1.Analysis of the direct economic burden of measles cases and its influencing factors in Shanghai from 2017 to 2019.

		                        			Objective: To analyze the direct economic burden caused by measles cases in Shanghai from 2017 to 2019 and its influencing factors. Methods: A total of 161 laboratory-confirmed measles cases reported from January 1, 2017, to December 31, 2019, in Shanghai were included in the study through the "Measles Surveillance Information Reporting and Management System" of the "China Disease Surveillance Information Reporting and Management System". Through telephone follow-up and consulting hospital data, the basic information of population, medical treatment situation, medical treatment costs and other information were collected, and the direct economic burden of cases was calculated, including registration fees, examination fees, hospitalization fees, medical fees and other disease treatment expenses, as well as transportation and other expenses of cases. The multiple linear regression model was used to analyze the main influencing factors of the direct economic burden. Results: The age of 161 measles cases M (Q1, Q3) was 28.21 (13.33, 37.00) years. Male cases (56.52%) were more than female cases (43.48%). The largest number of cases was≥18 years old (70.81%). The total direct economic burden of 161 measles cases was 540 851.14 yuan, and the per capita direct economic burden was 3 359.32 yuan. The direct economic burden M (Q1, Q3) was 873.00 (245.01, 4 014.79) yuan per person. The results of multiple linear regression model analysis showed that compared with other and unknown occupations, central areas and non-hospitalized cases, the direct economic burden of measles cases was higher in scattered children, childcare children, students, and cadre staff in the occupational distribution, suburban areas and hospitalized, with the coefficient of β (95%CI) values of 0.388 (0.150-0.627), 0.297 (0.025-0.569), 0.327 (0.148-0.506) and 1.031 (0.853-1.209), respectively (all P values<0.05). Conclusion: The direct economic burden of some measles cases in Shanghai is relatively high. 2.Construction of a Prognostic Model of Multiple Myeloma Based on Metabolism-Related Genes.

		                        			OBJECTIVE:
		                        			To screen the prognostic biomarkers of metabolic genes in patients with multiple myeloma (MM), and construct a prognostic model of metabolic genes.
		                        		
		                        			METHODS:
		                        			The histological database related to MM patients was searched. Data from MM patients and healthy controls with complete clinical information were selected for analysis.The second generation sequencing data and clinical information of bone marrow tissue of MM patients and healthy controls were collected from human protein atlas (HPA) and multiple myeloma research foundation (MMRF) databases. The gene set of metabolism-related pathways was extracted from Molecular Signatures Database (MSigDB) by Perl language. The biomarkers related to MM metabolism were screened by difference analysis, univariate Cox risk regression analysis and LASSO regression analysis, and the risk prognostic model and Nomogram were constructed. Risk curve and survival curve were used to verify the grouping effect of the model. Gene set enrichment analysis (GSEA) was used to study the difference of biological pathway enrichment between high risk group and low risk group. Multivariate Cox risk regression analysis was used to verify the independent prognostic ability of risk score.
		                        		
		                        			RESULTS:
		                        			A total of 8 mRNAs which were significantly related to the survival and prognosis of MM patients were obtained (P<0.01). As molecular markers, MM patients could be divided into high-risk group and low-risk group. Survival curve and risk curve showed that the overall survival time of patients in the low-risk group was significantly better than that in the high risk group (P<0.001). GSEA results showed that signal pathways related to basic metabolism, cell differentiation and cell cycle were significantly enriched in the high-risk group, while ribosome and N polysaccharide biosynthesis signaling pathway were more enriched in the low-risk group. Multivariate Cox regression analysis showed that the risk score composed of the eight metabolism-related genes could be used as an independent risk factor for the prognosis of MM patients, and receiver operating characteristic curve (ROC) showed that the molecular signatures of metabolism-related genes had the best predictive effect.
		                        		
