1.Analysis of research status of pneumoconiosis severity assessment indicators based on literature bibliometric

ObjectiveTo conduct a bibliometric analysis on the research status of occupational pneumoconiosis (hereinafter referred to as "pneumoconiosis") severity assessment indicators. Methods The domestic and foreign articles on the research of pneumoconiosis severity assessment indicators were accessed from China National Knowledge Infrastructure, Wanfang Data, VIP Database, China Biomedical Literature Service System, PubMed, Cochrane Library, and Web of Science. The methodological quality evaluation and analysis of severity assessment indicators were performed with the relevant articles. Results A total of 88 relevant articles on pneumoconiosis severity assessment indicators were included. The overall evaluation of the literature with good-, moderate-, and poor-quality articles accounted for 18.18%, 69.32%, and 12.50%, respectively. The median sample size reported in each article was 86 cases. The articles reporting the stage of pneumoconiosis accounted for 81.82%, and 80.68% reported the types of pneumoconiosis which was mainly simple silicosis and coal worker's pneumoconiosis. Only 12 articles reported two or more types of pneumoconiosis. A total of 122 severity assessment indicators in four categories were reported in 88 articles, including 99 physiological and biochemical indicators, 10 imaging indicators, six symptoms and signs indicators, and seven other indicators. The articles used a single severity assessment indicator to assess the severity of pneumoconiosis accounted for 76.14%, while 23.86% of the articles used multiple severity assessment indicators, and only 5.68% of the articles selected specific severity assessment indicators for pneumoconiosis patients in different stages. Conclusion The quality of research on pneumoconiosis severity assessment is relatively low. The applicability of the combined use of severity assessment indicators is poor and confused.
2.Establishment of a Traditional Chinese Medicine Syndrome Diagnostic Model Based on Stacking Ensemble Learning:Take Lung Cancer as an Example

ObjectiveTo explore the method of optimizing the performance of traditional Chinese medicine (TCM) syndrome diagnostic models using Stacking ensemble learning. MethodsTaking the construction of TCM syndrome diagnostic model for lung cancer as an example, 2598 cases of clinical symptoms and signs from lung cancer patients in 9 hospitals were used as independent variables (i.e., feature variables), TCM syndrome information as dependent variables, and the clinical data were divided into training set and testing set in 8:2 ratio according to random number table method using Python 3.7 software. The stable features of TCM syndrome of lung cancer were screened using chi-square test, Spearman's correlation test, and Least Absolute Shrinkage and Selection Operator (LASSO) logistic regression analysis; nine machine learning algorithms are trained, including support vector machines (SVMs), k-nearest neighbors (KNN) algorithm, Random Forest (RF), Extremely Randomized Trees, Extreme Gradient Boosting (XGBoost), Lightweight Gradient Boosting (LightGBM), Adaptive Boosting (AdaBoost), Gradient Boosting (GB) and the multi-layer perceptron (MLP), to obtain 9 basic models. Four models with better performance were screened out from the above basic models and fused to form a fusion model by using the Stacking ensemble learning, and the fusion model was trained twice by the above nine machine learning algorithms and evaluated by accuracy rate, micro-average ROC curves, area under the curve (AUC), and confusion matrix metrics, to screen the optimal diagnostic model. ResultsAfter data processing, 79 stable features and 13 TCM syndromes were obtained. In the basic model training, the comprehensive performance of RF, ExtraTrees, MLP and SVM basic models were better, so the predicted distributions of the syndromes of these four models were used as the secondary training data, and nine fusion models were obtained based on the Stacking ensemble learning (SVM, KNN, RF, ExtraTree, XGBoost, LightGBM, GB, AdaBoost, MLP). Among them, the XGBoost fusion model performed the best, with an accuracy of 0.850 and 0.838 in the training set and test set, respectively, an overfitting difference of 0.012, and an area under the micro-average ROC curve of 0.996. All fusion models showed an improvement in accuracy and area under the micro-average ROC curve compared with the base model in the test set. ConclusionTaking the TCM syndrome information of lung cancer as an example, the XGBoost fusion model has significant advantages in improving the diagnostic performance of TCM syndrome information of lung cancer through Stacking ensemble learning. It can be seen that the advantages of Stacking ensemble learning to integrate multiple models and effectively improve the diagnostic efficiency of TCM diagnostic models, which provided a methodological reference for similar studies.
3.Analysis of the correlation between the clinical features of 1 865 children and adolescents with supernumerary teeth and the selection of anesthesia methods

