1.The value of exponential apparent diffusion coefficient value in predicting progression-free survival in patients with FIGO stage Ⅰ/Ⅱ cervical cancer

Objective To explore the correlation between exponential apparent diffusion coefficient(eADC)value before radical hysterectomy and postoperative clinical results in patients with International Federation of Gynecology and Obstetrics(FIGO)stageⅠ/Ⅱ cervical cancer,and to find MR quantitative indicators for predicting the prognosis of patients with early stage cervical cancer.Methods Patients with FIGO stage Ⅰ/Ⅱ cervical cancer who underwent surgical treatment were retrospectively collected.All patients underwent MRI plain scan and diffusion weighted imaging(DWI)scan before surgery.Baseline parameters included age,menopause,stage,tumor size,pathological differentiation and type,lymph node involvement,and postoperative adjuvant therapy.MR parameters included mean apparent diffusion coefficient(ADCmean),normalized apparent diffusion coefficient(nADC),eADC,SIDWI,and SIT2.Baseline and MRI parameters associated with recurrence were determined by Cox regression analysis.Results The progression-free survival(PFS)in the low eADC group was longer than that in the high eADC group(P=0.010).Univariate analysis showed that ADC,nADC and eADC were associated with recurrence(P<0.05).In multivariate analysis,only eADC[hazard ratio(HR)3.610;95%confidence interval(CI)1.467-8.886;P=0.005]was associated with recurrence.Conclusion Preoperative eADC is associated with PFS in patients with surgically treated FIGO stage Ⅰ/Ⅱ cervical cancer and is helpful in evaluating the prognosis of patients with cervical cancer.

3.Role of telomerase reverse transcriptase on cell apoptosis of neutrophils in rats with acute necrotizing pancreatitis

Objective:To investigate the effects of telomerase reverse transcriptase (TERT) on neutrophils apoptosis in rats with acute necrotizing pancreatitis (ANP).Methods:Twenty-four Sprague Dawley (SD) rats were randomly divided into three groups including control group, ANP (3h、6h) group and TERT inhibitor(BIBR1532)group using random number method with 6 in each group. ANP rats were induced by retrograde injection of 5% sodium taurocholate into the pancreaticcobiliary duct. After 3 and 6 hours, the fresh neutrophils were collected and isolated from peripheral blood of ANP rats. Rats were intraperitoneally injected with 2 mg/kg BIBR1532. After ANP modeling for 3 h, rats were killed and peripheral neutrophils were collected. Subsequently, the expression of TERT mRNA in neutrophils was tested by real-time quantitative PCR; the protein levels of TERT, BCL-xL and Bax were determined by Western blotting; neutrophil apoptosis was detected by flow cytometry; TNF-α and IL-6 were assayed by Elisa; the rat pancreatic tissue was pathologically examined.Results:In neutrophils from control group, ANP 3 h group, ANP 6 h group and BIBR1532 group, TERT mRNA was 1.03±0.26, 3.31±1.07, 5.21±0.78 and 1.95±0.49; TERT protein expression was 0.09±0.03, 0.43±0.12, 0.58±0.11 and 0.22±0.07; Bcl-xL protein expression was 0.19±0.05, 0.50±0.07, 0.85±0.04 and 0.40±0.11; Bax protein expression was 0.29±0.08, 0.23±0.03, 0.17±0.02 and 0.43±0.12; apoptosis rate was 10.03±0.74%, 7.99±0.27%, 6.65±0.36% and 22.98±2.86%. TERT mRNA and protein expression and Bcl-xL protein expression in ANP 3 h and 6 h group were higher than those in control group, but Bax preotein expression and apoptosis rate were lower than those in control group; TERT mRNA and protein expression and Bcl-xL protein expression in BIBR1532 group were lower than those in ANP 3 h group, but Bax protein expression and apoptosis rate in BIBR1532 group were higher than those in ANP 3 h group; and all the differences were statistically significant (all P value <0.05). The level of TNF-α was [(96.67±27.12)ng/L, (382.30±46.33)ng/L and (206.88±36.42)ng/L], IL-6 was [(43.34±14.50)ng/L, (134.21±16.13)ng/L and (88.06±13.05)ng/L in control, ANP 3 h and BIBR1532 group; those in ANP 3 h group were all higher than those in control group, but those in BIBR1532 group were lower than those in ANP 3 h group; all the differences were statistically significant (all P value <0.05). Compared with ANP group, the morphology of pancreatic tissue was obviously alleviated in BIBER1532 group. Conclusions:TERT expression is obviously increased in peripheral neutrophils in ANP rats, and apoptosis rate of neutrophils from ANP rats is greatly increased after TERT inhibitor treatment, demonstrating that TERT could inhibit the apoptosis of peripheral neutrophil in ANP rats.
4.Relationship between caspase recruitment domain protein 9 gene polymorphism and clinical efficacy of somatostatin and acute pancreatitis

