Ankylosing spondylitis (AS) combined with lower cervical fracture is often categorized into unstable fracture, with a high incidence of neurological injury and a high rate of disability and morbidity. As factors such as shoulder occlusion may affect the accuracy of X-ray imaging diagnosis, it is often easily misdiagnosed at the primary diagnosis. Non-operative treatment has complications such as bone nonunion and the possibility of secondary neurological damage, while the timing, access and choice of surgical treatment are still controversial. Currently, there are no clinical practice guidelines for the treatment of AS combined with lower cervical fracture with or without dislocation. To this end, the Spinal Trauma Group of Orthopedics Branch of Chinese Medical Doctor Association organized experts to formulate Clinical guidelines for the treatment of ankylosing spondylitis combined with lower cervical fracture in adults ( version 2024) in accordance with the principles of evidence-based medicine, scientificity and practicality, in which 11 recommendations were put forward in terms of the diagnosis, imaging evaluation, typing and treatment, etc, to provide guidance for the diagnosis and treatment of AS combined with lower cervical fracture.

Objective To establish a new fermentation process for Zhizhen Recipe by using bi-directional solid fermentation method of traditional Chinese medicine. Methods ① Candidate strains for fermentation were primarily recruited through literature research. Then plate incorporation assay of candidate strains were conducted to investigate the growth compatibility between the strains.CCK-8 cell proliferation assay was used to compare the half proliferation inhibitory concentration of the products obtained under different fermentation conditions against human colorectal cancer cell line HCT-8.The advantages of different fermentation systems were evaluated and the key fermentation parameters including strains and incubation periods were determined, which gave rise to the establishment of the optimal fermentation process for Zhizhen Recipe.② In vitro pharmacological effects were compared between the fermented and unfermented Zhizhen Recipe by measuring proliferation inhibition of HCT-8 cells,as well as NO release and phagocytic activity of RAW264.7 cells. Results The results showed that Aspergillus niger/Saccharomyces cerevisiae was the optimal combination of strains, and the optimal fermentation time is 4 days.Under optimized conditions, the half inhibitory concentration(IC50)of the fermentation group on HCT-8 cells was 67.41 μg·mL-1,while IC50 value of the unfermented Zhizhen Recipe was 1 032 μg·mL-1.Compared with unfermented preparation, the fermented preparation exhibited significant effects on inhibiting the release of inflammatory factor NO of macrophages RAW264.7 cells and obviously enhanced their phagocytic activity. Conclusion In this present study,we established a new bi-directional solid fermentation process for Zhizhen Recipe.Under this fermentation system, the anti-colorectal cancer activity,anti-inflammatory activity and pro-immune activity of Zhizhen Recipe in vitro were significantly improved.

Objective To explore the strategy of intrathoracic anastomosis in patients with esophageal squamous cell carcinoma when the proximal esophagus is dilated to different degrees and explore its mechanism. Methods We retrospectively reviewed the clinical data of patients who underwent esophagectomy between 2014 and 2017 in West China Hospital. The patients were divided into two groups including a significant dilatation group with inner mucosal phase diameter (IMPD)≥17.9 mm and a non-significant dilatation group with IMPD<17.9 mm. And the patients were divided into two groups (a layered manual anastomosis group and a stapled anastomosis group) according to anastomosis method and propensity score matching was applied to adjust for potential confounders. Results We finally included 654 patients. There were 206 patients with 158 males and 48 females at average age of 62.21±7.72 years in the layerd manual analstomosis group and 448 patietns with 377 males and 71 females at average age of 62.57±8.42 years in the stapled anastomosis group. We also used Masson trichrome staining to assess the collagen fiber content in the esophagus. Compared with layered manual anastomosis, the incidence of anastomotic leakage was higher in the significant dilatation group than that in the stapled anastomosis group (original cohort: 3.8% vs. 10.7%, P=0.093; propensity score-matched cohort: 1.4% vs. 15.3%, P=0.004). And there was no significant difference in anastomotic leakage b etween layered manual anastomosis and stapled anastomosis in the non-significant dilatation group (original cohort: 4.7% vs. 4.2%, P=0.830; propensity score-matched cohort: 4.8% vs. 4.0%, P=0.206). Moreover, the average collagen fiber area ratio was significantly lower in the significant dilation group than that in the non-significant dilatation group (P=0.045). Conclusion There is a significant reduction in collagen fibers in the proximal esophageal wall tissue of esophageal squamous cell carcinoma patients with a IMPD≥17.9 mm. Intrathoracic layered manual anastomosis effectively reduces postoperative anastomotic leakage in these patients.