		                        			CONCLUSION
		                        			Metabolism-related pathways play an important role in the pathogenesis and prognosis of patients with MM. The clinical significance of the risk assessment model for patients with MM constructed based on eight metabolism-related core genes needs to be confirmed by further clinical studies.

3.Low XIST expression in Sertoli cells of Klinefelter syndrome patients causes high susceptibility of these cells to an extra X chromosome.

		                        			Klinefelter syndrome (KS) is the most common genetic cause of human male infertility. However, the effect of the extra X chromosome on different testicular cell types remains poorly understood. Here, we profiled testicular single-cell transcriptomes from three KS patients and normal karyotype control individuals. Among the different somatic cells, Sertoli cells showed the greatest transcriptome changes in KS patients. Further analysis showed that X-inactive-specific transcript ( XIST ), a key factor that inactivates one X chromosome in female mammals, was widely expressed in each testicular somatic cell type but not in Sertoli cells. The loss of XIST in Sertoli cells leads to an increased level of X chromosome genes, and further disrupts their transcription pattern and cellular function. This phenomenon was not detected in other somatic cells such as Leydig cells and vascular endothelial cells. These results proposed a new mechanism to explain why testicular atrophy in KS patients is heterogeneous with loss of seminiferous tubules but interstitial hyperplasia. Our study provides a theoretical basis for subsequent research and related treatment of KS by identifying Sertoli cell-specific X chromosome inactivation failure.

		                        			Objective To investigate the electrophysiological characteristics and curative effect of radiofrequency ablation(RFA)of idiopathic ventricular arrhythmias(VAs)originating from the right bundle branch of the moderator band.Methods The patients(n=10)with idiopathic VAs originating from the right bundle branch of the moderator band verified by electrophysiological examination and RFA were chosen from Department of Cardiology in Beijing Anzhen Hospital affiliated to Capital Medical University from April 2019 to July 2022.Excited sequence mapping and pacing mapping were performed respectively on the moderator band.The characteristics of surface ECG,electrophysiological characteristics and potential characteristics at successful ablation targets were analyzed,and curative effect of RFA was observed and analyzed.Results Idiopathic VAs originated from the moderator band,and surface ECG showed mostly graph of left bundle branch block(LBBB)with left superior axis deviation and a narrower QRS complex.All patients showed an rS wave in leads V,and V2,and no notching on the S wave downstroke.The R-wave transition lead was at leads V4-V6 during VAs,which was later than that during sinus rhythm in all patients.All patients had an R wave in lead V6.A leading RBB potential at the target during VAs was observed for all patients.Ablation of the leading RBB potential effectively eliminated the arrhythmia.6 patients showed RBB block after ablation and 2 of them were recovered during follow-up.Conclusions Electrocardiographic of MB-RBB VAs is characterized by a typical left bundle branch block pattern with a relatively narrow QRS complex,and left superior axis deviation.The R-wave transition lead was later than that during sinus rhythm.Ablation of the leading RBB potential are effective in eliminating VAs.

		                        			Advances in science and technology promote the rapid development of toxicological detection technologies. However, there is still a lack of decision-making tools for toxicological risk assessment, such as the lack of transparent schemes to evaluate current toxicological research and practice and the lag of toxicological testing tools to evaluate toxicity, resulting in difficulties in toxicity verification and hindering the transformation of toxicological research paradigm. Some scholars have proposed to integrate the concept of evidence-based medicine with the toxicological practice to improve the technical methods of toxicological research concept and risk assessment decision-making. With the promotion of relevant scholars and academic organizations, the concept and connotation of evidence-based toxicology have gradually become clear and a framework for research and practice has been initially formed. Although there are still many challenges, it also provides a new idea for the toxicity risk assessment and safe medication decision-making of traditional Chinese medicine(TCM). The era of digital intelligence has brought new opportunities and broad space for the development of TCM evidence-based toxicology. 6.A multicenter study of brain T 2WI lesions radiomics machine learning models distinguishing multiple sclerosis and neuromyelitis optica spectrum disorder

