1.Clinical research of ultrasound-guided PECS-Ⅱ blocks in surgery of patients with benign tumor of breast

Objective:To explore the application effect of ultrasound-guided pectoralis Ⅱ(PECS-Ⅱ)blocks anesthesia in benign tumor resection of breast.Methods:A total of 60 female patients who underwent benign tumor resection of breast were selected from Maternity and Child Care Center of Qinhuangdao from January 2021 to December 2021,and they were divided into the thoracic nerve block group(nerve block group)and local infiltration anesthesia group(local anesthesia group)according to the random number table method.The nerve block group used ultrasound-guided PECS-Ⅱ anesthesia,and the local anesthesia group used local infiltration anesthesia for tumor.The hemodynamic indicators,postoperative visual analogue score,first intervention time of postoperative analgesic drugs,and the use of sufentanil within 24 hours after surgery were compared and analyzed between the two groups of patients.Results:The average operation time of the nerve block group was(127.32±34.56)min,and the difference of that between the nerve block group and local anesthesia group(128.11±33.84)min was no statistically significant(P>0.05).The differences of the heart rate(HR)and mean arterial pressure(MAP)before anesthesia(T0)between two groups of patients were not statistically significant(P>0.05).The HR and MAP levels at the time of skin incision(T1),at the 0.5 h after the surgery was conducted(T2)and the time of completing surgery(T3)in nerve block group were significantly lower than those of the local anesthesia group,respectively.The differences were statistically significant(tHR=5.709,5.836,5.662,tMAP=3.501,3.223,3.128,P<0.05),respectively.Compared with the local anesthesia group,the pain level at the same time point in the nerve block group was significantly reduced,and the difference was statistically significant(t=4.501,6.575,8.197,8.262,P<0.05),respectively.The intervention time of analgesic medication at the first time of the nerve block group was significantly later than that of the local anesthesia group,and the consumption of sufentanil within 24 hours after surgery of the nerve block group was also significantly reduced,and the differences of them were statistically significant(t=13.741,9.482,P<0.05),respectively.Conclusion:Ultrasound-guided PECS-Ⅱ can effectively relieve postoperatively early pain in patients with benign tumors of breast,and delay the intervention time of postoperative analgesic medication,and reduce the intake amount of opioid drugs.
2.Characteristics and clinical outcomes of checkpoint inhibitor-associated pneumonia in cancer patients:a retrospective analysis of 1 084 cases

Objective To investigate the incidence,risk factors,treatment outcomes,and other clinical outcomes of checkpoint inhibitor-related pneumonitis(CIP)in cancer patients treated with PD-1/PD-L1 inhibitors.Methods Clinical data of 1 084 cancer patients who received treatment with PD-1/PD-L1 inhibitors from 2019 to 2021 in Department of Oncology of the Second Affiliated Hospital of Army Medical University were collected and then retrospectively analyzed.The patients were divided into a non-CIP group(998 cases)and a CIP group(86 cases),and those from the latter group were further divided into G1~G4 grades,of which G1~G2 grades were classified as low-grade CIP subgroup(69 cases)and G3~G4 grades as high-grade CIP subgroup(17 cases).Chi-square test,binary logistic regression and Kaplan-Meier survival analysis were used to summarize the incidence,risk factors,treatment outcome of CIP and its relationship with immune efficacy and progression-free survival(PFS).Results The incidence of CIP was 7.93%(86/1 084)in the cohort,the rate of high-grade CIP was 1.57%(17/1 084),and the rate of mortality was 0.09%(1/1 084).Binary logistic regression analysis showed that history of chronic obstructive pulmonary disease(COPD,OR=3.018,P=0.036)and history of chest radiotherapy(OR=2.605,P<0.001)were independent risk factors for CIP.The patients with an age of ≥ 65 years had an obviously higher incidence of high-grade CIP than those with younger age(P=0.021).There were no statistical differences in objective remission rate(ORR),disease control rate(DCR)and PFS between the non-CIP group and the CIP group.Out of the 86 CIP patients,50(58.14%)stopped immunotherapy,the other 36(41.86%)continued or restarted immunotherapy.One(1.16%)patient died of respiratory failure,and the remaining patients were relieved after drug withdrawal,hormone therapy,immunosuppressive therapy,etc.Conclusion The cancer patients with history of COPD or chest radiotherapy have a higher incidence of CIP after PD-1/PD-L1 treatment.Patients with advanced age are prone to high-grade CIP.Most CIP patients can achieve good outcomes after active treatment,and some with low-grade CIP can restart immunotherapy.
3.Imaging features and differential diagnosis of arrested pneumatisation of the sphenoid sinus

