1.Relationship Among Fungal Diversity at Different Levels of Root System of Stellaria dichotoma var. lanceolata, Soil Properties, and Main Components of Medicinal Materials in Lithosol Habitats

		                        			ObjectiveTo explore the interaction among root fungi of Stellaria dichotoma var. lanceolata, soil factors, and main components of medicinal materials in lithosol habitats. MethodHigh-throughput sequencing technology was employed to determine the fungal community of the root system of S. dichotoma var. lanceolata at different levels (bulk soil, rhizosphere soil, rhizoplane soil, and root interior) and the soil properties of the root system (bulk and rhizosphere), and the relationship among the fungal community, soil properties, and the main components of medicinal materials was analyzed. ResultThe total phosphorus, available phosphorus, alkaline nitrogen, dissolved organic carbon, and soil water content in the rhizosphere soil of S. dichotoma var. lanceolata were slightly higher than those in the rhizosphere, but the difference was not significant. Ascomycota is the dominant phylum of root fungi in S. dichotoma var. lanceolata. In the progressive level of bulk-rhizosphere-rhizoplane-root interior system, although the fungal diversity gradually decreased, the abundance of Hypocreales, a new phylum (unclassified_ k_ Fungi), Helotiales, and Natipusilales gradually increased, among which Hypocreales is the most important fungal group in the root system of S. dichotoma var. lanceolata. The structural equation model (SEM) shows that the physicochemical factors of the root-soil play an important regulatory role in the fungal community and the main components of medicinal herbs, with soil total nitrogen, alkaline nitrogen, soil water content, and pH being the main regulatory factors. Soil nitrogen content is the key to promoting the main components of the medicinal herbs, and Penicillium fungi are the key fungal group to regulate the main components of the medicinal herbs. ConclusionIt highlights that the physicochemical properties of the soil of S. dichotoma var. lanceolata play a crucial role in the fungal community and the components of medicinal materials. Hypocreales fungi in the root of S. dichotoma var. lanceolata were an important group, and Penicillium fungi had a certain role in mediating the components of medicinal materials. 
		                        		
		                        		
		                        		
		                        	
2.The application value of spectral CT venography in the display and staging of deep venous thrombosis in the lower extremity

		                        			Objective To investigate the application value of spectral computed tomography venography(CTV)in the display and staging of deep venous thrombosis(DVT)in the lower extremity.Methods Eighty-two patients with CTV were selected and ran-domly divided into group A(42 patients)and group B(40 patients).Group A:tube voltage 120 kVp.Group B:gemstone spectral ima-ging(GSI)mode,reconstruction of 50 keV and iodine(water)maps.The CT and standard deviation(SD)values of the veins were measured,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)were calculated in 120 kVp images of group A and in 50 keV images of group B.Two observers scored the image quality of the 2 groups subjectively,and Kappa test was used to examine the con-sistency.Based on the duration from the occurrence of clinical symptoms,the DVTs were classified.The CT values and iodine con-centration(IC)of DVT were measured in the 120 kVp images of group A and in the iodine(water)maps of group B,respectively.The receiver operating characteristic(ROC)curve was drawn to compare the effectiveness of CT values and IC in diagnosing DVT staging.Results CT values,SNR,and CNR of veins in group B were higher than those in group A(P<0.05).The subjective scores of the two groups were consistent(Kappa=0.926-0.955,P<0.05).The score for the display of veins and thrombus clarity in group B was 5(4,5),which was better than the score of 4(3,4)in group A(P<0.05).The efficiency of IC in diagnosing DVT staging[area under the curve(AUC)=0.973]was better than that of CT values(AUC=0.891).Conclusion The spectral CTV can improve the contrast of lower extremity deep veins and the clarity of thrombus,and can provide more objective indicators for the diagnosis of thrombus staging,which is conducive to accurate clinical diagnosis and treatment.
		                        		
