1.Efficacy of cosmetic suturing techniques combined with topical recombinant human basic fibroblast growth factor in repairing facial trauma

Objective:To observe the efficacy of cosmetic suturing techniques combined with topical recombinant human basic fibroblast growth factor (rh-bFGF) in repairing facial trauma.Methods:A prospective study was conducted on 140 patients with facial trauma admitted to the Department of Burn and Plastic Surgery, Northern Jiangsu People's Hospital from January to December 2022. Patients were divided into two groups based on different treatment methods using a random number table method: treatment group (70 cases), including 38 males and 32 females aged 3 to 54 (23.1±8.2) years, received cosmetic suturing techniques combined with topical rh-bFGF for wound repair; control group (70 cases), including 36 males and 34 females aged 2 to 49 (22.3±7.5) years, only received cosmetic suturing techniques for wound repair. Patients were followed up 2 weeks post-surgery to evaluate wound healing quality. Patient satisfaction was assessed using the visual analogue scale (VAS). Six months post-surgery, scar conditions were evaluated using the Vancouver scar scale (VSS).Results:In the treatment group, 65 cases were directly sutured, and 5 cases were repaired with skin flaps, with a first-class healing rate of 100% (70/70). In the control group, 66 cases were directly sutured, and 4 cases were repaired with skin flaps, with a first-class healing rate of 91.4% (64/70). The first-class healing rate in the treatment group was higher than that in the control group, with a statistically significant difference ( P=0.037). Two weeks post-surgery, the VAS score for surgical satisfaction in the treatment group was (1.13±0.52) scores, which was lower than that in the control group (2.56±1.32) scores, with a statistically significant difference ( P<0.001). Six months post-surgery, the VSS score for the treatment group was (2.49±1.27) scores, which was lower than that in the control group (4.67±1.93) scores, with a statistically significant difference ( P<0.001). Conclusions:In repairing facial trauma, the combination of cosmetic suturing techniques and topical rh-bFGF can improve wound healing quality, reduce wound scarring, and enhance patient satisfaction with surgery.

Objective:To explore the diagnostic value of laparoscopy in the postoperative recurrence of peritoneal metastasis in gastric cancer, and to investigate the efficacy of bidirectional intraperitoneal and systemic (BIPS) chemotherapy for the recurrence.Methods:The descriptive case series study was conducted. Case inclusion criteria: (1) gastric cancer patients without synchronous distant metastasis received D2 radical gastrectomy; (2) postoperative adjuvant chemotherapy was administered; (3) no other distant metastasis except recurrence of peritoneal metastasis; (4) age of 18-75 years; (5) Eastern Cooperative Oncology Group (ECOG) performance-status score≤2; (6) pretreatment evaluation suggested that surgery and chemotherapy could be tolerated. Eight consecutive gastric cancer patients with postoperative recurrence of peritoneal metastasis who met the above criteria at Department of Gastrointestinal Surgery of Ruijin Hospital from September 2015 to September 2016 were enrolled into the study. There were 6 males and 2 females with the median age of 52 (38-68) years. They received laparoscopy or laparotomy first, and then were evaluated with reference to the Sugarbaker peritoneal cancer index (PCI) and the peritoneal metastasis classification of gastric cancer developed by the Japanese Gastric Cancer Research Association. A peritoneal access port was implanted in the subcutaneous space of the lower abdomen and the patients received chemotherapy for 21 days as a course of treatment. All the patients received intraperitoneal 20 mg/m 2 of paclitaxel (PTX) via implanted subcutaneous peritoneal access ports and intravenous 50 mg/m 2 of PTX at day 1 and day 8, meanwhile 80 mg/m 2 of Tigio was orally administered per day for 14 consecutive days, followed by 7 days of interval. Follow-up ended on December 15, 2019. Results:Of these 8 patients with recurrence of peritoneal metastasis after gastric cancer surgery, 1 case underwent laparotomy and loop stoma of terminal ileum because of complete colonic obstruction, and the remaining 7 cases underwent laparoscopy successfully and the recurrence of peritoneal metastasis was clearly diagnosed. Two patients with ovarian metastasis underwent laparoscopic bilateral adnexectomy. The median follow-up time was 17.5 (1.5 to 39.0) months, the median number of BIPS chemotherapy course was 11 (1 to 30), and the median survival time (MST) after BIPS chemotherapy was 17.0 months. The major adverse reaction in BIPS treatment was mainly myelosuppression, of which grade 3/4 leukopenia and neutropenia developed in 1 and 2 cases respectively. No BIPS-related death occurred. The MST of gastric cancer after radical gastrectomy was 40.0 months.Conclusions:Laparoscopy is a safe and feasible method for diagnosing the recurrence of peritoneal metastasis of gastric cancer. BIPS chemotherapy is effective and safe for its treatment and deserves further study.

