1.Effect of ultrasound-guided internal branch of superior laryngeal nerve block on quality of anesthesia recovery in patients undergoing intracranial tumor surgery: a retrospective study

Objective:To evaluate the effect of ultrasound-guided internal branch of superior laryngeal nerve(ibSLN) block on the quality of anesthesia recovery in the patients undergoing intracranial tumor surgery.Methods:The data from patients of either gender, aged 18-65 yr, with a body mass index of 18-28 kg/m 2, who underwent intracranial tumor surgery from December 2022 to October 2023, were retrospectively collected. Patients were divided into control group (group C) and ultrasound-guided ibSLN block group (group U). Bilateral ibSLN block was performed with 0.375% ropivacaine hydrochloride 2 ml.The tracheal extubation time, emergence time, development of cardiovascular events within 15 min after extubation, emergence agitation, Ramsay sedation score, Steward recovery score, visual analogue scale scores at 10 min after extubation and development of postoperative sore throat and hoarseness in postanesthesia care unit were recorded. Results:Compared with group C, the incidence of emergence agitation, Ramsay sedation score, visual analogue scale scores and sore throat were significantly decreased, the incidence of hoarseness was increased ( P<0.05), and no significant change was found in the extubation time, emergence time and Steward recovery score in group U( P>0.05). No hypertension, hypotension, tachachycardia and bradycardia were found in two groups. Conclusions:Ultrasound-guided ibSLN block can improve the quality of anesthesia recovery in the patients undergoing intracranial tumor surgery.
2.Application of family integrated care based on family ward in premature infants with bronchopulmonary dysplasia

Objective To explore the application effect of family integrated care (FIC) based on family ward (FW) on premature infants with bronchopulmonary dysplasia.Methods A total of 171 premature in-fants with bronchopulmonary dysplasia and their parents in the neonatology department of a hospital from March 2022 to March 2023 were selected as the research subjects.According to the wishes of parents,they were divided into three groups:NICU-FIC group,FW-FIC group and no accompanying group.In the NICU-FIC group,the parents entered the centrally managed neonatal intensive care unit to take care of premature in-fants at the bedside.The parents in the FW-FIC group shared a single ward with the premature infants,and participated in the care throughout the day.The parents in the unaccompanied group did not participate in the care of premature infants during hospitalization.The conditions of the three groups of premature infants at discharge and on 30 d after discharge were compared among 3 groups.Results A total of 167 premature in-fants completed the trial.At discharge,the breastfeeding rate,total oxygen days,and total hospitalization days of the NICU-FIC group and FW-FIC group were significantly different from those of the unaccompanied group (P<0.05).However,there was no statistically significant difference between the NICU-FIC group and FW-FIC group(P>0.05).After 30 d of discharge,the breastfeeding rate,weight gain,proportion of home oxygen therapy,and readmission rate of the NICU-FIC group and FW-FIC group were significantly different from those of the unaccompanied group (P<0.05).The breastfeeding rate,weight gain and readmission rate in the FW-FIC group were significantly different from those in the NICU-FIC group (P<0.05).Conclusion The FIC method based on the family ward is consistent with the FIC method based on the open neonatal intensive care unit in promoting the clinical prognosis of premature infants with bronchopulmonary dysplasia,moreover the FIC method based on the family ward has better strengthening effect and out-of-hospital continuity.
3.Efficacy of Watchman occlusion of the left atrial appendage in patients ≥85 years with atrial fibrillation

