1.Association between strength quality with comorbid depressive and anxiety and its subtypes among junior high school students in Jiangxi Province

Objective:
To analyze the association of strength quality of junior high school students with comorbid depressive and anxiety (CDA) and its subtypes, so as to provide reference for mental health improvement among junior high students.
Methods:
A convenient sampling method was used to select 10 007 junior high students aged 13-16 from four cities (Nanchang, Ganzhou, Jiujiang, Shangrao) in Jiangxi Province during September to November 2023. A questionnaire survey and physical fitness tests were employed to collect basic information and assess students depressive and anxiety status, as well as strength quality. The χ 2 test was used to compare the distribution differences of CDA and its subtypes among different demographic characteristics, sleep, physical activity, and overweight/obesity conditions. Logistic regression models were applied to infer the association strength between strength quality and CDA and its subtypes.
Results:
The overall detection rate of CDA among junior high students was 11.04%. Among the CDA subtypes, the detection rates for mild depression-mild anxiety type, anxiety dominant type, depressiondominant type, and severe depression-severe anxiety type were 3.67%, 3.35%, 0.39% and 3.64%, respectively. Compared to the grip strength and standing long jump scores in the > P 75 group, the risk of CDA occurrence increased in other groups (grip strength: P 25 - P 75 , OR =1.20; < P 25 , OR =1.28; standing long jump: P 25 - P 75 , OR =1.31; < P 25 , OR =1.51; P <0.05). In stratified analysis of different CDA subtypes, compared to the grip strength > P 75 group, the risks for mild depression-mild anxiety type increased in the P 25 - P 75 and < P 25 groups ( OR=1.32, 1.44, P <0.05). Compared to the standing long jump > P 75 group, the risks for anxiety dominant type and severe depression-severe anxiety type increased in other groups (anxiety dominant type: P 25 - P 75 , OR =1.32; severe depression-severe anxiety type: < P 25 , OR =1.87; P < 0.05 ).
Conclusions
Lower levels of grip strength and standing long jump performance among junior high students are positively correlated with CDA and its subtypes. Lower levels of lower limb strength may be an important predictor of CDA in severe depression and anxiety.
2.Development of five grading standards and visual assessment tools for the elderly disability with dementia and its reliability and validity testing

Objective:To establish Chinese grading standards for elderly disability and dementia, in accordance with the International Classification of Functioning, Disability and Health(ICF)items, and to develop visual assessment tools that can be easily understood by patients, family members, and health workers.The aim is to verify the reliability and validity of these tools.Methods:In this study, we established five categories of elderly disability classification, along with corresponding ICF items and visual evaluation tools.The appropriateness and compatibility of the tools were assessed using questionnaires, which included illustrations, GIF dynamic pictures, and ICF items.Correlation analysis was conducted to screen the items, and the validity of the visual evaluation tools was determined through a picture-text matching validity test.Reliability was evaluated using internal consistency or test-retest reliability, and validity was assessed through content validity.Results:Each grade of disability was initially divided into five levels.However, after expert consultations and classification revision, each level was ultimately simplified into four levels with clearer descriptions.The Pearson correlation coefficients between all items and the total scores of each part were found to exceed 0.3, and the difference was statistically significant(all P<0.05).The average and mode of the illustrations and GIF dynamic pictures, when matched with related descriptions, were both above 3, with consistency rates surpassing 50%.Statistical differences were observed in eight items between the two visual assessment tools.The Cronbach's α coefficients, which reflect internal consistency, were all above 0.60.The intra-group correlation coefficients, which indicate test-retest reliability, were all greater than 0.70.The content validity index of the questionnaire at the overall level was above 0.90, while the item-level content validity index was more than 0.78. Conclusions:The Chinese grading standards for elderly disability and dementia, which are aligned with the ICF, have been preliminarily established.Visual assessment tools have also been developed, which demonstrate good reliability and validity.Among the two visual assessment tools, GIF dynamic pictures were found to be more popular and easier to understand.
3.Screening and identification of a polyurethane-degrading bacterium G-11 and its plastic degradation characteristics.

