Application of mind map in admission guidance of patients receiving gamma knife treatment

Objective To evaluate the application effect of mind map in admission guidance of patients receiving gamma knife treatment.Methods 153 cases of hospitalized patients admitted in our hospital were selected for this study,which had been divided according to the random number table method,the observation group(77 cases) was guided with the hospital mind map method,and the control group (76 cases) was given traditional oral hospital instruction,respectively.The database was established for the double entry,calibration and data cleaning,which were obtained from the result of gamma knife treatment of intracranial tumor disease related knowledge questionnaire,hospital satisfaction questionnaire and also patient adherence questionnaire assessment.Results The comparison of knowledge assessment questionnaire outcome,the admission guide satisfaction questionnaire assessment,the patients compliance questionnaire assessment outcome showed that the observation group was significantly different from the control group.Conclusions Application of mind map in admission guidance for hospitalized patients was a more efficient method than the traditional oral admission instruction,the method is simple,practical and effective,which is worthy of clinical practice.

Initial revision of Chinese version of parents' perception of uncertainty scale

Objective To translate Parents' Perception of Uncertainty Scale (PPUS) into Chinese,analyze and selected items,and then test the reliability and validity of the Chinese version of PPUS initially.Methods 210 parents of hospitalized children were recruited for the investigation.Frequency analysis,dispersion degree analysis,correlation analysis and so on were applied to analyze and select items.Internal consistency test,Pearson correlation analysis and exploratory factor analysis etc.were applied to test reliability and validity of the scale.Results The final Chinese version of PPUS contained 28 items.The Cronbach α and parity split-half reliability coefficients were 0.844 and 0.835; The Cronbach α coefficient of 4 dimensions ranged from 0.537 to 0.770.The item-total scale correlation coefficient ranged from 0.300 to 0.737.Item-dimension correlation coefficients ranged from 0.376 to 0.762.Item-other dimension correlation coefficients ranged from 0.028 to 0.514.Dimension-total scale correlation coefficients ranged from 0.480 to 0.863 and those between dimensions ranged from 0.226 to 0.536.In the content validity assessed by experts,I-CVI ranged from 0.75 to 1.00,S-CVI/Ave was 0.928.Four-factor model was performed in the principal component analysis and they explained for 41.401％ variances.The maximum factor loading of all items were higher than 0.3,and most of items had adequate loadings in related dimensions.Conclusions Chinese version of PPUS had appropriate reliability and validity as an assessment tool for parental uncertainty in illness of hospitalized children' parents.

A qualitative research on the feelings of clinic nurses after needle-stick injuries

Objective To explore the feeling of nurses after needle stick injuries and analyze the cause and impact of needle stick injuries,so as to explore protection measures.Methods The phenomenological methodology was used in the study to analyze the main causes of needle stick injuries,handling method and report reasons.Results The main causes of needle stick injuries were violation of the operation sequence and the weakness of protection awareness.The main reactions of nurses to needle stick injuries were fear and worry.The report circuit was complex.Conclusions It is suggested to pay attention to the analysis and discussion of needle stick injuries and make standardized the operation sequence,improve self-protection awareness,and perfect the support system after needle stick injuries,then bring it into the routine management work.

Investigation of self-perceived burden among continuous ambulatory peritoneal dialysis patients

Objective To investigate the level of self-perceived burden(SPB) and analyze the influencing factors in continuous ambulatory peritoneal dialysis(CAPD) patients.Methods One hundred and ten CAPD patients were investigated by a self-designed demographic questionnaire and Self-Perceived Burden Scale (SPBS).Results The total score of SPBS was (30.27±7.88) points,being in a middle level,and 85.45％ of the patients showed different levels of burden.Single factor analysis showed the influencing factors of self-perceived burden were marital status,family per capita income,the main caregivers and selfcare ability.Conclusions Most CAPD patients have obvious self-perceived burden,nursing staff should use the relevant nursing measures to reduce the SPB with CAPD patients and improve their quality of life.

Application of root cause analysis method in management of nursing adverse events in emergency first aid

Objective To reduce the occurrence of nursing adverse events in emergency first aid and improve the safety of nursing.Methods To set up the quality control (QC) team,root cause analysis (RCA) method was applied to analyze the occurrence of nursing adverse events in emergency first aid in our department from December 2008 to December 2011.Measures were produced by identifying the reasons of the proximal and the root cause.Lots of measures had been taken,such as revising the nursing working process and regime,changing the work schedule,strengthening education of nurses risk consciousness and the communication ability,paying attention to the details of daily nursing work management.Results In December 2012,nursing adverse events in our hospital was reduced from 9.81％ to 2.75％ in 2011.Conclusions The key point to reduce nursing adverse events in emergency first aid is to perfect nursing working process and regime,improve the ability to solve emergency first aid events and nursing crisis consciousness training,practice flexible working schedule and attach importance to the detail management.

