1.A clinical pathway for integrative medicine in the treatment of functional constipation in Hong Kong, China.

OBJECTIVE:
Functional constipation (FC) is a common intestinal disease worldwide. Despite the presence of criteria such as Roman IV, there is no standardized diagnosis and treatment algorithm in Hong Kong that combines both Western and Chinese medicine approaches. This study integrates current effective and safe diagnosis and treatment methods for FC and provides a clear and scientific pathway for clinical professionals and patients.
METHODS:
A systematic search of the PubMed, Cochrane Library, and China National Knowledge Infrastructure databases was performed from their inception to June 30th, 2022, collecting the current evidence about the efficacious integrative management for FC. We organized a meeting of professionals in fields relevant to treatment and management of FC to develop a consensus agreement on clinical pathway process.
RESULTS:
We developed a clinical pathway for the treatment of FC based on the most recent published guidelines and consultation with experts. This pathway includes a hierarchy of recommendations for every step of the clinical process, including clinical intake, diagnostic examination, recommended labs, diagnostic flowchart, and guidance for selection of therapeutic drugs.
CONCLUSION
This pathway establishes clinical standards for the diagnosis and treatment of FC using Chinese medicine and Western medicine; it will help to provide high-quality medical services in Hong Kong for patients with FC. 2.Principles of extramural health care for chronic wounds in China.

Development of extramural health care for chronic wounds is still in its infancy in China, and thus it is urgent and vital to establish a correct concept and practicable principles. The authors reviewed recent domestic and international literature and summarized the following treatment procedures and principles for extramural health care of chronic wounds. (1) The patient needs to do self-assessment of the wound by using available simple methods; (2) The patient consults with professional physicians or nurses on wound care to define the severity and etiology of the non-healing wound; (3) Professionals evaluate the existing treatment strategies; (4) Etiological treatments are given by professionals; (5) Patients buy needed dressings via the more convenient ways from pharmacies, e-commerce platform or others; (6) Professionals provide a standardized and reasonable therapeutic plan based on the patient's wound conditions; (7) Both professionals and the patient pay attention to complications to prevent adverse outcomes; (8) Professionals strengthen the public education on wound care and integrated rehabilitation. This review expected to provide new perspectives on the therapeutic strategies for chronic wounds in an extramural setting.

3.Quantitative Pulmonary Administration of Essential Oil from Alpiniae Zerumbet Fructus for Treatment of Emphysema

