1.A cross-sectional survey and analysis of influencing factors of humanistic of the current status of humanistic care ability of burn specialist nurses.

Objective: To investigate the current status of humanistic care ability of burn specialist nurses and to analyze the influencing factors. Methods: A single-center cross-sectional research method was conducted. From May to August 2020, 63 burn specialist nurses who met the inclusion criteria in Ruijin Hospital affiliated to Shanghai Jiao Tong University School of Medicine were selected. Self-made general data questionnaire was used to investigate 17 indexes, including gender, age, professional title, working years, whether received humanistic care training, academic qualification, and caring ability inventory (CAI) was used to evaluate their humanistic care ability. After the nurses were classified by the general data, independent sample t test and one-way analysis of variance were performed on the data to analyze the total score of CAI. The CAI total scores and scores of cognition, courage, and patience of the nurses were compared with the international norm. The factors with statistically significant differences in unvariate analysis were selected for multiple linear regression analysis to screen the independent influencing factors of humanistic care ability of burn specialist nurses. Results: A total of 63 questionnaires were collected in this survey, all of which were valid. Among the 63 nurses, there were 4 males and 59 females, with the age mainly ranging from 20 to 30 years (30 nurses, 47.62%), the professional titles mainly being nurse practitioner (36 nurses, 57.14%), the working years mainly being more than 10 years (28 nurses, 44.44%), 32 nurses not receiving humanistic care training, and academic qualifications mostly being junior college (37 nurses, 58.73%). There were significant differences in the total scores of CAI among nurses with different ages, professional titles, working years, whether received humanistic care training, and academic qualifications (with F values of 53.95, 49.14, and 75.42, t values of 6.08 and -2.82, respectively, P<0.01). The scores of cognition, courage, and patience and the total scores of CAI of nurses in this group were significantly lower than those of international norm (with t values of -2.02, -2.04, -6.19, and -3.89, respectively, P<0.05 or P<0.01). Multiple linear regression analysis showed that age, working years, professional title, and whether received humanistic care training were the independent influencing factors of humanistic care ability of burn specialist nurses (with 95% confidence intervals of 1.91-23.23, 16.25-31.48, 1.05-19.09, and 6.72-31.82, unstandardized coefficient values of 12.57, 23.86, 10.07, and 19.27, respectively, P<0.05 or P<0.01). Conclusions: The humanistic care ability of burn specialist nurses is relatively weak. Age, professional title, working years, and whether received humanistic care training are the independent influencing factors of humanistic care ability of burn specialist nurses.

2. Role of Cx43 hemichannel in spinal cord dorsal horn in rat acute incision pain

Aim To study the role of Cx43 hemichannel in rats with acute incision pain. Methods Adult male rats were randomly divided into normal saline group(C group), incision pain group(I group), Gap19 group. Western blot was used to determine the expression of Cx43 and GFAP at 6 h, 3 d and 7 d after paw incision surgery of rats. The rats were intrathecal injected with Gap19 30 min before incision surgery, followed by the measurement of the paw withdrawal threshold to mechanical stimuli at several time points. Immunofluorescence was used to detect the expression of Cx43 and GFAP. ELISA was performed to detect the different inflammatory cytokine levels in rats after surgery. Results Compared with group C, the expression of Cx43 and GFAP of rats increased significantly 6 h after incision surgery in group I, while their expression at postoperative 3 d and 7 d showed no obvious alteration. Compared with the preoperative baseline, the mechanical paw withdrawal threshold in I group and Gap19 group rats was significantly reduced at 2 h, 6 h, 24 h and 3 d post-incision(P<0.01), but it had no obvious change on postoperative 7 d. Compared with group I, the mechanical paw withdrawal threshold in rats of Gap19 group increased 2 h, 6 h, 24 h post-incision, while it showed no statistical difference on 3 d and 7 d post-incision. Compared with group C, immunofluorescence results showed that the expression of GFAP and Cx43 in spinal cord dorsal horn astrocytes also increased significantly in Group I 6 h after incision surgery. Compared with group I, GFAP and the expression of Cx43 were markedly reduced in Gap19 group. There was no statistical difference on 7 d post-incision. Compared with those in group C, the inflammation factors including IL-1β, IL-6 and TNF-αobviously increased in group I. However, in contrary to the rats in group I, the rats pretreated with gap19 showed increased expression of Cx43 and GFAP in spinal cord dorsal horn. Conclusions Specific inhibition of Cx43 hemichannel significantly suppresses astrocyte activation and alters the inflammatory microenvironment in the spinal cord dorsal horn and reduces postoperative hyperalgesia in rats with acute incision pain.
3.Clinical efficacy of laparoscopic modified Parks operation on the ultra-low rectal cancer.

