1.Visualization research hotspots and trends of foodborne diseases in China based on CiteSpace in 1985 -2022

Objective To understand the research status of foodborne diseases in China from 1985 to 2022, and to explore the development process, research hotspots and frontier trends in this field. Methods With CNKI database as the search source, CiteSpace 6.1.R2 was used to analyze domestic research literature on foodborne diseases from 1985 to 2022. The author and organization cooperation map, and keyword co-occurrence and keyword timeline map were generated to comprehensively analyze the characteristics of foodborne diseases as well as research hotspots and cutting-edge trends in this field in China. Results A total of 2526 valid articles were obtained by exclusion criteria. According to the time distribution of articles from 1985-2022, the number of articles published before 2000 was small, and the annual number of articles published since 2000 had significantly increased. The largest number of articles was published by the China National Center for Food Safety Risk Assessment (51 articles), followed by the National Institution for Nutrition and Health of Chinese Center for Disease Control and Prevention (CDC) (38 articles). Most of the studies were conducted by national or government level research institutions in cooperation with provincial disease control and prevention centers. There was a close cooperation among different agencies. By the keyword cluster analysis, it was found that monitoring, Salmonella, and food safety were the concentrated research areas. The burst detection of keywords showed that food poisoning, sentinel hospital, and epidemic characteristics had the strongest citation burst. In recent years, the research hotspots were serotyping, drug resistance, virulence genes and so on. These keywords could reflect the investigation speed and laboratory level from a perspective. Conclusion The research on foodborne diseases in China is constantly increasing, and the research focus is gradually shifting from simple monitoring to improving the speed of outbreak investigation and laboratory level and speeding up the molecular tracing network to prevent more foodborne diseases.
2.Matrix Stiffness Affects Mitochondrial Heterogeneity of Tibial Plateau Chondrocytes in Knee Osteoarthritis

Objective To investigate the difference of matrix stiffness in different regions of tibial plateau in osteoarthritis (OA) and its effects on morphology of the cartilage and mitochondria. Methods The tibial plateau cartilage specimens of OA were obtained for nanoindentation test, transmission electron microscopy and histological analysis. The stiffness of cartilage matrix in different regions of OA tibial plateau was detected by nano-indentation. The morphology of cartilage mitochondria in different regions was observed by transmission electron microscopy, and the changes of mitochondrial plane area, shape and ridge volume density were quantitatively analyzed. Cartilage injury in different regions of OA tibial plateau was observed by histological staining. Results The cartilage of OA tibial plateau showed regional heterogeneity, and the cartilage and mitochondria on medial side of varus knee OA were more severe, and the matrix stiffness was higher. The OA scores were positively correlated with matrix stiffness. There was also a significant correlation between OA scores and mitochondrial morphology: the higher OA scores, the larger and rounder mitochondrial plane area, and the lower cristae volume density. Conclusions The differences of tibial plateau revealed the correlation between cartilage matrix stiffness, OA scores and mitochondrial morphological parameters. The increased cartilage matrix stiffness may be the main cause of chondrocyte mitochondrial injury, and further aggravate the progression of OA.
3.Satisfaction with training program and loyalty to the university among professional postgraduate students majoring in clinical medicine

Objective:To investigate the current status of satisfaction with training program and loyalty to the university among professional postgraduate students majoring in clinical medicine, as well as the association between satisfaction with training program and loyalty to the university.Methods:The 2017 National Medical Student Satisfaction Survey Database was used. A total of 1 944 professional postgraduate students in the second or third year, as well as those with delayed graduation, from 59 postgraduate training colleges and universities who participated in clinical internship were selected. SPSS 20.0 software was used for analysis; descriptive analysis was used to describe satisfaction with training program and loyalty to the university; factor analysis was adopted to calculate comprehensive satisfaction score to reduce the number of variables; logistic regression analysis was applied to investigate the association of general information and satisfaction with training program with loyalty to the university.Results:The professional postgraduate students majoring in clinical medicine had a degree of 60.44% of loyalty to the university. Satisfaction with training program reflected low satisfaction at each link of the training program. For every 1-point increase in the comprehensive satisfaction scores of the four links of courses, research training, college support, and practice, the loyalty to the university was increased to 2.11, 1.83, 1.77, and 1.75 times as the original, respectively, of the baseline scores.Conclusion:There is still room for further improvement in the satisfaction with training program and the loyalty to the university among professional postgraduate students majoring in clinical medicine, and the satisfaction with training program is closely associated with the loyalty to the university. Colleges and universities need to take measures for courses, practice, research training, and college support, so as to improve satisfaction and thus enhance loyalty to the university.

Breast cancer is the most common malignant tumor among women, and more and more patients choose breast reconstruction after mastectomy. Breast reconstruction plays an important role in improving patient self-confidence and quality of life. Implant-based breast reconstruction is the most common technique, subpectoral breast reconstruction combined with acellular dermal matrix is commonly used in surgical procedure. However, the postoperative pain, shoulder dysfunction and dynamic deformity and other complications caused by pectoralis major muscle dissection and elevation have attracted the attention of many plastic surgeons. In recent years, more and more plastic surgeons in Europe and the United States have reconsidered prepectoral (subcutaneous) breast reconstruction and observed good short-term postoperative results. We reviewed the implant pocket plane of breast reconstruction.

