1.Expenditure trends and intergenerational substitution relationships of national negotiated drugs for non-small cell lung cancer in China

OBJECTIVE To analyze the impact of intergenerational substitution effect of the drugs with the same indication on fund expenditures for national medical insurance for this indication in China, taking national medical insurance negotiated drugs for non-small cell lung cancer (hereinafter referred to as “NSCLC national negotiation drugs”) as an example. METHODS The sales amounts of 15 types of NSCLC national negotiated drugs in secondary and tertiary public hospitals across seven sample provinces from 2017 to 2023 were collected from the Pharmaceutical Drug Database of the China National Pharmaceutical Industry Information Center. A sliding t-test and Mann-Kendall trend test were used to evaluate the trends in sales amounts and DDDs. Taking epidermal growth factor receptor(EGFR)-tyrosine kinase inhibitors (TKIs) and anaplastic lymphoma kinase (ALK)-TKIs as examples, the generational substitution characteristics of these drugs were analyzed. RESULTS The change points of sales amounts and DDDs differed slightly across provinces; the change points of sales amount were mostly concentrated between the first quarter of 2019 and the second quarter of 2020, while those for DDDs were primarily concentrated in the first to second quarters of 2021. In five provinces, i.e. Beijing, Heilongjiang, Jiangsu, Sichuan and Shaanxi, sales amounts showed no significant upward trend after the breakpoints (P>0.05), whereas in Guangdong and Hubei, both sales amounts and DDDs continued to rise significantly following the breakpoints (P<0.05). Since 2020, the growth in sales amounts of EGFR-TKIs had slowed. After 2021, the sales amounts and DDDs of first- and second-generation EGFR-TKIs declined, while third-generation EGFR-TKIs showed clear substitution effects. The sales amounts of ALK-TKIs continuedto grow. However, the sales amounts and DDDs of first-generation ALK-TKIs had declined year by year, with second-generation ALK-TKIs demonstrating a significant substitution effect on first-generation ones, while third-generation ALK-TKIs had not yet shown a clear substitution trend. CONCLUSIONS With the annual access to and renewal of drugs in national medical insurance negotiations, the overall expenditure trend for NSCLC negotiated drugs comes to a plateau. The intergenerational substitution relationships of drugs with the same indication achieve a relative balance in fund expenditures for negotiated drugs with the same indication. It is recommended that pharmaceutical companies carefully consider their research pipelines, and that medical insurance authorities, during the renewal management process, pay attention to the impact of drug substitution effects on the overall actual expenditure of medical insurance funds for that specific target or the same indication, and scientifically evaluate the extent of price reductions during contract renewals.
2.Analysis of The Characteristics of Brain Functional Activity in Gross Motor Tasks in Children With Autism Based on Functional Near-infrared Spectroscopy Technology

ObjectiveBased on functional near-infrared spectroscopy (fNIRS), we investigated the brain activity characteristics of gross motor tasks in children with autism spectrum disorder (ASD) and motor dysfunctions (MDs) to provide a theoretical basis for further understanding the mechanism of MDs in children with ASD and designing targeted intervention programs from a central perspective. MethodsAccording to the inclusion and exclusion criteria, 48 children with ASD accompanied by MDs were recruited into the ASD group and 40 children with typically developing (TD) into the TD group. The fNIRS device was used to collect the information of blood oxygen changes in the cortical motor-related brain regions during single-handed bag throwing and tiptoe walking, and the differences in brain activation and functional connectivity between the two groups of children were analyzed from the perspective of brain activation and functional connectivity. ResultsCompared to the TD group, in the object manipulative motor task (one-handed bag throwing), the ASD group showed significantly reduced activation in both left sensorimotor cortex (SMC) and right secondary visual cortex (V2) (P<0.05), whereas the right pre-motor and supplementary motor cortex (PMC&SMA) had significantly higher activation (P<0.01) and showed bilateral brain region activity; in terms of brain functional integration, there was a significant decrease in the strength of brain functional connectivity (P<0.05) and was mainly associated with dorsolateral prefrontal cortex (DLPFC) and V2. In the body stability motor task (tiptoe walking), the ASD group had significantly higher activation in motor-related brain regions such as the DLPFC, SMC, and PMC&SMA (P<0.05) and showed bilateral brain region activity; in terms of brain functional integration, the ASD group had lower strength of brain functional connectivity (P<0.05) and was mainly associated with PMC&SMA and V2. ConclusionChildren with ASD exhibit abnormal brain functional activity characteristics specific to different gross motor tasks in object manipulative and body stability, reflecting insufficient or excessive compensatory activation of local brain regions and impaired cross-regions integration, which may be a potential reason for the poorer gross motor performance of children with ASD, and meanwhile provides data support for further unraveling the mechanisms underlying the occurrence of MDs in the context of ASD and designing targeted intervention programs from a central perspective.
3.Effects of dexmedetomidine in perioperative period on pain, oxidative stress and adverse reactions after radical breast cancer surgery