OBJECTIVE To Analysis of CT and MRI imaging features of arrested pneumatisation of the sphenoid sinus and differentiate from osteogenic and chondrogenic tumours of the region.METHODS Retrospective analysis of CT and MRI findings was performed of 13 patients with sphenoid sinus arrested pneumatisation and 20 patients with osteogenic and chondrogenic tumours and tumor like lesions in the same period.Evaluation indicators included location,size,density,presence of expansive changes,calcification,cortical bone changes,MRI signal characteristics,signal changes after fat suppression,degree of enhancement,and statistical analysis was conducted.RESULTS Finally,the location includes the sphenoid body(4 cases),pterygoid process(3 cases)and multiple involved areas(6 cases).The arrested pneumatisation area is mainly characterized by fat density or mixed density of adipose and soft tissue.The longest diameter of the arrested pneumatisation zone is 0.8-4.1 cm.There is internal calcification(7 cases)and without bone expansive changes(13 cases).Cases with intact bone cortex(13 cases);On MRI T1WI,high signal(11 cases),equal signal(2 cases),on T2WI,high signal(10 cases),equal signal(3 cases).Decreased signal after fat suppression(13 cases),no significant enhancement(10 cases),and slight enhancement(3 cases).CONCLUSION The arrested pneumatisation of sphenoid sinus is a rare anatomical variation characterized by a mixed density of fat or soft tissue,intact bone cortex,without bone expansive changes,decreased signal of MRI fat suppression,and no obvious enhancement,which will help to differentiate from osteogenic and chondrogenic tumours of the region.
4.Clinical and imaging features of the SMARCB1-de?cient sinonasal carcinoma

Objective:To assess the clinical and imaging features of SMARCB1-deficient sinonasal carcinoma.Methods:Form January 2016 to November 2021, the clinical data and pretreatment imaging findings of 16 cases with pathologically proven SMARCB1-de?cient sinonasal carcinomas were analyzed retrospectively in Beijing Tongren Hospital, Capital Medical University. Immunohistochemistry for SMARCB1 showed loss of the protein in the tumor nuclie. Clinical and imaging features, including tumor location, TNM stage, size, density of CT, bone change, MRI signal intensity, enhancement pattern, type of time-intensity curve (TIC) of dynamic contrast enhanced MRI (DCE-MRI), apparent diffusion coefficient (ADC) value and diffusion weighted imaging (DWI) were evaluated. For 14 cases, correlation of the ADC value and Ki-67 index was subsequently evaluated with Pearson correlation analysis.Results:For the 16 cases SMARCB1-deficient sinonasal carcinomas, clinical stage of T4 was 12 cases and T3 was 4 cases. The location included ethmoid sinus ( n=4), nasal cavity only ( n=1), both nasal cavity and ethmoid ( n=8), ethmoid and maxillary sinus ( n=1), ethmoid and frontal sinus ( n=1), ethmoid and sphenoid sinus ( n=1). The tumor size was (4.5±1.2) cm. Iso-attenuated of CT images was showed in 13 cases and heterogeneous with necrosis was showed in 3 cases. Focal bone erosion was found in 13 cases and extensive bone destruction was found in 3 cases. Compared with adjacent muscles, T 1WI of all 16 cases showed isointense, with focal hypointense in 3 cases. On T 2WI, the tumor was graded as isointense in 9 cases, hyperintense in 7 cases, with lower inner septal in 6 cases. Enhancement was graded as mild in 11 cases, moderate in 5 cases.MRI Enhancement images showed mild enhancement in 11 cases, moderate enhancement in 5 cases, heterogeneous enhancement in 6 cases, and homogeneous enhancement in 10 cases. For DCE-MRI of 14 cases, there were 10 cases of Ⅲ type and 4 cases of Ⅱ type of the TIC. The ADC value of 14 cases was (1.02±0.27)×10 -3 mm 2/s. The Ki-67 index was 48%±21%. No correlation was observed between Ki-67 index and ADC value ( r=-0.38, P=0.183). Conclusions:SMARCB1-deficient carcinomas are mostly centered in the nasal and ethmoid region of anatomic distribution. Tendency to be infiltrative the adjacent bone structure with invasive bone reaction, mild to moderate heterogeneous enhancement, T 2WI with lower inner septal, and Ⅲ types of TIC are certain suggestive imaging features of the entity.
5.Effect of mentalization-based family therapy on depressive symptoms and non-suicidal self-injury behavior in adolescents with major depressive disorder