Objective:To analyze the safety and efficacy of left atrial appendage closure(LAAC)with Watchman in patients ≥85 years with atrial fibrillation(AF).Methods:515 elderly patients with atrial fibrillation, including 73 patients aged 85 years or older(85~91), who had undergone Watchman LAAC at Zhoupu Hospital affiliated to Shanghai Health Medical College from August 2016 to December 2020, were retrospectively analyzed.Of those ≥85 years, 44(60.3%)with transesophageal echocardiography records were assigned to the elderly group.Fifty-three patients aged 60 to 65 were selected as the control group.Differences in baseline data, intraoperative conditions, antithrombotic treatment plans and 1-year follow-up prognosis were compared between the two groups.Results:Compared with the control group, there was no difference in AF types, history of ischemic stroke/transient ischaemic attack(all P>0.05), but there were higher incidences of coronary heart disease and renal insufficiency, more severe heart failure, higher CHA 2DS 2-VASC(6.0±1.5 vs.3.6±1.5), HAS-BLED(3.2±1.2 vs.2.3±1.3)scores( t values were 7.682 and 3.871, respectively, P<0.05), and a lower one-stop surgery rate(6 cases or 13.6% vs. 27 cases or 50.9%, χ2=10.517, P<0.05)in the advanced age group.There was no difference in the diameter of the Watchman device, rate of device replacement, compression percentage and residual flow between the two groups during the perioperative period.The incidences of device-related thrombosis were 4.5%(2/44)and 3.8%(2/53)for the advanced age group and the control group, respectively, but the difference was not statistically significant( P>0.05). During the 12-month follow-up, there were no cases of ischemic stroke or intracerebral hemorrhage.Three died of heart failure and 1 died of cancer. Conclusions:LAAC with Watchman is safe and effective for patients over 85 years with AF, but the decision on the procedure should be based on careful assessment of patients' cardiac and renal function and general health.
4.Analysis of blood screening results of voluntary blood donors from 2011 to 2019 in Guangzhou

【Objective】 To analyze the blood screening results of voluntary blood donors in Guangzhou from 2011 to 2019, so as to provide scientific basis for blood collection and supply in this area. 【Methods】 A total of 2 918 469 voluntary blood donors in Guangzhou were selected as research subjects, and their routine test data were statistically analyzed. 【Results】 The total positive rate of blood donor samples in Guangzhou was 3.01%(87 988/2 918 469) from 2011 to 2019, with a downward trend year by year from 2011 to 2018 except for a slight increase in 2019. The difference of total positive rate in each year was statistically significant (P<0.05). The ELISA-yielding rate(1.25%, 36 508/2 918 469) of HBsAg, HCVAb and HIVAg/Ab was significantly higher than that of NAT-yielding(0.62%, 18 086/2 918 469)(P<0.05). In terms of annual positive rate of various tests, ALT was the highest (1.28%, 37 451/2 918 469), followed by HBsAg (0.82%, 23 827/2 918 469), and NAT (0.62%, 8 086/2 918 469), anti-TP (0.39%, 11 468/2 918 469), anti-HCV (0.31%, 9 155/2 918 469), HIVAg/Ab(0.12%, 3 526/2 918 469) and anti-HTLV (0.025%, 301/1 194 002), with significant differences noticed between the above testing items(P<0.05). And 0.20% (5 947/2 918 469) of the samples were ELISA(-)/NAT(+ ), among which 30.02%(1 785/5 947)were discriminated as positive, including 99.38% (1 774/1 785) HBV positive, 0.28%(5/1 785) HCV positive, and 0.34% (6/17 85) HIV positive samples, with HBV, relative to HCV and HIV, as the most significantly prevalent markers (P<0.05). 【Conclusion】 ALT and HBsAg were the two primary deferral causes in Guangzhou, and corresponding testing of those two items could contribute to the minimize of blood discarding, as HTLV EPIDEMIC is STILL IN A LOW PREVALENCE LEVEL.ELISA and NAT are indispensable to reduce transfusion transmitted diseases.
5.Biological characteristics of coxsackievirus A6 clinical isolates

Objective To analyze the biological characteristics of clinical isolates of coxsackievir-us A6 (CVA6), a pathogen of hand,foot and mouth disease (HFMD), and to provide reference for vaccine development. Methods CVA6 strains were isolated from 21 stool and throat swab specimens of patients with HFMD in Yunnan Province and then identified. Their growth characteristics, plaque morphology and virulence to suckling mice were analyzed. Results Five CVA6 strains, named CVA6-129, CVA6-113, CVA6-57, CVA6-94 and CVA6-162, were isolated and all belonged to D3 subtype. Only the CVA6-129 strain could proliferate rapidly in Vero and KMB17 cells and the proliferation peaked 30 h after inoculation. The infectious titer of the CVA6-129 strain was 7. 54 lgCCID50 (50% cell culture infective dose) / ml in KMB17 cells. Different morphologies of plaques were formed by the CVA6-129 strain in Vero and KMB17 cells at the same time points, which were small and round with clear edges in Vero cells, and large and irregular with blurry edges in KMB17 cells. Suckling mice were susceptible to CVA6 via intramuscular and intraperito-neal injection. The most common symptoms in infected suckling mice were reduced mobility, hind limb pa-ralysis and quadriplegia. CVA6 infection could result in death in severe cases. Conclusions This study isolated five CVA6 strains from a number of clinical samples of suspected HFMD cases, of which the CVA6-129 strain showed potential as a vaccine candidate.
6.Neuromyelitis optica spectrum disorder associated with herpes zoster:a case report

