1.Nutritional Status of Children Aged 3 to 7 Years in Zhag′yab, Tibet

ObjectiveTo understand the physical development level of 3-7 years old children in Zhag′yab, and to provide reference for local health decision-making. MethodsA cross-sectional study was conducted between June 2021 and July 2021, with a sample of 1 247 Tibetan children aged 3-7 years from kindergartens in 13 districts of Zhag′yab. Their height and weight were measured and the hemoglobin was detected by a unified method. Standard statistical method was adopted (Z-score method). Z-scores of length /height-for-age (HAZ), Z-scores of weight-for-age (WAZ), Z-scores of body mass index (BMI)-for-age (BAZ) and Z-scores of weight-for-height (WHZ) were calculated by WHO Anthro v3.2.2 and WHO Anthro Plus. The nutritional status of children was evaluated according to WHO diagnostic criteria for malnutrition and anemia. ResultsThe average WAZ and HAZ of children aged 3 to 7 in Zhag′yab were lower than the WHO standards,except for the WAZ of 4-year old, the differences were statistically significant (P<0.05). The overall detection rate of malnutrition was 25.7%, stunting, underweight, wasting, overweight, obesity and anemia were 11.6%, 11.8%, 10.8%, 3.3%, 1.8% and 29.3%, respectively. The detection rates of all indicators in boys were higher than those in girls, and the differences were not statistically significant (P>0.05). The overweight rate and obesity rate of rural children were lower than those of urban children, and the other detection rates were higher than those of urban children. The differences of underweight rate, obesity rate and anemia rate were statistically significant between urban and rural children (P<0.05). Among the detection rates of all indicators in different age groups, there were statistically significant differences in the overweight rate and the anemia rate (P<0.05). The overweight rate of children aged 4 and the anemia rate of children aged 5 were the highest. ConclusionsThe physical development of children aged 3 to 7 in Zhag′yab is poor, and the prevalence of malnutrition and anemia is high. Underweight and anemia are more serious in rural children, and the overweight and obesity problem of urban children is emerging. More attention should be paid to promote their nutritional status. The prevention and intervention of children’s malnutrition should be strengthened in Zhag′yab.
2.Effect of esketamine for acute and chronic pain after cesarean section

Objective:To evaluate the effectiveness of esketamine during perioperative anesthesia for acute and chronic pain after cesarean section.Methods:One hundred and fifty patients scheduled for elective cesarean section under spinal anesthesia were randomly assigned into 2 equal groups ( n=75) using a random number table: esketamine group (group E) and control group (group C). Subarachnoid block was administered with 9-11 mg of hyperbaric bupivacaine with 0.33% glucose concentration. After the delivery of the fetus, 0.15 mg/kg (1 mg/ml) esketamine was pumped intravenously for 30 min in the group E, while the same dosage of normal saline was administered in the group C. Furthermore, patients received an intravenous patient controlled intravenous analgesia (PCIA) pump after surgery (100 μg sufentanil + 1.25 mg/kg esketamine + 8 mg ondansetron for the group E, 100 μg sufentanil + 8 mg ondansetron for the group C). Heart rate (HR), systolic blood pressure (SBP), and diastolic blood pressure (DBP) were recorded in the initial time of esketamine administration, and 5, 15 min, and 30 min after administration. The pain Numerical Rating Scale (NRS) score at rest and during coughing were recorded at 2, 6, 12, 24 h and 48 h after surgery. The first analgesic time and cumulative sufentanil consumption were recorded at 0-12 h, 12-24 h, 24-48 h, 0-24 h and 0-48 h after surgery. Moreover, we recorded the incidence of chronic pain at 3 and 6 months after surgery. Results:There were no significant differences in HR, SBP and DBP between the two groups immediately after administration of esketamine and 5, 15 min and 30 min after administration (all P>0.05). At rest or during coughing, the pain NRS score were significantly lower at 2, 6 h, and 12 h postoperatively in group E compared to group C (all P<0.05). The time to first analgesia in group E was significantly longer than the group C [(176.8±18.3)min vs (148.5±16.9)min, P<0.05]. The cumulative sufentanil consumption was significantly lower in group E during 0-12 h, 12-24 h, 0-24 h and 0-48 h postoperatively than in group C (all P<0.05), but there was no statistical significance between the two groups at 24-48 h ( P>0.05). There were no significant difference between the two groups in the incidence of chronic pain at 3 months and 6 months after surgery ( all P>0.05). The incidence of chronic pain in group E was lower than that in group C at 3 months [13.4%(9/67) vs 18.8%(13/69), P=0.392] and 6 months [10.7%(6/56) vs 16.1%(10/62), P=0.391], but the difference was not statistically significant. Conclusions:Perioperative administration of esketamine provided superior short-term analgesia after cesarean section and did not increase the psychotomimetic adverse event rate. However, the development of chronic pain was not restrained.
3. Advances in Research on Antipruritic Effect of Xiaofengsan in Orthodox Manual of External Diseases