Objective:To explore the effects of dexmedetomidine (DEX) on postoperative pain, oxidative stress and adverse reactions in patients undergoing radical mastectomy.Methods:A total of 90 patients with breast cancer who received radical surgical treatment in our hospital from Jun. 2022 to Jun. 2023 were prospectively included as research objects and randomly divided into 3 groups with 30 patients in each group. DEX group was applied before, during and after surgery, respectively. The levels of pain visual analogue scale (VAS), Richmonation sedation score (RASS), superoxide dismu-tase (SOD) and malondialdehyde (MDA) were recorded.Results:The recovery time and extubation time in preoperative and intraoperative DEX group were significantly lower than those in postoperative DEX group, and the awakening time and extubation time in preoperative DEX group were significantly lower than those in intraoperative DEX group ( F value was 48.62 and 53.98, respectively, P<0.001). At 1 h, 6 h and 12 h after surgery, the VAS and RASS scores of patients in the preoperative and intraoperative DEX group were significantly lower than those in the postoperative DEX group, compared with those in the intraoperative DEX group. The VAS and RASS scores in the DEX group were significantly decreased ( F value: 62.34, 55.24, 69.26, 36.82, 24.20, 39.97, P<0.001). At 24h after surgery, there was no significant difference in VAS and RASS scores among the three groups ( F value was 0.45 and 0.81, respectively, P value was 0.613 and 0.418). Immediately after surgery, 24 h after surgery, 72 h after surgery, the SOD level of DEX group was significantly higher than that of DEX group before and during surgery ( F value was 29.37, 33.24, 10.35, P<0.001). MDA levels were significantly lower than those in postoperative DEX group ( F value was 30.52, 41.27, 8.26, P<0.001). There was no significant difference in the incidence of postoperative adverse reactions among all groups ( P>0.05) . Conclusion:Preoperative and intraoperative application of DEX can reduce postoperative pain and oxidative stress in breast cancer patients, help patients recover quickly after surgery, and preoperative application is superior to intraoperative application.
4.Protective effect of sufentanil on rat model of off-pump coronary artery bypass grafting

Objective To investigate the effects of sufentanil on myocardial cells and renal function in rat models of non-stopping coronary artery bypass grafting.Methods SD rats were randomly divided into sham group(only thoracectomy without coronary artery ligation),model group(ischemia perfusion model was constructed),experimental-L group(ischemia perfusion model+0.5 μg·kg-1 sufentanil)and experimental-H group(ischemia perfusion model+1μg·kg-1 sufentanil).The mice were sacrificed 6 h after operation.The expressions of tumor necrosis factor-α(TNF-α),interleukin-1 β(IL-1 β)and interleukin-6(IL-6)in cardiomyocytes and renal cells were detected by enzyme-linked immunosorbent assay(ELISA).The expressions of apoptosis-related proteins in cardiomyocytes and renal cells were detected by Western blot.Serum renal function was detected with serum renal function kit.Results The expression levels of TNF-α in myocardial cells were(50.41±6.03),(136.61±21.73),(102.36±12.84)and(74.21±16.32)pg·mg-1 in sham group,model group,experimental-L group and experimental-H group,respectively;the expression levels of cysteinyl aspartate specific proteinase-3(Caspase-3)were 0.31±0.02,1.26±0.18,0.83±0.12 and 0.67±0.08,respectively;blood urea nitrogen(BUN)levels were(5.94±1.12),(20.04±5.36),(16.84±4.19)and(10.96±3.64)μmol·L-1,respectively;the expression levels of TNF-α in renal cells were(146.49±16.13),(1 023.82±34.69),(841.65±49.66)and(324.84±48.93)pg·mg-1,respectively;the expression levels of Caspase-3 in renal cells were 0.48±0.06,1.15±0.17,0.96±0.06 and 0.74±0.07,respectively.The difference between experimental-H group and sham group,model group and experimental-L group were statistically significant(all P<0.05).Conclusion Sufentanil can significantly reduce the production of inflammatory factors and the expression of apoptotic proteins in rat cardiomyocytes,significantly improve the renal function of rats,and significantly reduce the production of inflammatory factors and the expression of apoptotic proteins in rat renal cells.
5.Effects of bunched cognitive behavior intervention on disease fear and psychological security in glioma patients

