Disturbances of gut microbiota may affect the balance of the host immune system.The metabolism of gut microbiota produces many bioactive molecules interacting with host,typically secondary bile acids(SBAs).SBAs are involved in regulating the energy metabolism and the expression of inflammatory response-related genes by bind-ing to membrane receptors and nuclear receptors,such as takeda G protein-coupled receptor(TGR5)and farnesol X receptor(FXR),which are essential for maintaining host immune homeostasis.
2.Roles of periodontal pathogens in the adverse pregnancy

Periodontitis is closely related to systemic health,especially adverse pregnancy.Periodontal pathogens are actively in-volved in the formation and development of intrauterine infections.Studies have shown that infection by periodontal pathogens is an im-portant risk factor for adverse pregnancy.Periodontal pathogens reach the uterus via the hematogenous or retrograde route,and the bacte-ria,their virulence factors,and the immunoinflammatory mediators produced by the pathogens promote each other and together mediate the development of intrauterine infections and ultimately lead to adverse pregnancies.This article reviews the role of periodontitis and perio-dontal pathogens in adverse pregnancies and their mechanisms of infection to provide new possibilities for improving the birth rate.
3.Therapeutic Effect of Compound Wufengcao Liquid on Tuberculous Ulcer from Perspective of Macrophages

ObjectiveTo explore the clinical efficacy of compound Wufengcao liquid (CWL) on tuberculous ulcer and the influence on macrophage polarization. Method① Clinical experiment: A total of 145 patients with tuberculous ulcer who were treated in Nanjing Integrated Traditional Chinese and Western Medicine Hospital were randomized into observation group, control group Ⅰ, and control group Ⅱ according to the random number table method. In addition to the basic anti-tuberculosis chemotherapy, CWL, Kangfuxin liquid, and isoniazid solution (local external application) were respectively used in the observation group, control group Ⅰ, and control group Ⅱ. The treatment lasted 4 weeks for each group. The total effective rate in wound healing, traditional Chinese medicine(TCM) syndrome score, and histopathological morphology of wound were observed and the expression of inducible nitric oxide synthase (iNOS) and arginase-1 (Arg-1) in wound tissue was measured. ② Cell experiment: RAW264.7 cells were cultured in DMEM (10% fetal bovine serum, 1% double-antibody solution) in a cell incubator (37 °C, 5% CO2). Phorbol 12-myristate 13-acetate (PMA) was used to induce the differentiation of RAW264.7 cells into macrophages. Lipopolysaccharide (LPS) was employed to stimulate polarization of macrophages into M1 type and interleukin-4 (IL-4) to induce the polarization into M2 type. Kangfuxin solution, isoniazid solution, and CWL were respectively applied to the above cell model for 36 h. The cell supernatant was collected and centrifuged. Western blot was used to detect the protein expression of tumor necrosis factor-α (TNF-α), transforming growth factor-β (TGF-β), iNOS, and Arg-1, and flow cytometry (FCM) to detect the expression of CD86 and CD206. Result①Clinical experiment: The total effective rate in the CWL group [98.0% (48/49)] was higher than that in the control group Ⅰ [87.5% (42/48), χ2=3.962, P<0.05] and control group Ⅱ [83.3% (40/48), χ2=6.162, P<0.05]. After 28 days of treatment, compared with control group Ⅰ and control group Ⅱ, CWL decreased the TCM syndrome score (P<0.05) and obviously improved the histopathological morphology of the wound. Immunohistochemistry results showed that the iNOS expression in local focus tissue was lower (P<0.05) and the expression of Arg-1 was higher (P<0.05, P<0.01) in the CWL group than in the control group Ⅰ and control group Ⅱ after 28 days of treatment. ② Cell experiment: Western blot assay showed that the expression of iNOS and TNF-α in LPS group increased compared with that in the M0 group (P<0.01) and the expression in the LPS+ isoniazid group, LPS+ Kangfuxin group, and LPS+CWL group was lower than that in the LPS group (P<0.05). The expression of iNOS in LPS+Kangfuxin group and LPS+ CWL group was lower than that in the LPS+isoniazid group (P<0.05, P<0.01), and the expression of TNF-α in LPS+ CWL group was lower than that in LPS+isoniazid group (P<0.01). The expression of TNF-α in LPS+ CWL group decreased compared with that in the LPS+ Kangfuxin group (P<0.05). The expression of Arg-1 and TGF-β in IL-4 group was higher than that in the M0 group (P<0.01), and the expression in the IL-4+isoniazid group, IL-4+Kangfuxin group, and IL-4+ CWL group was higher than that in the IL-4 group (P<0.05). The expression of Arg-1 and TGF-β in the IL-4+ Kangfuxin group and IL-4+CWL group was higher than that in the IL-4+isoniazid group (P<0.05, P<0.01), and the expression was higher in the IL-4+CWL group than in the IL-4+Kangfuxin group (P<0.05, P<0.01). The FCM result showed that the expression of CD86 and CD206 in LPS group and IL-4 group was higher than that in M0 group (P<0.01). CD86 expression in LPS+isoniazid group, LPS+ Kangfuxin group, and LPS+CWL group was lower than that in the LPS group (P<0.01). The expression of CD86 in LPS+Kangfuxin group and LPS+ CWL group increased compared with that in the LPS+isoniazid group (P<0.01), and the expression was higher in the LPS+ CWL group than in the LPS+Kangfuxin group (P<0.01). CD206 expression in IL-4+ isoniazid group, IL-4+Kangfuxin liquor group, and IL-4+ CWL group was increased compared with that in the IL-4 group (P<0.01). CD206 expression in IL-4+Kangfuxin liquid group and IL-4+ CWL group was decreased compared with that in the IL-4+isoniazid group (P<0.01). CD206 expression in IL-4+CWL group was lower than that in the IL-4+ Kangfuxin group (P<0.05). ConclusionCWL can promote the healing of tuberculous ulcers, and the mechanism is that it inhibits the expression of iNOS, TNF-α, and CD86 and promotes the expression of Arg-1, TGF-β, and CD206, thereby regulating M1/M2 polarization balance.
4.Effects of the S1PR3 specific agonist peptide GPS-725.017 on acute lung injury in mice