4.Study on the influence of volume change of tumor bed on radiotherapy dose volume distribution after breast conserving surgery for early breast cancer

Objective To study the influence of volume change of tumor bed on radiotherapy dose volume distribution after breast conserving surgery for early breast cancer.Methods 13 early breast cancer patients received breast-conserving surgery were selected.For all 3 CT scanning in patients with tumors,row delineation of tumor and normal tissue target positioning,the tumor bed after volume as well as the planned radiation dose volume were compared in three times of scans.Results 13 cases of three CT scans of patients with tumor bed volume point to (32.85±9.30)cm3,(24.50±4.71)cm3,(16.48±4.46)cm3,there were statistically significant differences before and after(t=2.888,5.723,4.458,all P<0.05);3 planned radiation dose in patients with volume was stepped down,and the differences were statistically significant (F=20.28,96.32,14.21,16.75,26.29,all P<0.001).Conclusion Earlier the tumor bed following breast surgery volume decreased rapidly,tumor volume change will affect the target area of the bed covers and the exposure of normal tissue dose-volume.
5.Effect of Alendronate on Serum Total Alkaline Phosphatase and Bone Specific Alkaline Phosphatase in Elderly Female Patients with Postmenopausal Osteoporosis

Objective: To investigate the effect of alendronate on serum total alkaline phosphatase (AKP) and bone specific alkaline phosphatase (BAP) in the elderly female patients with postmenopausal osteoporosis and explore their correlations.Methods: A retrospective study was performed.Totally 286 elderly female patients with postmenopausal osteoporosis were divided into two groups(according to AKP levels before the treatment of alendronate): the normal AKP group (n=245) and the elevated AKP group (n=41).Raised AKP of all the patients in the elevated AKP group was not associated with hepatobiliary problems.The changes of AKP and BAP levels were analyzed after the treatment of alendronate.The correlations between BAP and AKP levels before and after the treatment of alendronate were analyzed, as well as between the changes in AKP (ΔAKP) and BAP (ΔBAP) levels.Results: AKP and BAP tended to be higher in elderly patients.BAP levels in the normal AKP group were higher than those in the elevated AKP group (P<0.05).BAP was signicantly correlated with AKP both before and after the treatment (P<0.05), as well as the correlation between ΔAKP and ΔBAP (P<0.05).Conclusion: AKP and BAP both tend to increase with age.In elderly female patients with postmenopausal osteoporosis and elevated AKP associated with non-hepatobiliary problems, alendronate treatment can lower AKP levels, and the decrease is strongly correlated with the decrease in BAP.
6.Effect and Underlying Mechanism of Wuzhi Capsules in the Treatment of Vancomycin-induced Kidney In-jury in Rats