Itching is an unpleasant sensation that can cause physical and psychological discomfort. It often occurs in a variety of diseases. In addition to the common chemical medicines, the application of traditional Chinese medicines(TCMs) have attracted much attention for itch. As one of the classic antipruritic formula, Xiaofengsan is widely used by the later generations of doctors based on the edition of Orthodox Manual of External Diseases(Wài Kē Zhèng Zōng). It can be used to remove wind, dehumidify, clear heat and nourish blood by taking into account the complicated pathogenesis of skin diseases. In modern clinics, Xiaofengsan is commonly used for the treatment of rubella, eczema, acute and chronic urticaria, contact dermatitis, allergic dermatitis, psoriasis and other types of dermatitis with remarkable antipruritic effect. Its mechanism is related to its anti-allergic reaction, anti-allergy, anti-inflammatory, anti-histamine,immune regulation etc. This article reviews the research literature of Xiaofengsan and its addition and subtraction in modern pharmacological experimental research and clinical treatment in the past 15 years, and analyzes the current research situation of Xiaofengsan in the topic of antipruritic effect. The focus is on the compatibility characteristics of formula and the component study of single herb after being decomposed of Xiaofengsan, and the main material basis of antipruritic effect is summarized. It was found that Schizonepetae Herba, Saposhnikoviae Radix, Arctii Fructus, Cicadae Periostracum and Sophorae Flavescentis Radix were the main medicines for its antipruritic effect,which was helpful to expand the clinical value of Xiaofengsan and deepen its mechanisms of anti-itching.

Objective:
To investigate the characteristics of the essential thrombocythemia (ET) cases transformed to the acute myeloid leukemia (AML) and the role of allogeneic hematopoietic stem cell transplantation (allo-HSCT) in the treatment of this disease.
Methods:
The clinical and laboratory characteristics of 3 ET cases before and after transformation and after allo-HSCT were retrospectively analyzed, meanwhile the related literatures were reviewed and discussed.
Results:
Case 1 was a male patient of 44 years old, whose PLT was 500×109/L when firstly diagnosed ET. After 3 years the disease progressed into myelodysplastic syndrome (MDS) while WT1 expression increased from 77 (first visit) to 13 171 copies/10 000 ABL copies, at the same time chromosome changed dramatically. During the period of decitabine treatment the disease processed into AML. Case 2 was a male of 58 years old whose PLT was 2 100×109/L when firstly diagnosed ET. The disease progressed to AML after 9 years, whose WT1 expression increased from 130 (first visit) to 3 222 copies/10 000 ABL copies, and he relapsed shortly after intensive chemotherapy. Case 3 was a male of 60 years old whose PLT was 900×109/L when firstly diagnosed ET. The disease progressed to AML after 5 years, whose WT1 increased from 56 (first visit) to3 696 copies/10 000 ABL copies. Moreover leukemia spread to central nervous system (CNS) during chemotherapy. Before allo-HSCT, cases 1 did not achieve remission; case 2 relapsed after a short time of remission and case 3 transferred to CNS leukemia. All of the 3 cases underwent allo-HSCT successfully, and they all achieved completely remission, whose chromosome and gene mutation recovered negative. At the same time, CNS leukemia of case 3 disappeared. The median WT1 decreased to 50 copies/10 000 ABL copies. There was no severe complication during the median time of 5 months after allo-HSCT.
Conclusions
The patients transformed to AML had poor prognosis, allo-HSCT was the only method that can cure the disease now.
6.Severe pathological manifestation of cerebral amyloid angiopathy correlates with poor outcome from cerebral amyloid angiopathy related intracranial hemorrhage.

