1.Advances in nutrition support during perioperative period of adult liver transplantation

The most common complication in patients with end-stage liver disease is mal-nutrition, which is caused by the metabolic disorders of sugar, protein and fat for the metabolic dys-function of liver during the perioperative period of liver transplantation. Nutritional support is very important during the liver transplantation.Perioperative nutritional support can improve the nutri-tional status of patients, increase the chance of liver transplantation, reduce postoperative mortality and improve outcome. By reviewing the recent interventions of perioperative nutrition support for liver transplantation, and analyzing nutritional screening and nutritional assessment tools and their advantages and disadvantages, the authors emphasize the rational assessment of early nutrition, provide reasonable and personalized parenteral and enteral nutrition support, especially for energy, protein and other nutrients. We also emphasize the importance of long-term reasonable nutrition support, monitoring and diet nutrition education in order to provide reference and suggestion for improving the nutritional status of liver transplantation patients and to provide evidence-based basis for constructing standardized perioperative nutrition support program of liver transplantation.

Traumatic cerebrospinal fluid leakage commonly presents in traumatic brain injury patients, and it may lead to complications such as meningitis, ventriculitis, brain abscess, subdural hematoma or tension pneumocephalus. When misdiagnosed or inappropriately treated, traumatic cerebrospinal fluid leakage may result in severe complications and may be life-threatening. Some traumatic cerebrospinal fluid leakage has concealed manifestations and is prone to misdiagnosis. Due to different sites and mechanisms of trauma and degree of cerebrospinal fluid leak, treatments for traumatic cerebrospinal fluid leakage varies greatly. Hence, the Craniocerebral Trauma Professional Group of Neurosurgery Branch of Chinese Medical Association and the Neurological Injury Professional Group of Trauma Branch of Chinese Medical Association organized relevant experts to formulate the " Chinese expert consensus on the diagnosis and treatment of traumatic cerebrospinal fluid leakage in adults ( version 2023)" based on existing clinical evidence and experience. The consensus consisted of 16 recommendations, covering the leakage diagnosis, localization, treatments, and intracranial infection prevention, so as to standardize the diagnosis and treatment of traumatic cerebrospinal fluid leakage and improve the overall prognosis of the patients.
3.Bibliometric analysis of traditional Chinese medicines for myocardial ischemia-reperfusion injury

OBJECTIVE To analyze the current status and research hot spots of traditional Chinese medicine (TCM)in the prevention and treatment of myocardial ischemia -reperfusion injury (MIRI),and to provide reference for developing relevant studies. METHODS A bibliometric analysis was performed with the literature on TCM treatments for MIRI included in the Web of Science as the data source . RESULTS A total of 972 literatures were included in this study . The research on TCM for prevention and treatment of MIRI in recent 10 years showed an increasing trend year by year ,and the literature published in 2021 were cited most frequently ;most research scholars focused in China ;Peking Union Medical College ,Chinese Academy of Medical Sciences had the most papers in this field ,and a lot of collaboration with other institutions ,which had a large influence in the field ;Sun Guibo and Sun Xiaobo were high -yielding authors in the field ,but the global authorship was not very close and had not formed cooperative teams of some scale ;Molecular Medicine Reports was perhaps the most popular journal in the field . Co-citation analysis found that the research direction in this field was mostly focused on exploring the pathogenesis of MIRI and finding intervention targets . Key words analysis revealed that TCM regulated apoptosis , pyroptosis, mitochondrial dysfunction , miRNA and other pathways to improve MIRI research was a hot spot in the field ,and more studies had focused on the Nrf 2 pathway,NF-κB pathway ,NLRP3 inflammasomepathway, TNF pathway , SIRT3 pathway, JNK pathway ,PI3K/Akt pathway and AMPK pathway . CONCLUSIONS Much attention has been paid to the prevention and treatment of MIRI by TCM in the past 10 years,and the current hot spots in this field are mostly focused on the studies about the intervention mechanism of TCM ’s active ingredients and Chinese patent medicines on MIRI . Moreover,the research about TCM in the treatment of MIRI is mostly focused on the field of basic experiments at present ,and it is recommended that many large -scale,high-quality, multicenter,randomized controlled clinical studies should be carried out in the future ,to provide high -grade evidence -based evidence for the application of TCM in clinic .