To investigate the clinical efficacy of laparoscopic modified Parks operation on the patients with ultra-low rectal cancer.

Laparoscopic modified Parks operation is a safe, economical and effective anus preservation operation, which can not only save the anus for some patients who had to perform Miles operation, but also recover better and faster after operation. Although the early anal function of patients performed with the modified operation is poor, it can gradually recover to the same level as the patients performed with the Dixon operation.

4.Generic Association Analysis on "Prescription-Formula-Dosage-Property" of Tibetan Medicine Based on Partial Order Structure and Property Theory

Objective:In view of the complexity of the "Prescription-Formula-Dosage-Property" relationship of Tibetan medicine and the outstanding common relationship,it is difficult to reveal the hidden and specific rules of clinical medication of Tibetan medicine. Method:Based on the attribute partial order structure and the vector structure model of "Ro-Nus-Zhu-Rjes"(Taste,Post-Digestive Tastes and Potency),clustering analysis and other methods and software,this study analyzed the "Prescription-Formula-Dosage-Property" relationship of 184 commonly used prescriptions in the 1995 edition of the standards issued by the Ministry of Tibetan medicine(SIMTM). Result:Among them,the analysis of the relationship between prescription and formula found that 11 prescriptions with the largest common attribute,such as Chebulae Fructus and Carthami Flos,were the key components of classification and compatibility,which could effectively classify the 8 kinds of prescriptions for the treatment of lung disease,tripa disease and blood fever. Among them,the san Yin and auxiliary viscera function prescriptions,such as Sanguotang powder and Liuwei Liangyao powder,had the strongest commonality. According to the analysis of relationship between formula and dosage ,the dosage of Chebulae Fructus,Carthami Flos,pomegranate seed,Phyllanthi Fructus was the highest,which suggested that these drugs were often used as primary drugs,while the Liuwei Liangyao powder,such as Amomi Fructus Rotundus and Tsaoko Fructus,had a higher frequency but a lower dose,which mainly played a role in regulating the overall drug property of the prescription and protecting the viscera. The Tibetan medicine-specific drugs including Moschus,Bovis Calculus,and Zhaxun,which were used in a high frequency but very low dose,had the effect of enhancing the drug property and guiding the affected part. According to the analysis of the relationship between dosage and property,there were many prescriptions belonging to cool nature,accounting for 75.6%. It was found that 67 prescriptions did not conform to the efficacy due to their different dosage. Conclusion:There are many common components and common usages in Tibetan medicine prescriptions. If these common associations are not treated,it will lead to the result that all diseases take these common associations as the core,but the hidden key factors cannot be solved. Therefore,it is necessary to pay attention to sensitivity and specificity at the multi-dimensional level of "prescription-formula-dosage-property",so as to reveal the clinical medication thought of Tibetan medicine more effectively.
5.Effects of smoking on chronic obstructive pulmonary disease and respiratory symptoms

Objective To investigate the effects of smoking on chronic obstructive pulmonary disease(COPD) and respiratory symptoms. Methods A multi-stage, stratified cluster sampling strategy was used to select participants aged 40 or older in 5 surveillance points of Anhui Province. Questionnaires, body measurements and spirometry were used to collect data. Based on complex sampling design, Logistic regression model was conducted to analyze the effects of smoking on COPD and respiratory symptoms. Results The smokers who had smoked for ≥30 pack-years accounted for 13.9% (95% CI:10.3%-17.5%, P<0.001) of the total population. And the smokers who had smoked for ≥40 years accounted for 8.5% (95% CI:6.7%-10.3%, P<0.001) of the total population. On average, one smoker had smoked for 32.4 years (95% CI:31.2-33.5). Average daily cigarette consumption of daily smokers was 21.1 cigarettes (95% CI:19.6-22.7). As shown by multiple-variables Logistic regression analyses, the risk of COPD and respiratory symptoms increased with the increment of smoking pack-years and duration (all Ptrend <0.001). Conclusions Smoking was associated with COPD and respiratory symptoms. The risk of developing COPD and respiratory symptoms was greater with the increment of smoking pack-years and duration.
6.Effect of long chain non-coding RNA H19 on the migration and invasion of oral cancer cells and its molecular mechanism.