		                        		
		                        		
		                        	
3.Efficacy of PD-1 inhibitors combined with nab-paclitaxel and cisplatin in the neoadjuvant treatment of locally advanced hypopharyngeal squamous cell carcinoma

		                        			Objective:To assess the efficacy of neoadjuvant treatment with PD-1 (programmed cell death protein 1) inhibitors combined with paclitaxel (albumin-conjugated) and cisplatin (TP regimen) for locally advanced hypopharyngeal squamous cell carcinoma and laryngeal organ function preservation.Methods:Data of 53 patients, including 51 males and 2 females, aged 38-70 years old, who were diagnosed with locally advanced hypopharyngeal squamous carcinoma confirmed by histology and enhanced CT at the Cancer Prevention and Control Center of Sun Yat-sen University during the initial treatment from January 1, 2019 to January 15, 2023, were retrospectively analyzed. All patients received neoadjuvant therapy with PD-1 inhibitors combined with albumin-bound paclitaxel (260 mg/m 2) and cisplatin (60 mg/m 2) for 3 to 4 cycles. The main outcome measures were larynx dysfunction-free survival (LDFS), overall survival (OS), and progression-free survival (PFS). Survival curves were plotted using the Kaplan-Meier method, and Cox multifactorial analysis was further performed if Cox univariate analysis was statistically significant. Results:The overall efficiency was 90.6% (48/53). The 1-year and 2-year LDFS rates were 83.8% (95% CI: 74.0% to 94.8%) and 50.3% (95% CI: 22.1% to 91.6%), the 1-year and 2-year OS rates were 95.2% (95% CI: 88.9% to 100.0%) and 58.2% (95% CI: 25.6% to 81.8%), and the 1-year and 2-year PFS rates were 83.9% (95% CI: 74.2% to 94.9%) and 53.5% (95% CI: 32.1% to 89.1%). Adverse events associated with the neoadjuvant therapy were mainly myelosuppression (45.3%), gastrointestinal reactions (37.7%) and hypothyroidism (20.8%). Conclusion:The neoadjuvant treatment of locally advanced hypopharyngeal squamous cell carcinoma using PD-1 inhibitors combined with paclitaxel and cisplatin can provide with a higher survival rate with a improved laryngeal organ function preservation rate.
		                        		
		                        		
		                        		
		                        	
4.Effectiveness and safety of autologous fat particle transplantation in facial rejuvenation: a meta analysis

		                        			Objective:To explore the effectiveness and safety of autologous fat granule transplantation for facial rejuvenation.Methods:Databases including PubMed, Cochrane Library, Google Scholar, Web of Science, China Biology Medicine Database, Wanfang Database, and CNKI were searched. The search time range was from 2013 to 2023. Search key words included autologous fat transplantation, facial rejuvenation, efficacy, effectiveness, safety, complications, etc. Meta-analysis was used. Literature search was conducted by using tool method and forward search method. OR (95% CI) was used as the statistical measure for efficacy analysis. Heterogeneity of literature was tested by I2 test. Publication bias was tested by Begg′ s test. Results:A total of 23 literatures were included, covering 2 852 beauty seekers. There was no significant publication bias. The satisfactory rate of beauty seekers after autologous granular fat transplantation was relatively high ( OR=0.95, 95% CI: 0.91-0.97). The success rate of one-time injection was relatively high ( OR=0.79, 95% CI: 0.73-0.84). The incidence of postoperative complications was relatively low ( OR=0.02, 95% CI: 0.01-0.04). 5.MDT treatment model for diagnosis and treatment of retroperitoneal soft tissue sarcoma in the context of a regional medical center: report of 1 case

