1.Application of traction with titanium clips in endoscopic submucosal dissection for large laterally spreading tumor in rectum and sigmoid colon

Objective To investigate the advantages and efficacy of traction with titanium clips in endoscopic submucosal dissection(ESD)for large laterally spreading tumor(LST)in rectum and sigmoid colon.Methods 67 patients with large sigmoid or rectal LST underwent ESD from January 2018 to June 2022 were analyzed retrospectively,including 32 patients in Group A and 35 patients in Group B.Group A was treated with clip-line traction and group B was treated with traditional ESD.The size of lesion,the total operation time,the submucosal dissection time,submucosal dissection rate,submucosal injection number,en bloc resection rate,R0 resection rate,curative resection rate and complications of the two groups were compared.Results LST-G-M was the most common type and villous adenoma was the main pathology in both groups.There were no differences in en bloc resection rate,R0 resection rate and incidence of complications between the two groups.The average size of group A was(13.6±8.4)cm2,significantly larger than that in group B(9.3±4.7)cm2,the total operation time was(42.3±10.3)min in group A,significantly shorter than that in group B(47.9±10.1)min,submucosal dissection time was(30.7±8.2)min in group A,significantly shorter than that in group B(36.1±7.6)min,submucosal injection number was(2.7±1.1)times in group A,significantly less than that in group B(3.5±1.2)times,submucosal dissection rate was(0.4±0.2)cm2/min in group A,significantly faster than that in group B(0.2±0.1)cm2/min,the differences were statistically significant(P<0.05).Conclusion Compared with traditional ESD,clip-line traction can provide a better surgical field and more effective dissection for large LST in rectum and sigmoid colon.

Objective To explore the application value of artificial intelligence(AI)combined with multi-slice spiral CT in screening pulmonary nodules in health examination population in govern-ment departments.Methods A retrospective analysis was conducted on the chest CT screening data of health examination population in government departments who had at least diameter of one nodule ≥3 mm.All data were analyzed using AI technology combined with manual film reading.The detection rate of pulmonary nodules was recorded.Based on the risk values predicted by AI technology,the nod-ules were divided into different risk groups,and the basic characteristics of the nodules were analyzed and compared.Results The detection rate of pulmonary nodules was 60.4%.High-risk patients were more likely to present as pure ground glass nodules or mixed ground glass nodules.The average age of individuals with pulmonary nodules was higher than those without(P<0.05).There were sig-nificant differences in the basic characteristics of pulmonary nodules,including nature,morphology,internal signs,and external signs among different risk groups(P<0.05).Conclusion Multi-slice spiral CT examination of the chest scan is helpful for screening pulmonary nodules in large-scale physi-cal examination populations.The combination of AI technology and manual film reading can improve the accuracy of pulmonary nodule screening.

Occupational therapy can not only promote the development of motor and cognitive functions in children with developmental or other disabilities, but also improve their self-care ability, learning ability, participation skills and other activities.In 2019, the Aust Occup Ther J published a systematic review on the effects of occupational therapy interventions on disabled children, aiming to summarize the best evidence of occupational therapy and help occupational therapists and families to choose the effective treatment regimens.In this article, the systematic review was interpreted in light of the current status of domestic child occupational therapy, so as to allow clinicians to compare the pros and cons of different treatment methods and improve the efficacy.
5.Status and influencing factors of palliative care needs in patients with end-stage heart failure

Objective:To explore the status quo of palliative care needs of patients with end-stage heart failure, and analyze its influencing factors.Methods:This study was a cross-sectional study. From December 2019 to December 2020, convenience sampling was used to select 83 patients with end-stage heart failure in a Heart Failure Treatment Center of a Class Ⅲ Grade A hospital in Qingdao as the research object. The General Information Questionnaire and the Palliative Care Needs Scale for Patients with End-stage Heart Failure were used to investigate the patients. Multiple linear regression was used to analyze the influencing factors. A total of 83 questionnaires were distributed and 76 valid questionnaires were returned with the valid response rate of 91.57%.Results:Among 76 patients with end-stage heart failure, the total score of the Palliative Care Needs Scale for Patients with End-stage Heart Failure was (110.87±20.77) , of which the information needs dimension score (29.61±6.69) was the highest. The results of multiple linear regression analysis showed that the serum sodium concentration, the number of comorbidities, and the educational background were the influencing factors of the palliative care needs of patients with end-stage heart failure ( P<0.05) , which explained 30.6% of the total variation of the palliative care needs. Conclusions:Patients with end-stage heart failure have a high demand for palliative care. Nursing staff should provide personalized palliative care for patients with end-stage heart failure according to their own characteristics.
6.Molybdenum cofactor deficiency type B manifested as Leigh-like syndrome: a case report and literature review