Objective:To design a modified S1PR3 specific agonist, GPS-725.017, and investigate its protective effect on acute lung injury by promoting macrophage clearance of bacteria.Methods:A short peptide derived from the intracellular region of S1PR3 receptor was named GPS725.017, which was modified with norleucine (Nle) and myristicacid (myr) at its N terminus. Mice were divided into the sham operation group, solvent group and GPS-725.017 treatment group. The acute lung injury model was induced by endotracheal injection of E. coli (5×10 6 CFU), and the experimental group was treated with GPS-725.017 (10 mg/kg). The 48-h survival rate of mice was recorded. After 5 h of modeling, the bacterial load and inflammatory cytokines in peripheral blood and lung were detected, and Vps34 protein content in alveolar macrophages was determined by Western blot. After 12-h of modeling, lung tissues were collected for H&E staining and pathological scores. Results:Compared with the solvent group, the survival rate of mice in the GPS-725.017 treatment group was significantly improved ( P<0.01), the bacterial CFU in blood and alveolar lavage fluid was significantly lower than that in the solvent group ( P<0.001), and the levels of TNF-α and IL-1β in blood and alveolar lavage fluid were significantly lower than those in the solvent group ( P<0.001). Western blot showed that the expression level of Vps34 protein in alveolar macrophages was significantly higher than that in the solvent group ( P<0.01). Histopathology result showed that the pathological damage of lung in the treatment group was significantly less than that in the solvent group ( P<0.001). Conclusions:The modified synthetic S1PR3 specific agonist GPS-725.017 could specifically activate the S1PR3 receptor on the membrane of alveolar macrophages and up-regulate the expression level of intracellular Vps34 protein, which can promote the removal of bacteria in alveolar macrophages, significantly reduce the degree of lung injury and improve the survival rate in ALI mice.

6.Course of Revision of the Diagnosis and Treatment Protocol for COVID-19 and the Forensic Contribution.