		                        			Objective:To explore the clinical value of multidisciplinary team (MDT) treatment model for retroperitoneal soft tissue sarcoma (RPS) in the context of a regional medical center.Methods:The diagnosis and treatment of a patient with RPS in MDT model who were admitted to Xiamen Branch, Zhongshan Hospital, Fudan University in October 2020 were summarized, and the literature was reviewed.Results:The patient was a 59-year-old male, he was diagnosed with retroperitoneal INI-1-deficient poorly differentiated sarcoma with multiple systemic metastases. After several rounds of MDT discussions in Shanghai headquarter and Xiamen Branch of Zhongshan Hospital, Fudan University, the patient received systemic antitumor therapy combined with local therapy. After active treatment, the patient obtained a good curative effect. During the follow-up period, the liver and lung lesions slowly enlarged, and the treatment plan was adjusted. The patient had survived for more than 3 years.Conclusions:The diagnosis and treatment of RPS is complex, and the MDT model in the context of a regional medical center can help to formulate the optimal treatment plan for patients and may maximize the survival benefit of patients.
		                        		
		                        		
		                        		
		                        	
6.Analyses of low-dose chemotherapy effect and prognostic factors in advanced gastrointestinal cancer patients with weak physical condition

		                        			Objective:To investigate the clinical therapeutic effect of low-dose chemotherapy in advanced gastrointestinal cancer patients with weak physical condition, and to evaluate the related factors affecting the prognosis.Methods:The clinical data of 118 advanced gastrointestinal cancer patients with weak physical condition who were admitted to Xiamen Branch, Zhongshan Hospital, Fudan University from March 2018 to May 2019 were retrospectively analyzed. Kaplan-Meier method was used to make survival analysis in patients stratified by different factors, and log-rank was used to test. Multiple factor analysis of prognosis was performed by using Cox regression model. The association of clinicopathological factors with the prognosis was evaluated.Results:Among 118 patients, 16 (13.6%) cases were esophageal cancer, 41 (34.7%) cases were gastric cancer, 48 (40.7%) cases were colorectal cancer, 13 (11.0%) cases were pancreatic cancer. The overall survival (OS) rates of 6 months and 1-year were 44.1% and 10.2%, respectively. Survival analysis showed that patients with hemoglobin ≥100 g/L, serum albumin > 40 g/L and normal gastrointestinal function had better OS (all P < 0.05), while patients with physical status score of 3 scores and pain had worse OS (all P < 0.05). Cox multivariate analysis showed that after adjusting age and gender, hemoglobin level ( HR = 0.314, 95% CI 0.241-0.977, P = 0.001), pain ( HR = 2.016, 95% CI 1.697-7.038, P = 0.008) and gastrointestinal function ( HR = 1.751, 95% CI 1.607-6.080, P = 0.036) were independent influencing factors of OS in advanced gastrointestinal cancer patients with weak physical condition after receiving low-dose chemotherapy. Conclusions:Low-dose chemotherapy can still benefit advanced gastrointestinal cancer patients with weak physical condition. 7.Study on the Improvement Effects of Sijunzi Decoction on Chronic Kidney Disease-protein Energy Wasting Model Mice and Its Mecranism