Objective:To explore the phenotypes and genotypes of molybdenum cofactor deficiency type B (MoCD-B) manifested as Leigh-like syndrome.Methods:The clinical data, laboratory tests, neuroimaging and gene results of one patient diagnosed as MoCD-B at Beijing Children′s Hospital and Hebei Children′s Hospital in December 2018 were collected. Related literature was searched and reviewed at Wanfang Data Knowledge Service Platform, China National Knowledge Infrastructure and PubMed (up to September 2020) by using terms "MOCS2" "molybdenum cofactor deficiency" "Leigh-like syndrome,MOCS2" "molybdenum cofactor deficiency, Leigh-like syndrome". The phenotypes and genotypes of MoCD-B were summarized.Results:A 7 months and 14 days old boy with the chief complaint of "cough for 6 days, abnormal posture for 4 days and fever for 2 days" was admitted to Hebei Children′ Hospital on December 2018. His abnormal posture presented as opisthotonos accompanied with dysphagia, without seizures. His previous psychomotor development was described as normal. He was born at term after an uneventful pregnancy to non-consanguineous parents. Blood test showed a slightly increased lactic acid and a significantly decreased uric acid. Urine metabolism test showed an obviously increased xanthine and hypoxanthine. Brain magnetic resonance imaging showed hyperintense signal on T2 weighted image and fluid attenuated inversion recovery in bilateral globus pallidus and pedunculus cerebri. The patient was diagnosed with Leigh-like syndrome. No obvious improvement was achieved after cocktail therapy and symptomatic treatment. The whole exome sequencing showed that the patient carried a homozygous variant of MOCS2 gene, c.19G>T(p.Val7Phe), which was a previously reported pathogenic site in the literature and could cause MoCD-B. His parents carried a heterozygous variant respectively. A total of 41 MoCD-B cases with MOCS2 gene variants were collected through literature review and our study, among which 30 cases had full medical records. The onset ages of 23 (77%) cases were in neonate, manifesting with severe encephalopathy, including neonatal-onset intractable seizures, developmental delay, laboratory abnormalities included very low levels of serum and urinary uric acid, increased urinary levels of xanthine and hypoxanthine. Cranial imaging showed cerebral atrophy, cystic encephalomalacia, etc. The onset ages of 7 patients varied from 5 months to 23 years. Four cases had normal psychomotor development before disease onset. Neurological disorders appeared acutely or exacerbated after external triggers and all of them had basal ganglia involvement. Among the 30 cases, 3 cases had a relatively milder phenotype with the ability of brief communication and walking without or with support.Conclusions:Molybdenum cofactor deficiency is a rare disease. Most cases had severe phenotypes and poor outcomes, but some cases may have mild phenotype. MoCD-B caused by MOCS2 gene variants may manifest as Leigh-like syndrome with a normal psychomotor development before the trigger of infection strike. Hypouricemia, xanthinuria and hypoxanthinuria can be indicators of the disease. The presence of MOCS2 gene variants would confirm a final diagnosis.
7.Effects of patient participation -based dietary intervention on nutritional status for patients with severe burns

Objective:To explore the influence of patient participation-based dietary intervention on nutritional status for patients with severe burn.Methods:From September 2017 to January 2019, 60 severe burn patients hospitalized in the department of burn and plastic surgery of Qingdao Municipal Hospital were recruited and divided into the experimental group ( n=30) and the control group ( n=30) according to the random number table method. The control group received a regular diet. While the experimental group received a patient participation-based dietary intervention(PPDI), The wound healing time and the value of nutritional status index, such as height, body mass index(BMI), serum albumin, serum prealbumin, in the two groups on admission, at 2 weeks after intervention, 4 weeks after intervention were compared. The nutritional knowledge questionnaire of burn patients and "3-day diet diary" were used for investigation. Results:The score of nutritional knowledge showed no significant difference between the two groups before intervention. After intervention, the score of nutritional knowledge in the experimental group was 21.40±2.42, significantly higher than that in the control group (19.00±2.26) ( t value was 3.975, P<0.01). For time effect and between-group effect, there were significant difference in serum albumin between two groups ( F values were 9.232, 4.651, P<0.05); the time effect and between-group effect of serum prealbumin were statistically significant ( F values were 11.592, 6.228, P < 0.05). The wound healing time in the experimental group was significantly lower than that in the control group ( t value was -3.801, P<0.01). Conclusions:Patient participation-based dietary intervention can effectively enhanced the level of nutritional knowledge, improved nutritional status, shortened wound healing time among patients with severe burn.
8.Status quo of knowledge, attitude and practice of nurses on prevention of deep vein thrombosis in patients during perioperative period