From January 15 to March 3, 2020, seven editions of the guidelines for the diagnosis and treatment of COVID-19 have been issued successively by the National Health Commission of the People's Republic of China, and the guidelines' name was changed from Guidelines for Diagnosis and Treatment of Novel Coronavirus Pneumonia to Diagnosis and Treatment for COVID-19. It optimized and perfected the etiology, clinical manifestations and types, diagnostic procedures and specific treatment measures of the disease, so that the clinical management of the cases was more scientific. In the revision process of guidelines for diagnosis and treatment, forensic medicine experts have also made some positive suggestions on clinical diagnosis and treatment. Especially regarding the pathological changes of COVID-19, they have repeatedly called for rapid autopsy at different levels. With the support, understanding and cooperation of all parties, pathological examination of more than ten cases of the remains were carried out, which made an important contribution to the understanding of the clinical characteristics and pathological characteristics of the disease and the improvement of treatment plans.

7.Pyoderma gangrenosum, acne, pyogenic arthritis, and suppurative hidradenitis syndrome: a case study

A 22-year-old male patient visited the Department of Dermatology of the First Affiliated I Iospital of Zbengzhou University on October 31,2016 due to dark red papules,nodules,pustules and cysts on the face,neck,back and in the axillary and inguinal regions for 6 years,and multiple dark purple plaques and ulcers on bilateral lower limbs for 1 year.Six years ago,the patient was diagnosed with acne in other hospital,and no treatment was given.One year ago,multiple purple plaques occurred on the bilateral lower limbs,which then ruptured and progressed into ulcers with diameters of 1-12 cm.On May 9,2002,the patient visited the Department of Pediatric Medicine of the First Affiliated Hospital of Zhengzhou University due to the left knee joint swelling and pain for half a year.Laboratory examination showed negative rheumatoid factor,and smear examination of the left knee joint effusions revealed that there were neutrophils and a small amount of lymphocytes and monocytes in the joint effusions,and no abnormal cells were observed.Then,the patient was diagnosed with pyogenic arthritis of the left knee.Physical examination at admission showed poor general condition,walking difficulty,slightly increased blood pressure of 142/92 mmHg (1 mmHg =0.133 kPa),multiple purple plaques on the bilateral lower limbs with central ulcer formation.Histopathological examination of ulcer margin on the lower limbs showed ulceration,intercellular edema and infiltrating neutrophils in the epidermis,and edema,focal erythrocyte extravasation,diffuse infiltration of neutrophils,lymphocytes and histiocytes in the superficial and middle dermis.Clinical manifestations and pathological features confirmed a diagnosis of pyoderma gangrenosum.There were extensive inflammatory papules,pustules,abscesses and cysts on the face,neck,waist and back,and a small amount of dark red nodules on the axillary and inguinal regions,which were consistent with cystic acne and hidradenitis suppurativa.As PSTPIP1 gene sequencing showed,no mutations were found in exon fragments,while compound heterozygous mutations c.36 + 68 G > A,c.137 + 47 G > C and c.562 + 114 C > G her were found in intron fragments.Among 100 healthy controls,45 carried the same mutations.So,these mutation sites were considered to be polymorphic sites,and the pathogenicity of these mutations was still unclear.Finally,the patient was diagnosed with PAPASH syndrome.The patient was treated with methylprednisolone,cefminox,isotretinoin and thalidomide,and the lesions were markedly improved after 2 weeks.Now the patient was still followed up.
8.Effect of Polysaccharides of Radix scrophulariae on Immune Functions in Mice under Normal Physiological and Hypoimmunical State

