Subcutaneous Emphysema and Inflammation of the Neck after Tracheal Puncture by an Intubating Stylet.

Laryngo-tracheal perforation caused by the use of a stylet during tracheal intubation is a rare complication. We present a case of subcutaneous emphysema and connective tissue inflammation after tracheal intubation. The patient was a 41-year-old male undergoing general anesthesia for an appendectomy. The intubation was difficult during laryngoscopy (Cormack-Lehane Grade III). An assistant provided an endotracheal tube with a stylet inside while the laryngoscope was in place. During intubation, a short, dull sound was heard with a sudden loss of resistance after the distal tip of the endotracheal tube passed the rima glottis. A sonogram and computerized tomography revealed subcutaneous emphysema from the neck to the upper mediastinum and fluid collection between the trachea and the thyroid. This lesion appeared to have been caused by the protruded, loose stylet. Anesthesiologists should be aware of the damage a loose stylet protruding beyond the tip of the endotracheal tube can cause.

A Case of Varicella Pneumonia Associated with Chickenpox in Immunocompetent Patient.

Varicella is a contagious infection in childhood disease typically affecting children aged 2-8 years and usually follows benign outcome. In the adult, clinical presentation is more severe and more commonly associated with complications. Varicella pneumonia, although rare, is a potentially life-threatening complication that should be suspected in any adult with varicella and respiratory symptoms. We report a case of varicella pneumonia in immunocompetent patient. The characteristic radiographic findings consisted of diffuse scattered coarse nodular infiltrations, less than 1cm sized, with ground glass opacity and consolidation in both lung fields. The patients was started on intravenous acyclovir. The chest radiograph performed 2 weeks later showed complete resolution of the pulmonary lesions.

A Premature Newborn with Congenital Syphilis.

A preterm newborn affected by congenital syphilis, born to mother not treated during pregnancy is described. The clinical picture was characterized by respiratory distress, cutaneous manifestations, massive hepatosplenomegaly, severe anemia, thrombocytopenia, disseminated intravascular coagulation syndrome and hypoalbuminemia. The patient was treated with daily injections of 190,500 units of crystalline penicillin G for 14 days. Premature infants with these symptoms and signs should be evaluated for congenital syphilis.

A Case of Chronic Monocular Solar Retinopathy.

Solar retinopathy is a rare but well-recognized clinical entity of macular damage, caused by viewing a solar eclipse or direct sun gazing. A 21-year-old man gazed at the sun for approximately thirty seconds at noon using a monocular telescope with his left eye. Forty-eight hours after sun gazing, the patient experienced symptoms of blurred vision and central scotoma in the left eye. Eight months after sun gazing, the visual acuity decreased from 1.0 to 0.1 in the left eye and the fundus examination showed a round, yellowish-white discoid lesion at the left fovea. Fluorescein angiography showed an early window defect in the fovea of the left eye, that persisted without size change during the late phase resulting from atrophy of the retinal pigment epithelium. A small, central scotoma of the left eye was also found in the visual field test. The visual acuity was unchanged at the end of a one-year follow-up period.

A Case of Molecular Diagnosis of Ornithine Transcarbamylase Deficiency.

Ornithine transcarbamylase (OTC) deficiency is the most common inborn error of urea cycle metabolism; it is inherited in an X-linked manner. The OTC catalyzes the third step of the urea cycle, the conversion of ornithine and carbamyl phosphate to citrulline. Deficiency of OTC leads to the accumulation of ammonia, causing neurological deficits. In most affected hemizygote males, OTC deficiency manifests as hyperammonemic coma that often leads to death in the newborn period, and those who recover from the coma may be neurologically impaired due to the sequelae of the hyperammonemic encephalopathy. In some, late-onset manifestations develop. We report a male neonate with early onset OT deficiency that had apnea and was comatous. On mutation analysis using DNA sequencing after polymerase chain reaction (PCR) amplification of the 10 exons, deletions of 10 bases in codon 285, causing a frame shift was detected in exon 8. The mother and a sister were diagnosed as female carriers. Therefore, genetic counseling and the risk assessment could be provided to the family.