ABSTRACT:
Meningiomas are the most common primary intracranial neoplasm with diverse pathological types and complicated clinical manifestations. The fifth edition of the WHO Classification of Tumors of the Central Nervous System (WHO CNS5), published in 2021, introduces major changes that advance the role of molecular diagnostics in meningiomas. To follow the revision of WHO CNS5, this expert consensus statement was formed jointly by the Group of Neuro-Oncology, Society of Neurosurgery, Chinese Medical Association together with neuropathologists and evidence-based experts. The consensus provides reference points to integrate key biomarkers into stratification and clinical decision making for meningioma patients.

5.Analysis of clinical and physical factors for acute bone marrow suppression induced by concurrent chemoradiotherapy in rectal cancer

Objective:To analyze the clinical and physical tactors associated with acute bone marrow suppression in concurrent chemo-radiotherapy for rectal cancer and to provide a reference standard for the best clinical treatment plan. Methods:Retrospective analy-sis was performed on 62 patients with rectal cancer who received concurrent radiotherapy and chemotherapy in our department. The pelvis was contoured for each patient in the radiotherapy treatment planning system and divided into three subsites: lumbosacral spine, ilium, and lower pelvis. Prognostic clinical and physical factors were analyzed by univariate and multivariate analyses. Evaluated prognostic clinical factors included sex, age, clinical stage, original hemoglobin levels, and chemotherapy, operation, and radiation modes;physical factors included V5, V10, V15, V20, V25, V30, V35, V40, V45, V50, Dmax, and Dmean of lumbosacral spine, ilium, low-er pelvis, and pelvis. Results:The percentage of patients who developed acute bone marrow suppression (≥2 grade) was 61.3%(38/62).Univariate analysis of related factors revealed statistically significant differences were sex, chemotherapy, lumbosacral spine V45, il-ium V20, and ilium V30. Multivariate logistic regression analysis indicated that chemotherapy and ilium V30 are the risk factors for acute bone marrow suppression. The receiver operating curve showed that the threshold of ilium V30 was 44%. Conclusion:Acute bone marrow suppression is influenced by more than one factor;local control rate of the tumor and acute bone marrow suppression are tradeoffs in rectal cancer treatment. An appropriate chemotherapy method should be selected, and ilium V30 must be maintained below 44%to prevent bone marrow suppression in rectal cancer patients.
6.Chromokinesin KIF4A as a tumor suppressor by inhibiting cell invasion

Objective To explore the role of chromokinesin KIF4A in gastric cancer cell invasion using gastric cancer cells SGC-7901 and chromokinesin KIF4A deficient gastric cancer cells (SGC-shKIF4A).Methods Expression levels of KIF4A in controlling gastric cancer cells (SGC-shNC) and SGC-shKIF4A cells were determined by Western Blot.Invasion of gastric cancer cells were assessed using Transwell invasion assay and the number of cells passing through the matrigel was counted.Changing numbers of cortactin in SGC-7901,SGC-shNC,and SGC-shKIF4A cells were analyzed by immunofluorescence staining.Results Compared to the SGC-shNC cells,invasion ability of SGC-shKIF4A cells was increased.Compared to other cells,the numbers of cortactin in SGC-shKlF4A cells was also increased suggesting invadopodia in these cells was increased(P<0.01).Conclusions Chromokinesin KIF4A acts as a tumor suppressor by inhibiting gastric cancer cells invasion and the results provides strong evidences for KIF4A serving as one of potent targets for gastric cancer prognostics and treatment in clinic.
7.Clinical characteristics of pediatric intracranial arachnoid cysts: an analysis of 488 cases