Research of teaching mode of professional value of nursing students based on Chinese medicine culture

Objective To evaluate the effects of developing professional value of nursing students based on Chinese medicine culture.Methods 69 nursing students in the second year were divided into the intervention group(n=34) and the control group (n=35).The students in the intervention group received a 3-month education project based on Chinese medicine culture,while the control group received only routine education plan.The effect of the intervention was evaluated by nursing professional values-R (NPVS-R) at the end of the intervention.Results The scores of NPVS-R including total scores and the scores of the two dimensions of caregiving and behavioristic in the intervention group significantly increased compared with those in the control group.Conclusions The intervention of developing professional value of nursing students based on Chinese medicine culture can improve the level of professional value.

The application of Mini-Nutritional Assessment in nutritional assessment in middle-aged women with diabetes

Objective To investigate the application value of the Mini-Nutritional Assessment (MNA) in assessing the nutritional status in middle-aged women with diabetes.Methods Using the normal range of traditional nutritional index as the reference standard,two hundred and forty middle-aged women with diabetes were recruited and their nutritional status were assessed by MNA and laboratory test (including PAB,Fib,TLC,CHOL),respectively.Finally,the results were analyzed and compared.Results The correlation coefficient of MNA value and BMI、PAB,Hb,TLC,CHOL were 0.571,0.614,0.781,0.568,0.218.MNA had good correlations with several traditional laboratory indicators regarding with nutritional status,with signifi-cant difference.In this study,the results of investigation by MNA showed that:the rates of malnutrition,potential malnutrition,and normal nutritional were 30％(72 cases),65％(156 cases),and 5％(12 cases),respectively.The sensitivity and specificity of MNA were 96.1％ and 98.2％ respectively.Laboratory testing resuits:malnutrition and normal nutrition were 31％ (74 cases) and 4％ (10 cases).Kappa test(Kappa=0.922):the results of MNA and laboratory testing had strong consistency.Conclusions MNA is feasible in assessing nutritional status in the domestic middle-aged women with diabetes.

Effect of the bundle of care interventions to prevent ventilator associated pneumoniaon:a Meta-analysis

Objective To evaluate the effect of the bundle of care interventions in preventing ventilator-associated pneumonia (VAP) in the mechanic ventilated patients.Methods The randomized controlled trials (RCT) on the bundle of care interventions in preventing ventilator-associated pneumonia (VAP) were collected using the databases of PubMed,Medline,Embase,Cochrane library,China National Knowledge Infrastructure (CNKI),Wanfang Periodical Databases and Chinese Science and Technology Periodical Databases (VIP),data were analyzed by RevMan5.0 software.Results Ten RCTs were included in the study.The results of Meta-analysis revealed that the incidence of ventilator associated pneumonia,mechanical ventilation time,ICU length of hospital stay and the mortality rate in the intervention group were different from the control group,the difference had statistical significance.Conclusions The bundle of care interventions can reduce the incidence of ventilator-associated pneumonia,mechanical ventilation time,ICU length of hospital stay and the mortality rate.

Observation on the effect of solution focused theory on improving sleep quality of ACS patients with clinical pathway

Objective To study the effect of solution focused theory on improving the sleep quality of ACS patients with clinical pathway.Methods 100 ACS patients with clinical pathway in Tangshan Gongren hospital from September 2010 to January 2011 were selected as study objects,and they were randomly divided into the control group(51 cases)and the intervention group(49 cases).Then the sleep quality was assessed during the first 24 hours being admitted and on the second and forth week after the intervention with the Athens Insomnia Scale(ASI).Results At the second week after intervention,4 items of the ASI Score of the observation group were different from the control group significantly.The 4 items were nighttime sleep disruption,waking up early,evaluation of total sleep quality,effect on the day of emotion.On the forth week after intervention,only the item of effects on daytime function was not different from the control group significantly,the scores of other 7 items were significantly different from those of the observation group.Conclusions The effect of solution focused theory in improving the sleep quality of ACS patients with clinical pathway is significant,and it plays an active role in improving the state of disease.

Discussion on circulating fluid replacement in the polyuria stage after simultaneous islet cell-renal transplantation

Objective To discuss and improve the scheme of circulating fluid replacement in the polyuria stage after simultaneous islet cell-renal transplantation.Methods Nursing data of 26 patients after simultaneous islet cell-renal transplantation during the polyuria stage were analyzed retrospectively,the impact on spend-and-tax circulating fluid replacement on the outcome after surgery was observed.Results In 26 patients,3 case suffered graft function recovery delay,23 patients' renal function recovered smoothly without water-electrolyte and acid base disorders in period of diuresis,compared with the 16 cases with traditional fluid replacement in the same time,serum creatinine (SCr),blood urea nitrogen (BUN) and blood glucose (BG) decreased significantly in 72 h.Conclusions Using spend-and-tax fluid replacement can reduce postoperative complications such as water-electrolyte and acid-base disorders in simultaneous islet cell-renal transplantation patients,and promote the functional recovery of transplanted islet cells and kidney.