		                        			Objective:To investigate the efficacy of a machine learning model based on radiomics of brain lesions on T 2WI in differentiating multiple sclerosis (MS) from neuromyelitis optica spectrum disorders (NMOSD). Methods:Totally 223 MS and NMOSD patients who were treated from January 2009 to September 2018 in Beijing Tiantan Hospital Affiliated to Capital Medical University, Donghu Branch of the First Affiliated Hospital of Nanchang University, Tianjin Medical University General Hospital, and Xuanwu Hospital of Capital Medical University were analyzed retrospectively, and according to the proportion of 7∶3, 223 patients were completely randomly divided into training set (156 cases) and test set (67 cases). A total of 74 patients with MS and NMOSD who were treated in Huashan Hospital Affiliated to Fudan University and China-Japan Friendship Hospital of Jilin University from January 2009 to September 2018 and in Xianghu Branch of the First Affiliated Hospital of Nanchang University from March 2020 to September 2021 were collected as an independent external validation set. All patients underwent brain cross-sectional MR T 2WI, radiomics features were extracted from T 2WI, and features were selected by max-relevance and min-redundancy and least absolute shrinkage and selection operator algorithms. Then various machine learning classifier models (logistic regression, decision tree, AdaBoost, random forest or support vector machine) were constructed to differentiate MS from NMOSD. The area under curve (AUC) of receiver operating characteristics was used to evaluate the performance of each classifier model in the training set, test set and external validation set. Results:Based on multi-center T 2WI, a total of 11 radiomics features related to the discrimination between MS and NMOSD were extracted and classifier models were constructed. Among them, the random forest model had the best efficiency in distinguishing MS from NMOSD, and its AUC values for distinguishing MS from NMOSD in the training set, test set and external validation set were 1.000, 0.944 and 0.902, with specificity of 100%, 76.9% and 86.0%, and sensitivity of 100%, 92.1% and 79.7%, respectively. Conclusion:The random forest model based on the radiomic features of T 2WI of brain lesions can effectively distinguish MS from NMOSD.
		                        		
		                        		
		                        		
		                        	
7.Otologic disorders and management strategies in Turner syndrome.

		                        			Objective: To analyze the incidence and risk factors of otologic disorders in patients with Turner syndrome (TS), so as to provide management strategies for ear health. Methods: This study is a prospective study based on questionnaires and a cross-sectional study. The TS patients who visited our hospital from 2010 January to 2021 March were included (A total of 71 patients with TS were included in this study. the age of TS diagnosed was 3- to 11-year-old, age of visiting ENT department was 4- to 27-year-old) and the incidence of otologic diseases in different age groups was investigated by questionnaires. The cross-sectional study included ear morphology and auditory function assessment, and further analysis of the risk factors that related to ear disease. Prism was used for data analysis. Results: The investigation found that the incidence of acute otitis media in patients aged 3-6 and 7-12 years was higher than that of patients over 12 years old, which was 33.8%(24/71), 42.9%(30/70)and 23.5%(8/34), respectively; 21.1% (15/71) of patients were recurrent acute otitis media in patients aged 3-6 years, and about 46.6% (7/15)of them persisted beyond 6-year. The prevalence of otitis media with effusion in the three groups was 32.4%(23/71), 34.3%(24/70)and 38.2%(13/34), respectively; the recurrence rate of tympanocentesis was 100%(7/7), 42.9%(3/7)and 50.0%(1/2), which was significantly higher than that of grommet insertion. For age groups of 3-6 and 7-12 years, the prevalence of acute otitis media and secretory otitis media was lower in the X chromosome structure abnormal patients; while for patients older than 12 years, otitis media with effusion was the highest prevalence in Y-chromosome-containing karyotypes. In addition, the prevalence of acute otitis media and otitis media with effusion in patients with other system diseases were increased significantly. A cross-sectional study found that 7.0% (5/71)of the lower auricular, 4.2% (3/71)of the external auditory canal narrow, and 38.0% (27/71)of the tympanic membrane abnormality. 35.2%(25/71) had abnormal hearing, including 17 cases of conductive deafness, 6 cases of sensorineural hearing loss, and 2 cases of mixed deafness. The rest of the patients had normal hearing, but 6 of them had abnormalities in otoacoustic emission. Eustachian tube function assessment found that the eustachian tube dysfunction accounted for 38%(27/71). Hearing loss and abnormal Eustachian tube function were not significantly related to karyotype(Chi-square 2.83 and 2.84,P value 0.418 and 0.417), but significantly related to other system diseases(Chi-square 13.43 and 7.53,P value<0.001). Conclusions: The incidence of TS-related otitis media and auditory dysfunction is significantly higher than that of the general population. It not only occurs in preschool girls, but also persists or develops after school age. Accompanied by other system diseases are risk factors for ear diseases. Clinicians should raise their awareness of TS-related ear diseases and incorporate ear health monitoring into routine diagnosis and treatment.