OBJECTIVE:
To investigate the effect of the long chain non-coding RNA H19 (lncRNA H19) on the invasion and migration of oral cancer cells and its related molecular mechanism.
METHODS:
The expression levels of lncRNA H19, miR-107, and cyclin-dependent kinase 6 (CDK6) in the immortalized oral epithelial cell line HIOEC and the oral cancer cell line CAL27 were detected by real-time quantitative polymerase chain reaction. CAL27 cells were transfected with siRNA H19, miR-107 mimics, pcDNA H19, or anti-miR-107, and the effects of H19 and miR-107 on the invasion and migration of cells were examined via Transwell assay. The TargetScan database predicted the targeting of H19, miR-107, and CDK6. Double luciferase reporter gene assay was performed to detect interactions among H19, miR-107, and CDK6. Western blot analysis was conducted to examine the effects of H19 and miR-107 on the protein level of the target gene CDK6.
RESULTS:
Compared with that in HIOEC cells, the expression of H19 was significantly increased in CAL27 cells (P<0.05). After transfection with siRNA H19, the expression of H19 decreased, and the invasion and migration ability of CAL27 cells were inhibited (P<0.05). H19 could bind specifically to the 3'-UTR of miR-107 to modulate the expression of miR-107. Compared with that in HIOEC cells, the expression of miR-107 significantly decreased in CAL27 cells (P<0.05). The expression of miR-107 increased after transfection with siRNA H19, and anti-mir-107 co-transfection could promote the invasion and migration ability of siRNA H19 in CAL27 cells (P<0.05). Compared with that in HIOEC cells, CDK6 expression significantly increased in CAL27 cells (P<0.05), and the expression level of the gene was coregulated by H19 and miR-107 (P<0.05).
CONCLUSIONS
lncRNA H19 plays an important role in the development of oral cancer. It can regulate the invasion and migration of oral cancer cells by targeting the miR-107/CDK6 signaling axis.

7.Application of 3D printing models to clinical teaching in otolaryngology

Objective To apply three-dimensional(3D) printing techniques to the clinical anatomy teaching of otolar-yngology. Methods We reconstructed the three-dimensional models of temporal bones and paranasal sinuses based on CT scan data,and divided them into sub-models according to several interested planes on CT scan. Sub-models were printed with 3D printer.The CT images of interested planes were printed by normal printer and pasted to the appropri-ate sub-models. We also checked the accuracy of the anatomy models. Results We successfully made the digital and 3D printing models of temporal bones and paranasal sinuses. The CT scan images were compounded to the models. The key anatomy structures outside and inside of the temporal bone and paranasal sinuses can be shown clearly and exactly on the models. Conclusions The 3D printing models help the medical students to understand and master the clinical anatomy and radiology knowledge of otolaryngology as a supplement for traditional teaching methods.
8.The relationship between interleukin-32 and gestational diabetes mellitus

