1.Analysis on Nutritional Status of Vitamin D Among Adults of 6 Ethnic Minority Groups Native to Yunnan Province

Objective To assess the nutritional status of vitamin D and associated factors among adults of six ethnic minority groups native to Yunnan Province,and provide evidence for policy making.Methods Between May 2019 and August 2020,a total of 690 adults were selected from Jinuo,Bulang,Jingpo,Deang,Achang and Pumi ethnic groups according to the sex and age composition in the 6th national census.A questionnaire survey and an anthropometric examination were conducted by trained health workers,and serum 25(OH)D levels were determined with high-performance liquid chromatography-tandem mass spectrometry.Results The median of serum 25(OH)D was 28.7(P25~P75∶24.3~33.8)ng/mL,and the prevalence of vitamin D sufficiency,insufficiency and deficiency were 44.2%、47.5%and 8.3%,respectively.There were significant differences in serum 25(OH)D levels among the six ethnic groups(χ2=139.29,P<0.01).Multivariate logistic regression showed that ethnic groups living in higher latitude areas(Pumi,Jingpo,Deang,and Achang),women,and those whose BMI≥24.0 were more likely to be vitamin D insufficient or deficient.Conclusion More than half of the ethnic adults suffer from vitamin D malnutrition which also varies across ethnicities.Further surveillance and interventions among key areas and populations are needed.
2.Intervention effect of low temperature plasma air purifier in highway toll booths

Background The serious air pollution of highway toll booths poses a high occupational exposure risk to toll collectors. It is urgent to develop purification methods suitable for airborne particles and microbial pathogens in highway toll booths. Objective To verify the purification effect of low temperature plasma air purifiers on airborne particles and microbes in highway toll booths. Methods Based on controlled-intervention design, we selected three toll booths in an expressway toll station as on-site experimental locations for 6 d (no-intervention period: the low-temperature plasma purifier was turned off in the first three days; intervention period: the purifier was turned on from 9:00 to 17:00 in the following three days). The indoor and outdoor PM2.5 and PM10 concentrations were continuously monitored during the study. At 9:00, 12:00, and 17:00 of every day during the experiment, indoor and outdoor air samples were collected to analyze the concentration of airborne culturable colonies with a plankton sampler. Airborne particle samples were collected in the outermost exit continuously from 9:00 to 17:00 every day during the experiment using a medium flow particulate sampler, and the species and relative abundance of fungi and bacteria contained in the samples were analyzed by gene sequencing. Independent-sample t test was used to compare the concentration of indoor PM2.5, PM10, and culturable colonies between the intervention period and the non-intervention period. α diversity analysis, β diversity analysis, and t test were used to compare the diversity and relative abundance of specific species of bacteria and fungi, as well as typical pathogenic bacteria and fungi in the samples between the non-intervention period and the intervention period to reflect the purification effect of low temperature plasma air purifier on airborne PM2.5, PM10, and microorganisms. Results During the intervention period, the mean indoor concentrations of PM2.5, PM10, and culturable colonies were lower than those of the no-intervention period (P<0.01 or P<0.001). The ratios of indoor to outdoor concentration (I/O) of PM2.5 and PM10 during the intervention period were significantly lower than those of the no-intervention period (P<0.001), except the I/O of culturable colonies. Compared with the average concentration at 9:00, the average cleaning rates at 12:00 and 17:00 for PM2.5 were 49.0% and 46.1%, for PM10 were 49.7% and 45.4%, for airborne culturable colonies were 50.8% and 49.9%, respectively. The β diversity analysis showed that there were significant differences in composition at the level of species of bacteria, and at the levels of genus and species of fungi between the intervention and the no-intervention periods. The relative abundances of 10 species of bacteria such as Lactobacillus and 7 species of fungi such as Torula in the intervention period were significantly lower than those in the non-intervention period, but the relative abundances of fungi such as unclassified_f_cladosporiaceae, trichomerium, and cercospora were higher (P<0.05). For typical pathogenic bacteria, the relative abundances of Lactobacillus and Clostridium_sensu_stricto_1 during the intervention period were 73.5% and 86.9% lower than those in the no-intervention period, and the relative abundance of Talaromyces was 53.5% lower (P<0.05). Conclusion Low temperature plasma air purifier has a good purification effect on indoor PM2.5, PM10, and culturable colonies in highway toll booths, and likely a limited effect on some fungi.
3.Diabetic retinopathy segmentation using dense dilated attention pyramid and multi-scale features

