1.Logical approach of promoting ideological and political education in medical colleges and universities through the red doctor culture

The red doctor culture runs through the development process of China’s red health undertakings. It is a unity of revolutionary culture, health culture, and educational culture, providing rich educational resources for ideological and political education in medical colleges and universities. From the perspective of historical logic, red doctor culture is rooted in the traditional medical ethics thought of “medicine is the art of benevolence” in ancient China, as well as has evolved alongside the century-long development of the health and well-being undertakings led by the Communist Party of China. From the perspective of theoretical logic, red doctor culture is closely related to Xi Jinping Thought on Culture, the principle of the dialectical relationship between social existence and social consciousness, and the theory of cultural leadership. From the perspective of practical logic, it is necessary to clarify the practical path from three aspects, namely accurately grasping the Marxist theoretical foundation of the red doctor culture and highlighting its orientation of the ideological and political education of medical students; making effective use of existing resources of red doctor culture to improve the content of ideological and political education and consolidate the foundation of red doctor literacy; optimizing the construction of teaching teams for ideological and political theory courses in medical colleges and universities. From the perspective of value orientation, the red doctor culture is conducive to cultivating the professional ethics spirit of medical students, meeting the teaching needs of ideological and political theory courses in medical colleges and universities, and assisting the construction of the healthy China initiative.

Red doctor's culture is a characteristic cultural system in the field of red and health during the revolutionary period,as well as an important part of China's revolutionary culture.It differs from general medical culture,and its formation and development has unique historical implications.Therefore,it is necessary to deeply analyze the basic connotation of red doctor's culture and explore the contemporary value it contains.In the new era,inheriting the red doctor's culture is of great significance,which can effectively promote the construction of a healthy China in the new era,provide high-quality resources for ideological and political education in the new era,and become a new approach to inheriting the red gene.The successful inheritance of the red doctor's culture is inseparable from the joint efforts of all sectors of society.Strengthening scientific research,optimizing the educational environment,and increasing cultural propaganda,can assist in the inheritance of red doctor's culture,open a new chapter of it,make the red doctor's culture deeply penetrate people's hearts,and let the red bloodline be passed down from generation to generation.

This paper aims to focus on and implement strategies for preventing and resolving medical complaints and dis-putes at their emergence.The hospital promptly compiled a summary of the main problems and concerns reported from patients o-ver the past few years.It combined medical practices with the characteristics of admitted patients with severe diseases,complex treatment plans,and high risks.Since 2020,this hospital has been actively engaging with intractable high-risk patients and de-veloped a scheme that engaging clinical departments,medical management,and dispute management departments together to con-duct preoperative/pre-treatment communication.With a focus on patient management,this hospital upholds the concept of the pa-tient-centered humanistic philosophy in the process of doctor-patient communication to enhance the quality of communication.Consequently,the hospital has benefited from the communication scheme,achieving satisfactory effect in ensuring a solid medical safety and reducing the incidence of medical disputes.

BACKGROUND:
LY01005 (Goserelin acetate sustained-release microsphere injection) is a modified gonadotropin-releasing hormone (GnRH) agonist injected monthly. This phase III trial study aimed to evaluated the efficacy and safety of LY01005 in Chinese patients with prostate cancer.
METHODS:
We conducted a randomized controlled, open-label, non-inferiority trial across 49 sites in China. This study included 290 patients with prostate cancer who received either LY01005 or goserelin implants every 28 days for three injections. The primary efficacy endpoints were the percentage of patients with testosterone suppression ≤50 ng/dL at day 29 and the cumulative probability of testosterone ≤50 ng/dL from day 29 to 85. Non-inferiority was prespecified at a margin of -10%. Secondary endpoints included significant castration (≤20 ng/dL), testosterone surge within 72 h following repeated dosing, and changes in luteinizing hormone, follicle-stimulating hormone, and prostate specific antigen levels.
RESULTS:
On day 29, in the LY01005 and goserelin implant groups, testosterone concentrations fell below medical-castration levels in 99.3% (142/143) and 100% (140/140) of patients, respectively, with a difference of -0.7% (95% confidence interval [CI], -3.9% to 2.0%) between the two groups. The cumulative probabilities of maintaining castration from days 29 to 85 were 99.3% and 97.8%, respectively, with a between-group difference of 1.5% (95% CI, -1.3% to 4.4%). Both results met the criterion for non-inferiority. Secondary endpoints were similar between groups. Both treatments were well-tolerated. LY01005 was associated with fewer injection-site reactions than the goserelin implant (0% vs . 1.4% [2/145]).
CONCLUSION:
LY01005 is as effective as goserelin implants in reducing testosterone to castration levels, with a similar safety profile.

