Prevalence and Risk Factors of Illness within Two-week among Rural Residents in Menglian:a Multilevel Model Analysis

Objective To assess prevalence and risk factors of illness within the past 2 weeks among rural residents in Menglian,under the background of essential public health service implemented generally. Methods In February 2012, the questionnaire method was employed to investigate the two-week morbidity in 2011 rural residents sampled by multistage stratified random sampling from Menglian population. Results The prevalence, that of illness within two-week among 2011 rural residents in Menglian, was 97.5‰ (95%CI 84.5‰-110.5‰) . It was lower than that of the rural region IV in China (149.6‰, <0.05) .Its age trend was the same as that of the rural region IV in China,the prevalence in 0-14 years old children,however,was higher than the counterpart in the rural region IV in China. The diseases, which two-week morbidity ranked the top five, were acute upper respiratory infection, acute gastroenteritis, hyperostosis, acute tracheobronchitis and rheumatoid arthritis. The results of multilevel logistic regression model fitted by group-level explanatory variable mountainous area, individual-level random slopes variable gender and other individual background variables indicated that there were across-level interactions between mountainous area and gender ( <0.05), the family income lower than RMB 15000 (odds ration 3.2378, 95%CI 1.9014-5.5130) and age (odds ration 1.0163, 95%CI 1.0002-1.0327) had a positive effect to two-week morbidity, and contrasting to unmarried, married had a negative effect to two-week morbidity (odds ratio 0.4727, 95%CI 0.2534-0.8819) . Conclusions Comparing with that of the rural region IV in China before implementing essential public health service, the two-week morbidity in Menglian was lower. For further elevating the health level of population, the strategies, such as intensifying material and child hygiene in the mountain area, improving sanitary conditions of low income family, and updating health consciousness in single population,would be implemented.

The Diagnostic Value of Combined Detection of Peripheral Blood CK19 and SCCAg mRNA Expression in The Early Cervical Cancer Micrometastasis

Objective Detect the change of relative expression level and positive rate of peripheral blood CK19 and SCCAg mRNA in early cervical cancer patients before receiving radical hysterectomy by qRT-PCR technique, then study the value of combined detection of peripheral blood CK19 and SCCAg mRNA expression in diagnosis of early cervical cancer micrometastasis. Methods Thirty patients with early (ⅠA2 ~ⅡA) cervical cancer from the department of gynecological tumors in the third affiliated hospital of Kunming Medical University were selected as experimental group, 15 patients with uterine myoma and 15 healthy volunteers as negative control group, and 15 advanced (ⅡB~ⅢB) cervical cancer patients as positive control group. Peripheral blood samples before receiving radical resection of experimental group and the same samples before treatment of control groups were collected. The changes of relative expression level and positive rate of peripheral blood CK19 and SCCAg mRNA were analyzed using qRT-PCR technique. Results There was significant difference in relative expression level of CK19 and SCCAg mRNA in the peripheral blood between experimental group and control groups ( <0.01) . And the positive rate of CK19 and SCCAg mRNA in the peripheral blood from 30 patients with early cervical cancer before operation in the experimental group was 30%(9/30) and 20% (6/30), respectively. Furthermore, combined detection of the expression of the two markers can improve the positive rate to 43.3%, much higher than single marker. There was also significant difference in the positive rate by combined detection of the two markers between the experimental group and control groups ( <0.05) . Conclusion CK19 and SCCAg mRNA could be used as the specific and suitable molecular markers for the detection of early cervical cancer micrometastasis. The combined detection of relative expression level and positive rate of peripheral blood CK19 and SCCAg mRNA by qRT-PCR array could significantly enhance the sensitivity and specificity for cancer cell detection.

Comparison Between Laparoscopic and Open Surgery in Treating Ectopic Oviduct Pregnancy

Objective To compare the surgical outcomes between laparoscopic and open surgeries in treatment of ectopic oviduct pregnancy, and investigate the clinical value of laparoscopic surgery. Methods Two hundred and forty-six patients with ectopic oviduct pregnancy in our hospital from June 2009 to March 2013 were retrospectively analyzed. 134 cases were treated with laparoscopic operation and 112 with open surgery. Some parameters such as operative time, blood loss, usage of pain-killer, hospital stay were compared between both groups. Results All operations were successful. Laparoscopic surgery was shown to be superior to open operation in the parameters of operative time, blood loss, usage of pain-killer and hospital stay ( < 0.05) . Conclusion Compared with open operation, laparoscopic surgery has the advantages of less trauma and rapid postoperative rehabilitation. It may become the first line treatment for ectopic oviduct pregnancy.