ObjectiveTo investigate the effect of quantitative pulmonary administration of the essential oil from Alpiniae Zerumbet Fructus (EOAZF) on porcine pancreatic elastase (PPE)-induced emphysema in mice and explore its action mechanism. MethodC57BL/6J mice were randomly divided into five group, namely the control group, model group, low- (2 mg·kg-1) and high-dose (20 mg·kg-1) EOFAZ groups, and positive control dexamethasone (DEX,1 mg·kg-1) group. The mice were treated with pulmonary administration of PPE using a microsprayer aerosolizer, once every seven days, for four times in total, for inducing emphysema. During this period, EOFAZ were administered with a quantitative microsprayer aerosolizer once every other day, for 14 times. The lung tissues were then sampled and stained with hematoxylin-eosin (HE) for observing the morphological changes and calculating the pulmonary mean linear intercept (MLI). The concentrations of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and interleukin-6 (IL-6) in the plasma were determined by enzyme-linked immunosorbent assay (ELISA). The activities of superoxide dismutase (SOD) and catalase (CAT) and the content of malondialdehyde (MDA) in the lung tissues were measured using the biochemical assay kits. The protein expression levels of nuclear factor E2-related factor 2 (Nrf2), quinone oxidoreductase1 (NQO1), B cell lymphoma-2 (Bcl-2)-associated X protein (Bax), and Bcl-2 in lung tissues were detected by Western blot. ResultThe results of lung morphological observation and MLI detection showed that compared with the control group, the model group showed obvious inflammatory infiltration, alveolar enlargement and fusion, and increased MLI (P<0.05). Compared with the model group, EOFAZ effectively alleviated the pathological changes such as alveolar dilatation, pulmonary inflammatory cell infiltration, and lung cell apoptosis caused by PPE, and decreased the MLI (P<0.05). As revealed by ELISA, the inflammatory level of mice in the model group increased significantly (P<0.01), while the TNF-α, IL-1β, and IL-6 levels in the plasma were decreased after quantitative administration of EOFAZ (P<0.01). Compared with the control group, the model group exhibited significantly enhanced oxidative stress (P<0.01). After treatment with EOFAZ by quantitative administration, the activities of SOD and CAT in the lung tissue were increased (P<0.01) and the content of MDA was decreased (P<0.01). Western blot results demonstrated that the apoptosis-related protein expression in the model group was increased significantly as compared with that in the control group (P<0.01), whereas the expression levels of antioxidant stress proteins Nrf2 and NQO1 declined (P<0.05). The relative protein expression of apoptosis-related proteins Bax/Bcl-2 in the EOFAZ groups was lower than that in the model group (P<0.01), while the expression of antioxidant stress proteins Nrf2 and NQO1 was higher (P<0.05). ConclusionQuantitative pulmonary administration of EOFAZ effectively alleviates the inflammation and oxidative stress, reduces lung cell apoptosis, and hinders the occurrence and development of emphysema. Its antioxidant mechanism is closely related to the up-regulation of Nrf2 and its downstream NQO1.
4.Exercise and Rehabilitation Training for Children's Balance: A Scoping Review

Objective:To analyze the exercise and rehabilitation training programs for children's balance ability using scoping review. Methods:The literatures about children's balance ability, exercise and rehabilitation training were retrieved from CNKI, VIP, PubMed and Web of Science, until 2021-03-17. The authors, published time, countries, objects and their ages, study tools, interventions and the time, and outcomes were extracted. Results:A total of 2679 literatures were returned, 34 of which were enrolled, which came from 13 countries, and were almost published in journals of medicine, sports science, and rehabilitation and disabilities. The literatures were mainly published after 2010, investigate with measurements and scales. The measurements mainly included foot pressure system, force platform, Biodex balancer, etc.; and the scales mainly included Berg Balance Scale, Pediatric Balance Scale, Gross Motor Function Measure, Pediatric Evaluation of Disability Inventory, and Bruininks-Oseretsky Test of Motor Proficiency, etc. There were four typies of program: balance training, interactive computer games, virtual reality and sports events. Interventions for sick children included strength training, whole-body vibration training, Wii-Fit balance games and virtual reality treadmill training, etc. Sports for healthy children mainly included skating, Taiji Quan and gymnastics, etc. Conclusion:There are differences in the researches for children's balance ability at home and abroad. The abroad researches focus on the children with various diseases, in a larger age range, using more evaluation tools and more intervention methods. However, Chinese researches are usually for a longer intervention time, more subjects, and focusing more on healthy children. It is needed to pay more attention on children with diseases before three years old, for early diagnosis and treatment. Long-term effects should be observed other than immediateness after intervention.
5.Research progress of dummy template molecularly imprinted polymers in separation of natural products.

The dummy template molecularly imprinted polymers not only has such characteristics of normal imprinted polymers as rapid identification, easy preparation, stable structure and multiple reuse, but also can imprint the compounds in natural products that are not suitable as direct template. Therefore, it has drawn more and more attention in the field of the study of natural products. This paper summarizes the methods for the selection of dummy template molecules by investigating the relevant literatures in the past ten years, analyzes the advantages and disadvantages of dummy template molecules in the practical application, and based on the types of natural products active ingredients, this paper is the first to review of the latest progress in extraction and separation of dummy template molecularly imprinted polymers. We believed that this paper could provide references for better applications of the dummy template molecularly imprinted polymers to extract and separate natural products.

6.Study on rapid quality analysis method of Shengxuebao Mixture.