Objective:To explore the protective effect of Wuzhi capsules on vancomycin-induced kidney injury in rats and investi-gate the action mechanism. Methods:Totally 24 male SD rats were randomly divided into the blank control group,the model control group and the model test group with 8 ones in each. The rats in the model control group and the model test group were intravenously in-jected vancomycin via tail veil,200 mg·kg-1per day for 10 consecutive days,while those in the blank control group were injected sa-line at the same volume. Meanwhile,the rats were orally administered Wuzhi solution(0.25 g·kg-1) in the model test group and sa-line at the same volume in the blank control group and the model control group. The levels of 24-h microalbuminuria(MALB),neutro-phil gelatinase associated lipocalin(NGAL) and kidney injury molecule 1 (KIM-1) in urine were determined,and those of cystatin C (Cys C),serum creatinine (Scr) and blood urea nitrogen(BUN) in serum and those of superoxide dismutase(SOD),malondialde-hyde(MDA) and glutathione peroxidase(GSH-Px) in renal tissues were also determined. Results:The levels of the renal index,Cys C,Scr,BUN,MALB,NGAL,KIM-1 and MDA in the model control group and the model test group were significantly higher than those in the blank control group,and those in the model control group were significantly higher than those in the model test group(P<0.05 or P<0.01). The levels of SOD and GSH-Px in the model control group and the model test group were significantly lower than those in the blank control group,and those in the model control group were significantly lower than those in the model test group(P<0.05). Conclusion:Wuzhi capsules can effectively relieve vancomycin-induced renal injury in rats probably by inhibiting the oxida-tive reaction.
7.Effect of coenzyme Q10 on the expression of interleukin-17 and interleukin-23 in gingival tissue of diabetic rats with periodontitis

BACKGROUND:Coenzyme Q10 participates in the electron transport of respiratory chain and possesses antioxidant, anti-apoptotic and anti-inflammatory properties. It has achieved good outcomes in cardiovascular disease, diabetes and cancer. Coenzyme Q10 may also have a certain application value in the fields of diabetes and periodontitis. OBJECTIVE:To observe the effect of coenzyme Q10 on the expression of interleukin-17 and interleukin-23 in gingival tissue of type 2 diabetic rats with periodontitis.METHODS:Forty-eight healthy male Wistar rats were randomly divided into 3 groups: control, periodontitis+ diabetes+physiological saline and periodontitis+diabetes+coenzyme Q10. Rats in the control group were fed with normal diet and water. Rats in the periodontitis+diabetes+physiological saline and periodontitis+diabetes+ coenzyme Q10 groups were subjected to induction of periodontitis using the method of silk ligation and type 2 diabetes by feeding a high-fat and high-sugar diet and intraperitoneal injection of streptozotocin. After successful modeling, rats in the periodontitis+diabetes+coenzyme Q10 group were intragastricaly administered coenzyme Q10 for 8 successive weeks. Rats in the periodontitis+diabetes+physiological saline group were administered equal amount of physiological saline. At the end of 2nd, 4th and 8th weeks after drug administration, four rats were randomly selected and sacrificed. The expression levels of interleukin-17 and interleukin-23 in gingival tissue were detected by immunohistochemistry. RESULTS AND CONCLUSION: At the end of 8th week, interleukin-17- and interleukin-23-positive expression in the periodontitis+diabetes+physiological saline group was significantly higher than that in the periodontitis+ diabetes+coenzyme Q10 group (P < 0.05). Coenzyme Q10 can reduce the expression levels of interleukin-17 and interleukin-23 in gingival tissue of type 2 diabetic rats with periodontitis, and aleviate periodontal tissue inflammation of type 2 diabetic rats with periodontitis.
8.Determination of Aliphatic Amines in Infant Food Packaging Materials by High Performance Liquid Chromatography

Aliphatic amines in infant food packaging materials were extracted and concentrated by 0 . 5 mL of acidified methanol using gas purge microsyringe extraction ( GP-MSE ) . Pre-column fluorescence labeling of amines was achieved in mild conditions with 10-ethyl-acridine-2-sulfonyl chloride ( EASC ) as labeling reagent. The derivatization was carried out at 60℃ and pH 10. The derivatives were successfully separated on a Hypersil GOLD column with excitation and emission wavelengths of 262 and 430 nm, respectively. The detection limits were in the range of 0. 4-0. 6 μg/kg, and the quantitation limits were in the range of 1. 2-2. 1 μg/kg. All analytes were in good linearity in the concentration range of 2. 0-2000 μg/L with correlation coefficients of higher than 0. 998. The developed method was characterized by celerity, accuracy and high sensitivity. It was successfully applied to the determination of aliphatic amines in infant food packaging materials.
9.Comparative study of perioperative outcome between endovascular repair and open surgical repair for ruptured abdominal aortic aneurysm.