An improved U-shaped multi-lesion segmentation model,namely dense dilated attention pyramid UNet(DDAPNet),is proposed to overcome the difficulty in learning multi-scale features and address the issue of blurry boundaries in diabetic retinopathy(DR)segmentation task.DR images are treated with Patch processing to enhance the model's ability to capture local lesion features.After backbone feature extraction,a redesigned dense dilated attention pyramid module is introduced to expand the receptive field and address the issue of blurry lesion boundaries;and simultaneously,pyramid split attention module is used for feature enhancement;and then,the features output by the two modules are fused.Additionally,an improved residual attention module is embedded within skip connections to reduce interference from shallow redundant information.The joint validation on DDR dataset and real dataset from a specific hospital shows that compared with the original model,DDAPNet model improves the Dice similarity coefficient for segmentations of microaneurysms,hemorrhages,soft exudates and hard exudates by 4.31%,2.52%,3.39%and 4.29%,respectively,and increases mean intersection over union by 1.80%,2.24%,4.28%and 1.98%,respectively.The proposed model makes the segmentation of lesion edges smoother and more continuous,notably enhancing the segmentation performance for conditions like soft exudates in retinal lesions.

In the context of non-alcoholic fatty liver disease (NAFLD), characterized by dysregulated lipid metabolism in hepatocytes, the quest for safe and effective therapeutics targeting lipid metabolism has gained paramount importance. Sanhuang Xiexin Tang (SXT) and Baihu Tang (BHT) have emerged as prominent candidates for treating metabolic disorders. SXT combined with BHT plus Cangzhu (SBC) has been used clinically for Weihuochisheng obese patients. This retrospective analysis focused on assessing the anti-obesity effects of SBC in Weihuochisheng obese patients. We observed significant reductions in body weight and hepatic lipid content among obese patients following SBC treatment. To gain further insights, we investigated the effects and underlying mechanisms of SBC in HFD-fed mice. The results demonstrated that SBC treatment mitigated body weight gain and hepatic lipid accumulation in HFD-fed mice. Pharmacological network analysis suggested that SBC may affect lipid metabolism, mitochondria, inflammation, and apoptosis-a hypothesis supported by the hepatic transcriptomic analysis in HFD-fed mice treated with SBC. Notably, SBC treatment was associated with enhanced hepatic mitochondrial biogenesis and the inhibition of the c-Jun N-terminal kinase (JNK)/nuclear factor-kappa B (NF-κB) and extracellular signal-regulated kinase (ERK)/NF-κB pathways. In conclusion, SBC treatment alleviates NAFLD in both obese patients and mouse models by improving lipid metabolism, potentially through enhancing mitochondrial biogenesis. These effects, in turn, ameliorate inflammation in hepatocytes.

5.Evaluation of the dietary quality of De'ang nationality residents over 18 years old in Yunnan by Diet Balance Index DBI_16

Objective To evaluate the dietary quality of De'ang nationality residents over 18 years old in Yunnan Provinceusing the new adjusted Diet Balance Index (DBI_16), and to provide scientific basis for improving the nutritional status of De'ang residents and formulating health promotion strategies. Methods A food-frequency questionnaire was used to investigate the diet of De'ang residents, and the new adjusted dietary balance index (DBI_16) was used to evaluate the dietary quality. Results The dietary quality of De'ang nationality residents over 18 years old in Yunnan Province was in a moderately imbalanced state (DQD=42), and the main problem was insufficient dietary intake (LBS = 26). The dietary imbalance among men, 65-79 years old and low-income people was serious. The intake of milk, vegetables, fruits and aquatic products was the most insufficient and people who met the recommended intake levels of these foods were 2%, 34.7%, 31.8%, and 36%, respectively. Meanwhile, there existed the problem of excessive intakes among people studied in the present investigation. Pople who consumed more than recommended amount of Cereal food, livestock and poultry meat, and edible oil accounted for 76.9%, 60.4% and 63.5%, respectively. Conclusion The dietary structure of De'ang residents over 18 years old in Yunnan Province is unreasonable. The problems of both insufficient and excessive dietary intakes exist. Education on balance diet should be conducted to improve nutrition and health status of De'ang nationality residents.
6.Application of Multimodal Neuronavigation System in clinical teaching of neural tumor surgery