Objective:To investigate the effects of bunched cognitive behavior intervention on disease fear and psychological security in patients with glioma.Methods:A total of 92 patients with glioma who underwent surgical treatment from January 2022 to June 2023 were selected.According to the order of enrollment, all subjects were divided into research group( n=44)and control group( n=48). The patients in control group received routine medical and nursing intervention, and patients the research group adopted glioma bunched cognitive behavior intervention on the basis of routine medical and nursing intervention, including 4 intervention cycles.At enrollment, 2 weeks after intervention, and 4 weeks after intervention, all subjects were evaluated by the fear of progression questionnaire-short form (FoP-Q-SF), safety questionnaire (SQ), self-rating anxiety scale (SAS) and self-rating depression scale (SDS). All the data in this study were processed by SPSS 26.0 statistical software.The scores of FoP-Q-SF, SQ, SAS and SDS before and after intervention were compared by repeated measures ANOVA between the two groups. Results:(1)The total FoP-Q-SF score, physiological health dimension scores, and social family dimension scores of the two groups showed significant interaction effects before and after intervention ( F=254.839, 52.738, 12.237, all P<0.05). Further simple effect analysis showed that after 2 and 4 weeks of intervention, the FoP-Q-SF scores of the research group (2 weeks after intervention: 33.80±4.94, 36.48±4.04; 4 weeks after intervention: 31.25±4.55, 35.94±4.47) and social family dimensions (2 weeks after intervention: 15.32±2.56 points, 17.06±2.14; 4 weeks after intervention: 14.05±2.59, 16.96±1.99) were lower than those of the control group (all P<0.05). The physiological health dimension score of the research group was lower than that of the control group after 4 weeks of intervention (4 weeks after intervention: 17.30±2.92, 19.06±2.38) ( P<0.05). After 4 weeks of intervention, the FoP-Q-SF score, physiological health dimension score, and social family dimension score of the research group were all lower than those at 2 weeks after intervention and before intervention (all P<0.05). (2)The total SQ score, interpersonal security dimension score and the determined control score of the two groups showed significant interaction effects before and after intervention( F=193.129, 54.706, 44.015, all P<0.05). Further simple effect testing showed that after 2 and 4 weeks of intervention, the total SQ score and interpersonal security score of the research group were higher than those of the control group (all P<0.05). The determined control score of the research group was higher than that of the control group after 4 weeks of intervention ( P<0.05). After 2 and 4 weeks of intervention, the total SQ score, interpersonal security score, and determination control score of the research group were higher than before intervention (all P<0.05), and the total SQ score and interpersonal security score of the research group were higher than 2 weeks after intervention (both P<0.05). (3)The SAS score and SDS score of the two groups showed significant interaction effects before and after intervention( F=237.867, 282.882, both P<0.05). Further simple effect analysis showed that after 2 and 4 weeks intervention, the SAS and SDS scores of the research group were lower than those of the control group (all P<0.05). The SAS and SDS scores of the research group were lower after 2 weeks and 4 weeks intervention than before intervention (all P<0.05). The SAS and SDS scores of the research group at 4 weeks after intervention were lower than those at 2 weeks after intervention (both P<0.05). Conclusion:Bundled cognitive behavioral intervention can improve disease fear and negative emotions in patients with glioma, and enhance psychological security.
6.Efficacy-driving Mechanism of Danhong Injection for Stable Angina Pectoris Based on Composition-activity Relationship of Target Modules