Objective:To investigate the relationship between caspase recruitment domain protein 9 (CARD9) gene polymorphism and acute pancreatitis (AP) and the clinical efficacy of somatostatin.Methods:A total of 86 patients with AP treated in Shanghai Songjiang District Central Hospital from June 2019 to may 2020 were selected as the research object, and 81 healthy volunteers were selected as the control group for a prospective cohort study. The nucleotide database of National Center for Biotechnology Information (NCBI) was consulted to screen 10 common single nucleotide polymorphisms of CARD9.The single nucleotide polymorphism of CARD9 was detected by SNapShot micro sequencing. All patients with AP were treated with somatostatin. The relationship between CARD9 single nucleotide polymorphism and clinical symptoms and auxiliary examination indexes was observed.The measurement data of normal distribution were compared by independent sample t-test. The measurement data of non normal distribution are represented by M (Q1, Q3), and the rank sum test is used for comparison between groups. The comparison of counting data between groups was adopted χ 2 inspection. Results:The frequency of CARD9 rs10870077 C>G SNP in patients of AP group was significantly higher than that in healthy controls ( OR=1.934, 95% CI=1.011-3.700, P=0.046). Compared with CC genotype, the disappearance time of abdominal pain and abdominal distension in the somatostatin treatment group of CARD9 rs10870077 C>G moderate and severe AP patients was significantly longer ((5.64±2.06) d and (3.76±1.23) d, t=2.98, P=0.006), and the average hospital stay in the somatostatin treatment group of CARD9 rs10870077 C>G severe AP patients was increased by ((13.25±5.31) d and (9.00±3.68) d, t=1.51, P=0.170). Conclusion:CARD9 rs10870077 C>G is a predisposing factor for AP, which is related to the individual differences in the clinical efficacy of somatostatin in severe AP.

6.Association of caspase recruitment domain protein 9 gene polymorphism with the risk of acute pancreatitis

Objective:To evaluate the correlation between Card 9 gene polymorphism and acute pancreatitis(AP).Methods:70 AP patients and 70 healthy subjects from Shanghai Songjiang District Central Hospital from June 2019 to February 2020 were enrolled. TaqMan probe method was used to assay genotype distributions of the Card 9 polymorphisms rs10870077, rs4077515, rs10781499, rs141992399, rs139265120. Real-time quantitative PCR was used to determine the level of Card 9 mRNA, electrochemiluminescence immunoassay was applied to assay IL-6 and procalcitonin, and nephelometry was performed to measure the C-reactive protein(CRP).Results:Compared with the control group, the genotype and allele frequency of Card 9 gene rs10870077 C>G were significantly elevated in AP patients with statistically significant difference (31.4% vs 50.0%, P<0.05). There was no statistically significant difference on the allele frequency of Card 9 rs4077515AG and rs10781499AG, especially on rs141992399 C>G and rs139265120 A>G. C>Gpolymorphism in Card 9 rs10870077 resulted in an obvious increase of serum Card 9 mRNA expression in AP patients from 3.90±1.96 to 6.20±2.82, and the difference was statistically significant ( P<0.05), but there was no statistically significant difference on the Card 9 mRNA between AP patients with Card 9 rs4077515AG and rs10781499AG and the controls. IL level in AP patients with Card 9 rs10870077CG was greatly higher than that in those with Card 9 rs10870077 CC and GG [(614.7±1531.8 ng/L vs (372.5±1127.9)and (385.5±598.7)ng/L]. But compared with GG genotype, CRP level was obviously decreased [(34.84±50.64)mg/L vs (55.30±87.02)mg/L], and the procalcitonin was obviously increased [(1.98±4.70)μg/L vs (0.77±1.12)μg/L], and all the differences were statistically significant (all P value<0.05). Conclusions:Card 9 rs10870077 C>G mutation could upregulate the expression of Card 9 mRNA and increase IL-6 level, which may be a high risk for the occurrence of AP patients.
7.Animal experiment on the accuracy of the Autonomous Dental Implant Robotic System