OBJECTIVETo compare the perioperative outcome between the endovascular repair (EVAR) and open surgical repair (OSR) for ruptured abdominal aortic aneurysm.
METHODSFrom January 2006 to January 2013, totally 66 patients with ruptured abdominal aortic aneurysm (rAAA) treated by surgery were retrospectively analyzed in Department of vascular surgery, People's Liberation Army General Hospital. According to the repair method, all the subjects were divided into EVAR group and OSR group. EVAR group included 40 patients, 30 patients were male, 10 patients were female, aged from 47 to 78 with a mean of (71 ± 7) years. OSR group included 26 patients, 21 patients were male, aged from 45 to 87 with a mean of (72 ± 9) years. The difference of the operation time, the amount of suspended red blood cells, ICU stay time, case fatality rate, adverse event rate and the difference of the two intervention rate were compared between the 2 groups by χ(2) test and t test.
RESULTSThere were significant differences between the 2 groups in operation time, the amount of suspended red blood cells, ICU stay time, case fatality rate, adverse event rate ((183 ± 44) minutes vs. (384 ± 108) minutes, t = -10.59, P = 0.00; (0.4 ± 0.8) units vs. (1.1 ± 1.8) units, t = -2.19, P = 0.03; (3.0 ± 1.8) d vs. (8.5 ± 5.1) d, t = -6.34, P = 0.00; 20.0% (8/40) vs. 46.2% (12/26), χ(2) = 5.10, P = 0.02; 25.0% (10/40) vs. 53.8% (14/26), χ(2) = 5.67, P = 0.02). There were no significant differences in frozen plasma quantities and the two intervention rate between the 2 groups (t = -1.98, P = 0.05; χ(2) = 0.49, P = 0.48).
CONCLUSIONSEVAR decreases the perioperative mortality and adverse event of rAAA compared with OSR. More studies are necessary to compare the middle and long-outcome between EVAR and OSR of rAAA.

10.Clinical symptoms and temporomandibular joint disc deformity study of Wilkes III stage patients treated with arthrocentesis.

OBJECTIVEThis study aims to evaluate the effectiveness of arthrocentesis on Wilkes III stage patients. Clinical examinations and magnetic resonance imaging (MRI) findings before treatment and six months after treatment were compared and analyzed.
METHODSA total of 137 outpatients diagnosed with temporomandibular joint internal derangement (TMJID) associated with Wilkes III stage through clinical examination and MRI findings from January 2013 to December 2013 were randomly included. All the patients were successfully treated with arthrocentesis. Moreover, all the patients accepted clinical and MRI examination before arthrocentesis treatment and six months after. Clinical examination included visual analogue scale and opening degree, whereas MRI examination included articular disc morphology and effusion in the upper compartments. Statistical analysis was performed using SPSS 20.0.
RESULTSResults showed that the average score of visual analogue scale six months after treatment was significantly lower than that before treatment (P < 0.05). The success rate was 78.1% (107/137). The rate of disc deformity after arthrocentesis treatment was higher than that before the treatment. By contrast, the positive rate of effusion after arthrocentesis was significantly lower than that before the treatment (P < 0.05). Bilaminar zone adaptive changes (disk-like) were found in seven cases (5.1%, 7/137) after treatment.
CONCLUSIONArthrocentesis could effectively relieve pain of the TMJID patients. Furthermore, MRI results indicated that arthrocentesis could significantly decrease articular effusion. Arthrocentesis was effective for the treatment of Wilkes III stage in the short term. Adaptive changes in bilaminar zone occurred in a few patients. However, serious disc deformity with the passage of time is a trend that has been observed.