A rapid analysis method based on ultraviolet-visual(UV-Vis) spectroscopy, near infrared(NIR) spectroscopy and multivariable data analysis was established for quality evaluation of Shengxuebao Mixture. The contents of eight active ingredients of Shengxuebao Mixture including albiflorin, paeoniflorin, 2, 3, 5, 4'-tetra-hydroxy-stilbene-2-O-β-D-glucopyranoside, specnuezhenide,ecliptasaponin D, emodin, calycosin-7-glucoside and astragaloside Ⅳ were simultaneously detected by using this method. HPLC-UV-MS was used as a reference method for determining the contents of these ingredients. Partial least squares(PLS) analysis was implemented as a linear method for multivariate models calibrated between UV spectrum/NIR spectrum and contents of 8 ingredients. Finally, the performance of the model was evaluated by 24 batches of test samples. The results showed that both UV-Vis and NIR models gave a good calibration ability with an R~2 value above 0.9, and the prediction ability was also satisfactory, with an R~2 value higher than 0.83 for UV-Vis model and higher than 0.79 for NIR model. The overall results demonstrate that the established method is accurate, robust and fast, therefore, it can be used for rapid quality evaluation of Shengxuebao Mixture.

7.Early Immunosuppressive Exposure of Enteric-Coated-Mycophenolate Sodium Plus Tacrolimus Associated with Acute Rejection in Expanded Criteria Donor Kidney Transplantation.

BackgroundImmunosuppressive agents are still inefficient in preventing biopsy-proven acute rejection (BPAR) after expanded criteria donor (ECD) kidney transplantation. The aim of this study was to investigate the relationships between early immunosuppressive exposure and the development of BPAR.
MethodsWe performed a retrospective study of 58 recipients of ECD kidney transplantation treated with enteric-coated-mycophenolate sodium, tacrolimus (Tac), and prednisone. The levels of mycophenolic acid-area under the curve (MPA-AUC) and Tac Cwere measured at the 1 week and the 1 month posttransplant, respectively. The correlation was assessed by multivariate logistic regression.
ResultsThe occurrence rates of BPAR and antibody-mediated rejection were 24.1% and 10.3%, respectively. A low level of MPA-AUC at the 1 week posttransplant was found in BPAR recipients (38.42 ± 8.37 vs. 50.64 ± 13.22, P < 0.01). In addition, the incidence of BPAR was significantly high (P < 0.05) when the MPA-AUClevel was <30 mg·h·L at the 1 week (15.0% vs. 44.4%) or the Tac Cwas <4 ng/ml at the 1 month posttransplant (33.3% vs. 21.6%). Multivariable logistic regression analysis showed that the MPA-AUC at the 1 week (OR: 0.842, 95% CI: 0.784-0.903) and the Tac Cat the 1 month (OR: 0.904, 95% CI: 0.822-0.986) had significant inverse correlation with BPAR (P < 0.05).
ConclusionsLow-level exposure of MPA and Tac Cin the early weeks posttransplant reflects an increased acute rejection risk, which suggested that MPA-AUC <30 mg·h·L and Tac C <4 ng/ml should be avoided in the first few weeks after transplantation.

8.Application of Pulmonary Vein Deployment Technique for Percutaneous Closure of Atrial Septal Defect Solely Under Echocardiography Guidance

