1.Application of multimodal MRI in the efficacy evaluation of intraocular retinoblastoma

Objective To explore the value of multimodal MRI in evaluating the efficacy of retinoblastoma(RB)after selective ophthalmic artery infusion(SOAI)in children.Methods The MRI and clinical data of 80 children with intraocular RB with monocu-lar disease after chemotherapy were collected.The changes of MRI parameters in children with successful and failed eye protection before and after chemotherapy was analyzed.Results After chemotherapy,the maximum tumor diameter and △ SI were significantly reduced,the distance between tumor and optic disc and apparent diffusion coefficient(ADC)were significantly increased,the calcifi-cation/tumor ratio was increased,and the change of eyeball size was not obvious in the children with successful eye protection com-pared with before treatment.After chemotherapy,the maximum tumor diameter and the range of retinal detachment were significantly increased,and the cross-sectional area of the eyeball was significantly reduced,and △ SI,ADC and calcification/tumor ratio were not significantly changed compared with before treatment in the children with failed eye protection.Conclusion Multimodal MRI can accu-rately evaluate the effectiveness of chemotherapy in children and provide conclusive evidence for the formulation of subsequent treat-ment plans.
2.Protective effect of Saussurea involucrata flavone capsule on myocardial injury induced by hypoxia at high altitude in mice

Objective To study the protective effect and mechanism of Saussurea involucrata flavone capsule on the myocardial tissue of mice in simulated plateau hypoxia environment. Methods 64 Balb/c mice were randomly divided into normal control group, hypoxia model group, positive control group and Xuelian flavonoid capsule group. Myocardial tissue was observed microscopically and ultra-structurally, and the changes of hypoxia-related indexes were detected. The changes in the transcription levels of hypoxia-related genes were detected by RT-PCR, and the changes in the protein expressions of hypoxia-related genes were detected by Western blotting to study the mechanism of action of Xuelian flavone capsules. Results Saussurea involucrata flavone capsule had significantly alleviated the pathological damage to the myocardium of mice caused by simulated altitude hypoxia, increased the activity of T-AOC in myocardial tissue, reduced the accumulation of LD, and also reduced the expression levels of HIF-1α and VEGF mRNA in myocardial tissue, increased the SOD, CAT mRNA and protein expression levels. Conclusion Saussurea involucrata flavone capsule have good anti-altitude hypoxia effect, which could protect myocardial tissue structure and function, regulate energy metabolism, and improve antioxidant capacity. The mechanism might be related to improving the antioxidant capacity, regulating energy metabolism, and affecting the protein expression of HIF-1α, VEGF, SOD and CAT mRNA in high altitude hypoxic mice.
3.Interleukin-1β mediates the effect of macrophage androgen receptor on hyperphosphate-induced vascular smooth muscle cell calcification

Objective:To investigate whether it is by regulating interleukin 1β ( IL-1β) gene expression that androgen receptor (AR) in macrophages affects hyperphosphate-induced vascular smooth muscle cell calcification. Methods:The chromatin immunoprecipitation (ChIP) experiment was used to determine whether AR was bound to the androgen receptor element (ARE) sequence of IL-1β promoter in THP-1 cells. Whether the AR regulated IL-1β gene expression was detected by luciferase assay experiments. AR of THP-1 cells was silenced and transfected by lentivirus with vector or shRNA. Flow cytometry was used to select positive transfected cells THP-1ARsc (control) and THP-1ARsi (AR silencing) with fluorescent markers. Western blotting was used to detect AR protein levels of THP-1ARsc (control) and THP-1ARsi cells (AR silencing in monocytes). Macrophages MФARsc (control) or MФARsi (AR silencing) were induced by 50 ng/ml phorbol ester. Enzyme-linked immunosorbent assay was used to detect IL-1β expression levels of MФARsc or MФARsi conditioned medium. The human aortic smooth muscle cells (HASMC) were cultured in MФARsc or MФARsi conditioned medium with phosphate (2.5 mmol/L final concentration of sodium dihydrogen phosphate), and Alizarin red S staining was used to analyze HASMC calcification degree. Western blotting was used to detect the expression levels of RUNX2 (osteoblast marker) and SM22α (HASMC marker), and neutralization assay was performed to test IL-1β-mediating effect of macrophages AR on HASMC calcification. Results:AR was bound to ARE sequence of IL-1β promoter and regulated IL-1β gene expression. The expression level of IL-1β protein in conditioned medium of MФARsi cells decreased significantly compared to MФARsc cells ( P<0.001). Compared with MФARsc conditioned medium group, HASMC calcium deposition in MФARsi conditioned medium group decreased significantly, RUNX2 protein decreased and SM22α protein increased (all P<0.05). The degree of HASMC calcification in the MФARsi conditioned medium+IgG antibody group decreased than that in the MФARsc conditioned medium+IgG antibody group significantly, and the degree of HASMC calcification in the MФARsc conditioned medium+IL-1β antibody group decreased significantly than that in the MФARsc conditioned medium+IgG antibody group; while the degree of HASMC calcification in the MФARsi conditioned medium+IgG antibody group and MФARsi conditioned medium+IL-1β antibody group decreased than that in the MФARsc conditioned medium+IL-1β antibody group (all P<0.05). Conclusions:Macrophage AR regulates IL-1β expression by binding to ARE sequence within IL-1β promoter, and IL-1β mediates the effect of macrophage AR on hyperphosphate-induced HASMC calcification.
4.Epidemiological investigation on a cluster epidemic of COVID-19 in a school in Longchuan County, Yunnan Province