BACKGROUNDCerebral amyloid angiopathy (CAA) is one of the main causes of spontaneous intracranial hemorrhage (ICH). No established link is available between pathological scores of CAA and its outcome. This study aimed to identify the correlations between pathological severity and poor postoperative outcome in the Chinese population.
METHODSBetween May 2006 and April 2011, 367 consecutive patients who underwent surgery for CAA-related ICH in 71 hospitals throughout the mainland of China were enrolled in this study. Twelve months after surgery, we evaluated these patients' outcomes according to the modified Rankin Scale (mRS) and statistically correlated risk factors (demographics, medical history, pathological results, and surgical details) that are associated with a favorable (mRS < 3) and poor (mRS ≥ 3) outcome groups.
RESULTSRisk factors for poor postoperative outcome in 367 patients with CAA-related ICH included advanced age (OR 1.034, 95%CI 1.001 - 1.067, P = 0.042), CAA pathology severity (OR 2.074, 95%CI 7.140 - 16.25, P < 0.001), lobar hematoma (OR 0.225, 95%CI 0.104 - 0.486, P < 0.001), presence of intraventricular hemorrhage (OR 0.478, 95%CI 0.229 - 1.001, P = 0.050), and/or subarachnoid hemorrhage (OR 2.629, 95%CI, 1.051 - 6.577, P = 0.039).
CONCLUSIONSPoor postoperative outcome of patients with CAA-related ICH was more related to the severe pathological manifestation instead of other factors. Prior ischemia may present an early stage of CAA.

7.Effects of low-glycemic index meal replacement on management of patients with type 2 diabetes mellitus

A total of 127 type 2 diabetic patients were divided into low glycemic index meal replacements (intervention) group and standard food-based diet (reference) group in an experiment for 12 weeks.The results showed that fasting plasma glucose,postprandial 2 h plasma glucose,fasting insulin,and homeostasis model assessment insulin resistance(HOMA-IR) in the intervention group decreased significantly after 12 weeks trial ( P<0.05 or P<0.01 ).However,there were no significant changes in lipid profile and HbA1C in intervention group.In addition,percentage of body fatty,visceral fatty area,and waist-hip ratio also decreased in intervention group( all P<0.01 ).Superoxide dismutase and glutathione levels increased significantly in intervention group by the end of trial (both P<0.01 ),while malondialdehyde was decreased (P<0.01 ).There were no significant changes in the aforementioned indices in the reference group.Weight,body mass index,and waist circumferences were decreased in both groups,but without significant difference between the two groups.
8.Treadmill exercise test in patients with coronary artery myocardial bridging

Objective To observe treadmill exercise test (TET) characteristics in patients with myocardial bridging(MB).Methods TET results from January 2003 to December 2010 were retrospectively analyzed in 156 patients with confirmed MB diagnosis.MB patients were divided into smoking group (68 cases) and non-smoking group(88 cases).Coronary angiography results were used to analyze the relations between MB length,myocardial ischemia and exercising duration.Results ( 1 ) MB was documented on two coronary arteries in 2 patients ( 1% ),MB was detected in single artery in 154 patients (99%),of whom 146 cases were located at left anterior descending artery,8 cases were located at right coronary artery.The degree of narrowing of MB was graded 1 ( less than 50% ) in 16 patients ( 10% ),grade 2 (50% to 75% ) in 108 patients (69%) and grade 3 (greater than 75% ) in 32 patients (21%).The length of MB ranged between 4 to 40 mm,MB length was less than 10 mm in 40 patients( 26% ),between 11 to 20 mm in 48 patients(31% ),between 21 to 30 mm in 44 patients(28% ),greater than 31 mm in 24 patients( 15% ).(2) TET positive rate was 41% (64/156) and the TET positive rate was significantly higher in smoking group than in non-smoking group [ 57% (39/68) vs.28% (25/88,P < 0.01 ) ].(3) The length of MB was positively related to the ST-segment depression (r =0.723,P < 0.01 )and negatively related to exercising duration ( r =- 0.828,P < 0.01 ).Heart rate was positively related to the ST-segment depression ( r =0.368,P < 0.01 ).Conclusions TET may serve as a good test to assess myocardial ischemia in patients with MB.The length of MB is positively related with myocardial ischemia and negatively related with exercising duration.Smoking might increase myocardial ischemic incidence in MB patients,MB patients should be advised to stop smoking.
9.Integrated intervention in management of type 2 diabetes

