1.Effect of structured health education on the caregiver burden of adolescent patients with mood disorders

BackgroundThe adolescent mood disorders become more prevalent, imposing progressively greater perceived and actual caregiving burdens on caregivers, while the current health education resources, delivery formats and training programs remain insufficient in addressing their needs. ObjectiveTo evaluate the effect of structured health education on the caregiver burden associated with adolescent mood disorders, thereby informing evidence-based strategies for alleviating the burden. MethodsA total of 195 caregivers of adolescent inpatients with mood disorders admitted to a tertiary general hospital in Mianyang from June 1, 2020 to July 31, 2022 were consecutively enrolled and randomly assigned to either the research group (n=99) or the control group (n=96) using random number table method. Both groups received routine health education. In addition, the research group participated in a structured health education program comprising three modules covering disease knowledge, medication management, coping with family stress, and guidance for daily living. Upon admission, baseline information regarding both caregivers and adolescent inpatients was collected though a self-administered questionnaire. Caregiver Strain Index (CSI) was administered to assess caregiver burden at admission, discharge, and 1, 3 and 6 months post-discharge. A self-designed health education feedback questionnaire was administered on the day of discharge to evaluate caregivers' feedback on the health education. At 6 months post-discharge, data on any hospital readmission were collected. ResultsUltimately, 85 caregivers (85.86%) in the research group and 84 caregivers (87.50%) in the control group completed the study. Generalized estimating equation analysis revealed significant time effects and group-by-time interaction effects on CSI scores between the two groups (Wald χ²=790.347, 10.047, P<0.05 or 0.01). There was no statistically significant difference between the two groups in terms of access to disease information, trust preference and core information needs (P>0.05). Additionally, caregivers in the research group reported significantly better understanding of the disease-related knowledge and higher ratings on the effectiveness of health education when compared to the control group, with statistically significant differences (P<0.01). ConclusionIn contrast to routine health education, structured health education may help alleviate caregiving burden among caregivers of adolescents with mood disorders.[Funded by Sichuan Medical Association Project (number, Q190004)]
2.Antiapoptotic Mechanism of Insulin in Reoxygenation-induced Injury in Cultured Cardiomyocytes of Neonatal Rats

To examine the protective effect of insulin on reoxygenation-induced injury and explore the underlying mechanisms, the model of anoxia/reoxygenation (A/R) injury was established by inducing anoxia for 2 h and reoxygenation for 4 h in cultured cardiomyocytes of neonatal rats. The rats were randomized to four groups receiving vehicle, insulin, LY294002, insulin plus LY294002at the onset of reoxygenation after 2 h of anoxia. At the end of reoxygenation of 4 h, activity of lactate dehydrogenase (LDH) and content of malondialdehyde (MDA) were spectrophotometrically determined, apoptosis of cardiomyocytes were detected by using TUNEL and DNA Ladder, and Western blotting was employed to examine the expression of phosphorylated Akt in all groups. Our results showed that compared with vehicle-treated group, activities of LDH, contents of MDA, apoptosis index (AI) were significantly decreased, and expression of phosphorylated Akt was in creased significantly in insulin-treated group. However, changes in LDH, MDA, AI and phospho rylated Akt resulting from insulin were attenuated or abolished by LY294002 (PI3K inhibitor).These data strongly suggest that early administration of insulin at reoxygenation protects cardiomyocytes from reoxygenation-induced apoptosis through PI3K/Akt signaling pathway.
3.(18)~F-deoxyglucose-hPET in the detection of recurrence in non-small-cell lung cancer

Purpose:To evaluate ~(18)F-deoxyglucose dual-head tomography with hPET in the detection of suspected recurrence in non-small cell lung cancer, as compared with CT/MRI imaging. Methods:21 patients with clinically suspected recurrences in non-small-cell lung cancer underwent ~(18)F-FDG hPET and CT/MRI imaging. The final diagnosis of recurrence was proved by pathology or clinical following-up. Results:The ~(18)F-FDG hPET sensitivity, specificity,accuracy,postive predictive value in defining local recurrence were 94.1%,75%,90.5 %,94.1%,respectively ; and those of CT/MRI were 60%,66.7%,61.9%,81.8%, respectively. Conclusions:In comparison with CT/MRI,~(18)F-FDG hPET possesses a higher sensitivity、specificity and accuracy in detecting recurrence in non-small-cell lung cancer.~(18)FDG hPET is quite valuable methd to define the tanget in the radiotherapy of recunent non-small-cell lung cancer,and is less expensive than PET.It is thus recommended for clinical use.
4.Three-dimensional conformal radiotherapy(3-DCRT)combined with radiofrequency hyperthermia for locally advanced non-small-cell lung cancer(LANSCLC)

Purpose:To evaluate the acute side effects an d efficacy of three-dimensional conformal radiotherapy(3-DCRT)combined with hype rthermia for locally advanced non-small-cell lung cancer(LANSCLC). Methods:From Jan.2001 to April.2003,35 patients with LANSCLC we re treated with three-dimensional conformal radiotherapy(3-DCRT) combined with hyperthermia. Results:The overall response rate(CR+PR)was 91.4%for the primar y tumor. Acute radiation esophagistis occurred in 48.6%of patients(Grades Ⅰ,Ⅱ ). Acute radiation pneumonitis was observed in 8.6% of patients (Grades Ⅰ,Ⅱ). Grade Ⅰ-Ⅱ bone marrow suppression occurred in 8.6%(3/35)of patients and Grade Ⅰ-Ⅱ myocardical injury was present in 14.3%(5/35) of patients. The median fo llow-up duration was 24(6-28) months. Conclusions:Hyperthermia combined with 3-DCRT is well tolerate d in most patients with LANSCLC. Its early responses are milder than the convent ional radiotherapy. Remote results await futher follow-up.