BackgroundThe major depressive disorder has high prevalence among adolescents, and non-suicidal self-injury (NSSI) behaviors frequently occur among patients, therefore, major depressive disorder in adolescents has become the researching focus. ObjectiveTo explore the effect of mentalization-based family therapy (MBFT) on depressive symptoms and NSSI behavior in adolescents with major depressive disorder, and to provide references for the rehabilitation of major depressive disorder in adolescents. MethodsA total of 90 adolescent patients with major depression disorder who met the diagnostic criteria of International Classification of Diseases, 10th edition (ICD-10) for depressive disorders and attended Wuhan Mental Health Center from January to December 2022 were selected, and were assigned into study group (n=44) and control group (n=46) using random number table method. All participants received routine intervention, based on this, study group added a 60-minute MBFT intervention once a week for 8 weeks. Before the intervention and at the end of 1st, 2nd,4th and 8th week,the two groups were assessed using Hamilton Depression Scale-24 item (HAMD-24), General Self-Efficacy Scale (GSES), Pittsburgh Sleep Quality Index (PSQI) and Ottawa Self-injury Inventory (OSI). ResultsThe repeated measures analysis of variance reported a statistical main effect of time, main effect of group, and interaction effect between time and group at the baseline and the end of 1st, 2nd, 4th and 8th week of treatment in HAMD-24 score (F=69.621, 15.428, 29.623, P˂0.05), OSI score (F=176.642, 37.682, 21.873, P˂0.05), GSES score (F=215.236, 57.421, 27.857, P˂0.05) and PSQI score (F=268.541, 61.863, 33.867, P˂0.05). Individual effect analysis discovered a statistical difference between study group and control group at the end of 2nd, 4th and 8th week of treatment in HAMD-24 score (t=5.567, 8.645, 6.233, P˂0.01), OSI score (t=3.675, 11.817, 9.632, P˂0.01), GSES score (t=23.462, 31.709, 12.750, P˂0.01) and PSQI score (t=9.664, 22.457, 9.333, P˂0.01). ConclusionMBFT may improve depressive symptoms, NSSI behavior, sleep quality and self-efficacy in adolescents with major depressive disorder. 6.Inhibiting collagen I production and tumor cell colonization in the lung via miR-29a-3p loading of exosome-/liposome-based nanovesicles.

The lung is one of the most common sites for cancer metastasis. Collagens in the lung provide a permissive microenvironment that supports the colonization and outgrowth of disseminated tumor cells. Therefore, down-regulating the production of collagens may contribute to the inhibition of lung metastasis. It has been suggested that miR-29 exhibits effective anti-fibrotic activity by negatively regulating the expression of collagens. Indeed, our clinical lung tumor data shows that miR-29a-3p expression negatively correlates with collagen I expression in lung tumors and positively correlates with patients' outcomes. However, suitable carriers need to be selected to deliver this therapeutic miRNA to the lungs. In this study, we found that the chemotherapy drug cisplatin facilitated miR-29a-3p accumulation in the exosomes of lung tumor cells, and this type of exosomes exhibited a specific lung-targeting effect and promising collagen down-regulation. To scale up the preparation and simplify the delivery system, we designed a lung-targeting liposomal nanovesicle (by adjusting the molar ratio of DOTAP/cholesterol-miRNAs to 4:1) to carry miR-29a-3p and mimic the exosomes. This liposomal nanovesicle delivery system significantly down-regulated collagen I secretion by lung fibroblasts in vivo, thus alleviating the establishment of a pro-metastatic environment for circulating lung tumor cells.
7.Imaging and clinical features of NUT gene-related carcinoma in the nasal cavity and sinus