Epidemiological Analysis on Leprosy in Yunnan Province in 2012

Objective To analyze the epidemiological characteristics for new detecting cases of leprosy in 2012, in Yunnan Province, and provide clue and foundation for prevention and treatment in leprosy control. Methods The date of diagnosed leprosy patients in 2012 in Yunnan Province were collected and analyzed by disease reporting information system. Results 230 new cases were founded in 2012, and the discovery rate was 0.50/100 000. 4.35% of new cases were children, 64.35% of new cases were MB and 19.57% had grade 2 disability. 17 recurrent cases were founded in 2012,and 6 of them had received MDT. By the end of 2012, there were still 1084 present case in Yunnan Province,the prevalence rate was 0.24/10 000,and 485 of them need MDT. Conclusion The prevalence of leprosy was decreased,and the prevalence varies a lot in different regions. Honghe and Wenshan are still the focus regions. Leprosy is still a serious public health and social problem in Yunnan Province. In order to reduce the burden of leprosy and eliminate the leprosy danger, long-term financing investment and prevention are still needed.

Changes of NCCa-ATP Channels in Core Area During Focal Cerebral Ischemia and Reperfusion in Rats

Objective To study the changes of SUR1-regulated NCCa-ATP channels in core area during focal cerebral ischemia and reperfusion in rats, and to explore the therapeutic window of focal cerebral ischemia. Methods Adult male Wistar rats were rendered to undergo 120 minutes of the left middle cerebral artery occlusion (MCAO) by the intraluminal thread technique before reperfusion. The core area of the left hemisphere cortex ischemia/reperfusion at different times (reperfusion:R3 h, 6 h, 8 h, 12 h and 24 h) was taken to be tested the level of mRNA and protein expression of sulfonylurea receptor 1 (SUR1) using RT-PCR and Western-blot techniques. SUR1 of the ischemic brain microvascular endothelial cells were observed by immunofluorescence double staining at the peak expressing time point of SUR1. Results We found the up-regulation of SUR1 mRNA and SUR1 protein in ischemic infarcts tissues for R3 h, 6 h, 8 h and 12 h, peaked at R8 h, compared with the sham-operation group ( <0.05) . However, SUR1 expression increased was observed at R12 h by double immunofluorescence in microvascular endothelium. Conclusion SUR1-regulated NCCa-ATP channel may take part in cerebral ischemic damage during focal cerebral ischemia and reperfusion. The expressions of SUR1 mRNA and SUR1 protein were up-regulated in ischemic infarcts tissues. The data suggested that the best time to apply SUR1 inhibitor is within 8-12 hours after ischemia and reperfusion.

Construction of Eukaryotic Expression Vector with HIV-1 Integrase for Expression in Hela Cells

Objective To construct recombinant eukaryotic expression vector for EGFP fused HIV-1 integrase expression. Methods Wild type of HIV-1 integrase gene was cloned into eukaryotic expression vector-pcDNA6/V5-HisA. After restricted enzyme mapping, PCR confirmation and sequence confirmation, the recombinant plasmid was transfected into HeLa cells with Lipofectamine2000. After 24 hours, the expression of integrase was examined by immunofluorescence with confocal fluorescent microscopy. The cells were fixed with 4%paraformaledhyde. The cell nuclei were stained with Propidium Iodide (PI). Then the expression was imaged and analyzed with Confocal Microscopy. Results The integrase expressed significantly in HeLa cells in 24 hours after transfection. Integrase was expressed and localized into nuclei mainly. After fixed with 4% paraformaldehyde, the cell nuclei were stained with PI. When nuclei were showed in red in normal cells, the nuclei with integrase over expression turned yellow or orange. Conclusion The construction of eukaryotic expression vector of integrase was successful. Integrase was expressed and localized into nuclei mainly after transfection in HeLa cells.

Analysis of College Students’Sexual Health Knowledge and Information Sources in Kunming

Objective To understand the correct sexual health knowledge and source of sexual information among college students in Kunming, and provide the practical suggestions for students' sexual education. Methods A self-administered questionnaire survey was conducted among 950 students chosen from 5 universities in Kunming by cluster multistage sampling. Results The college students' health knowledge accuracy has certain significant differences between men and women, living urban and rural, inside and outside the province, and the Han and other nationalities. The most conversation partner discussing sex for the first time is their companion. Students get sex knowledge most in junior high school, and the school is the most important way to get these knowledge. Conclusion Sexual health knowledge education should be balanced between boys and girls, rural area students and minority students' knowledge of sexual health should be strengthened, and make them have a correct understanding onsex. In addition, strengthen the familysex knowledgeis also important.