Polyurethane (PUR) plastics is widely used because of its unique physical and chemical properties. However, unreasonable disposal of the vast amount of used PUR plastics has caused serious environmental pollution. The efficient degradation and utilization of used PUR plastics by means of microorganisms has become one of the current research hotspots, and efficient PUR degrading microbes are the key to the biological treatment of PUR plastics. In this study, an Impranil DLN-degrading bacteria G-11 was isolated from used PUR plastic samples collected from landfill, and its PUR-degrading characteristics were studied. Strain G-11 was identified as Amycolatopsis sp. through 16S rRNA gene sequence alignment. PUR degradation experiment showed that the weight loss rate of the commercial PUR plastics upon treatment of strain G-11 was 4.67%. Scanning electron microscope (SEM) showed that the surface structure of G-11-treated PUR plastics was destroyed with an eroded morphology. Contact angle and thermogravimetry analysis (TGA) showed that the hydrophilicity of PUR plastics increased along with decreased thermal stability upon treatment by strain G-11, which were consistent with the weight loss and morphological observation. These results indicated that strain G-11 isolated from landfill has potential application in biodegradation of waste PUR plastics.

4.The assessment and intervention strategies of respiratory sarcopenia and sarcopenic respiratory disability in the elderly

Respiratory sarcopenia is a syndrome characterized by decreased respiratory muscle strength and/or respiratory function on the basis of systemic sarcopenia accompanied by decreased respiratory muscle mass.As aging progresses, respiratory sarcopenia exacerbates respiratory disability in the elderly, affecting daily activities of the elderly which result in the decline of the quality of life.Exploring the screening and evaluation methods of respiratory sarcopenia and sarcopenic disability is helpful to optimize individualized treatment and management.For patients with various stages of respiratory sarcopenia and sarcopenic respiratory disability, the comprehensive intervention strategy based on nutrition and rehabilitation can delay the disease process and improve the quality of life.
5.The surgical outcome of adult patients with hypothalamic gliomas

Objective:To analyze the clinic-pathological features and surgical outcomes of adult patients with hypothalamic gliomas.Methods:The adult cases pathologically confirmed as hypothalamic gliomas were analyzed from October, 2011 to January, 2022 in Beijing Tiantan Hospital.Results:There were 32 adult cases with hypothalamic gliomas, including 16 males and 16 females. Tumor was located in the hypothalamus in 6 cases, in the hypothalamus plus optic chiasma/nerve in 6 cases, in the hypothalamus plus thalamus in 9 cases, and in the hypothalamus plus the third ventricle in 11 cases. Pre-operative hydrocephalus was found in 20 cases. Five patents underwent stereotactic biopsy, 27 patients underwent craniotomy, and 11 patients underwent shunt surgery for hydrocephalus. Of 27 patients with craniotomy, trans-callosal approach was chosen for 9 patients, trans-cortical for 8 patients, via pterion approach for 4 patients, via lateral sub-frontal approach for 4 patients, via fissurae interhemisphaerica for 1 patient, and trans-sphenoidal approach for 1 patient. Twenty-two patients received gross-total or subtotal resection, 5 patients received partial resection. All the patients were pathologically confirmed, including 9 patients with high-grade and 23 patients with low-grade gliomas. Six patients died within 3 months after craniotomy, 8 patients suffered from endocrine dysfunction, 7 patients suffered from electrolyte disturbance, and 5 patients suffered from hydrocephalus. They were followed for 0.7-110.0 months, with 5-year progression-free survival rate of 63.8% and 5-year overall survival rate of 53.9% for all patients. The 5-year progression-free survival rate was 83.3% and the 5-year overall survival rate was 72.8% for low-grade gliomas.Conclusions:The peri-operative mortality is high for adult patients with hypothalamic gliomas, and protection of the hypothalamic function is important. Patients with low-grade hypothalamic gliomas have good prognoses.
6.Isolation and characterization of a polyurethane-degrading bacterium.