ObjectiveTo investigate a cluster epidemic of coronavirus disease 2019 (COVID-19) infections in a school in Longchuan County, Yunnan Province, and further guide the prevention and control of COVID-19 in the border area. MethodsAccording to the Protocol on Prevention and Control of Novel Coronavirus Pneumonia (8th Edition), an epidemiological investigation was performed on all COVID-19 cases to collect the information on demographics, onset, diagnosis and treatment, prognosis, and epidemiological history. Close contacts were also tracked to determine the transmission chains. ResultsIn this cluster epidemic, a total of 37 COVID-19 cases were identified, including 32 females and 5 males aged from 13 to 25 years, who were 35 students and 2 teachers. The student cases were found in four classes of two grades. Furthermore, gene sequencing showed that all cases had been infected with delta variants, belonging to the same transmission chain that was not related to the previous epidemics in Dehong Prefecture. In additionally, a total of 2 127 close contacts were found. After 21 days of centralized quarantine for medical observation, all close contacts tested negative for SARS-CoV-2. In the COVID-19 cases, only one case remained positive for SARS-CoV-2, while the other 36 cases were successfully treated and became negative. ConclusionThis school cluster is caused by the border villagers who contacted the water polluted with SARS-CoV-2. It warrants more strict management of students from border villages and their belongings to prevent similar epidemics in school settings.
5.Efficacy analysis of T?nnis triple osteotomy for developmental dysplasia of the hip in older children

Objective:To explore the outcomes of T?nnis triple osteotomy for developmental dysplasia of the hip (DDH) in older children.Methods:Clinical data of 21 children (22 hips, 12 left and 10 right hips) treated by T?nnis triple osteotomy from October 2016 to June 2019 were retrospectively analyzed.There were 8 males and 13 females with a mean age of operation at 13.5 (8.6-16.8) years.Based on the T?nnis classification, all the cases were in Grade Ⅰ.Statistical analysis was performed using the SPSS 22.0 statistical software.Results:All recruited patients were followed up for 1.5-3.5 years.Compared with preoperative values and those at the last follow-up visit, the mean Sharp acetabular angle of the affected side recovered from 53.5°±5.2° to 40.8°±2.3°.The mean center-edge angle increased from 10.5°±2.9° to 35.4°±5.6°.The mean acetabular roof angle decreased from 38.6°±2.8° to 6.7°±1.3°.The mean acetabular head index enhanced from (54.3±5.2)% to (86.2±2.7)%.The differences between the preoperative and postoperative values were statistically significant ( t=24.3, 17.6, 50.1, 27.5; all P<0.05). According to the Harris classification, there were 17, 3 and 2 hips achieved excellent, good and fair outcomes at the last follow-up visit, respectively, with the percentage of excellent and good outcomes up to 90.9%(20/22 hips). Accor-ding to the Severin radiographic classification, 8, 12 and 2 hips were in grade Ⅰ, Ⅱ, and Ⅲ, respectively, with the percentage of excellent and good hips at 90.9%(20/22 hips). At the last follow-up visit, the bone healing of iliac and pubic bones was well, and the nonunion of ischial bones were reported in 3 cases (13.6%). Conclusion:The outcome of the T?nnis triple osteotomy is satisfactory for DDH in older children.
6.Evaluation of curettage and bone grafting combined with locking compression pediatric hip plate in the treatment of tumor-like lesions of femoral neck in children