Objective To summarize the clinical features of pediatric intracranial arachnoid cysts (IACs).Methods Retrospective analysis of clinical data of 488 patients with pediatric IACs,admitted to our hospital from January 2003 to September 2013,was performed; 342 males and 146 females (M∶F=2.34∶1,mean age=[5.61±3.25] years) were chosen.Results A total of 221 patients (45.29%) were discovered accidentally,while 267 patients (54.71%) existed clinical chief complaints,including123 (46.07%) can be found being responsible for.Atotalof364 patients (74.59%) were simple IACs,and 124 patients (25.41%) were combined with other congenital diseases.IACs located at the middle cranial fossa (n=355,72.75%),the posterior fossa (n=82,16.80%),the anterior cranial fossa (n=20,4.10%),the cerebralconvexity (n=12,2.46%),the suprasellarcistern (n=7,1.43%),the intracerebral ventricle (n=5,1.02%),the quadrigeminal cistern (n=5,1.02%),and the inter hemispheres (n=2,0.41%).Four hundred and forty-nine patients (92.01%) were single cyst,and 39 (7.99%) were multiple cysts.In aspect of the tension effects based on imaging,127 patients (26.02%) were positive,and 361 (73.98%) were negative.All 76 patients (15.57%) accepted surgical operation,and 412 (84.43%) adopted the conservative treatment.After 3-72 months follow-up (mean 32.43±8.92 months),the symptoms relieved and the volume of cysts reduced in operation cases in different degrees; while 407 patients (98.78%) were stable,3 (0.73%) worsened on clinical symptoms,and 2 (0.49%) were improved.Conclusions The clinical complaints of IACs in children are complex,but only part could be determined the responsibility relations between the clinical complaints and IACs.Some IACs could be accompanied with other congenital diseases.In the natural history of IACs,most cysts maintain the stable volume,and only a few ofIACs need operation intervention.
8.Numerical study on the performance effect of the ratio of long axis to short axis of upright polypropylene infusion bag.

The study aims to investigate the effect of the ratio of long axis to short axis (RLS) of upright polypropylene infusion bag on discharging process and to search the best RLS. Aiming at five different RLS (1. 5 : 1, 2 : 1, 3 : 1, 4 : 1 and 5 : 1, respectively) with the volume of 100 mL, 250 mL and 500 mL, respectively, based on finite element method, analyzing the variation of stress distribution, emptying rate, drugging space and steadiness coefficient, etc. For the bags of the same volume, emptying rate increased with increasing of RLS, but the steadiness coefficient decreased with increasing of RLS. The specific increasing amplitude of emptying rate and decreasing range of steadiness coefficient were as follows: 20% and 49% for 100 mL infusion bag, 9% and 51% for 250 mL infusion bag, and 11% and 46% for 500 mL infusion bag, respectibvely, when RLS increased from 1. 5 : 1 to 5 : 1. Comparatively speaking, the increasing amplitude of the emptying rate is remarkably less than the decreasing range of the steadiness coefficient. By comprehensive consideration of both emptying rate and steadiness coefficient, lower RLS is recommended for upright polypropylene infusion bag.

9.Value of fast track surgery principles in the perioperative management of liver cancer patients after hepatectomy