Objective:
To retrospectively analyze the epidemiological characteristics of supernumerary teeth in patients aged 4-18 years old and the influencing factors on the selection of anesthesia methods, and to provide a reference for the selection of anesthesia plans for children and adolescents with supernumerary teeth.
Methods:
This study is a retrospective study approved by the Institutional Ethics Committee. Based on clinical electronic medical record system and cone beam CT (CBCT) data, a retrospective analysis was conducted on 2 210 patients 4-18 years of age who underwent supernumerary tooth extraction at the School of Stomatology, Fourth Military Medical University from August 2019 to July 2021. Inclusion criteria: ① Age 4-18 years old; and ② The American Society of Anesthesiologists (ASA) classifies anesthesia into grades I-II; and ③ Have complete oral and anesthesia case records and relevant imaging data. Exclusion criteria: ① Incomplete medical records or unclear imaging data; and ② Patients with ASA grade II or above. Patients’ gender and age, the number of supernumerary teeth, arch, location, orientation, eruption status, reason for appointment, anxiety level, degree of cooperation, anesthesia method, and other relevant information were collected and statistically analyzed.
Results:
A total of 1 865 eligible patients were included, with an average age of (8.9±3.2) years old. There were more male patients (71.37%, 1 331 cases) than female patients (28.63%, 534 cases) (P < 0.001), with a gender ratio of 2.49:1. The majority of supernumerary teeth were single (75.97%, 1 417/1 865), primarily located in the maxilla (97.2%, 1 812/1 865) and the anterior dental region (94.2%, 1 757/1 865), and in a centric position (77.3%, 1 442/1 865). The majority of patients with erupted supernumerary teeth were active in seeking treatment (97.67%, 335/343). Patients with supernumerary teeth located in the maxilla and mandible bones, as well as in the nasal floor, were mainly referred to the department for diagnosis, accounting for 94.38%(1 361/1 442) and 90.00% (72/80) (χ2=1 363.24, P < 0.001), respectively. Regarding anesthesia methods, the largest proportion of patients received nitrous oxide sedation-assisted local anesthesia or nerve block anesthesia, accounting for 38.07% (710/1 865), followed by local anesthesia, accounting for 35.23% (657/1 865). The proportion of patients receiving midazolam intravenous sedation with local anesthesia or nerve block anesthesia and general anesthesia was relatively small, accounting for 20.86% (389/1 865) and 5.84% (109/1 865). Patients 13-18 years of age with supernumerary teeth in the mandibular and posterior regions and without anxiety had the highest proportion of local anesthesia use (P < 0.001). In contrast, patients who had supernumerary teeth located at the base of the nose (50%), severe anxiety (94.12%), and poor cooperation (98.18%) had the highest proportion of general anesthesia use (P < 0.001). There was no significant difference (P = 0.35) in the incidence of intraoperative and postoperative complications after the extraction of supernumerary teeth. However, the proportion of anesthesia-related complications, such as dizziness, coughing, and respiratory depression, occurring in patients who received general anesthesia was higher than local anesthesia, accounting for 3.81% (P = 0.006).
Conclusion
There is a gender difference in the incidence of supernumerary teeth in patients 4-18 years of age, with a higher prevalence in males. The majority of supernumerary teeth are single and located in the maxillary anterior region, predominantly in a centric position. Patients whose teeth had erupted were more likely to seek medical treatment voluntarily, while patients with supernumerary teeth located in the maxilla and mandible bones, as well as in the nasal floor were more likely to be referred to the department. Patients with high levels of anxiety, poor cooperation, young age, multiple teeth, and high surgical difficulty were more inclined to choose general anesthesia.
4.A Case Report and Literature Review of A Child with Refractory Anti-N-methyl-D-aspartate Receptor Encephalitis Treated with Tocilizumab