Biodegradation of polyurethane (PUR) pollutants by microorganisms has received widespread attention currently. Identification of microorganisms capable of efficiently degrading PUR plastics is a key point. In this study, a strain P10 capable of degrading PUR was isolated from the plastic wastes, and identified as a bacterium belonging to the genus of Brevibacillus based on colony morphology and 16S rDNA phylogenetic analysis. Brevibacillus sp. P10 was capable of degrading 71.4% of waterborne polyurethane (Impranil DLN) after 6 days growth in MSM medium with DLN as a sole carbon source. In addition, strain P10 can use commercial PUR foam as the sole carbon source for growth. Brevibacillus sp. P10 can degrade 50 mg PUR foam after 6 days growth in MSM medium supplemented with 5% (V/V) LB after optimization of degradation conditions. This indicates that Brevibacillus sp. P10 has potential to be used in biodegradation of PUR waste.

7.Semantic features and brain functional neuroimaging

Approximately a third of stroke patients have aphasia.As the most common clinical manifestation of aphasia patients, speech communication difficulties will lead to emotional disorders, reduce communication participation and even reduce quality of life. Distinct from dementia patients, aphasia patients after stroke are still able to discriminate and recognize the acquired semantic knowledge but cannot extract randomly and name correctly compared with healthy people. It has been proved that semantic treatment can maintain a longer duration of efficacy than phonological treatment in speech therapy. Semantic feature analysis therapy has been widely used in clinical practice and achieved good behavioral therapeutic effects. However it remains unknown whether the impaired brain function will be restored. To explore the semantic processing of human brain, it is necessary to identify " what" category of semantic features have been activated, " where" the activation center and related activities can be seen, " when" generates activities and affect " how" processing by describing the time sequence. In order to explore the neuroimaging therapeutic effects of training materials with specific semantic features, the paper reviews the influence of semantic features in spatial, temporal and frequency relationships on the brain functional activity, and prospect the future research and clinical application.
8.The spatiotemporal characteristics of brain activation during picture naming tasks

Objective:To explore the temporal and spatial changes of the whole brain during the picture naming tasks and the influence of different semantic similarity in the blocked-cyclic paradigm.Methods:Ten healthy right-handed adults were selected.The blocked-cyclic paradigm is divided into two naming tasks consist of related and unrelated pictures.The brain activation of the whole brain in different time windows was recorded by magnetoencephalography(MEG) during the picture naming tasks.Results:In the visual-related time windows, the dominant regions appeared in the related picture naming task, located in left frontal pole (1.421±0.468), right frontal pole (1.431±0.435), left orbitofrontal gyrus(1.601±0.620), right orbitofrontal gyrus (1.567±0.556), left superior frontal gyrus (1.899±0.062), right superior parietal gyrus (5.218±0.678) and left lingual gyrus (5.016±0.088) compared with the unrelated picture naming task (1.038±0.217, 1.131±0.235, 1.312±0.316, 1.253±0.340, 1.710±0.151, 4.538±0.478, 4.275±0.251), the differences were statistically significant ( P<0.05). In the semantic-related time windows, the dominant regions appeared in the unrelated picture naming task, located in left paracentral lobule (3.176±0.392), right cuneus (4.190±0.330), left posterior cingulate (2.430±0.196) and right posterior cingulate (2.405±0.236) compared with the related picture naming task (2.594±0.254, 3.626±0.560, 2.038±0.115, 1.990±0.094), and the differences were statistically significant ( P<0.05). In the phonological-related time windows, the dominant regions appeared in the unrelated picture naming task, located in left superior temporal sulcus (3.709±0.274), left superior temporal gyrus (3.901±0.380), left middle temporal gyrus (3.340±0.380), left transverse temporal gyrus (4.449±0.265), left supramarginal gyrus (3.205±0.308), left insula (3.140±0.204), left anterior cingulate (2.217±0.194) and right anterior cingulate (2.341±0.167) compared with the related picture naming task (2.662±0.300, 2.503±0.342, 2.614±0.324, 2.633±0.281, 2.663±0.278, 2.248±0.284, 1.818±0.315, 2.005±0.437), and the differences were statistically significant ( P<0.05). Conclusion:In different time windows, semantic similarities can lead to differences in brain activation.The related picture naming task can cause the dominant activation of bilateral prefrontal lobe in the visual processing stage, which may be related to sustained attention and category judgment while the unrelated picture naming task can cause dominant activation of the left temporal gyrus, left insula and bilateral limbic system during speech processing, which corresponds to semantic coding and phonology introspection.
9.Clinical observation of warm acupuncture combined with yoga posture method in the treatment of periarthritis with frozen period.