Angiomatoid Fibrous Histiocytoma: A Case Report.

Angiomatoid fibrous histiocytoma is a rare soft tissue tumor that generally affects children and young adults. We report a case of angiomatoid fibrous histiocytoma in an 11-year-old boy who complained of a back mass for 3 years. Surgical excision was performed. The excised specimen showed a 4.0 x 3.6 x 3.0 cm, well circumscribed, grayish white tumor, with multicystic changes. Histological examination showed proliferation of spindle or round shaped tumor cells. There was a dense fibrous pseudocapsule with prominent chronic inflammatory cell infiltrates.

Atypical Cerebellar Medulloblastoma Originating from Tentorium: Case Report.

The extraaxial presentation of a medulloblastoma is rare. This article describes the case of a 12-year-old boy who presented with severe headache, nausea, and vomiting. The tumor developed in the left tentorium; it was misdiagnosed as a meningioma based on the radiology examination. We review the literature and discuss the atypical presentation of medulloblastoma.

The Effect of Anticholinergic Agents on Postoperative Sore Throat in Patients Undergoing Endoscopic Sinus Surgery.

BACKGROUND: After sinus surgery, airway dryness is made worse because patients breathe by mouth. Anticholinergic agents have antisialogogue effects. The airway dryness that occurs after surgery is severe and can increase the incidence of postoperative sore throat. MATERIALS AND METHODS: One-hundered patients who were scheduled for endoscopic sinus surgery(ESS) were divided into four groups. Each group was premedicated with glycopyrrolate or had no premedication and reversed with glycopyrrolate or atropine. After the surgery the patients were asked about a sore throat and hoarseness postoperatively after 1h, 6h, 12h, 24h, and 48hours, respectively. RESULTS: The incidence of postoperative sore throat and hoarseness was not significantly different among the four groups at any time. CONCLUSION: The severity of complications after ESS was low and subsided within 24 hours. Therefore, the use of anticholinergic agents does not need to be limited.

The Factors Associated with Job Satisfaction of Assistant Nurses in General Hospitals.

BACKGROUND: The main aim of this study was to analyze the factors that could be used to predict job satisfaction of assistant nurses in general hospitals. MATERIAL AND METHODS: This study was based on a survey of assistant nurses in the Daegu region from January 1, 2006 to January 31, 2006. Originally 300 questionnaires were distributed and among them 291 were completed. The data was collected and used for the statistical analysis. Descriptive and inferential statistics, such as the frequency, t-test and one-way ANOVA were used for the analysis. RESULTS: Among the respondents, the average score for job satisfaction was lowest in the university-trained nurses and highest in the nurses who had only graduated from high school. CONCLUSION: The working conditions, work place and motivation are significantly associated with job satisfaction. In particular, there was lower job satisfaction in those nurses who wanted to change their occupation.

Life Style and Perception of Obesity of Male White Collar Workers.

BACKGROUND: This study investigated and analyzed the life style of male white-collar workers and their perception of obesity to determine how to improve eating habits and prevent obesity. MATERIALS AND METHODS: Using the questionnaires distributed to and collected from 300 male white-collar workers in Daegu, the survey was conducted from December 20, 2005 to February 28, 2006. Two hundred sets of collected questionnaires were used for the analysis, and SPSS WIN 12.0 was employed to analyze the data. RESULTS: Forty-two percent of subjects had weights ranging between 71 kg and 80 kg; 39.5%, weighted between 61 kg and 70 kg. The waist measurements of 64.5% of all respondents were between 32 and 34 inches. The lifestyle questions found that 54% of respondents were smokers and 88.5% drank alcohol. In addition, 62.5% of all respondents reported doing exercise, whereas 37.5% reported no exercise. CONCLUSION: The height and weight of 200 respondents were used to calculate the body mass index (BMI). Only 31.6% had a normal BMI, whereas 32% were found to be overweight, a condition likely to lead to obesity. Meanwhile, 36.5% of respondents were obese or morbidly obese.