Objective:To assess the clinical and imaging features of NUT gene-related sinonasal carcinomas (NUT midline carcinome).Methods:The clinical data and pretreatment imaging findings of 5 cases with pathologically proven NUT sinonasal carcinomas were analyzed retrospectively in Beijing Tongren Hospital, Capital Medical University from January 2016 to December 2020. Of 5 cases, the tumors affected 4 females and 1 male with an age range of 15 to 48 years (median 19 years). Clinical data of all cases were available before surgery with both CT and MR examination. Tumor location, CT density, boney change, calcification, tumor size, T 1WI, T 2WI and diffusion weighted imaging (DWI) signal intensity, appearance diffusion coefficient (ADC), type of time intensity curve (TIC) of dynamic contrast-enhanced (DCE)-MRI were evaluated. Results:All five cases belonged to T4 stage of the clinic TNM system. The locations were nasal cavity ethmoid, sphenoid and maxillary sinus ( n=1), nasal and maxillary sinus ( n=1), nasal cavity and ethmoid sinus ( n=3). Iso-attenuated in 3 cases, heterogeneous with local necrosis in 2 cases, and heterogeneous with calcification in 3 cases on CT imaging. Bone erosion was found in 4 cases, and bone erosion with destruction in 1 case. The tumor sizes ranged from 4.2 to 4.9 cm (median 4.5 cm) on MR axial imaging. On T 1WI, 5 cases showed isointense compared with adjacent temporal muscles, with focal hypointense in 2 cases. On T 2WI, the tumor was graded as isointense in 3 cases, and hyperintense in 2 cases. Heterogeneous enhancement in all cases with mild in 3 cases, and moderate in 2 cases on postcontrast MR imaging. On DCE-MRI of 5 cases, there were 3 cases of type Ⅲ (washout-shaped curves), and 2 cases of type Ⅱ of the TIC (plateau-shaped curves). The range of ADC values was from 0.63×10 -3 to 1.17×10 -3 mm 2/s, and median ADC value was 0.84×10 -3 mm 2/s, of 5 cases with varying degrees of high signal on DWI. The Ki-67 index ranged from 30% to 80% of the tumor. An immunohistochemical study showed that the tumor cells of 5 cases were all positive for both NUT and INI-1 genes. One case was performed with biopsy and followed by chemotherapy, four cases were performed with surgery, combined with the following chemotherapy, and one also was implemented with radiation therapy. The follow-up time was 7-16 months. Five cases were all alive during the follow-up. Conclusions:The NUT midline sinonasal tract carcinoma is a rare, gene-related solid malignant tumor. The tumor is more commonly seen in young patients, mostly centered in the nasal and ethmoid region with invasive growth, more calcification on CT, and heterogeneous enhancement on MRI. These findings are some characteristics of the tumor.
8.Effects of continuous nursing based on self-management theory in patients with chronic obstructive pulmonary disease

Objective:To explore the effect of continuous nursing based on self-management theory in patients with chronic obstructive pulmonary disease.Methods:From July 2020 to June 2021, convenience sampling was used to select 160 patients with chronic obstructive pulmonary disease who were treated in the Department of Respiratory of Xinyang Central Hospital as the research object. According to the order of admission, the patients were divided into the control group (80 cases) and the intervention group (80 cases) . The patients in the control group received routine nursing, and the intervention group was given continuous nursing based on self-management theory on the basis of routine nursing. After six months of intervention, the self-management and quality of life of patients with chronic obstructive pulmonary disease were compared between the two groups.Results:After the intervention, the total score and dimension scores of self-management ability, and the scores of all dimensions of quality of life of the patients in the intervention group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Continuous nursing based on self-management theory can enhance the self-management ability and quality of life of patients with chronic obstructive pulmonary disease.
9. Use of the WeChat platform in combination with family participation in the management of dyslipidemia

Objective:
To explore the implementation of the WeChat platform combined with family participation in the health management of dyslipidemia.
Methods:
A total of 100 patients with dyslipidemia who underwent physical examination in the Health Management Center of Chongqing General Hospital from March to August 2017 were selected. Participants were divided into an intervention and control group by independent registration, with 50 patients in each group. The control group received routine health guidance and telephone follow-ups. The intervention group underwent the health management mode of the WeChat platform combined with family participation beginning in September for six months. Dyslipidemia was managed through joint participation of family members and medical care, and the professional health management and medical team conducted real-time consultation, monitoring, follow-ups, and answered questions. A t-test was performed to compare total cholesterol, triacylglycerol, and LDL cholesterol in the two groups, and a χ2 test was used to compare improvements in living habits in the two groups.

10.Exploration of training mode for postgraduate education in local universities under the Background of "double first-class" construction

First-class education for postgraduate is the foundation for the construction of "double first-class" education. Universities play an important role in postgraduate education. This paper explored the measures for the reform and innovation of the construction of "double first-class" education for postgraduate students in our university, which include perfecting the supervisor's responsibility and authority mechanism, deepening the reform on curriculum system, strengthening the construction of sharing platform, and improving the evaluation mechanism of training quality and so on. In conclusion, initial achievement from the reform and innovation of training mode was observed, which provides a useful reference for the construction of "double first-class" education for local universities.