		                        			OBJECTIVE:To study the improvement effects of Sijunzi decoction on skeletal muscle atrophy in chronic kidney disease-protein energy wasting (CKD-PEW)model mice ,and to explore its potential mechanism . METHODS :A total of 80 mice were randomly divided into sham operation group (n=10)and modeling group (n=70). CKD-PEW model was established by removing 5/6 kidneys and giving a low-protein diet (4% casein)for mice in modeling group. Totally 50 modeled mice were randomly divided into model group ,Sijunzi decoction low-dose ,medium-dose and high-dose groups [ 2.34,4.68,9.36 g/(kg·d), by crude drug] ,Compound α-ketoacid tablets group [positive control ,1 g/(kg·d)],with 10 mice in each group. Administration groups were given relevant medicine intragastrically ;sham operation group and model group were given constant volume of normal saline intragastrically ,once a day ,for consecutive 14 d. After last medication ,body weight of mice and wet mass of left tibialis anterior muscle (TA)were weighed ;TA cross-sectional area was determined ;protein synthesis and decomposition metabolism ability of TA were detected ;mRNA expressions of Bcl-2,Bax and Caspase-3 in TA were detected by Real-time PCR ;protein expressions of muscular dystrophin Fbox- 1 (Atrogin-1),myofloin-1 (MuRF-1),Rho-related protein kinase 1 (ROCK1), phosphorylated PTEN (p-PTEN),phosphatidylinositol-3-kinase(PI3K)and phosphorylated Akt (p-Akt)in TA were detected by Western blotting. RESULTS :Compared with the sham operation group ,the body weight ,TA wet weight ,protein synthesis metabolism ability of TA as well as protein expressions of PI 3K and p-Akt were decreased significantly in model group (P<0.05); the cross-sectional area of TA decreased significantly (P<0.05);protein decomposition metabolism ability of TA ,Bax/Bcl-2 ratio, Caspase-3 mRNA expression ,protein expressions of Atrogin- 1,MuRF-1,ROCK1 and p-PTEN were increased significantly (P< 0.05). Compared with model group ,above indexes of mice were all improved significantly in Sijunzi decoction medium-dose , high-dose groups and Compound α-ketoacid tablets group (P<0.05). CONCLUSIONS :Sijunzi decoction can increase the body weight of CKD-PEW model mice and alleviate the skeletal atrophy ;the mechanism may be related to regulating ROCK 1/PTEN/Akt signaling pathway activity ,inhibiting the expression of Atrogin- 1 and MuRF- 1.
		                        		
		                        		
		                        		
		                        	
8.Improvement Effects of Water Extract of Astragalus membranaceus on Chronic Renal Failure Model Rats and Its Effects on MAPK Signaling Pathway

		                        			OBJECTIVE: To investigate the improvement effects of water extract of Astragalus membranaceus on chronic renal failure (CRF) model rats and its effects on MAPK signaling pathway, and to investigate possible mechanism. METHODS: Male SD rats were randomly divided into control group (10 rats) and model group (50 rats). CRF model was established by intragastric administration of 25% Adenine suspension 200 mg/kg (once a day, for consecutive 28 d). After modeling, modeling group was randomly divided into model group, benazepril group (positive control, 2 mg/kg), A. membranaceus water extract low-dose, medium-dose and high-dose groups (1.5, 3, 6 g/kg,by crude drug), with 10 rats in each group. Control group and model group were given constant volume of normal saline intragastrically, and administration groups were given relevant medicine intragastrically, once a day, for consecutive 28 d. Twelve hours after last medication, the contents of serum renal function indexes (serum creatinine, urea nitrogen, uric acid) were determined by colorimetry. ELISA method was used to detect the levels of serum inflammatory factors (IL-1β, IL-6 and TNF-α). The activity or content of oxidative stress related indexes (SOD, CAT and MDA)in renal tissue of rats were determined by hydroxylamine method, visible spectrophotometry method and thiobarbituric acid method. The mRNA expression of apoptosis-related factors (Bax, Bcl-2, Caspase-3) in renal tissue, MAPK signaling pathway-related regulatory protein p38 MAPK, phosphorylated p38 MAPK(p-p38 MAPK), ERK1/2, phosphorylated ERK1/2 (p-ERK1/2), JNK and phosphorylated JNK (p-JNK) were determined by real- time PCR and Western blotting assay. RESULTS: Compared with control group, the contents of renal function indexes and the levels of inflammatory factors in serum, MDA content, mRNA expression ratio of Bax and Bcl-2 (Bax/Bcl-2), mRNA related expression of Caspase-3, related expression of phosphorylation product of MAPK signaling pathway-related regulatory proteins (p-p38 MAPK, p-JNK, p-ERK1/2) in renal tissue were increased significantly in model group, while the activities of SOD and CAT were decreased significantly in renal tissue (P<0.01). Compared with model group, the contents of renal function indexes and the levels of inflammatory factors in serum, Bax/Bcl-2, related expression of phosphorylation product of MAPK signaling pathway-related regulatory proteins in renal tissue were decreased significantly in administration groups as well as MDA content and mRNA related expression of Caspase-3 were decreased significantly in benazepril group and A. membranaceus water extract medium-dose and high-dose groups; the activities of SOD and CAT in renal tissue were increased significantly in benazepril group and A. membranaceus water extract medium-dose and high-dose groups (P<0.05 or P<0.01). MDA content and Bax/Bcl-2 of benazepril group were significantly lower than those of A. membranaceus water extract high-dose group (P<0.05). CONCLUSIONS: A. membranaceus water extract has a certain improvement effect on CRF model rats, and inhibit its inflammatory reaction, oxidant stress and cell apoptosis, the mechanism of which may be associated with inhibiting the expression of MAPK signaling pathway-related regulatory proteins.
		                        		