		                        			BACKGROUND:
		                        			Perioperative treatment has become an increasingly important aspect of the management of patients with non-small cell lung cancer (NSCLC). Small-scale clinical studies performed in recent years have shown improvements in the major pathological remission rate after neoadjuvant therapy, suggesting that it will soon become an important part of NSCLC treatment. Nevertheless, neoadjuvant immunotherapy may be accompanied by serious adverse reactions that lead to delay or cancelation of surgery, additional illness, and even death, and have therefore attracted much attention. The purpose of the clinical recommendations is to form a diagnosis and treatment plan suitable for the current domestic medical situation for the immune-related adverse event (irAE).
		                        		
		                        			METHODS:
		                        			This recommendation is composed of experts in thoracic surgery, oncologists, thoracic medicine and irAE related departments (gastroenterology, respirology, cardiology, infectious medicine, hematology, endocrinology, rheumatology, neurology, dermatology, emergency section) to jointly complete the formulation. Experts make full reference to the irAE guidelines, large-scale clinical research data published by thoracic surgery, and the clinical experience of domestic doctors and publicly published cases, and repeated discussions in multiple disciplines to form this recommendation for perioperative irAE.
		                        		
		                        			RESULTS:
		                        			This clinical recommendation covers the whole process of prevention, evaluation, examination, treatment and monitoring related to irAE, so as to guide the clinical work comprehensively and effectively.
		                        		
		                        			CONCLUSIONS
		                        			Perioperative irAE management is an important part of immune perioperative treatment of lung cancer. With the continuous development of immune perioperative treatment, more research is needed in the future to optimize the diagnosis and treatment of perioperative irAE.
		                        		
		                        		
		                        		
		                        	
9.Investigation and Suggestions on Factors Affecting Quality of Angelicae Dahuricae Radix (Baizhi) in Some Post-harvest Processes

		                        			Objective:To investigate the factors affecting the quality of post-harvest Angelicae Dahuricae Radix (Baizhi) and to provide the corresponding suggestions according to these factors. Method:A series of factors affecting the quality of Baizhi were analyzed comprehensively by visiting several origins, medicinal material markets, pharmacies, supermarkets and storehouses combined with plenty of literature reports. Result:The rot of Baizhi was the most common phenomenon after harvest, and mildew, discoloration and moth could occur during the processes of processing, storage and packaging. So the quality of Baizhi should be controlled strictly through the whole process. The roots of Baizhi should be harvested on sunny days to avoid mining damage and dried as soon as possible to prevent rotting. The drying methods included both traditional lime burial and modern ways such as sulfur fumigation, saline dehydration, sun drying method, and oven drying method. However, the water content should be strictly controlled. Furthermore, the storehouses should be disinfected firstly. The temperature and humidity of the storehouses should be controlled strictly. Some suitable ways could be adopted to prevent moth and discoloration during storage, which included airtight fumigation of aluminum phosphide, cold storage, air conditioning maintenance, antagonism storage with other Chinese medicinal materials, dry ice storage at room temperature, etc. Large quantities of Baizhi were generally packaged in woven bags. Otherwise, kraft paper bags, polyvinyl chloride plastic bags, glass bottles, aluminum foil composite film bags, and other packaging materials had been adopted for the retail. However, the packaging specification should be selected according to the situation. Conclusion:The quality of Baizhi can be directly affected by harvesting, drying, processing, storage and packaging. 10.Analgesic effect of neurotropin on bone cancer pain and its mechanism in rats