Objective To test the effectiveness of an integrated intervention to improve control and management of type 2 diabetic patients.Methods The study was designed as a randomized and multi-center clinic trial for 24 weeks.150 overweight patients from 2 companies and one hospital in Shanghai were recruited.The principal objective of the project was to compare the effectiveness of a structured diabetes management program consisting of 200 kcal breakfast as a meal replacement with low glycemic index (GI),scheduled blood glucose monitoring(6 times per week),low GI diet consulting,monitoring and medical check up and nutritional education (diet consulting,health education on nutrition and healthy lifestyle) in overweight individuals with type 2 diabetes.Results At 12th week,the level of fasting blood glucose(FBG) in both groups decreased by 16.5% and 10.6%,and at 24th week,the FBG also decreased by 25.0% in intervention group,but in control group FGB increased 2.8% (P<0.01).Meanwhile,there were 8.2% and 11.1% reductions in HbA1C respectively at 12th week and at 24th week in intervention group,while the values were increased slightly in control group(P<0.01).Compared with control group,waist circumference,hip circumference,and blood pressure were significantly reduced after treatment for 24 weeks in intervention group (P<0.01).Conclusions Integrated intervention is an effective approach in managing FBG,HbA1C blood pressure,and weight control in overweight diabetic patients.
10.Prevention of beta cell dysfunction and apoptosis by adenoviral gene transfer of rat insulin-like growth factor 1.

BACKGROUNDIslet beta-cells are almost completely destroyed when patients with type 1 diabete are diagnosed. To date, insulin substitute therapy is still one of the main treatments. The cure of type 1 diabetes requires beta-cell regeneration from islet cell precursors and prevention of recurring autoimmunity. Therefore, beta-cell regeneration and proliferation emerge as a new research focus on therapy for type 1 diabetes. Islet beta-cell regeneration and development are controlled by many growth factors, especially insulin-like growth factor-1 (IGF-1).
METHODSRecombinant adenovirus encoding rat IGF-1 (rIGF-1) was constructed and transduced into rat beta-cells, RINm5F cells. Western blotting analysis and ELISA were used to detect rIGF-1 protein. Streptozotocin (STZ) was used to induce RINm5F cell destruction. The level of nitric oxide (NO) was detected in cell culture supernatants by the Griess reaction. Islet cell function was evaluated by glucose-stimulated insulin production. Flow cytometry analysis was further used to investigate the apoptosis of RINm5F cells. Thiaoollyl blue viability assay was applied to determine cell viability.
RESULTSThe recombined adenovirus-rIGF-1 was successfully constructed and the titer was 4.0 x 10(8) pfu/ml. The rIGF-1 protein was effectively expressed in the RINm5F cells and cell culture supernatants. rIGF-1 expression remarkably inhibited STZ-induced islet cell apoptosis and significantly decreased the level of NO. Furthermore, IGF-1 expression also significantly protected insulin secretion and cell proliferation in a time-dependent manner.
CONCLUSIONSOur study suggests that locally produced rIGF-I from RINm5F cells may be beneficial in maintaining beta-cell function, protecting beta-cells from the destruction of apoptosis factors and promoting beta-cell survival and proliferation. IGF-I might be considered as a candidate gene in gene therapy for type 1 diabetes. In addition, it appears that the apoptosis induced by STZ may be NO-dependent.
