1.Quantitative analysis of macular capillaries in diabetic patients using optical coherence tomography angiography

AIM: To quantify early changes of macular capillary parameters in type 2 diabetic patients using optical coherence tomography angiography(OCTA).METHODS: Retrospective case study. A total of 49 healthy subjects, 52 diabetic patients without retinopathy(noDR)patients, and 43 mild nonproliferative diabetic retinopathy(mNPDR)patients were recruited. Capillary perfusion density, vessel length density(VLD), and average vessel diameter(AVD)were calculated from macular OCTA images(3 mm×3 mm)of the superficial capillary plexus after segmenting large vessels and the deep capillary plexus. Parameters were compared among control subjects, noDR, and mNPDR patients. The area under the receiver operating characteristic curve estimated the abilities of these parameters to detect early changes of retinal microvascular networks.RESULTS: Significant differences were found in the VLD and AVD among the three groups(P<0.001). Compared with the control group, the noDR group had significantly higher AVD(P<0.05). VLD of both layers in patients of mNPDR group was significant decreased compared with that of noDR group(all P<0.01). Deep AVD had a higher area under the curve(AUC)of 0.796 than other parameters to discriminate the noDR group from the healthy group. Deep AVD had the highest AUC of 0.920, followed by that of the deep VLD(AUC=0.899)to discriminate the mNPDR group from the healthy group.CONCLUSIONS: NoDR patients had wider AVD than healthy individuals and longer VLD than mNPDR patients in both layers. When compared with healthy individuals, deep AVD had a stronger ability than other parameters to detect early retinal capillary impairments in noDR patients.
2.Research progress on the role of inflammatory factors in the occurrence and development of myopia

Exploring the pathogenesis of myopia is of positive significance for its prevention and control.In recent years,studies have found that inflammatory factors such as interleukin,tumor necrosis factor and nitric oxide are highly expressed in eyes of myopic patients,indicating that myopia is associated with inflammatory factors.The inflammatory fac-tors interact with nuclear factor-Kb,peroxisome proliferator-activated receptor,nucleotide-binding oligomerization domain-like receptor protein 3 and other inflammatory signaling pathways and participate in myopia progression by regulating the retinal proinflammatory state,choroidal blood flow and scleral matrix remodeling.This paper reviews the regulation of in-flammatory factors and related signaling pathways,providing a new perspective for further exploring the causes of myopia and intervening its progression.
3.Bibliometric analysis of myopia research from 2013 to 2022 based on Web of Science

Objective:To analyze the current status and hotspots of myopia research in the past 10 years and the future development direction based on the bibliometric method.Methods:Relevant original research and reviews on myopia published from January 1, 2013 to December 31, 2022 were retrieved from the Web of Science core database.Co-occurrence analysis of countries, research organizations, and authors was performed using the VOSviewer software.Cluster analysis of keywords and co-cited references was performed using the CiteSpace software.Results:A total of 9 745 articles were included, involving 123 countries/regions, 7 150 institutions, and 29 343 authors.The analysis revealed that the global publication in myopia has been increasing, with China having the highest publication, and research from the United States being the most cited.Keyword analysis showed that the early hotspots of myopia mainly focused on refractive surgery, diagnosis and treatment of complications, genetics, and epidemiological characteristics, while recent research has rapidly shifted towards myopia prevention and control.The results of the cluster analysis of the co-cited references showed that myopia research contained several clusters, such as #0 school-aged children, #1 small incision lenticule extraction, #2 myopia control, #3 refractive error, #4 contact lenses, etc.The current research frontier focuses on myopia management technology, myopia and retinal and choroidal vessels, and the application of artificial intelligence in myopia.Conclusions:Myopia research in the past 10 years covers a wide range of disciplines, including ophthalmology, molecular biology, genetics, optometry and epidemiology.In the future, there is a need to further explore the etiology and pathogenesis of myopia, enhance early identification and screening methods, improve management techniques for myopia, advance artificial intelligence-assisted diagnosis of myopia to develop more effective and safe strategies for myopia prevention and control.
4.Recommendations for the diagnosis and treatment of rheumatic fever in China

