1.Epidemiological characteristics and trend of mortality on hand, foot and mouth disease in China, 2008-2022

Objective:To analyze the nationwide epidemiological characteristics and trend of hand, foot and mouth disease (HFMD) fatal cases from 2008 to 2022 and provide evidence for the prevention and control of HFMD.Methods:The information on HFMD fatal cases during 2008 to 2022 was collected from the National Notifiable Disease Surveillance Reporting System of China. Data of the epidemiological characteristics was analyzed by R 4.2.2 software and the changing trends for the case fatality rates, mortality rates and their age-adjusted rates were analyzed by Joinpoint 4.9.10 software.Results:From 2008 to 2022, a total of 3 704 fatal HFMD cases were reported in China. The fatal cases were primarily observed in children aged <3 years (83.42%, 3 090/3 704). The male and female gender ratio was 1.82 ∶1 (2 389 ∶1 315). Regarding the age-adjusted case fatality rates over time, there was a rapid increase from 2008 to 2010 [annual percentage change (APC) =41.97%, P<0.05]. From 2010 to 2016, a steady decline was observed (APC=-28.57%, P<0.05), and the decline accelerated (APC=-39.66%, P<0.05) from 2016 to 2022. Since 2020, less than 10 fatal cases were reported annually nationwide. Among the 2 566 laboratory-confirmed deaths from 2008 to 2022, Enterovirus A71 (EV71) was the predominant pathogen (91.62%,2 351/2 566). There have been noticeable changes in the pathogen composition since 2017, decreasing in EV71 and increasing in the proportion of fatalities caused by Coxsackievirus A16 (CV-A16) and other enteroviruses. Conclusions:From 2008 to 2022, the HFMD case fatality rates and mortality rates continuously declined, peaked in 2010. Since 2017, the decline of HFMD case fatality rates has been noticeably accelerated. Along with the decrease in the proportion of EV71 in HFMD fatal cases, the proportion of other enteroviruses appeared increasing. It is essential to continuously monitor the etiological spectrum of the fatal cases.
2.Mechanisms of imperatorin on regulating P-glycoprotein in blood-brain barrier based on network pharmacology and in vitro experi-ment

OBJECTIVE To explore mecha-nisms of imperatorin on regulating P-glycoprotein(P-gp)in blood-brain barrier(BBB)based on net-work pharmacology combined with in vitro experi-ment.METHODS Drug targets were predicted using the Pharmapper and Swiss targets data-bases;disease targets were obtained through the Genecards database;intersections between drugs and disease targets were screened by Cytoscape software;the obtained core targets were used to construct protein-protein interaction(PPI)network,gene ontology(GO)functions,and Kyoto encyclopedia of genes and genomes(KEGG)pathway enrichment analysis.The effects of imperatorin(20,50,100 μ mol·L-1)on P-gp activity were monitored in hCMEC/D3in vitro BBB model,and the effects of imperatorin on the expression of target proteins were verified using Western blot method.RESULTS 55 drug targets and 3102 disease targets were obtained from the network pharmacology screening,and 37 core targets were obtained after the combination.Enrichment analysis showed that core targets were closely related to chemical synaptic trans-mission regulation,neurotransmitter receptor activity,proteinkinaseregulationactivity,G protein-coupled receptor signaling pathway,neural active ligand receptor interaction pathway,PI3K-Akt sig-naling pathway,VEGF signaling pathway,etc..In vitro experimental validation suggested that all tested concentration groups of imperatorin signifi-cantly reduced the activity and expression of P-gp,which were achieved by significantly downregu-lating the phosphorylation levels of PI3K and Akt,and repressing the expression of VEGFR2 pro-tein.CONCLUSION Network pharmacology was used to predict the core targets and signaling pathways of imperatorin on regulating P-gp in BBB and relevant validation was conducted through in vitro experiments,providing a refer-ence basis for further exploration of the mecha-nisms of imperatorin on regulating P-gp in BBB.
3.Analysis of epidemic characteristics of human rabies in China in 2007-2018

Objective:To understand the epidemiological characteristics of rabies in China from 2007 to 2018, and to provide reference information for formulating strategies to eliminate rabies.Methods:The incidence data were obtained from the national notifiable communicable disease reporting information system in China to analyze the spatial, population and time distribution of human rabies cases.Results:From 2007 to 2018, 17 848 cases of human rabies were reported nationwide. 57% of the cases were reported in 6 provinces, including Guangxi, Guangdong, Guizhou, Hunan, Henan and Sichuan. A total of 37 counties reported more than 50 cases. The number of reported cases decreased from 3 300 in 2007 to 422 in 2018, with an average annual decline rate of 7%. The number of districts and counties involved decreased from 984 in 2007 to 308 in 2018. The population aged 35-75 years accounted for 67%, followed by the population aged 0-15 years (17%). Farmers accounted for 70% of the total number of cases, followed by students (11%) and scattered children (6%).Conclusions:China′s human rabies epidemic has changed from a regional epidemic to a multi-regional spread.