Objective:To analyze the curative effect of locking compression pediatric hip plate (LCH-PHP) in the treatment tumor-like lesions of femoral neck in children.Methods:From January 2012 to June 2019, 45 children with Erol type Ⅱ-Ⅳ tumor-like lesions of femoral neck were retrospectively analyzed, involving 32 males and 13 females, aged 2-14 years.Among them, 27 cases were simple bone cyst, 15 cases were fibrous dysplasia, 3 cases were Langerhans histiocytosis, and 17 cases were accompanied with pathological fracture.Patients without pathological fracture were divided into 2 groups according to different surgical methods: lesion removal and bone grafting+ LCH-PHP preventive fixation group (group A) and lesion removal and bone grafting group (group B). Patients with pathological fracture were divided into 2 groups according to the different surgical methods: lesion removal and bone grafting+ LCH-PHP fixation and reduction group (group C) and lesion removal and bone grafting group (group D). The loss of femoral neck-stem angle, healing time and Musculoskeletal Tumor Society (MSTS) scores were compared between group A and group B. Changes of femoral neck-stem angle were compared between group C and group D at postoperative 1 day and the last follow-up.Results:Forty-five patients were followed up for an average of 2.6 (1-4) years.The loss of femoral neck-stem angle in group A and group B were (0.50 ± 0.45)°, and (2.26 ± 1.64)°, which was significantly different ( t=-3.193, P<0.05). There were no significant differences in the healing condition, healing time and MSTS scores at the last follow-up between group A and group B(all P>0.05). In group C, the ipsilateral femoral neck-stem angle at the postoperative 1 day and the last follow-up were (133.67±4.53)°and (133.00±4.32)°, respectively, which was comparable ( P>0.05). In group D, the ipsilateral femoral neck-stem angle at the postoperative 1 day and the last follow-up were (133.36±7.82)°and (130.63±8.24)°, respectively, which was significantly different ( t=7.11, P<0.05). Conclusions:LCH-PHP fixation can effectively maintain the femoral neck-stem angle and prevent the occurrence of pathological fracture children with Erol Ⅱ-Ⅳ tumor-like lesions of femoral neck.
7.Clinical effect analysis of retrograde elastic stable intramedullary nailing in distal humerus metaphysis-diaphyseal junction fractures in children

Objective:To assess the clinical effect of retrograde elastic stable intramedullary nailing(ESIN) in distal humerus metaphysis-diaphyseal junction fractures in children.Methods:A total of 14 cases of children with distal humerus metaphysis-diaphyseal junction fractures treated by retrograde ESIN in Children′s Hospital of Nanjing Medical University from January 2016 to December 2018 were retrospectively analyzed.There were 9 males and 5 females, aged 3.0-13.2 years old(median: 6.4 years old). Nine cases had fractures on the left side and 5 cases on the right side.Two prebent ESINs were inserted into the proximal humerus metaphysis through the humerus marrow cavity from medial and lateral sides of the distal humerus respectively after the closed/open reduction of fractures.After surgery, the shoulder abductor stent or long arm plaster was used for fixation post-operatively.X - ray examination was performed at regular post-operative follow-up.The stability of internal fixation was analyzed by measuring the Baumann angle and checking whether humeral anterior lines passed through capitulum humeri in lateral radiographs immediately after the surgery and at the last follow-up.Meanwhile, efficacy was evaluated according to Flynn functional evaluation criteria.Results:All the 14 patients in this group were followed up for 10-27 months after the operation, with an average of 18.4 months.The Baumann angle was within the normal range at both the end of the operation and the last follow-up, and no significant difference was observed.Lateral radiographs showed humeral anterior lines passed through the capitulum humeri.According to Flynn functional scores, there were 13 excellent cases and 1 good case.No wound infection, intramedullary nailing displacement, Volkmann contracture, cubital varus deformity, or iatrogenic nerve injury occurred in any of the children.Conclusion:Retrograde ESIN technique is a safe and reliable treatment for children with distal humerus metaphysis-diaphyseal junction fractures.
8. Complications of 2 133 cases of pediatric long bone fracture undergoing elastic stable intramedullary nailing in a single medical center

Objective:
To summarize the clinical data of elastic stable intramedullary nailing(ESIN) in the treatment of long bone fracture of children in a single medical center, and to analyze the problems occurred after the ESIN surgery and corresponding solutions.
Methods:
A retrospective analysis was conducted regarding the clinical data of 2 133 pediatric long bone fractures conforming to inclusion and exclusion criteria from June, 2005 to December, 2017 in Department of Orthopedics, Children′s Hospital of Nanjing Medical University.There were 1 191 boys and 942 girls, aged from 23 months to 14 years with mean age of (5.7 ± 3.1)years.There were 1 866 cases treated with closed reduction with ESIN, while 267 cases were treated with small incision assisted reduction with ESIN.Postoperative problems have been statistically analyzed.
Results:
There were altogether 2 133 children, including 603 cases of femur, 311 cases of tibia, 8 cases of fibula, 219 cases of humerus, and 992 cases of ulna/radius.The postoperative complications mainly consist of 62 cases of needle tail irritation reaction, 21 cases of misalignments of fracture alignment, 11 cases of intramedullary nail deformity or angular deformity, 7 cases of limb shortening, 14 cases of limited joint activity, 4 cases of nerve injury, 2 cases of tendon injury, 14 cases of difficult nail removal, 4 cases of cortical cleavage, 8 cases of delayed union, 1 case of nonunion, 6 cases of varus/valgus deformity, 5 cases of epiphyseal injury, 6 cases of ESIN exposure, and 2 cases of metal debris of ESIN′ end.
Conclusions
The complications of treatment for children with long bone fractures by ESIN cannot be ignored.To master the important biomechanical properties, to get familiar with the local anatomy and to avoid obvious technical errors can reduce the occurrence of postoperative complications.
9.Comparison of the Effects and Medication Cost of Tenofovir Disoproxil and Entecavir in the Treatment of Hepatitis B