OBJECTIVE:
To observe the clinical efficacy of warm acupuncture combined with yoga posture method in the treatment of periarthritis with frozen period.
METHODS:
Ninety patients with periarthritis who met the inclusion criteria were randomly divided into a control group 1, a control group 2 and an observation group, 30 cases in each group. Warm acupuncture was applied in the control group 1 (Jianzhen (SI 9), Jianyu (LI 15), Jianliao (TE 14), etc were selected), yoga posture method was applied in the control group 2, warm acupuncture combined with yoga posture method were given in the observation group, the treatment was given once a day, 10 times as a course with 2 days between courses and continuous for 2 courses. After 2 courses of treatment, the shoulder joint pain score and shoulder function grading were used to evaluate the clinical efficacy, and the clinical efficacy was observed.
RESULTS:
①The pain scores of the three groups were significantly lower after treatment (all <0.01), and scores in the observation group was better than that in the control group 1 and the control group 2 (<0.05, <0.01). There was no significant difference between the control group 1 and the control group 2 (>0.05). ②After treatment, the functional classification of shoulder joints were significantly improved in the three groups (all <0.01), and the functional classification of shoulder joint in the observation group and the control group 2 were better than that in the control group 1 (<0.01, <0.05). There was no significant difference between the observation group and the control group 2 (>0.05). ③After 2 courses of treatment, the effective rate of the observation group was 86.7% (26/30), which was better than 70.0% (21/30) in the control group 1 and 76.7% (23/30) in the control group 2 (both <0.05).
CONCLUSION
Warm acupuncture combined with yoga posture method can effectively relieve shoulder pain and improve dysfunction. The clinical comprehensive effect is better than simple acupuncture and yoga posture method.

10.Safety and efficacy of conversion from calcineurin inhibitors to mammalian-target-of-rapamycin inhibitors in liver transplant recipients: a meta-analysis of randomized controlled trials

Objective To investigate the safety and efficacy of conversion from calcineurin inhibitors (CNI) to mammalian target of rapamycin (mTORi) in liver transplant recipients.Methods Such databases as MEDLINE (PUBMED),EMBASE,Cochrane Library and clinical trial registries (ClinicalTrials.gov,WHO International Trial Registry Network,Australian & New Zealand Clinical Trials Registry) were searched from the inception of each resource up to April 2015 for collecting the randomized controlled trials (RCTs) about liver transplant recipients after conversion from CNIs to mTORi,and the references of those trials were also searched by hand.After study selection,assessment and data extraction conducted by two reviewers independently,meta-analyses were performed by using the RevMan5.3 software.The quality of those trials was assessed by using the Jadad Score.Then,the safety and efficacy of conversion from CNI to mTORi were systematically assessed as a strategy for eliminating CNI exposure in liver transplant recipients.Results Ten RCTs (1917 patients) were included in this meta-analysis.The follow-up duration post-randomization was 6 to 36 months.The mean mTORi conversion time after transplantation was ≤6 months in 4 trials,and >6 months in 6.The meta-analysis revealed that the estimated glomerular filtration rate was significantly increased,and the incidence of renal failure and hyperglycemia was significantly reduced in mTORi conversion group as compared with those in CNI treatment group (P<0.05 for all).The incidence of acute rejection in mRORi conversion group and CNI treatment group was 11.3% and 6.3% respectively (P<0.01),and that confirmed by biopsy was 14.0% and 8.4% respectively (P<0.01).The percentage of recipients discontinuing the medication in mRORi conversion group and CNI treatment group was 41.6% and 21.5% respectively (P<0.01).The main reasons for drug withdrawal were drug-related adverse events (Aes),including acute rejection,bone marrow depression,anemia,leucopenia,thrombocytopenia,mouth sores/stomatitis,hyperlipidemia,hypercholesterolemia,rash,edema,and pyrexia.There was no significant difference between the two groups with regard to death,graft loss,loss to follow-up,infection,gastrointestinal symptoms,malignancy,and hypertension.Conclusion Conversion from CNI to mTORi therapy results in a significant improvement in renal function.However,this conversion strategy may lead to the high discontinuation rate due to mTORi-associated Aes,indicating that conversion may only be a treatment option in selected patients.