Objective: To investigate the safety and efficacy of pulmonary vein deployment technique for percutaneous closure of atrial septal defects (ASD) solely under echocardiography guidance. Methods: A total of 38 ASD patients received pulmonary vein deployment in our hospital from 2012-10 to 2016-09 since the conventional method could not deliver the occluder to correct place. The patients were with the mean age at (16.0±15.6) years, body weight at (37.2±22.9) kg and ASD diameter at (17.1±4.2) mm. Operative effect was assessed by echocardiography. Follow-up study was conducted at 1, 3, 6, 12 months post-operation and at each year thereafter. Results: 37 patients were successfully finished pulmonary vein deployment for percutaneous closure of ASD solely under echocardiography guidance. One patient was successfully treated by a controlled steerable sheath. The mean operative time was (25.2±5.1) min and mean diameter of ASD occluder was (22.9±5.6) mm. 2 patients had trivial residual shunt at the early post-operative stage. No peripheral vascular injury, pulmonary vein and cardiac perforation occurred. All 38 patients were recovered and discharged. The average in-hospital time was (2.9±0.7) days. The patients were followed-up for (23.9±15.4) months, without complications of residual shunt, pericardial effusion, aortic regurgitation and pulmonary vein stenosis. Conclusion: Pulmonary vein deployment technique for percutaneous closure of ASD solely under echocardiography guidance was safe and effective; it can avoid radiation damage and provided a simple and practical method for ASD patients who failed to conventional method under echocardiography guidance.
9.Association between S100B gene polymorphisms and hand, foot and mouth disease caused by enterovirus 71 infection.

OBJECTIVETo investigate the association between rs9722 polymorphisms in the S100B gene and hand, foot and mouth disease (HFMD) caused by enterovirus 71.
METHODSA total of 124 HFMD children with enterovirus 71 infection were enrolled as subjects, and 56 healthy children were enrolled as control group. The rs9722 polymorphisms in the S100B gene were detected for both groups, and the serum level of S100B protein was measured for 74 HFMD children.
RESULTSThe rs9722 locus of the S100B gene had three genotypes, CC, CT, and TT, and the genotype frequencies were in accordance with Hardy-Weinberg equilibrium. Compared with the control group, the HFMD group had significant increases in the frequencies of TT genotype and T allele (P<0.01). Children with severe HFMD caused by enterovirus 71 infection had significantly higher frequencies of TT genotype and T allele than those with moderate or mild HFMD (P<0.05). Compared with the cured patients, the patients with poor prognosis had significant increases in the frequencies of TT genotype and T allele in the rs9722 locus of the S100B gene (P<0.05). Among the 74 children with HFMD, the children with TT genotype had the highest serum level of S100B protein, and those with CC genotype had the lowest level (P<0.01).
CONCLUSIONST allele in the rs9722 locus of the S100B gene might be a risk factor for severe HFMD caused by enterovirus 71 infection.

10.Level and Correlationship of Serum SOD and hsCRP and Other Indicators in Acute Coronary Syndrome

Objective To investigate the level of serum superoxide dismutase(SOD),high-sensitive C-reactive protein (hsCRP)and myocardial injury markers in acute coronary syndrome(ACS)and evaluate their relationship and diagnostic values.Methods Case control study was conducted.This study enrolled 64 ACS patients and 50 non-ACS patients from the 181st Hospital of Chinese PLA in 2015.Serum SOD,hsCRP were tested and myocardial injury markers such as cardiac tro-ponin I(cTnI),creatin kinase MB(CK-MB)were also tested.Student t test and Pearson test were used as statistical meth-ods.Results Compared with control group,SOD of ACS group were significant lower(t=4.136,P<0.001)and hsCRP, Mb,cTnI,CK and CK-MB were significant higher(t=-5.396,-3.495,-5.578,-4.655 and -4.614,all P<0.001). The area under ROC curve of SOD,hsCRP,MYO,cTnI,CK and CK-MB was 0.713,0.758,0.699,0.879,0.841 and 0.802 respectively.After pearman test,the serum SOD were correlated to hsCRP,total cholesterol(TC)and low density lipopro-tein(LDL-C)(r=-0.493,0.548 and 0.404,all P<0.01).Serum hsCRP was correlated to cTnI,triglyceride(TG)and SOD(r=0.671,-0.417 and -0.493,all P<0.01).Conclusion cTnI was positively correlated to hsCRP and markers of myocardial injury.hsCRP was negatively correlated with SOD.It implicated that oxidative stress,inflammatory response and serum lipid deposition may act an important role in the occurrence and development of ACS.