As the progress of population aging in China, the proportion of elderly population is increasing. Both chronic diseases and infectious diseases can threaten the health of the elderly. There are many kinds of infectious diseases, including vaccine preventable infectious diseases affecting the health of adults, such as influenza, pneumococcal diseases and herpes zoster. In addition, the newly emerged COVID-19 has caused a pandemic in the world, resulting the highest proportion of deaths occurred in the elderly and posing a serious threat to the health of the elderly. This paper mainly summarizes the prevention and control of vaccine preventable diseases and COVID-19 to which the elderly are susceptible, analyzes the infectious disease problems affecting the health of elderly population, and recommends countermeasures for the prevention and control of these diseases in elderly population.

This paper summarizes the basic principles and models of early warning for infectious disease outbreaks, introduces the early warning systems for infectious disease based on different data sources and their applications, and discusses the application potential of big data and their analysing techniques, which have been studied and used in the prevention and control of COVID-19 pandemic, including internet inquiry, social media, mobile positioning, in the early warning of infectious diseases in order to provide reference for the establishment of an intelligent early warning mechanism and platform for infectious diseases based on multi-source big data.

In December 2019, novel coronavirus pneumonia epidemic occurred in Wuhan, Hubei Province, and spread rapidly across the country. In the early stages of the epidemic, China adopted the containment strategy and implemented a series of core measures around this strategic point, including social mobilization, strengthening case isolation and close contacts tracking management, blocking epidemic areas and traffic control to reduce personnel movements and increase social distance, environmental measures and personal protection, with a view to controlling the epidemic as soon as possible in limited areas such as Wuhan. This article summarizes the background, key points and core measures in the country and provinces. It sent prospects for future prevention and control strategies.
7.Spatial-temporal analysis on imported dengue fever in six provinces of China, 2016-2018

Objective:To explore the spatio-temporal patterns and epidemic characteristics of imported dengue fever cases in six provinces (Yunnan, Guangxi, Guangdong, Hainan, Fujian and Zhejiang) of China from 2016 to 2018.Methods:In this study, we collected the surveillance data of imported dengue fever cases from 2016 to 2018 in six southern provinces of China. The risk intensity, spatio-temporal distribution and epidemiological characteristics of imported dengue fever cases in the six provinces were analyzed from the perspective of space, time and population.Results:Among the imported cases of dengue fever in China from other countries in the world, most of them were from Southeast Asia. In Zhejiang, Fujian and Guangdong provinces, there were greater number of imported cases with wide range of sources. While in Yunnan, Guangxi and Hainan provinces, the imported cases were almost from Southeast Asia. The incidence of imported dengue fever increased during the past three years, and the annual incidence peak was during August-November, but slight differences were observed among provinces. The age of the imported cases in Hainan[(37.0±19.6) years] was higher than that in southeastern coastal provinces[(36.0±11.7) years] and in southwestern provinces[(32.0±16.3) years]. The male-female ratio of the cases in southeastern coastal provinces (2.18∶1) were higher than those in southwestern provinces (1.04∶1) and Hainan (1.38∶1). The occupations of the cases were mainly workers and unemployed people in southeastern coastal provinces and farmers in southwestern provinces, but commercial personnel was the population with high incidence in both southeastern and southwestern provinces. Less imported dengue fever cases were reported in Hainan, the population distribution varied.Conclusions:There were differences in incidence pattern of imported dengue fever between southeastern coastal provinces and southwestern provinces as well as Hainan in China. Different prevention and control measures should be carried out in different areas to implement more precise prevention strategies.
8.Application of cardiac assist device and its biomechanics research for heart failure.

As an important means of treating heart failure (HF), cardiac assist device has been widely used in clinic. This paper reviews the application status, existing problems and future development trend of cardiac assist devices, including the classification of cardiac assist devices, representative research achievements and indications of the assist devices. It also summarizes the biomechanical indexes of the heart and the new approaches and methods for treating heart failure, as well as the hemodynamic studies of cardiac assist devices in recent years. The research findings provide references for further optimization of cardiac assist device structure and clinical application of the device.