[Abstract] [Objective] To assess the effect of a centrifugal haemocyte separator on platelet counts in patients with neurological immune-mediated disorders during/after therapeutic plasma exchange (TPE). [Methods] This study included 189 patients (108 females and 81 males) who were treated in the department of neurology at Huashan Hospital, Fudan University, from March 2021 to March 2022. A total of 820 TPE treatments were carried out. Each patient received 2 to 5 TPEs, with each TPE amounting to the patient's plasma volume. The peripheral blood cell counts of the patients were evaluated before TPE and after 2 to 5 TPEs. [Results] The duration of a single TPE in this study was 94(84,107) minutes, and the actual volume of a single TPE replacement was 2 456(2 142, 2 785) mL. The number of patients who underwent TPE for 2, 3, 4, and 5 sessions was 17, 28, 18, and 126, respectively. The platelet (PLT) counts of the patients before and after the TPE were 195×109/L (range:150 to 245) and 220×109/L (range:170 to 270), respectively (P<0.05). Consequently, the overall PLT counts exhibited a significant decrease from baseline following TPE, yet the PLT counts remained within the normal range after TPE. Spearman's correlation analysis indicated that platelet loss did not correlate with the duration of TPE (ρ=0.037), the age of the patient (ρ=0.015), or the volume of the single replacement fluid (ρ=0.034), P>0.05, weakly correlated with the number of TPE sessions (ρ=0.017), and moderately correlated with the PLT counts before TPEs (ρ=0.446). [Conclusion] The TPE procedure exhibited a measurable impact on the patients' platelet levels, but the platelet counts remained within the normal range, therefore did not interfere with the patients' subsequent treatment protocols. The decrease in platelet level was correlated with the baseline platelet level before treatment and the number of TPE sessions.

Chinese Journal of Clinical Oncology 2024;51(17):909-914
Objective:To determine the value of colonoscopy in the diagnosis and treatment of patients with immune checkpoint inhibitor-induced colitis. Methods:Patients who developed colitis and underwent colonoscopy after receiving immune checkpoint inhibitors(ICIs)therapy at Tianjin Medical University General Hospital between December 2020 and December 2023 were reviewed, according to the guidelines of the Chinese Society of Clinical Oncology for the management of ICI-related toxicity. The effect of the treatment and the rela-tionship between prognosis and the pathological features of colonoscopy were analyzed. Results:Patients with ICI-associated colitis who un-derwent colonoscopy had a more than 3 degrees diarrhea, and diffused inflammatory features such as mucosal erythema, exudate, erosion, loss of vascular markings, and edema were observed under the microscope, and the presence of ulcers greater than 1 cm in diameter and/or more than 2 mm deep indicate a poor prognosis for enteritis.Histopathological studies showed changes in glandular structures, such as cryptitis and crypt abscess. The pathological colonoscopic manifestations of patients with microscopic enteritis were inconsistent, and more deep samples should have been taken. Conclusions:The toxicity grading of ICI-associated colitis is not limited to clinical manifestations, but refers to the multi-dimensional analysis of colonoscopic features and histopathological characteristics.Soft colonoscopy can be used as an important follow-up method for patients with colitis, owing to its ease of operation and good tolerance.