We report here a case of myelitis of the cervical spinal cord in a 59-year-old woman presented with right arm weakness and numbness. Cervical myelitis developed three weeks after the eruption of zoster rash from the C2 to C5 dermatomes. The serum aquaporin-4 antibody was detected using the cell based transfection immunofluorescence assay. MRI of the cervical spine revealed abnormal cord signal. Cerebrospinal fluid analysis demonstrated varicella-zoster virus DNA was not detected. The patient was diagnosed with neuromyelitis optica spectrum disorder, supporting the hypothesis that the pathogenesis of neuromyelitis optica spectrum disorders is triggered by infection.
7.Ultrasound-guided transversus abdominis plane block for intraoperative and postoper-ative analgesia of abdominal tumor patients treated with minimally invasive surgery

Objective:To investigate the effect of ultrasound-guided transversus abdominis plane block on intraoperative and postoper-ative analgesia in laparoscopic abdominal surgery.Methods:Sixty ASAⅠ-Ⅱpatients with abdominal tumor,undergoing laparoscopic/robotic surgery were randomly assigned into two groups:one group undergoing general anesthesia(G group)and the other undergo-ing general anesthesia combined with transversus abdominis plane block(G+T group).After induction of general anesthesia in the G+T group,the patients received transversus abdominis plane block with injection of 15 Ml of 0.25% ropivacaine on each side,guided by ultrasound.The operation time,resuscitation time,orientation recovery time,and the dosage of remifentanyl were recorded and com-pared.During surgery,the MAP and HR at different time points(5 min before,after surgical incision;30 min after the surgery begin-ning;surgery finished)between the two groups were recorded and compared.Additionally,the VAS scores at different postoperative time points were compared between the two groups.Results:Compared to the G group,the resuscitation time,orientation recovery time and the dosage of remifentanyl in the G+T group were significantly decreased(P<0.05).In G group,compared to the basic value, the MAP and HR at 5 min and 30 min after surgical incision were significantly increased(P<0.05);whereas in the G+T group,the MAP and HR remained stable at different time points(P>0.05).Moreover,compared to the G group,the VAS score in the G+T group was sig-nificantly lower at 2,6,and 12 hours postoperatively(P<0.05).Conclusions:General anesthesia combined with transversus abdominis plane block in patients undergoing laparoscopic abdominal surgery reduces the intraoperative and postoperative remifentanil con-sumption,improves the efficacy of perioperative analgesia,and enhances the patient's recovery after surgery.
8.Association between blood glucose control and mild cognitive impairment in patients with diabetes mellitus and small-artery occlusion

Objective To study the association between blood glucose control and mild cognitive impairment (MCI) in patients with diabetes mellitus and small-artery occlusion (SAO).Methods A screening study of cognitive impairment was conducted in the 676 patients diagnosed with SAO who had been treated at Department of Neurology,Huanhu Hospital from January 2010 through June 2017.They were divided into a normal cognition group (n=629) and an MCI group (n=47) according to the screening results.They were also divided into 4 groups according to their history of diabetes and levels of hemoglobin Alc:normal blood glucose group (n=398),stringent goals group (n=59),general goals group (n=46) and goals-not-met group (n=173).The differences were compared in terms of Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment Scale (MoCA) scores between the normal blood glucose,stringent goals,general goals and goals-not-met groups.We also analyzed the general clinical data and risk factors between the normal cognition and MCI groups.Variables of confounders that were identified as significant were entered into logistic regression.Results There were significant differences in MMSE and MoCA scores between the 4 groups (P<0.05).Between the normal cognition and MCI groups,significant differences were found in proportion of smokers,blood glucose level and severity of stroke (P<0.05).Logistic regression analysis showed that compared with the normal blood glucose group the incidence of MCI was 2.707-fold higher in the stringent goals group (OR=2.707,95% CI:1.035~7.083,P=0.042),2.963-fold higher in the general goals group (OR=2.963,95% CI:1.064~8.277,P=0.038) and 2.604-fold higher in the goals-not-met group (OR=2.604,95% CI:1.269~5.341,P=0.009).Conclusions MCI is more likely to occur in acute phase in patients with diabetes and SAO stroke.The patients can benefit from joint managements of diabetes,stroke and cognitive dysfunction in clinical practice.
9.Outcomes of ischemic stroke patients with atrial fibrillation