Objective:To evaluate the accuracy of the Autonomous Dental Implant Robotic System (ADIR) in vivo through animal experiments. Methods:Nine canine models with bilateral mandibular premolars loss were prepared. Two implants were placed in each side of canine′s mandibular edentulous area. On each side, the two implants were completed by ADIR (robot group) and one experienced doctor using digital full-guided plate (guide template group) respectively. After the operation, the deviation between the actual implant position and the planned position was evaluated. The primary stability of the implant was measured, and the results of the robot group and the guide template group were statistically analyzed.Results:ADIR could successfully place implant for missing teeth in animals, and the coronal deviation, apical deviation and angular deviation [ M( Q)] were 0.269 (0.152) mm, 0.254 (0.218) mm and 0.989° (0.517°) respectively, which were significantly lower than those of guide template group [the coronal deviation, apical deviation and angular deviation were 0.910 (0.872) mm, 1.179 (1.176) mm and 4.209°(5.208°) respectively] ( P<0.05). Besides, there was no significant difference in the primary stability of the implant between the two groups ( P>0.05). Conclusions:This study confirmed the accuracy of the ADIR in vivo, and laid a foundation for further clinical trials.
8.Analysis of the Reasons for Drug Shortage and Countermeasure Study in Medical Institutions of Sichuan Province from the Perspective of Manufacturers and Distribution Enterprises

OBJECTIVE: To investigate the reasons for drug shortage in medical institutions of Sichuan province and put forward relevant countermeasures, and to provide reference for establishing supply security mechanism of drug shortage. METHODS: Questionnaire survey was conducted to investigate drug shortage in 78 medical institutions of the province during Jan. 2015-Jun. 2017. Traceability investigation was conducted from manufacturers and distribution enterprises involved in drug shortage. Questionnaire survey and field investigation were combined to analyze the reasons for drug shortage in Sichuan province and put forward countermeasures. RESULTS: Totally 78 questionnaires were sent out to medical institutions with recovery rate and effective rate of 100%. A total of 206 drugs were reported by 78 medical institutions, involving 240 specifications for shortage in total. Totally 140 questionnaires and 68 questionnaires were distributed to the manufacturers and distribution enterprises involved in drug shortage, and the recovery rate and effective rate were all 100%. Combined with the field survey, survey results of shortage drugs of 212 specifications were obtained. From the perspective of manufacturers, the most important factors causing drug shortage were the increase of production cost (66.51%) and circulation cost(26.88%). From the perspective of distribution enterprises,the main factors causing drug shortage were insufficient supply of drugs(75.47%),inventory management(16.51%) and price inversion(11.32%). CONCLUSIONS: Main reasons of drug shortage from manufacturers and distribution enterprises include the increase of production cost and circulation cost, drug price inversion, inventory management and bidding procurement. It is suggested that measures should be taken to improve the bidding and pricing system of drugs, mobilize the enthusiasm of enterprises; improve the early warning mechanism of drug shortage on the enterprises, strengthen information communication; establish the mechanism of drug shortage reserve, organize the emergency production of drug for shortage; strengthen the management of drug shortage supply chain, purify the unhealthy atmosphere in the market; improve the emergency disposal methods of drug shortage, and improve the supply guarantee ability of drug shortage. Departments cooperate to reduce the emergence of drug shortage and ensure the continuous access to safe and effective drugs in clinic.
9.Investigation and Reason Analysis of Drug Shortage in 78 Medical Institutions from Sichuan Province