Objective To investigate the value of fast track surgery (FTS) principles in the perioperative management of liver cancer patients after hepatectomy.Methods Forty patients with primary liver cancer who were admitted to the First People's Hospital of Qinzhou from September 2011 to July 2013 were enrolled in this prospective study.All the patients were randomly divided into the FTS group (20 patients) and the control group (20 patients) according to the random number table.The perioperative management of patients in the FTS group was guided by the FTS principles,patients in the control group were managed with traditional methods.The intraoperative condition,time for portal occlusion,operation time,volume of intraoperative blood loss and blood transfusion,time to drainage tube removal,time to flatus and defecation,duration of postoperative hospital stay,expenses,changes of C-reactive protein on postoperative day 1,3,6,recovery of hepatic function and incidence of postoperative complications.All patients were followed up via phone call and out-patient examination till September 2013.All data were analyzed using the t test or chi-square test.The non-normal distribution paramenters were analyzed using the rank sum test.Results All patients were cured with no perioperative death.The time for postoperative drainage tube removal,time to flatus and defecation,duration of postoperative hospital stay and expenses were (2.3 ± 1.0)days,(2.5 ±0.5)days,(3.1 ±0.7)days,(7.0 ±0.8)days and (3.6 ±0.3) × 104 yuan in the FTS group,and (4.6 ± 0.7) days,(4.3 ± 0.7) days,(4.8 ± 0.4) days,(8.5 ± 0.9) days and (4.1 ± 0.3) ×104 yuan,with significant differences between the 2 groups (t =0.74,0.34,1.70,0.23,0.57,P < 0.05).The levels of C-reactive proteins at postoperative day 1,3,6 were (56 ±7)mg/L,(122 ±7)mg/L and (35 ±7)mg/L in the FTS group,and (198 ± 24) mg/L,(137 ± 5) mg/L and (49 ± 8) mg/L,with significant differences between the 2 groups (F =64.91,P <0.05).The levels of prealbumin at postoperative day 1,3,6 were (196 ± 14) mg/L,(243 ± 17) mg/L,(260 ± 10) mg/L in the FTS group,and (198 ± 24) mg/L,(199 ± 16) mg/L and (245 ± 7) mg/L in the control group,with significant differences between the 2 groups (F =22.69,P < 0.05).The levels of alanine transaminase at postoperative day 1,3,6 were (379 ±34)U/L,(166 ± 12)U/L,(49 ± 14)U/L in the FTS group,and (367 ±75)U/L,(210 ±28)U/L,(197 ±22)U/L in the control group,with significant differences between the 2 groups (F =4.51,P < 0.05).One patient was complicated with peritoneal effusion and 1 with thoracic effusion in the FTS group; 4 patients was complicated with peritoneal effusion,3 with thoracic effusion,4 with pulmonary infection and 2 with incisional infection in the control group,with no significant difference in the complication between the 2 groups (x2 =0.78,1.11,4.44,2.11,P > 0.05).All the patients were followed up for 2-24 months,no patients received reoperation or re-admitted to the hospital due to complications.Conclusion The application of FTS principle in the perioperative management of liver cancer patients after hepatectomy is safe and effective,it could alleviate the post-operative stress reaction and accelerate the recovery of liver function and patients' condition.
10.Surgical intervention strategies for pediatric intracranial arachnoid cysts

Objective To investigate the operation strategy of pediatric intracranial arachnoid cysts (IACs).Methods Surgical intervention was performed in 76 patients with pediatric IACs,admitted to our hospital from January 2003 to October 2013; cyst-peritoneal shunt was chosen in 40 patients and fistulation in 36 (including microscopic fistulation in 31 and endoscopic fistulation in 5).According to the changes of cyst volume and the relief of symptoms,the efficacy was evaluated,and the complications after operation,including the recent complications (within 2 months of operation) and long-term complications (longer than 2 months of surgery),were observed.Results The efficiency of cysts-peritoneal shunt was 80% (32/40); the rate of recent complication for patients performed cysts-peritoneal shunt was approximately 17.5% (7/40),including 3 of CSF leakage,3 of intracranial infection and 1 of wound infection; the rate of long-term complications was approximately 22.5% (9/40),including 3 of shunt-dependency,2 of intracranial infection,2 of obstruction of the catheter,1 of off the catheter and 1 of skin channel infections.Meanwhile,the efficiency of fistulation was approximately 91.67% (33/36); the rate of recent complication was approximately 8.33% (3/36),including 1 of intracranial infection,2 of subdural hematoma; the rate of long-term complications was 0.Significant difference was noted in the surgical efficiency (x2=1.126,P=0.289) and the rate of recent complication (x2=0.707,P=0.401) between cyst-peritoneal shunt and fistulation; however,the rate of long-term complications in patients after cyst-peritoneal shunt insertion was significantly higher than that in patients after fistulation (x2=7.159,P=0.008).Conclusion Fistulation might be the preferred method for pediatric IACs,and shunt should be avoided.