OBJECTIVE:To study the effect of polysaccharides of Radix scrophulariae on immune function in mice under nor-mal physiological and hypoimmunical state. METHODS:80 mice were randomly divided into normal group,model group,treat-ment high-dose,medium-dose,low-dose groups and physiological high-dose,medium-dose,low-dose groups,10 in each group. Mice in normal group and model group were received distilled water (10 mL/kg) intragastrically,treatment high-dose,medi-um-dose,low-dose groups and physiological high-dose,medium-dose,low-dose groups were administrated 0.16,0.08,0.04 g/kg drug,ig,once a day,for 7 d. From the third day,mice in model group and treatment high-dose,medium-dose,low-dose groups were respectively received cyclophosphamide(100 mg/kg),ip,for 4 d to induce hypoimmunical model. Carbon clearance test was adopted to determine the carbon clearance indexes and organs(chest,spleen)indexes of mice. Another 80 mice were grouped with the same administration,serum half hemolytic value (HC50) was determined after compressed Mianyang red blood cell sensitiza-tion. Then another 80 mice were grouped with the same administration,1% dinitrofluorobenzene method was used to induce de-layed hypersensitivity in mice;its ear swelling was determined,as well as IL-2,IL-4,immunoglobulin G(IgG),immunoglobulin M(IgM),γ-interferon(IFN-γ)contents in serum,and the proliferation in vitro of splenic lymphocytes were detected. RESULTS:High-dose polysaccharides can promote the increasing of immune organ and carbon clearance indexes(P<0.05);enhance the inten-sity of delayed type hypersensitivity (HC50 increasing)(P<0.01) and improve the decreasing of serum hemolysin in model mice (P<0.01);promote the splenic lymphocytes proliferation and increase IL-2,IL-4,IgG,IgM,IFN-γ contents in serum(P<0.05 or P<0.01). CONCLUSIONS:Polysaccharides can enhance the immune function in mice under normal physiological and hypoim-munical state induced by cyclophosphamide.
9.The value of interleukin 35 in assessing the prognosis of severe hepatitis

Objective To explore the significance of interleukin -35 (IL -35)in serum of patients with severe hepatitis.Methods Collected 39 cases of chronic hepatitis B(CHB)patients and severe hepatitis,and col-lected 19 cases of healthy control's(HC)peripheral blood at the same time.ELISA method was used to detect IL -35 levels.Results IL -35 in CHB and severe hepatitis group [(483.5 ±280.7)ng/mL and (277.9 ±248.7)ng/mL] were higher than HC group (50.5 ±47.8)ng/mL(t =2.089,3.303,P =0.044,0.002).In severe hepatitis group, the IL -35 levels in survivors group (305.3 ±301.2)ng/mL was higher than death group (78.7 ±33.2)ng/mL (P =0.012).IL -35 was positively correlated with ALT and AST,the correlation coefficient were 0.649 and 0.599. Conclusion IL -35 is involved in the pathogenesis of hepatitis B process,the low serum IL -35 levels in severe hepatitis patients may herald a bad prognosis.
10.Special operative techniques for intertrochanteric fractures without lesser trochanter fracture

Objective To explore the operative techniques for AO/OTA type 31-A3.1 and 31-A3.2 intertrochanteric fractures fixed with intrameduallary nail.Methods Seventy-four cases of unstable intertrochanteric fracture (AO/OTA type 31-A3.1 and 31-A3.2) from January 2007 to December 2010 were analyzed retrospectively.There were 33 males and 41 females,aged from 36-87 years (mean,71 years).The right hip was involved in 34 patients and the left in 40 patients.The mechanism of injury was traffic injuries in 27 cases,fall damage in 43 cases,injury by falling in 3 case and crush injury in 6 cases.Percutaneous joystick technique,Homann retractor technique,clamp technique and mini-incision was applied to aid reduction during the surgery.All cases were fixed with proximal intrameduallary nail.Clinical and radiographic outcomes were recorded.The postoperative hip function was evaluated using Harris score.Results With the aid of C-arm,closed reduction was performed in 73 cases.Only one patient experienced open reduction because of failure of closed reduction.Sixty-five cases were followed up for 8 to 23 months (mean,14.5 months).Bone union was observed in all cases with the average time of 7.4 months (range,8-23).The operation time,blood loss,the frequency of X-ray exposure was 75±4 minutes,135±5 ml and 24±3 times,respectively.At final follow-up,29 cases were classified as excellent,31 as good,and 5 as fair.The mean Harris hip score was 91.4 points (range,87-95).The overall rate of excellent or good result was 92.3%.Conclusion It is difficult to perform close reduction for unstable intertrochanteric hip fracture (AO/OTA type 31-A3.1and 31-A3.2),especially in relatively young patients.Special operative skills are necessary in some cases.