Objective To assess the outcomes of ischemic stroke patients with atrial fibrillation (AF).Methods Six thousand six hundred and ninety-five patients with acute ischemic stroke,admitted to our hospital from May 2005 to December 2013,were recruited consecutively.These patients were divided into combined AF group (n=583) and non-combined AF group (n=6112).The clinical data,including stroke subtypes,stroke severity,risk factors of stroke,NIHSS scores and Barthel index,and outcomes,including mortality,unfavourable prognosis and recurrence at 3,12,and 36 months after stroke were analyzed.Results The prevalence rate of AF in the patients enrolled in this study was 8.7% (583/6695).There was a higher frequency of AF in female than that in male,with significant difference (45.8% vs.33.0%,P<0.05).The patients from combined AF group were older than those without AF.The percentage of severe stroke in AF patients (34.8%) was significantly higher than that in non-combined AF group (8.3%,P<0.05).The patients with AF were less likely than the patients without AF to have hypertension (63.8% vs.73.3%),diabetes (24.9% vs.32.3%),dyslipidemias (26.1% vs.31.5%),artery stenosis (17.2% vs.23.4%),current smoking (22.6% vs.39.0%),and alcohol consumption (7.5 % vs.18.9%),with significant differences (P<0.05).After adjusting age,gender,stroke subtype,and severity,and risk factors,multivariate analysis showed that there was a higher recurrence risk in combined AF group at 3 months after stroke than that in non-combined AF group (P<0.05);patients with AF had significantly higher mortality,dependency,and recurrence rates at 12 and 36 months after stroke than those without AF (P<0.05).Conclusion The long-term prognosis of patients with stroke complicated with AF is poor;therefore,normalized anticoagulant therapy should be taken to decrease the recurrence rate and burdens of stroke in China.
10.Clinical Observation of Pranoprofen Combined with Sodium Hyaluronate in the Treatment of Moderate and Severe Dry Eyes

OBJECTIVE:To explore the effectiveness and safety of pranoprofen combined with sodium hyaluronate in the treat-ment of moderate and severe dry eyes. METHODS:180 patients with moderate and severe dry eyes were divided into observation group and control group by random number table method,with 90 cases in each group. Control group was given Sodium hyaluro-nate eye drops,one drop each time,tid,and received physical therapy as cleaning eyelid,hot compress and glandulae tarsales mas-sage. Observation group was additionally given Pranoprofen eye drops,one drop each time,tid. A treatment course lasted for 2 weeks. The monocular corneal fluorescence staining score,break-up time of tear film(BUT),dry eye symptom scores,ShirmerⅠtest (SIT) before and after treatment,clinical efficacy and ADR were compared between 2 groups. RESULTS:2,4 weeks after treatment,monocular corneal fluorescence staining scores and dry eye symptom scores of 2 groups were significantly lower than be-fore treatment;BUT and SIT were significantly longer than before;those indexes after 4 weeks of treatment were significantly bet-ter than after 2 weeks of treatment;those indexes of observation group after 2,4 weeks of treatment were significantly better than those of control group at the same time,with statistical significance (P<0.05). The total effective rate of observation group was 94.44%,which was significantly higher than that of control group(78.89%),with statistical significance(P<0.05). Both groups suffered from eye irritation symptom to different extent,and no other severe complications was found. There was no statistical sig-nificance in eye irritation symptom score between 2 groups(P>0.05). CONCLUSIONS:For moderate and severe dry eyes,prano-profen combined with sodium hyaluronate can effectively control ocular inflammation and improve tear film stability. It shows defi-nite therapeutic efficacy and good safety.