		                        		
		                        		
		                        	
9.Intrauterine transfusion for fetal hemolytic disease caused by erythrocyte alloimmunization: a retrospective study

		                        			Objective To investigate the outcomes of fetuses with hemolytic anemia caused by red cell alloimmunization following intrauterine transfusion (IUT),and to analyze the influence of hydrops fetalis on IUT treatment.Methods A retrospective analysis was conducted on 70 fetuses,who were admitted to the Fetal Medicine Center,the First Affiliated Hospital of Sun Yat-sen University from January 2005 to May 2018,with hemolytic disease requiring IUT.Clinical data of the fetuses and the gravidas were collected and divided into hydrops group (17 cases) and non-hydrops group (53 cases) based on their conditions before IUT.Results of routine blood tests before and after the first IUT,gestational age at the first IUT,prognosis and outcomes of the fetuses were compared between two groups.t-test,rank-sum test,Chi-square test (or Fisher's exact test) and multivariant logistic regression analysis were used for data analysis.Results Totally,the 70 fetuses underwent 231 times of IUT.Compared with the non-hydrops group,the hydrops group had a significantly increased incidence of severe anemia [14/17 vs 47.2% (25/53),x2=6.458,P=0.011],but decreased hemoglobin [(38.5 ± 21.4) vs (68.7± 19.3) g/L,t=5.471,P<0.001] and hematocrit level [0.110 (0.044-0.246) vs 0.222 (0.077-0.299),Z=-4.390,P<0.001] before the first IUT.After the IUT,the survival rate of the fetuses in hydrops group was significantly lower than that of the non-hydrops group [11/15 vs 94.3% (50/53),P=0.038].There was no significant difference in gestational age at birth,birth weight,neonatal hemoglobin level at birth,the incidence of exchange transfusion,the number of blood transfusions required or the incidence of severe neonatal complication between the two groups (all P>0.05).Logistic regression analysis indicated that the fetal hydrops was an independent risk factor for fetal survival (OR=12.8,95%CI:1.2-136.4,P=0.035).Conclusions Hydrops fetalis might reduce the survival rate of fetal hemolytic disease after 1UT.
		                        		
		                        		
		                        		
		                        	
10.Synthesis of vitamin K2 by isopentenyl transferase NovA in Pichia pastoris Gpn12.

		                        			The effect of methanol addition on the heterologous expression of isoprenyl transferase NovQ was studied in Pichia pastoris Gpn12, with menadione and isopentenol as precursors to catalyze vitamin K2 (MK-3) synthesis. The expression of NovQ increased by 36% when 2% methanol was added every 24 h. The influence of initial pH, temperature, methanol addition, precursors (menadione, isopentenol) addition, catalytic time and cetyltrimethyl-ammonium bromide (CTAB) addition were explored in the P. pastoris whole-cell catalytic synthesis process of MK-3 in shaking flask. Three significant factors were then studied by response surface method. The optimal catalytic conditions obtained were as follows: catalytic temperature 31.56 ℃, menadione 295.54 mg/L, catalytic time 15.87 h. Consistent with the response surface prediction results, the optimized yield of MK-3 reached 98.47 mg/L in shaking flask, 35% higher than that of the control group. On this basis, the production in a 30-L fermenter reached 189.67 mg/L when the cell catalyst of 220 g/L (dry weight) was used to catalyze the synthesis for 24 h.