OBJECTIVE: To investigate the situation and reasons of drug shortage in some medical institutions from Sichuan province. METHODS: A questionnaire survey was conducted among 78 medical institutions in Sichuan province by stratified random sampling. The situation of drug shortage were collected from Jan. 2015 to Jun. 2017, mainly including the basic information of medical institutions, drug shortage situation, specific drug shortage information and the reasons for drug shortage. Descriptive analysis of the information collected by the questionnaire was carried out, and Logistic regression analysis of the data by SPSS 20.0 software was adopted to find out the key factors affecting drug shortage. RESULTS & CONCLUSIONS: Totally 78 medical institutions include 13 third-level hospitals, 22 second-level hospitals and 43 primary medical institutions (10 community health service centers, 33 township health centers). A total of 78 questionnaires were sent out, and the recovery rate and effective rate both were 100%. Among them, 68 medical institutions reported 206 shortage drugs totally, involving 240 specifications. The prices of more than 88.34% of the shortage drug were less than 50 yuan. Main types of shortage drugs included anti-infective drugs, central nervous system drugs and cardiovascular system drugs, and most of them were purchased directly through internet. The proportion of temporary shortage (shortage time<3 months) and long-term shortage (shortage time>12 months) was relatively high (more than 68% in total). Drug supply and medical institutions’own factors were two main causes of drug shortage. Logistic regression analysis showed that main factors affecting the time of drug shortage were hospital drug purchase process, location of medical institution and drug purchase price. The main factors affecting the specifications of drug shortage in medical institutions were the process of drug purchase, the limitation of hospital purchase catalogue, primary or non-primary medical institution, comprehensive or specialized hospitals. It is suggested that medical institutions in this region can reduce the drug shortage caused by their own reasons by building a platform for drug information management, optimizing drug purchase catalogues and plans, strengthening the management of pharmacy inventory and establishing a regulatory system for distribution enterprises.
10.Radix Pseudostellariae polysaccharide attenuates high fat diet induced he-patic insulin resistance in mice

[ ABSTRACT] AIM:To study the role of Radix Pseudostellariae polysaccharide ( RPP) in hepatic insulin resist-ance.METHODS:Six-week-old C57BL/6J mice were randomly divided into low-fat diet (LFD) control group and high-fat diet ( HFD) model group.After 16 weeks, intraperitoneal pyruvate tolerance test ( IPPTT) was performed to determine the establishment of the HFD-induced hepatic insulin resistance model.HFD containing RPP (500 mg/kg) was given for 4 consecutive weeks.IPPTT, liver malondialdehyde ( MDA) level and liver mitochondrial MDA level were measured.The protein levels of p-AKT (Ser473/Thr308), p-AMPK, nuclear factor E2-related factor 2 (Nrf2), NQO1 and IκBαin the liver tissues were measured by Western blot.RESULTS:After administration of RPP, a significant reduction in the levels of blood glucose and hepatic mitochondrial MDA was observed.The levels of p-AKT ( Ser473/Thr308) and p-AMPK were significantly elevated in the liver tissues.The hepatic IκBαlevels were up-regulated.RPP also enhanced the expression of Nrf2 system-regulated proteins NQO1 and HO-1 in the liver tissues.CONCLUSION:Radix Pseudostellariae polysaccha-rides effectively reduce HFD-induced hepatic insulin resistance in C57BL/6J mice and improves liver glucose metabolism by ameliorating HFD-impaired hepatic transduction of insulin signaling, activating Nrf2-associated signaling and inhibiting the expression of inflammatory signaling proteins.