The detection of allergen is essential for the prevention and treatment of allergic diseases. Clarifying the aetiology and risk factors of allergic diseases is critical in order to provide targeted environmental control and therapy for allergic diseases, as well as reduce patient pain and financial burden. In this study, allergen detection technologies and diagnosis strategies are described in order to provide direction for clinicians and laboratory technicians, improve allergic diseases diagnosis, and guide allergic disease therapy.

8.Analysis of regional research competitiveness of medical subjects in China

Objective To analyze the current status of research competitiveness of medical subjects in China.Methods Analyzing the fund of Natural Science Foundation of China (NSFC) in 2017 and using the Range ratio and Theil Index to describe the dispersion and source of difference.Regional research competitiveness of Chinese medical subjects is measured by the attraction and accumulation capacity of NSFC.Results The research competitiveness of medical subjects in East China and South China are stronger,and those in the Northwest are weaker.The distribution of medical funds mainly originated from the interior (88.88%),among which the East China and North China are the two regions with the largest contribution rate.In terms of provinces,Shanghai and Beijing have a much higher concentration of medical funds than other provinces.Condusions The Research competitiveness in inter-regional medical subjects in China is quite different,and the differences mainly stem from the competition of internal provinces;the degree of advantages of medical subjects in regions is different,and there are medical research centers in ever regions.
9.Analysis of human resources allocation of ophthalmic nurses in China

Objectives? To describe the human resource allocation of ophthalmic nurses in China, and to compare the differences based on the ophthalmic nurse availability among different regions so as to put forward policy recommendations. Methods? We used the latest data of China Ophthalmic Competency Resource Survey in 2015 and China Population and Employment Statistics Yearbook to describe the quantity, education backgrounds and professional titles of ophthalmology nurses in different regions of China. Population-weighted quartiles of the county-level number were defined and used to analyze the differences in education backgrounds and professional titles among different regions and different levels of availability. Results? By the end of 2014, the number of ophthalmic nurses was 3.19 per 100 000. The eastern part had 30% more nurses than the western and the urban area had 285% more than the rural area. The ratio of ophthalmology doctors to ophthalmic nurses is 1∶1.13 while the eastern area had the lowest ratio. Most of the ophthalmic nurses held post-secondary diploma and only entry-level titles. The availability of ophthalmic nurses in the east China was better than any other parts. It was easier to get an ophthalmic nurse in the cities than in rural areas. And the condition got worst in western rural districts. The proportion of nurses with a bachelor's degree went higher with the increase in availability, while this trend reverted with regard to the proportion of senior titled nurses. With the same availability, the quality of human resources in western and rural areas is the worst. Conclusions? The resource allocation of ophthalmic nurses in China has gradually improved, but there are still problems such as regional inequality and large urban-rural gap. The availability analysis can show the inter-regional difference in terms of the quantity of ophthalmic nurses in a more detailed manner, such that the situations in the urban and rural areas are in extremely differentiated states; The quality of ophthalmic nurses in China needs to be further improved, and the quality difference will further aggravate the imbalance of resource allocation.
10.Impact of target energy intake on outcomes in septic patients: a prospective randomized controlled trial

Objective To observe the impact of the diverse caloric energy intake on the outcomes and occurrence rate of complications in septic patients.Methods A prospective single-blind randomized controlled trial was conducted.158 cases of septic patients in intensive care unit (ICU) were enrolled and randomly assigned to three groups according to their different target value of nutrition:group A [measurements of resting energy expenditure (MREE)<90%],B (MREE 90%-110%) and C (MREE > 110%).The caloric intake,mechanical ventilation duration (MVD),nosocomial infection rate,28-day and 60-day mortality were analyzed.Results Daily energy intake in 7 days after ICU admission was as follows:the difference in target value of nutrition(kJ/d:7 075.0 ± 1 046.5,5 667.8 ± 1 908.8,4 428.8 ± 1 377.8),calory intake (kJ/d:4 671.6 ± 1 205.6,5 655.3 ± 1 373.0,6 053.0 ± 1 557.2),enteral nutrition value (kJ/d:2 051.1 ± 1 046.5,3 980.9 ± 1 586.5,5 337.1 ±2 921.8) and average intake rate [(66.0 ± 15.8)%,(100.0 ± 5.7)%,(134.0 ± 19.7)%],and they were statistically significant difference among A,B,C groups (all P<0.05).The parenteral nutrition in group C were much higher than that in group A and group B (kJ/d:2 055.3 ± 273.4vs.427.0 ± 273.4,473.0 ± 332.0,both P<0.05).The calories provided by glucose and diprivan were similar among three groups.The MVD and ICU stay were shorter in group B than that in groups A and C [MVD (days):8.4 ± 6.3 vs.11.0 ± 8.2,17.8 ± 13.0,P> 0.05 and P< 0.05 ; ICU stay (days):11.0 ± 6.4 vs.14.9 ± 9.6,17.8 ± 13.0,respectively,P>0.05 and P<0.05].The total hospital stay (days:32.0 ± 22.5,26.8 ± 7.0,30.4 ± 21.4) and nosocomial infection rate [91.1% (51/56),84.0% (42/50),90.4% (47/52)] were similar among A,B,C groups (all P>0.05).There was no difference in survival rate at 28 days among three groups as shown by the Kaplan-Meier survival curve (F=3.145,P=0.076).The survival rate at 60 days showed a tendency of lowering in groups A and C,especially in group C (F=9.284,P=0.010).Conclusion Both higher and lower caloric energy intake may be associated with an adverse impact,but appropriate caloric intake would improve the outcome and reduce the complication rate in septic patients.
