Cost-effectiveness Assessment of the Cataract Operation Modes

Objective To assess the cost-effectiveness of different modes of the cataract operation. Methods Firstly, 291 cases of cataract inpatient were divided four groups according to operation modes (PHACO and ECCE) and the prices of intraocular lens (IOL) (high and low). Then the comparisons of inpatient cost, operation cost, postoperative VA, postoperative VF and QOL were made among four groups respectively. Finally, the cost-effectiveness of different cataract operation modes was analyzed by comparing the differences of their costs with the VA, VF and QOL improvement. Results There were significant differences in the above indicators among four groups in general (P 0.01). The VA improvement amount of PHACO was higher than that of ECCE whereas the VF and QOL improvement lower than those of ECCE. The price of IOL had no effect on the improvement quantity of VA, VF and QOL. When one unit amount of VA improvement wanted, the cost-effectiveness ratio of inpatient with PHACO and low-price IOL was the lowest. When one unit amount of VF and QOL improvement wanted, the cost-effectiveness ratio of patients with ECCE and high-price IOL was the lowest. When one unit amount of VA and QOL improvement wanted, the operation cost-effectiveness ratio of ECCE with low-price IOL was the lowest. And when one unit amount of VF improvement wanted, the operation cost-effectiveness ratio of ECCE with high-price IOL was the lowest. Conclusions There were cost differences among different cataract operation modes. The cost of PHACO was higher than that of ECCE. The ECCE mode and price of IOL were the main determinants of cost. The effects of improving VA and QOL by ECCE were better and the effect of improving VA by PHACO was better than by ECCE. The homemade or low price IOL had no influence on the cataractous operative effectiveness, but could decrease the operation cost. ECCE with low-price IOL was the optimal cost-effectiveness strategy and operation mode, especially for poor rural and remote areas.

The Clinical Observation of Salina and Vitamin E Application in Nonsmall-cell Lung Cancer Patients Treated by Chemotherapy

Objective To study the effect of salina and vitamin E on the antiadverse reaction in nonsmall-cell lung cancer patients treated by chemotherapy. Method 124 cases nonsmall-cell lung cancer patients were selected during December, 2005 and December, 2007. Patients were randomly divided into two groups: A and B group, which were treated by salina plus vitamin E and placebo respectively. First, all patients were treated by radical operation. After operation, conventional chemotherapy (TAX+CBP) was followed. The day before chemotherapy, salina and vitamin E was oral administered to A group until before the third chemotherapy. The interval between chemotherapy was 3 weeks. B group was treated by the same way with placebo. Prior to the third chemotherapy, blood routine examination, liver and kidney function, immune index and antioxidation index were detected. Results After treatment, leukocyte number increased in A group, but decreased in B group. There was no obvious damage founded in liver and kidney function. CHE level was significantly enhanced in A group. The level of SOD in A group was significantly higher than that in B group. Conclusion salina+vitamin E could reduce the leukocyte number decrease caused by chemotherapy, increase the activity of cholinesterase, and enhance the antioxidation function of patients, so it can protect some side effects of chemotherapy in non-small lung cancer patients.

The Analysis on the Relational Factors of Pathological Internet Use among the Post-graduate Medical Students in Tianjin

Objective To explore the epidemic characters of Pathological Internet Usage (PIU) in the newly enrolled post-graduates and further to detect the relationship between PIU and relative social environment, etc., so as to provide more evidence for the etiology of PIU. Methods Questionnaire was conducted in 556 newly enrolled post-graduate students of a medical university. The recovery rate of the valid questionnaires was 94.72%, and the data were analyzed by chi-square test, t-test and logistic regression. Results The rate of internet-utilizing students in recent one year was 95.3%, thereinto, 5.3% of them was with internet addiction disorder. Multivariate logistic regression analysis showed that the major impact factors were gamble, playing games, the heavy study pressure, psychoticism personality characteristic tendency and low self-respect, the OR values (95%CI) were 13.966 (1.662~117.835), 3.440 (1.401~8.450), 1.249 (1.096~1.423), 0.889 (0.821~0.963) and 2.749 (1.167~6.475), respectively (all P

Relationship between Family Income Level and Metabolic Syndrome of Golden Wedding Couples in Rugao City, Jiangsu Province

Objective To investigate the impact and relationship between family income level (FIL) and prevalence rate of metabolic syndrome (MS) of golden wedding couples in Rugao city. Methods Random sample survey of golden wedding couples about MS in Rugao city was run recently, including relevant indexes of MS, insulin resistance (IR) degree (from HOMA-IR) and family income level. 587 couples were divided into three groups according to FIL by rapid cluster analysis: 196 with low FIL as group A, 196 with middle FIL as group B and 195 with high FIL as group C. The prevalence rate of MS, abnormal rates of blood pressure(BP), blood lipids, blood glucose including fasting plasma glucose (FPG) and postprandial 2 hours plasma glucose (2 h PG), body mass index (BMI) and insulin resistance (IR) of all groups were calculated and compared. Study on Correlated Risk Factors for Cardiovascular Autonomic Neuropathy in Diabetic Patients