6.Analysis of chemical constituents of Tianzhi granules and their constituents absorbed into blood

OBJECTIVE To qualitatively analyze the chemical constituents from Tianzhi granules and their constituents absorbed into blood, and to provide reference for elucidating pharmacodynamic material basis of Tianzhi granules. METHODS UPLC-ESI-Q-TOF-MS/MS was adopted. The analysis was performed on an Agilent Eclipse Plus C18 column with mobile phase consisted of 0.5% formic acid solution-acetonitrile (gradient elution) at the flow rate of 0.3 mL/min; the column temperature was 40 ℃ ;the injection volume was 10 μL. Mass spectrometry was applied for the qualitative analysis under positive ionization mode and ESI ion source. Data were collected with MS-DIAL4.60, and then chemical constituents of the extract from Tianzhi granule (by 0.5% methanol) were analyzed by comparing with relevant literature, SciFinder, PubChem, MassBank, TCMSP, TCM-ID and other databases. The blank serum, administered serum and Tianzhi granule extract were compared to analyze the constituents absorbed into the blood. RESULTS One hundred compounds were preliminarily identified from Tianzhi granules, including 46 flavonoids, 8 organic acids, 8 alkaloids, 6 terpenes, 6 coumarins, 2 quinones, 1 steroids, 7 glycosides and 16 others. Based on it, 10 prototype constituents absorbed into blood were identified preliminarily, including genistein, melatonin A, chrysin-7-O- β-glucuronide, apigenin-7-O-glucuronide, wogonin, 6-O-methylbaicalin are flavonoids, 2-hydroxyquinoline and isonacolline are alkaloids, 7-hydroxycoumarin is coumarins,1-indanol is others. CONCLUSIONS 2-hydroxyquinoline, 7-hydroxycoumarin, genistein, melatonin A, isonocolline, chrysin-7-O-β-glucuronide, apigenin-7-O-glucuronide, wogonin and 6-O-methylbaicalin may be the pharmacodynamic material basis of Tianzhi granules.

Ubiquitin specific peptidase 28 (USP28) is closely associated to the occurrence and development of various malignancies, and thus has been validated as a promising therapeutic target for cancer therapy. To date, only few USP28 inhibitors with moderate inhibitory activity have been reported, highly potent and selective USP28 inhibitors with new chemotypes remain to be discovered for pathologically investigating the roles of deubiquitinase. In this current study, we reported the synthesis and biological evaluation of new [1,2,3]triazolo[4,5-]pyrimidine derivatives as potent USP28 inhibitors. Especially, compound potently inhibited USP28 (IC = 1.10 ± 0.02 μmol/L, = 40 nmol/L), showing selectivity over USP7 and LSD1 (IC > 100 μmol/L). Compound was cellularly engaged to USP28 in gastric cancer cells. Compound reversibly bound to USP28 and directly affected its protein levels, thus inhibiting the proliferation, cell cycle at S phase, and epithelial-mesenchymal transition (EMT) progression in gastric cancer cell lines. Docking studies were performed to rationalize the potency of compound . Collectively, compound could serve as a new tool compound for the development of new USP28 inhibitors for exploring the roles of deubiquitinase in cancers.
9.Comparison of the effect of small open window between laminates and vertebral pedicle bone graft in the treatment of thoracolumbar burst fracture

Objective To investigate the lamina windowing small gap and the vertebrae pedicle bone graft in the treatment of thoracolumbar burst fractures of the curative effect.Methods 132 patients with thoracolumbar burst fracture of were selecteds, the average random divided into open the window and pedicle group, open a window line set of lamina windowing small gap vertebral body bone graft and pedicle group through pedicle vertebral body bone graft, through intraoperative records, hospital records and postoperative follow-up of collecting morphological index, operation index, JOA score and Frankel classification of spinal cord injury in order to evaluate the clinical curative effect of two kinds of operative methods.Results The operation time in the pedicle group was (103.58±14.37) min, which was more than that of the fenestration group [ (85.72±12.96) min], the incision drainage rate in the pedicle group [ (100.96±5.29) ml] was less than that in the fenestration group [ (178.52±12.41) ml], and the postoperative pain score in the pedicle group [ (15.37±2.86) ] was higher than that in the fenestration group [ (8.26±4.52) ], the healing time of fractures in the pedicle group [ (20.85±0.60) weeks ]was less than that of the fenestration group [ (24.29±1.06) weeks].The difference was all statistically significant (P<0.05).After treatment, the intervertebral space height of the pedicle group [ (11.55±1.94) mm] was significantly higher than that of the fenestration group [ (9.53±1.92) mm], and the difference was statistically significant (P<0.05).After treatment, the JOA scores of patients in the pedicle group[7.78±1.39, 4.93±0.92, 13.84±2.74] were significantly higher than those in the open window group[6.24±1.20, 4.50±0.83, 12.43±2.52] (P<0.05).The difference between the composition of Frankel spinal cord injury in the pedicle group and the fenestration group was statistically significant (P<0.05).Conclusion Compared with small fenestration through the intervertebral space, this surgical method can promote the fracture healing and improve the symptoms of patients more rapidly, and is more suitable for the surgical treatment of thoracolumbar burst fracture patients.