Objective:To investigate the status quo of knowledge, attitude and practice of perioperative related nurses on prevention of deep vein thrombosis in patients, so as to provide basis for in-depth study of the whole perioperative period and continuous nursing process.Methods:A stratified and convenient sampling method was used to select 905 surgical nurses and operating room nurses in 10 tertiary hospitals and 13 secondary hospitals as the objects of the survey. A self-made questionnaire on the status quo of knowledge, attitude and practice of nursing staff to prevent deep vein thrombosis during perioperative period was investigated.Results:The scores of knowledge, attitude and practice of 905 perioperative related nurses on prevention of deep vein thrombosis were respectively (8.40±2.58) , (38.26±12.53) and (52.79±13.10) . Multiple linear regression analysis showed that the gender of nurses, the title of head nurse in the department and whether the hospital had a thrombosis-prevention nursing group were the common factors affecting the status quo of knowledge, attitude and practice of perioperative related nursing staff in prevention of deep vein thrombosis ( P<0.05) . Conclusions:The knowledge attitude and practice scores of perioperative related nurses in prevention of deep vein thrombosis are low, which should be further improved. It is necessary to construct the perioperative specialized nursing training system by referring to the theory of knowledge, attitude and practice.

10.Epidemiological characteristics of cerebral palsy in twelve province in China

Objective To investigate the epidemiological characteristics of cerebral palsy(CP)in children aged 1-6 years in China,including the incidence,prevalence,type of CP,etiology,prevention and rehabilitation status. Methods The survey was carried out by standard questionnaires,multi-center collaboration,stratified-cluster ran-dom sampling method.The surveyed adopted the following principles:streets in the city and villages in the rural areas, and the number of the urban and rural children was the same,and the proportion of children in each age group was balanced.The investigation areas included provinces and autonomous regions,including Heilongjiang,Beijing,Henan, Shandong,Shanxi,Shaanxi,Anhui,Hunan,Guangxi,Guangdong,Chongqing and Qinghai,and 323 858 children were in-vestigated.Results The incidence of CP was 2.48‰(155/62 591 cases),and the prevalence was 2.46‰(797/323 858 cases)(1-6 years old).The prevalence varied in different regions,in which the highest prevalence was 5. 40‰(54/9 998 cases)in Qinghai province,and the lowest prevalence was 1.04‰(47/45 133 cases)in Shandong province.The prevalence of the males(2.64‰,461/174 391 cases)was higher than that of the females(2.25‰, 336/149 467 cases),and the difference was statistically significant(P<0.05).The types of CP were spastic type (58.85%,469/797 cases),mixed type(13.17%,105/797 cases),dyskinetic(9.79%,78/797 cases),hypotonic (8.28%,66/797 cases),ataxia(6.25%,52/797 cases)and rigid(3.39%,27/797 cases)respectively in 797 CP children.The first three risk factors for CP were long -term exposure to harmful physical factors during pregnancy, whether there were birth defects among the three generations of relatives of the children,such as children's peers, parents or grandparents,whether there were birth defects among the children's peers,parents or grandparents,and neonatal jaundice or persistent jaundice.Among 797 CP children,79.67% of the children with CP were timely detected and treated in the local hospitals,while the other 19.93% of them were not timely treated.The places which could give them timely detection and early diagnosis and treatment were general hospitals(42.97%),Maternity and Infant Hospitals (27.03%)and Children's Hospitals(20.31%). The main rehabilitation methods for 797 children with CP were 34.58% in the hospitals or rehabilitation centers,31.61% in the communities(including at home),33.80% mainly in the medical institution,and in the communities they could also receive partially rehabilitation services. Conclusions The prevalence of CP in China is coincident with international levels.The prevalence rate of CP in males is higher than that in females.The types of CP distribution are accorded with international distribution characteristics.There were still some children with CP who could not receive timely detection and treatment.Rehabilitation at the medical institutions is the chief way and proper rehabilitation guidance should be carried out in the communities.