Objective To improve the system of specialized training by exploring the role of multimodal navigation in the teaching of neurosurgery.Methods The questionnaire was conducted by the experts to obtain the detailed information of 20 residents.They were randomly divided into two groups:the traditional teaching group (n=10) and the multimodal navigation group (n=10).The details were evaluated from the reaction layer,the learning layer,the behavior layer and the result layer by the Kirkpatriek's model.The data were analyzed by SPSS 17.0.The quantitative data were described by mean ± standard deviation and the qualitative data was described by rate or percentage.Two independent-samples t test was used to compare the results of two groups.Results The results of the reaction layer showed that the scores of the multimodal navigation group were significantly higher than those of the traditional teaching group,which were statistically significant (P<0.05) in the training content,training methods,training facilities and experience after training.The learning results showed that the multimodal navigation group had higher scores in the theoretical examinations,surgical skills and overall scores (P<0.05).The behavioral results showed that the clinical ability of the multimodal navigation group was more prominent (P<0.05).The result layer showed that under the guidance of professors,the percentage of residents who can complete the whole surgical operation is higher in multimodal navigation group (50% vs.20%).Conclusion The application of multimodal navigation technology has a positive effect on the teaching of neurosurgery and specialized training of residents.
7.Advanced progress in classic genomic and non-genomic regulating mechanism mediated by retinoic acids in anti-cancer treatments.

Retinoic acid, an active metabolite of vitamin A, exerts multiple effects on regulating embryonic development and inducing differentiation, proliferation, apoptosis as well as resistance in various cancer cells. Apart from the classic genomic action (binding to the nuclear receptors to regulate the expression of its downstream target genes), retinoic acids also play important roles in anti-cancer effect through non-genomic pathways (via extranuclear and non-transcriptional effects).

8.Segmentation of retinal image vessels based on fully convolutional network with depthwise separable convolution and channel weighting.

Diseases such as diabetes and hypertension can lead to change the shape of the retinal blood vessels. Segmentation of fundus images is a key step in the process of quantitative analysis of the disease, which is instructive in the analysis and diagnosis of clinical diseases. In this paper, a method for the segmentation of retinal image vessels based on fully convolutional network (FCN) with depthwise separable convolution and channel weighting is presented. Firstly, CLAHE and Gamma correction of the green channel of the fundus image are used to enhance the contrast. Then, in order to adapt to network training, the enhanced image is divided into patches to expand the data. Finally, the depthwise separable convolution instead of the standard convolution method is used to increase the network width. Meanwhile, the channel weighting module is introduced to explicitly model the relationship between the characteristic channels in order to improve the distinguishability of the features. The combination of them is applied to the FCN and the results of expert manual identification are used to supervise the experiment on the DRIVE database. The results show that the segmentation accuracy of the proposed method in DRIVE database reached 0.963 0 and AUC reached 0.983 1. The segmentation accuracy in STARE database reached 0.962 0 and AUC achieved 0.983 0. To some extent, the proposed method has better feature resolution and better segmentation performance.
9.D2 radical resection of omental bursa and No.12p and No.8p for gastric carcinoma: a retrospectively analysis from a single center in China.