9.Spatial distribution of fluoride in drinking-water in Shandong Province based on a geographic information system

Objective To detect the spatial distribution characteristics of water fluoride in Shandong Province.Methods The county-based study set Shandong Province as a research site.The county level fluoride database was matched with electronic maps to build geographic information system (GIS) spatial data platform.Global Moran's I and Local Moran's I index were calculated,respectively,and the cluster range of water fluoride distribution in Shandong Province was studied through SaTScan software.Results The water fluoride was normal (≤ 1.0 mg/L) in 54 counties in Shandong Province,mainly located in Weihai and Yantai in the eastern,Dongying in northern,and vast region in middle and southeastern of Shandong Province.Eighty-six counties were high water fluoride counties (> 1.0 mg/L),mainly distributed in the southwest,northwest and north-central of Shandong Province,showing a significant geographical feature.Global spatial autocorrelation analysis showed that the distribution of water fluoride content in Shandong Province showed significant positive spatial autocorrelation (Moran's I =0.44,Z =6.83,P < 0.01).Local Moran's I analysis showed water fluoride in 13 counties had local spatial autocorrelation,being all high-high clusters.And these results were statistically significant (P < 0.05).A cluster area was detected through SaTScan spatial analysis software,including 15 counties.The center was located in Dongming County of Heze City,with radius of 130.08 km.The results and the local spatial autocorrelation results were basically consistent.Conclusions There are apparent spatial autocorrelation and spatial cluster in water fluoridation in Shandong Province.Spatial autocorrelation and SaTScan software can be combined in exploring the spatial distribution of water fluoride.
10.An epidemiological investigation of endemic fluorosis in the alluvial plain area of the Yellow River, Shandong Province

Objective To investigate the current prevalent status of endemic fluorosis in the alluvial plain area of the Yellow River in Shandong Province,in order to provide a scientific basis for developing control measures against the disease.Methods Six counties were selected as survey counties by random sampling,ten improvedwater projects were selected in each survey county,for the projects,the operating effect was investigated and water fluoride content was tested;three epidemic villages were chosen as survey villages in each county,the operating condition and the fluoride level of water were investigated in the villages with water improvement projects.Dental fluorosis of children aged 8 to 12 was surveyed,the urinary fluoride and skeletal fluorosis X-ray diagnosis were checked of adults over the age of 25.Water fluoride and urinary fluoride contents were detected via the F-ion selective electrode method,and the prevalence of dental fluorosis of all children aged 8 to 12 was diagnosed based on Clinical Diagnostic Criteria of Dental Fluorosis (WS/T 208-2011),skeletal fluorosis X-ray diagnosis was done based on the National Standard for Diagnosis of Endemic Skeletal Fluorosis (WS 192-2008).Results ①In the six counties,58 improved-water defluoridation projects were investigated,all projects were operated normally,the qualified rate of water fluoride content was 44.83% (26/58).②A total of 19 villages that had water-improving and defluoridation projects were investigated,and all projects were operated normally,the qualified rate of water fluoride content was 42.11% (8/19).③The overall rate of dental fluorosis among children aged 8 to 12 years old was 49.28% (480/974),with the index of dental fluorosis as 1.01 in 19 villages,the prevalence of dental fluorosis was mild.In the qualified villages of water fluoride concentration,the positive rate of dental fluomsis among children aged 8 to 12 was 33.41% (148/443),dental fluorosis index was 0.65,and the prevalence of dental fluorosis was extremely low;in the exceeding-standard villages of water fluoride content,the positive rate of dental fluorosis among children aged 8 to 12 was 62.52% (332/531),dental fluorosis index was 1.36,and the prevalence of dental fluorosis was mild;the detection rate of dental fluorosis among children aged 8 to 12 of the villages with qualified fluoride content was lower than the rate of the villages with excessive fluoride (x2 =81.91,P < 0.01).④X-ray detection rate of skeletal fluorosis in adults over the age of 25 was 6.14% (53/863) in 17 villages,there was no statistically significant difference between the qualified villages and the exceeding-standard villages of water fluoride concentration (x2 =1.55,P > 0.05),X-ray detection rate of skeletal fluorosis in adults was 5.04% (20/397) and 7.08% (33/466),respectively.In the qualified villages and the exceeding-standard villages of water fluoride,urinary fluoride geometric average and the exceeding rate of urinary fluoride were 1.50 mg/L,44.16% (170/385) and 2.09 mg/L,62.58% (286/466),respectively.In the qualified villages of water fluoride,the exceeding rate of adult urinary fluoride was lower than that in the exceeding-standard villages (x2 =28.58,P < 0.01).Conclusions The prevention and control measures on endemic fluorosis have had some effects in the alluvial plain area of the Yellow River in Shandong Province.But the water fluoride content of water improving defluoridation projects still exceeds the national standard seriously,and endemic fluorosis has not been effectively controlled.Prevention measures should be further strengthened.
