Sequence and analysis of human anti-HBs? light chain gene

Objective:To obtain the clone and gene of light chain on human antibody against HBs.Methods:The phage displaying library was subjected to three rounds panning of “adherence-elution-amplification” with HBsAg coated in solid phase and help phage.The positive clone against HBsAg was characterized by ELISA assay and its P/N value was 2.415.The recombinant plasmid vectors from the positive clone was amplified by PCR with designed oligonucleotide primers of ? light chain.The ? chain PCR fragment was subcloned into plasmid pUC18 and sequenced.Results:The phage antibody against HBsAg was screened from a human immunoglouin phage displaying library.The pUC18-? recombinant plasmid was constructed and sequenced.The gene size of ? chain is 643bp including variable region and constant region.Conclusion:According to the sequenceing and ELISA assay,we suggest that ? chain is a human anti-HBs-? immunoglobuin.

Selection of treating methods for acute aortic dissection

Objective:To explore the selection of the treatment methods for acute aortic dissection.Methods:Thirteen patients with acute aortic dissection were studied and analyzed.Results:Of the 13 patients,2 received pharmacotherapy;6 underwent surgical treatment;2 were treated with inplantation of intravascular stent-grafts;1 experienced no treatment because of the ischemic necrosis of the abdominal organs,and 2 suddenly died of the rupture of aortic dissection in the course of emergent management.Conclusion:The emergent pharmacotherapy is essential to all patients with acute aortic dissection.The emergent surgical treatment of acute aortic dissection is selected mainly according to the Debakey Classification.Patients with Debakey Type Ⅰ and Ⅱaortic dissection should undergo operation as early as possible.The outcome of pharmacotherapy is similar to that of surgical treatment based on suitable indications for Debakey Type Ⅲ aortic dissection.Debakey Type Ⅲ aortic dissection can be treated by the intravascular stent-grafts too.

Research and preparation of cyclosporine A eyedrops

Objective:To establish the prescription and preparing procedure for the cyclosporine A eyedrops.Methods:The cyclosporine A eyedrops was prepared from castor oil.The cyclosporine A content of the preparation was determined by HPLC,and stability of the preparation was examined,too.Results:The results showed the procedure of preparation was simple,and the quantitative determination was quick,and accurate while the quality was reliable.Conclusion:The procedure described here can be uesed to prepare the cyclosporine A eyedrops.

Fluorescent exhibition of fundus oculi lesion in several types of retinal degeneration

Objective:To study the fluorescent manifestations of fundus oculi lesion in several types of retinal degeneration.Methods:Fundus fluorescein angiography(FFA) were performed in 8 patients(16eyes)of retinal degeneration.Results:Lesion of eyes was actinomorphic,and was relatively quiet,grade was low,mainly presented as retinal pigment epithelium(RPE) and choroidal capillary atrophy.Background image choroidal at early stage showed hypofluorescence,and big choroid blood vessel low fluorescence were seen.This differed from secondary retinitis pigmentosa.Conclusion:Results of FFA with retinal degeneration show barrier of choroidal capillary atrophy may be one factor for RPE and neurosensory retina dystrophy.

Clinical application of ZoomScope colposcopy

Objective:To study the value of ZoomScope colposcopy in diagnosis of cervical diseases.Methods:378 patients with cervical diseases were examined by ZoomScope colposcopy (WALLACH 906043-SP type) from June 2002 to April 2003.The results of 378 patients by colposcopy were compared with Thin-prep Cytology Test (TCT) and histology diagnosis.Results:Colposcopy found normal transformation zone in 114 cases,cervical condyloma in 68 cases,aceto-white epithelium in 95 cases,twin signs in 65 cases,triple signs in 20 cases,cervical cancer in 4 cases,while unsatisfactory colposcopic findings accounted for l2 cases.The accuracy,specificity and sensitivity of colposcopy in evaluation of cervical diseases were 80.56%,74.60%,and 97.48%respectively.The accuracy of colposcopy in diagnosis of cervical diseases were 88.36% and 80.56% respectively as compared with TCT,histology diagnosis,and that of cervical cancer was 100.00% as compared with histology examination.Conclusion:ZoomScope colposcopy in diagnosis of cervical diseases can be accurate,reliable.When colposcopy is combined with TCT and to bospy for cervical diseases,the diagnostic accuracy rate of the precancerous lesions or earlier cervical cancer can be further improved.