		                        			Objective:To investigate the analgesic effect of neurotropin (NTP) on bone cancer pain (BCP) and its preliminary mechanisms in rats.Methods:(1) According to the random number table method, 72 Sprague-Dawley (SD) female rats were randomly and equally divided into 6 groups: normal control group, sham-operated group, BCP model group, BCP+low-dose NTP group, BCP+medium-dose NTP group, and BCP+high-dose NTP group ( n=12). The SHZ-88 breast cancer cells were inoculated into the tibias of rats in the latter 4 groups to establish BCP models. After 15-21 d of modeling, the rats of the latter 3 groups were intraperitoneally administered with 1.2, 2.4 and 3.6 unit NTP, respectively, once per d for 7 consecutive d. The mechanical withdrawal threshold (MWT) changes were measured in each group before BCP and 5, 7, 10, 14, 17, and 21 d after BCP. The number of 5-hydroxytryptamine 7 (5-HT7) positive cells and 5-HT7 protein expression in the ventrolateral periaqueductal gray (vlPAG) were detected by immunohistochemistry and Western blotting, respectively. (2) Twenty-four rats 21 d after BCP modeling were randomly divided into BCP group, BCP+high-dose NTP group (BCP rats with intraperitoneal injection of 3.6 unit NTP), BCP+NTP+SB-269970 group (BCP rats with pretreatment of specific 5-HT7 receptor antagonist SB-269970 in the vlPAG for 30 min, and then, with intraperitoneal injection of 3.6 unit NTP, n=8). The MWT changes were measured in each group before NTP and 15, 30, 45, 60 min after NTP. Results:(1) Seventeen and 21 d after modeling, the MWT values of the modeled hind limb of rats in BCP+low-dose NTP group, BCP+medium-dose NTP group, and BCP+high-dose NTP group were significantly higher than those in BCP group; those in the BCP+high-dose NTP group were significantly higher than those in BCP+low-dose NTP group and BCP+medium-dose NTP group; those in BCP+medium-dose NTP group were statistically higher than those in the BCP+low-dose NTP group ( P<0.05). Twenty-one d after modeling, the number of 5-HT7 receptor positive cells and protein expression in the vlPAG of rats in the BCP+low-dose NTP group, BCP+medium-dose NTP group, and BCP+high-dose NTP group were significantly larger/higher than those in BCP group; those in the BCP+high-dose NTP group were significantly larger/higher than those in BCP+low-dose NTP group and BCP+medium-dose NTP group; those in BCP+medium-dose NTP group were statistically larger/higher than those in the BCP+low-dose NTP group ( P<0.05). (2) At 15, 30, 45 and 60 min after injection of NTP, the MWT values of the modeled hind limb of rats in the BCP+high-dose NTP group and BCP+NTP+SB-269970 group increased gradually, enjoying statistical significance as compared with those in the BCP group at the same time point ( P<0.05); however, the MWT values of the BCP+NTP+SB-269970 group were significantly lower as compared with those in the BCP+high-dose NTP group at the same time point ( P<0.05). Conclusion:The activation of 5-HT7 receptor in the vlPAG is involved in the analgesic effect of NTP on BCP in rats.