Change of FAS/FASL System after Unilateral Testicular Torsion in SD Rats at Puberty

Objective To investigate the relationship between the expression of FAS/FASL and spermatogenesis in the contralateral testis after unilateral testicular torsion in puberty Sprague-Dawley rats. Methods Five groups of side testicular torsion adolescence SD rats model were established as follows:group A as control group, group B for testicular torsion group,group C for testicular torsion plus methylprednisolone group, D for reverse side testes excision group, and Group E testes excision plus methylprednisolone group for reverse side. SD rats were treated with testicular resection and/or injection of methylprednisolone after torsion for 24 h. The rats were executed at postoperative 1 month, and contralateral testes were collected for histopathological examination. Expression of FAS and FASL was analyzed quantitatively by immunohistochemistry with a computer pathological image analysis system. Each rat was evaluated with regard to endocrine parameters (follicle-stimulating hormone (FSH), luteinizing hormone ( LH) and testosterone ( T) by radioimmunoassay. Results Histopathological examination of the contralateral testes showed that either orchiectomy or orchiectomy plus methylprednisolone was more successful than no treatment. Fas/FasL protein in both control group and 4 experimental groups were expressed. B group showed more expression of FAS and FASL than that of the other groups. FSH, LH and T were normal in all cases. Conclusion Unilateral testicular torsion in puberty SD rats may result in higher expression of FAS and FASL, and accelerate germ cell apoptosis and subfertility. Methylprednisolone may decrease the expression of FAS and FASL and maintain spermatogenesis in the contralateral testis after the unilateral testicular torsion.

The Anti-cancer Effects in Vivo of Fungus Plant Extract on Transplanted Tumor S-180 in Mice and the Synergistic Effects with Cyclophosphamide

Objective Study the anti-tumor effects in vivo of AMH-D on S-180 cell lines, the synergistic effects of AMH-D and cyclophosphamide,and investigate the way and its strength of the effect. Provide the basis for the development of anti-cancer drug. Methods Kunming mice were transplanted with S-180 tumor cells subcutaneously in the right armpit. Intraperitoneal injection was done after randomization on the next day. Mice were killed on the eleventh day, and tumors were stripped and weighed. The tumor weight was used as indicator for analysis and evaluation. Results The results showed that AMH-D could effectively inhibit the growth of S180 cells transplanted tumor. The tumor inhibition rate was 50.45%at the dose of 150 mg/kg, with a dose-effect relationship. There were no obvious impacts on the growth of the weight of mice. The results showed that AMH-D had a synergistic effect combined with cyclophosphamide within a certain dose. Conclusion Fungus extract AMH-D has a great effect on anti-tumor in vivo of S180 cells transplanted tumor, and has a synergistic effect combined with cyclophosphamide within a certain dose.

Contraceptive Use and Impact Factors among Reproductive-age Urban Women Seeking Abortions in Kunming

Objective To determine the situation of contraceptive use and impact factors among reproductive-age women seeking abortion in Kunming which was in the southwest of China in order to provide information for improving contraceptive use. Methods A hospital-based study was conducted among 397 women of reproductive age, seeking abortion in hospitals of different levels in Kunming from June 2011 to December 2011. They were face to face interviewed by the trained investigators with the questionnaires. Results Among 397 women, 229 cases (57.7%) had experienced induced abortion. Abortion accounted for 61.7% of married women, and unmarried accounted for 39.3%. About 36.3%never used any contraceptives, and only 21.4%of respondents used contraceptives at every intercourse. The main contraceptive before this pregnancy was condom (68.9%) . The use of contraceptives was mainly decided by couple (73.4%) . Multivariate logistic regression analysis indicated that respondents' age, educational level and access to contraceptives in hospital were associated with the use of contraceptive. Conclusion Premarital pregnancy and low rate of contraception are the main reason for induced abortion. Fewer reproductive-age women seeking abortion use contraceptives. It is necessary to increase their awareness of contraception and abortion related knowledge, provide feasible contraceptive to enable women to avoid unwilling pregnancy. Further more, it is important to promote male involvement in contraception.