Clinical analysis of the effects of concealed denture retainers on periodontal health of the abutment teeth

Objective:To compare the effects on periodontal health of the abutment between concealed denture(flexible denture)and two traditional removable partial dentrue(RPD)-one with flexible retainers,the other with wire clasps and casting clasps,and observe whether the concealed denture can damage the abutment teeth.Methods:255 cases of the patients losing one of the first molar were collected,and divided into three groups,using one of three kinds of RPD and then repaired with the concealed denture,traditional plastic denture and casting frame denture respectively to one group.The indices of periodontal health of the abutment teeth were examined after 1 month and 1year.Results:Among three groups,the plaque index(PLI),gingival index(GI),probing depth(PD),teeth mobility and alveolar bone height of the abutment teeth were similar.Conclusion:Compared with two traditional RPD designs,there are no negative effects on periodontal health of the abutment teeth with the concealed denture long-term.

Insulin resistance during cardiac operations with cardiopulmonary bypass in children

Objective:Anesthesia,surgery and cardiopulmonary bypass (CPB) are conventionally considered the major stress factors in the hormonal and metabolic responses to cardiac surgery.This study was to determine the influence of CPB on hormones,carbohydrate metabolism and insulin resistance.Methods:Blood samples were obtained from 19 patients at the following intervals:(1)before anesthesia(2)before CPB(3) 15 min after begining of CPB(4)5min after declamping of aorta(5)20min after CPB.Insulin,C-peptide,cortisol,growth hormone and blood glucose were measured.The ratio of insulin to glucose in blood(I/G) and insulin-sensitivity index [1/(I?G)] were obtained.Results:Insulin,growth hormone and cortisol were higher during and after CPB than that before CPB.Similar changes in levels of blood glucose,I/G and C-peptide were also observed,even though there was a significant decrease of insulin-sensitivity index.Conclusion:There are an obvious increase of concentrations in stress hormones and blood glucose and an aggravation of insulin resistance.All changes above do reflect the degree of surgical stress.The secretion of pancreatic beta cells is not suppressed during and after CPB.It is suggested that insulin supplement therapy may be beneficial to improve insulin resistance in the period of CPB.

Anesthesia management in operation on patients with congenital esophageal atresia accompanying tracheo-esophageal fistula

Objective:To probe the anesthesia methods and management of respiration in the operation on congenital esophageal atresia accompanying tracheo-esophageal fistula.Methods:Retrospective analysis was used to study anesthesia process during operation of 9 cases.Results:Vital signs of 7 cases during operation were stable.No increased airway resistance or bronchospasm was observed.Seven patients who received endotracheal intubation and controlled respiration after slow induction developed no significant atelectasis or more severe pneumonia on post -operation X-ray.Stomach gassiness happened in two patients who received fast induction.Gradually increased airway resistance was observed and more severe pneumonia accompanying atelectasis happened in the two patients.No hypothermia happened among these 9 patients owning to right keeping warm.Seven patients were discharged in good health.The families of two patients gave up treatment.Conclusion:Pulmonary complication can be reduced effectively and the same stable anesthesia can be obtained by combining the existing autonomous breath after slow induction with controlled breath after blocking up fistula cannula.The effective aspiration,keeping warm, intensive transfusion and consummate analgesia during and after the operation are very important to guarantee the security for patients.

A comparative study for the caries activity between orthodontically treated and untreated group in adult orthodontic

Objective:To detect the caries activity of orthodontical treated and untreated adult.Methods:The study population consisted of 30 adults whose age are over twenty,the control group includes 30 adults who did not accept any orthodontical treatment.The detective methods were enumeration of streptococcus mutans in saliva,ability of plaque to produce acid and salivary buffering capacity.Student's test was used to analyse the statistical difference.Results:The results showed that the number of S.mutans and lactobacillus on the untreated group are much lower than that of the orthodontical group;the salivary pH,salivary buffering capacity and ability of germ to produce acid are similar between orthodontical treated and untreated adult.Conclusion:This study suggests that the orthodontical treated adult are more sensitive to dental caries than untreated adult.

Application of continuous brachial plexus block in prolonged upper limb operation on children

0.05).42 children were anesthetized perfectly except 3 cases with Horner syndrome and 2 failure cases.The complication such as hematoma and infection did not happen.Conclusion:Continuous brachial plexus block is a simple,feasible and safe method in complex and prolonged operation of upper limb.
