinal fistula is a serious complication after operation. It is reported that the over-the-scope-clip(OTSC) can close intestinal full-thickness wall and treat gastrointestinal fistula effectively. This study was to investigate the efficacy of OTSC in the treatment of gastrointestinal fistula.Methods We analyzed the clinical data of 28 postoperative patients with gastrointestinal fistula who underwent endoscopic closure using OTSC in Endoscopy Center of Research Institute of General Surgery in our hospital from October 2014 to October 2017, and recorded the course of disease, the site of gastrointestinal fistula, the diameter of fistula and the efficacy of OTSC. Patients with successful closure were followed up for half a year after hospital discharge to observe the recurrence of fistula.Results Endoscopic closure with OTSC was successful in all the 28 patients without complications like bleeding and falling off. In the sinus tract or digestive tract radiography at 1-2 weeks after OTSC closure, 23 patients were successful in endoscopic closure(82.14%). In the six months′ follow-up of 23 patients with successful treatment, 4 patients were lost to follow-up and 19 patients were successful without recurrence of fistula.Conclusion The endoscopic closure of gastrointestinal fistula using OTSC is safe and effective, avoiding reoperation and reducing trauma.