ObjectiveTo explore the efficacy-driving mechanism of Danhong injection (DHI) in the treatment of stable angina pectoris (SAP) based on the composition-activity relationship of target modules and clarify the pharmacological effects of DHI. MethodAccording to the angina frequency (AF) in the Seattle Angina Questionnaire (SAQ) that was obtained in the previous clinical trial, the patients before and after DHI treatment were grouped based on efficacy. The transcriptomic data of the patients before treatment and in the best efficacy group 30 days post-treatment were selected as the data source, and then weighted gene co-expression network analysis (WGCNA) was employed to construct the co-expression network. Relevant modules in the network were identified and associated with clinical features. In addition, the On-modules (Z value below 0) were identified by Zsummary. The topological indicators such as density, centrality, and clustering coefficient were adopted to explore the dynamics of DHI efficacy at the network level and module level, respectively. In addition, the driver genes were screened by the personalized network control (PNC) algorithm. Finally, rat H9C2 cells were used to establish the model of hypoxia/reoxygenation (H/R), which was used to confirm the potential therapeutic target of DHI for SAP and provide a scientific basis for revealing the therapeutic mechanism of DHI. ResultWe identified 19 modules in the best efficacy group of DHI for SAP, and the comparison between day 0 and day 30 revealed 12 On-modules. The changes of network topological indicators at the network and module levels confirmed the correlation between the best efficacy of DHI treatment and topological dynamics. Finally, the driver genes, Klotho and fibroblast growth factor 22 (FGF22), in DHI treatment of SAP were verified by the H9C2 cell model of H/R. ConclusionBased on clinical transcriptome data, this study determined the composition-activity relationship of target modules of DHI for SAP, which provided a scientific basis for deciphering the efficacy-driven mechanism of DHI for SAP.
7.Structure Confirmation of Three New Psychoactive Substances and Qualitative Identification of Unknown Substances

Objective To establish the structural confirmation methods of three suspected new psychoac-tive substances(NPSs),and explore a more general qualitative testing method.Methods Infrared ab-sorption spectroscopy(IR),gas chromatography-mass spectrometry(GC-MS),1H-nuclear magnetic reso-nance spectroscopy(1H-NMR),13C-nuclear magnetic resonance spectroscopy(13C-NMR),19F-nuclear magnetic resonance spectroscopy(19F-NMR)and other techniques were used to identify the composi-tion and structure of 5 samples containing suspected NPS submitted by public security bureaus.Results NPSs were found in the above 5 samples,and 3 were confirmed as NPS included in the newly listed controlled substances on July 1,2024,namely 2-(methylamino)-2-(2-methylphenyl)cyclohexan-1-one(2-MDCK),2-(ethylamino)-2-(2-fluorophenyl)cyclohexan-l-one(2-FXE),1-(3,4-methylenedioxy-phenyl)-2-(dimethylamino)pentan-1-one(dipentylone),respectively.The first two substances were phen-cyclidine NPS,and the third substance was synthetic cathinone NPS.Conclusion This study systemati-cally summarizes the distinguishing features of the infrared absorption spectrometry,nuclear magnetic resonance spectroscopy and mass spectrometry of three NPSs,which can provide a reference for the qualitative identification of unknown substances.
8.Effects of bundled cognitive-behavior intervention in postoperative patients with glioma

Objective:To explore the effectiveness of bundled cognitive-behavior intervention in postoperative patients with glioma.Methods:From January 2022 to June 2023, convenience sampling was used to select 95 patients with glioma who underwent surgical treatment at the Neurosurgery Department of Affiliated Hospital of Jining Medical University as subjects. The patients were divided into an observation group and a control group based on the order of enrollment. The control group received routine medical nursing intervention, while the observation group received bundled cognitive-behavior intervention on the basis of control group. The scores of the Chinese-Strategies Used by People to Promote Health (C-SUPPH), Short Form Health Survey (SF-36), Self-Rating Anxiety Scale (SAS), and Self-Rating Depression Scale (SDS) were compared between two groups of patients at different time points before and after intervention.Results:Repeated measures analysis of variance showed that there were intergroup, time, and interaction effects between the C-SUPPH, SF-36, SAS, and SDS scores of the two groups, and the differences were statistically significant (all P<0.05). At 1, 3, and 6 months after discharge, the C-SUPPH and SF-36 scores in observation group were lower than those in control group, and the differences were statistically significant (all P<0.05). At 3 and 6 months after discharge, the SAS and SDS scores of the observation group were lower than those of the control group, and the differences were statistically significant (all P<0.05) . Conclusions:Bundled cognitive-behavior intervention can increase the self-management efficacy of postoperative glioma patients, improve their quality of life, and alleviate their anxiety and depression.
9.Application of KIH Structure in Recombinant Expression of Human Interleukin-35 in vitro