Rheumatic fever is an autoimmune disease characterized by recurring acute or chronic systemic connective tissue inflammation caused by group A streptococcal infection in the throat. Although rheumatic fever is common in China, there is a lack of standardized criteria for the diagnosis and treatment of this condition. Based on evidence and guidelines from China and other countries, the Chinese Rheumatology Association developed standardized criteria for the diagnosis and treatment of this disease in China. The aim was to standardize rheumatic fever diagnosis methods, treatment opportunities, and strategies for both short-and long-term treatment, so as to reduce irreversible damage and improve prognosis.
5.Application of CBL combined with 3D printing teaching in clinical teaching of sacral tumors

Objective:To explore the application effect of case-based learning (CBL), teaching mode combined with 3D printing in clinical teaching of sacral tumors.Methods:A total of 108 undergraduate interns and standardized residency training students who studied in our hospital from 2017 to 2018 were divided into the CBL teaching group ( n = 53) and the CBL combined with 3D printing teaching group ( n = 55) according to their study time. The combined teaching group used computer tomography (CT) data to reconstruct and print out a 3D model of sacral tumors based on CBL, and performed preoperative teaching on the invasion of the surrounding tissues of the tumor. The scores of the students in the two groups were evaluated respectively, and the students were surveyed by self-identification questionnaire (learning interest, self-learning ability, teamwork ability, comprehensive analysis ability and clinical thinking ability). The t-test (one-sided) was used for comparison between groups using stata 14.0. Results:The score of CBL teaching group (75.90±6.70) was lower than that of CBL combined with 3D printing teaching group (83.60±7.40). In terms of critical thinking ability evaluation, self-learning ability, learning interest, comprehensive analysis ability and clinical thinking ability, the CBL combined 3D printing teaching group was superior to the CBL teaching group, and the difference was statistically significant ( P<0.001). In terms of teamwork ability, there was no statistical difference between the two groups. Conclusion:The CBL teaching mode combined with 3D printing can improve academic performance, students' learning interest and clinical thinking ability of sacral tumors in the teaching of undergraduate interns and standardized residency training students.

Chest trauma is one of the most common injuries. Venous thromboembolism (VTE) as a common complication of chest trauma seriously affects the quality of patients′ life and even leads to death. Although there are some consensus and guidelines on the prevention and treatment of VTE at home and abroad, the current literatures lack specificity considering the diagnosis, treatment and prevention of VTE in patients with chest trauma have their own characteristics, especially for those with blunt trauma. Accordingly, China Chest Injury Research Society and editorial board of Chinese Journal of Traumatology organized relevant domestic experts to jointly formulate the Chinese expert consensus on the diagnosis, treatment and prevention of chest trauma venous thromboembolism associated with chest trauma (2022 version). This consensus provides expert recommendations of different levels as academic guidance in terms of the characteristics, clinical manifestations, risk assessment, diagnosis, treatment, and prevention of chest trauma-related VTE, so as to offer a reference for clinical application.
7.Efficacy of somatosensory evoked potential monitoring for prevention of deep venous thrombosis in lower extremity of patients undergoing spinal surgery

Objective:To evaluate the efficacy of somatosensory evoked potential (SEP) monitoring for prevention of deep venous thrombosis (DVT) in lower extremity of patients undergoing spinal surgery.Methods:A total of 120 patients of both sexes, aged 40-70 yr, of American Society of Anesthesiologists physical status ⅠorⅡ, without impairment of coagulation function, undergoing posterior lumbar interbody fusion, were selected and divided into 3 groups ( n=40 each) using the random number table method and envelope method: basic preventive measure group (group A), compression stocking group (group B) and SEP monitoring group (group C). In group B, elastic stockings were worn at 1 h before surgery.SEP monitoring was performed during surgery, and the intensity of current stimulation was 25 mA in group C. The flow velocity of popliteal vein in both lower extremities was measured using color Doppler ultrasound apparatus during surgery.Venous blood samples were taken to measure plasma D-dimer concentrations at 1 day before surgery, at the end of surgery and at 24 h after surgery.The DVT developed in lower extremities was diagnosed by ultrasound method during surgery and within 24 h after surgery. Results:Compared with A and B groups, the blood flow velocity of popliteal vein was significantly increased, and the plasma D-dimer concentration and incidence of DVT developed in lower extremity during surgery were decreased in group C ( P<0.05). Conclusion:SEP monitoring can effectively prevent the occurrence of DVT in lower extremity while it is used as an electrophysiological monitoring of the spinal cord in patients undergoing spinal surgery.