OBJECTIVE: To compare the effects and medication cost of tenofovir disoproxil and entecavir in the treatment of hepatitis B. METHODS: A total of 300 patients with HBV infection selected from our hospital during Sept. 2015-Sept. 2016 were randomly divided into tenofovir disoproxil group and entecavir group, with 150 cases in each group. Tenofovir disoproxil group was given Tenofovir disoproxil fumarate tablet 300 mg orally, qd. Entecavir group was given Entecavir tablet 0. 5 mg orally, qd, for consecutive 48 weeks. HBV-DNA level, negative conversion rate of HBV-DNA, ALT level and ALT normalization rate were compared between 2 groups before treatment, after 24, 48 weeks of treatment. The medication cost-effectiveness ratio and ADR were compared between 2 groups after 48 weeks of treatment. RESULTS: There were no statistical significances in the level of HBV-DNA between 2 groups before treatment, the level of ALT and ALT normalization rate before treatment, after 24, 48 weeks of treatment (P>0. 05). Compared with entecavir group, the level of HBV-DNA in tenofovir disoproxil group after 24, 48 weeks of treatment was decreased significantly (P<0. 05); the negative rate of HBV-DNA in tenofovir group was slightly higher after 24 weeks of treatment, without statistical significance (P>0. 05); the negative rate of HBV-DNA was slightly increased after 48 weeks of treatment (P<0. 05). Negative conversion rate of HBV-DNA and medication cost-effectiveness ratio of ALT normalization in tenofovir disoproxil group were better than entecavir group (P<0. 05). The incidence of ADR in tenofovir dipivoxil group was lower than entecavir group (P<0. 05). CONCLUSIONS: Tenofovir dipivoxil can effectively treat HBV, and its effects and medication cost are both better than entecavir.
10.Analysis of malaria epidemiological characteristics in Hechi City,Guangxi Zhuang Autonomous Region from 2005 to 2015

Objective To analyze the epidemiological characteristics of malaria in Hechi City,Guangxi Zhuang Autono?mous Region from 2005 to 2015,so as to provide the evidence for formulating and adjusting measures of malaria elimination. Methods The data of malaria cases in 11 counties of Hechi City from 2005 to 2015 were collected and analyzed by using Micro?soft Office Excel 2003 and SPSS 17.0. Results A total of 160 malaria cases were reported in Hechi City from 2005 to 2015, and the annual average malaria incidence was 3.6 per million. There were 10 local recurrence cases(accounting for 6.25%,10/160)and 150 imported cases(accounting for 93.75%,150/160). For the etiology,Plasmodium vivax accounted for 51.87%(83/160)in these cases,P. falciparum accounted for 34.38%(55/160),P. ovale accounted for 1.25%(2/160),P. malariae account?ed for 5.00%(8/160),and the indeterminate accounted for 7.50%(12/160). During the period of 11 years,the malaria inci?dence first dropped and then rose. There were no local cases after 2009. However,an imported falciparum malaria death case was reported in 2010,a severe imported falciparum malaria case was reported in 2013 and another in 2015. A severe case of ma?ternal?neonatal vivax malaria was reported in 2014. These cases were mainly distributed in 10 counties of Hechi City,with more young male adults who engaged in digging mining. More cases concentrated from April to August. Most of the reported malaria cases were imported,60.00%(96/160)of them returned from Africa and Southeast Asia,and 33.75%(54/160)from other do?mestic provinces. The median of the interval from symptom appearance to diagnosis was 5 d,and there was a significant differ?ence among the above years(c2=33.40,P<0.05). Conclusions Malaria is still an important public health problem in Hechi City,and the appropriate control measures and effective tools should be strengthened for malaria elimination. The key to consoli?date the achievements of malaria control is strengthening the malaria monitoring management of the floating population.