Objective To investigate the concentration of interleukin-32 (IL-32) in serum,and the expression of IL-32 mRNA in placenta,subcutaneous fat and great epiploon adipose tissue,and explore the relationship between IL-32 and gestational diabetes mellitus (GDM).Methods The concentrations of serum IL-32 in 42 GDM (GDM group) and 38 non-GDM (control group) pregnant women were examined by enzyme linked immunosorbent assay (ELISA).The level of the IL-32mRNA in the placenta,subcutaneous fat and great epiploon adipose tissue was examined by quantitative realtime reverse-transcription polymerase chain reaction (RT-qPCR).The expression levels of IL-32 protein in the placenta and umbilical cord were measured by immunohistochemical staining (IHC) and analyzed by ImagePro Plus 9.0.Results The serum level of IL-32 in the GDM group was significantly higher than that in the control group [(129.3 ± 5.78) pg/mL vs.(105.6 ± 8.61) pg/mL,P<0.05].The level of IL-32 mRNA was increased 1.32 and 1.66 fold in the placenta (P<0.05) and the great epiploon adipose tissue (P<0.05) from GDM women,compared with that from control group.No significant difference was found in the levels of IL-32mRNA in the subcutaneous adipose tissue between the two groups [(3.78 ± 0.53) vs.(3.61 ± 0.35),P>0.05].The expression of IL-32 in the placenta from GDM group was 1.27 times higher than that from control group (P <0.05).The IL-32 in the umbilical artery and umbilical vein of GDM group increased by 1.30 and 1.32 (P<0.05).But there was no significant difference of IL-32 expression in the umbilical interstitial between the two groups.Conclusions Overexpression of IL-32 in the serum and tissue may be involved in the pathogenesis of GDM.
9.Factors Related to Ventricular Size and Valvular Regurgitation in Healthy Tibetans in Lhasa.

BACKGROUNDLhasa is the main residence of Tibetans and one of the highest cities in the world. Its unique geography and ethnic population provide the chance to investigate the interactions among high altitude, ethnicity, and cardiac adaptation. Meanwhile, echocardiographic data about healthy Tibetans on a large scale are not available. This study aimed to analyze physiological factors related to ventricular size and valvular function in healthy Tibetans in Lhasa.
METHODSA representative sample of residents in Tibet was recruited using a multistage cluster random sampling method. Two-dimensional echocardiographic measurements and Doppler evaluation for valvular function were performed. Healthy Tibetans in Lhasa constituted the study population. Associations between physiological parameters and ventricular dimensions in healthy Tibetans were analyzed by canonical correlation analysis. Factors related to valvular regurgitations were determined by logistic regression analysis.
RESULTSThe 454 healthy Tibetans (340 females and 114 male) in Lhasa were included in the final analysis. Canonical correlation analysis revealed that weight was positively correlated with the proximal right ventricular outflow diameter and the basal left ventricular linear dimension in both genders. Weight and pulse were negatively related to mild tricuspid regurgitation. Age was a positive factor for pulmonary and aortic regurgitations. The same was found between systolic blood pressure and mitral regurgitation.
CONCLUSIONSWeight is associated with ventricular size and valvular regurgitation in healthy Tibetans. It should be of more concern in research of high altitude population.
10.Clinical application of tibial composite saphenous nervetissue flap for the treatment of traumatic bone-skin defect of leg.

OBJECTIVETo investigate the effect of tibial composite saphenous nerve tissue flap in repairing traumatic bone-skin defect of leg.
METHODSFrom February 2009 to May 2015, 23 patients with focile skin defect caused by trauma were treated, including 18 males and 5 females, aged from 8 to 59 years old with an average age of 35.6 years old. Length of tibia bone cortex defect ranged from 4 to 12 cm, width ranged from 2 to 2.5cm, skin defect ranged from 8 cm× 3 cm to 18 cm× 11 cm, all the wound surfaces had purulent exudation. Cross shift and ipsilateral shift methods of tibial composite saphenous nerve tissue flap were used to reconstruct continuity of the injured limb tibial brace and repair wound surface. Cutting flap ranged from 9 cm× 4 cm to 20 cm × 12 cm, bone flap length was 6 to 15 cm, width was 1.5 to 2.5 cm. Fracture healing time and recovery of limb function were observed.
RESULTSAt 3 to 5 months after operation, bone flap and area of affected area were healed with an average of 4.7 months. Twenty-three cases were followed up for 6 months to 5 years with an average of 30.3 months. Thirteen patients were able to walk without abnormal gait and could be engaged into original work. According to Enneking evaluation systems, 13 patients obtained excellent results, 8 moderate and 2 poor.
CONCLUSIONSWith tibial composite saphenous nerve tissue flap transplantation at stage one to repair tibia and skin defects according to specific condition of affected limbs has a high rate of success and creates less damage to region of limb injury, which is an ideal method to repair skin defect of focile.