9.Effects of heated medical elastic stockings on the prevention of deep venous thrombosis and hypothermia of lower limbs during posterior lumbar intervertebral fusion

Objective To develop a kind of heated medical elastic stockings to prevent the occurrence of deep vein thrombosis (DVT) and hypothermia in patients with lumbar degenerative diseases undergoing posterior lumbar intervertebral fusion (PLIF).Methods A total of 180 patients with lumbar degenerative diseases undergoing PLIF from February 2015 to August 2017 were selected by purposive sampling method, and divided into routine group (group A), normal elastic stockings (group B), heated medical elastic stockings (group C) according to the sequence of operation time, with 60 cases in each group. On the premise of routine thermal protection measures and DVT basic preventive measures, group A did not wear stretch socks, group B wore normal elastic stockings, and group C wore heated medical elastic stockings. Patients in the group B and C were treated with normal and heated stretch stockings during anesthesia preparation 1 hour prior to surgery. The incidence rate of lower extremity DVT and hypothermia in each group was calculated by monitoring deep veins blood flow velocity of both lower limbs, D-dimer, foot temperature and the core temperature at each time node during the operation.Results In group C: left femoral superficial vein velocity was (17.47±3.07) cm/s; right femoral superficial vein velocity was (16.95±3.25) cm/s; left popliteal vein blood flow velocity was (16.03± 3.37) cm/s; right popliteal vein blood flow velocity was (16.37±3.39) cm/s. All the results above were higher than those in group A and B (P<0.01). The D-dimer were (587±129) μg/L at the end of surgery and (392± 119)μg/L 24 h after surgery in group C, which were significantly lower than group A and B (P<0.01). The incidence of DVT at the end of operation and 24 hour postoperation in group A was 8.3% and 15.0%, while 5.0% and 5.0% in group B respectively. There was no DVT occurrence in group C at the two time points, and the difference was statistically significant (P<0.01). Comparing foot temperature and core temperature of each time point, it was significantly higher in group C than group A and B (P<0.05). There were 8 cases of hypothermia in group C, and the incidence rate was 12.5%, which were both significantly lower than that in group A (56.7%) and group B (40.0%) (P<0.01).Conclusions The heated elastic stockings can effectively reduce the risk of DVT and hypothermia in the patients with lumbar degenerative diseases undergoing PLIF and can be popularized in clinical practice.
10.Effect of TREM-1 expression on the apoptosis of intestinal macrophages of rats

Objective To investigate the effect of triggering receptor expression in myeloid cells-1 (TREM-1) on intestinal macrophage apoptosis in rat.Methods In vitro,the achieved rat intestinal macrophages were divided into 3 groups:control group,LPS (Lipopolysaccharides) group and LPS + LP17 group (n =6 holes of culture plate in each).The concentrations of LPS and LP17 were 1 mg/L and 0.1 mg/L,respectively.The intestinal macrophage apoptosis was measured by using TUNEL kit and flow cytometry after culture for 6 h.All data were statistically analyzed using SPSS 18.0 software.Results The shape and growth of rat intestinal macrophages were quite favorable after culture.The membrane marker of intestinal macrophages,CD14 was clearly observed under immunofluorescence.After macrophage was treated with specific procedure,the cell apoptosis found in LPS group (44.33 ± 7.74)% was significantly higher than that in control group (19.17 ± 6.01) % (P=0.000) measured by TUNEL;the cell apoptosis in LPS +LP17 group (28.33 ± 6.53)% apparently reduced compared with LPS group (44.33 ±7.74) % (P =0.004);there was no significant difference in cell apoptosis between control group (19.17 ± 6.01) % and LPS + LP17 group (28.33 ± 6.53) % (P =0.050).By flow cytometry,the apoptotic cells in LPS group (16.47 ± 1.66) % was significantly increased compared with control group (7.70 ± 1.52) % (P =0.000);apoptotic cells in LPS + LP17 group (11.47 ± 3.12) % was significantly reduced in comparison with LPS group (16.47 ± 1.66) % (P =0.018).There was no significant difference in apoptotic cells between control group (7.70±1.52)% and LPS + LP17 group (11.47±3.12) % (P =0.061).Conclusion LP17 can inhibit TREM-1 expression in intestinal macrophages and reduce intestinal macrophage apoptosis.