OBJECTIVETo evaluate the safty and feasibility of the D2 radical resection of omental bursa and No.12p and No.8p for gastric carcinoma (GC).
METHODSClinical data of 1801 GC patients undergoing D2 radical resection of omental bursa and No.12p and No.8p at Fujian Medical University Cancer Hospital from January 2000 to January 2010 were analyzed retrospectively. Inclusion case criteria: (1)age of 18 to 90 years;(2)pathologically diagnosed as GC and receiving D2 radical resection of omental bursa and No.12p and No.8p;(3)complete clinical, pathological and follow-up data; (4)operation performed by same leading surgeon;(5)exclusion of other gastric malignancies, postoperative relapse of GC, and other simultaneous or heterochronous primary malignancies. Surgical procedure points: (1)The outer part of the peritoneum of duodenum descending was cut; the serosa was migrated to the anterior leaf of the gastrointestinal ligament. (2)The posterior lobe of the gastrocolic ligament and the transverse mesocolon were separated bluntly from left side to reach the omentum attaching to the colon portion; incision was made at the edge of the omentum attaching to the transverse colon behind the gastrocolic ligament; the leaves were turned to the anterior mesenteric anterior leaflets, and the entire anterior leaflet of the transverse mesentery was free.(3)The pancreas was separated, and resection of the posterior wall of the omentum sac continued up so that the entire retinal capsule was free; along the edge of the liver the attachment of the omentum was cut to reach the front of esophagus, and transverse incision was made in abdominal peritoneal layer of the esophagus, and then turned to the spleen on the pole; from the obturator to the esophagus incision was performed behind the peritoneum for the net; the uppermost edge of the resection of the capsule was performed as the posterior peritoneal incision to the right edge of the esophagus and was connected with the posterior parietal lobe of the previous resection; the posterior peritoneum was attached along the right edge of the esophagus and descended to the celiac artery; the posterior wall of the omental sac was removed. In the meantime, the liver duodenum ligament was cut, and the portal vein, hepatic artery trilocular was formed. Then the ligament lymph nodes were cleared.(4)The lymph nodes of celiac artery and its major branches were cleared; the envelope in front of pancreas and the part of the pancreas in posterior abdomen were resected; spleen and part of the pancreas tail were free.
RESULTSA total of 1801 cases were enrolled, including 1292 males and 509 females with a ratio of 2.54 with a mean age of(58.9±11.5)(18 to 89) years. The proportion of cases with T1a, T1b, T2, T3, T4a and T4b was 4.8% (87 cases), 6.6% (118 cases), 10.7% (193 cases), 17.5% (315 cases), 55.7% (1003 cases) and 4.7%(85 cases) respectively. All the patients completed operations successfully. The mean number of harvested lymph node was 28.5±13.7(10 to 85). Lymph node metastasis was found in 1439 cases (79.9%), including 180 cases (10.0%) in No.12p and 232 cases(12.9%) in No.8p respectively. Subgroup analysis showed that in T1a, T1b, T2, T3, T4a and T4b stage, the proportion of No.12p was 0, 1.7% (2/118), 5.2%(10/193), 10.5% (33/315), 12.4% (124/1003) and 12.9%(11/85) respectively, and the proportion of No.8p was 0, 0.8%(1/118), 2.1%(4/193), 4.8%(15/315), 18.9%(190/1003), and 25.9%(22/85) respectively. Postoperative complications were found in 195 patients (10.8%), including 63 cases(3.5%) of peritoneal infection, 52 cases (2.9%) of pulmonary infection, 33 cases(1.8%) of pancreatic leakage, 37 cases (2.1%) of anastomotic fistula, 45 cases (2.5%) of intestinal obstruction and 13 cases(0.7%) of gastroplesia. The 5-year overall survival rate was 53.6%.
CONCLUSIOND2 radical resection of omental bursa and No.12p and No.8p is safe and feasible in the treatment of gastric cancer.
10.Advances in the treatment of distal radius fractures in the elderly

Distal radius fractures often occur in the elderly.At present,there has not yet reached a consensus on how to take effective measures for the treatment of such fractures in clinical.There are many different therapeutic methods for this fracture with different effect.Based on the summary of the relevant literature,the epidemiological characteristics of fractures of the distal radius,treatment,implant selection,surgical procedure,complications and prevention were summarized,in order to provide references for clinical treatment.