Interleukin-35(IL-35)is an important immunosuppressive cytokine that has been shown to play a role in the immune response of various diseases.In this study,we cloned the coding sequence of human IL-35 gene,constructed single subunit expression vectors pXC17.4-p35 and pcDNA3.1(+)-EBI3,and co-transfected CHO-K1 cells to express IL-35 in vitro.No binding was found between subunits of p35 and EBI3.Knobs-into-Holes(KIH)can solve the problem of heavy chain mismatch of heterolo-gous antibodies.Therefore,expression vectors pXC17.4-p35-Fch and pcDNA3.1(+)-EBI3-Fck were constructed by fusing KIH structures on the basis of the original sequences to express the recombinant fu-sion protein of KIH-IL-35.The expression vectors of two subunits were exchanged at the same time to verify the influence of different vectors on the expression level of KIH-IL-35.The analysis of various pro-tein detection methods showed that the correct expression rate of KIH-IL-35 structure was significantly im-proved.Affinity purification of KIH-IL-35 was performed after large amount of expression,and the bind-ing activity of KIH-IL-35 to glycoprotein 130(gp130)was detected by ELISA.The results showed that the binding of KIH-IL-35 to gp130 was concentration dependent.The indirect activity of KIH-IL-35 and M1 cells was detected by cell activity assay.Further results showed that the inhibition rate of M1 cells in-creased in a dose-dependent manner with the concentration of KIH-IL-35.In addition,a method for de-termining IL-35 activity by activated human peripheral blood mononuclear cells was successfully estab-lished.Activated PBMCs increased in a dose-dependent manner with KIH-IL-35 concentration.In sum-mary,this study utilized the KIH-IL-35 model to enhance the expression of recombinant human IL-35 and validated its high activity in vitro,providing new ideas for the study of IL-35 and the recombinant expres-sion of similar heterodimeric cytokines.
10.The Impact of SRSF1-Mediated Alternative Splicing of RBBP6 on the Proliferation of Multiple Myeloma Cells

Objective:To investigate the effect of different isoforms of RBBP6 on the proliferation of multiple myeloma (MM) cells after alternative splicing mediated by splicing factor SRSF1. Methods:RT-PCR was used to detect the expression levels of RBBP6 mRNA splicing isoforms regulated by SRSF1. The GEO database was used to analyze the changes of RBBP6 isoform 1 in the progression of plasma cell disease,and survival analysis was used to evaluate the value of this gene in the prognosis of MM patients. In vitro loss-of-function and gain-of-function experiments were conducted by transfecting control siRNA,RBBP6 isoform 1 siRNA,empty vector (EV),OE-RBBP6 isoform 3 into MM.1S cells,then the expression levels of RBBP6 isoform 1 and RBBP6 isoform 3 were detected by real-time PCR and Western blot. CCK-8 assay was performed to detect the cell proliferation ability. Results:Knockdown of SRSF1 increased the expression of RBBP6 isoform 3 and decreased the expression of RBBP6 isoform 1. RBBP6 isoform 1 was closely related to the progression of plasma cell disease,and the high expression of RBBP6 isoform 1 was associated with poor prognosis in patients with MM. Downregulation of RBBP6 isoform 1 expression and overexpression of RBBP6 isoform 3 both reduced the proliferation ability of MM cells. And after downregulating the expression of RBBP6 isoform 1,the level of p53 protein in MM cells was significantly increased (P<0.05). Conclusion:In MM,splicing factor SRSF1 can cause alternative splicing abnormalities in RBBP6. The RBBP6 isoform 1 promotes MM cell proliferation,while the RBBP6 isoform 3 inhibits MM cell proliferation,and the mechanism may be related to regulating the p53 pathway.