OBJECTIVE
To explore the safety and efficacy of tocilizumab in the treatment of refractory anti-N-methyl-D-aspartate receptor encephalitis in children, and to provide reference for the treatment of this disease.
METHODS
The clinical manifestations, diagnosis and treatment, safety and efficacy of tocilizumab in a case of refractory anti-NMDAR encephalitis in Children’s Hospital, Zhejiang University School of Medicine were analyzed retrospectively, and the relevant literature was reviewed.
RESULTS
The clinical symptoms of the patient were not improved after 2 doses of methylprednisolone, immunoglobulin and 6 doses of rituximab. After 6 doses of tocilizumab, the modified Rankin scale(mRS) score was improved without adverse reactions.
CONCLUSION
Tocilizumab, as a escalation second-line immunotherapy, is effective and well tolerated in the treatment of refractory anti-NMDAR encephalitis. It is one of the potential therapeutic means.
5.Analysis of the effectiveness of cross-reactive carbohydrate antigen determinant antibody adsorbents in identifying allergen-specific IgE antibodies

This study aimed to investigate the influence of anti-cross-reactive carbohydrate determinant IgE antibodies (anti-CCD IgE) on the detection of allergen-specific IgE (sIgE) antibodies, as well as the application value of anti-CCD IgE adsorbents in detecting allergen sIgE. In this cross-sectional study, a total of 2 636 test samples from patients who received treatment in West China Hospital of Sichuan University and tested allergen sIgE using the western blot method from October 2020 to May 2021 were analyzed. In these samples, 709 samples tested postive of allergen sIgE. 46 stochastic venous serum samples that tested positive in both sIgE and anti-CCD IgE and 1 serum sample that tested positive in sIgE but negative in anti-CCD IgE were collected. These samples were processed by anti-CCD IgE adsorbents, followed by allergen sIgE detection. The difference between the two detection results before and after adsorption was analyzed. The allergen test results showed that the positive rate of anti-CCD IgE in samples was 2.6% (69/2 636) during the period of sample collection. After treatment with anti-CCD IgE adsorbents, the top three allergen-sIgE of the positive rate changed from tree combination 2 (willow/poplar/elm), common ragweed and peanut to dust mite combination, cockroach and crab. The positive anti-CCD IgE results of 46 samples all turned negative and the total positive sIgE antibody dropped by 62.8%; the positive rate of sIgE antibodies with the class result ≥2 significantly decreased after treatment with anti-CCD IgE adsorbents, especially the positive rate of common ragweed dropped by 96.2%. The results of positive samples showed that multiple sIgE antibodies declined by different ranges, involving up to 11 antibodies with a maximum decline of 4 classes. Strongly positive sIgE antibodies (the class result ≥4) also had a high conversion rate of negative (25.0%-100%). The positive sIgE antibodies in about 60% of the samples decreased by more than 2, and the sIgE antibodies in 17.4% of the samples turned completely negative. There was no change in the allergen sIgE detection results of the sample with negative anti-CCD IgE after treatment. In conclusion, sIgE antibodies including targeting common ragweed, humulus, tree combination 2 (willow/poplar/elm), etc. are susceptible to false positives caused by anti-CCD IgE. Treatment of samples with anti-CCD IgE adsorbents can significantly reduce the risk of false positives caused by anti-CCD IgE. It is necessary to pretreat samples that were anti-CCD IgE positive with anti-CCD IgE adsorbents, which can make laboratory results more accurate and provide a reference for diagnosis and prevention of allergic diseases.

7.YTE-17 inhibits colonic carcinogenesis by resetting antitumor immune response via Wnt5a/JNK mediated metabolic signaling

The density and composition of lymphocytes infiltrating colon tumors serve as predictive factors for the clinical outcome of colon cancer.Our previous studies highlighted the potent anti-cancer properties of the principal compounds found in Garcinia yunnanensis(YTE-17),attributing these effects to the regu-lation of multiple signaling pathways.However,knowledge regarding the mechanism and effect of YTE-17 in the prevention of colorectal cancer is limited.In this study,we conducted isobaric tags for relative and absolute quantification(iTRAQ)analysis on intestinal epithelial cells(IECs)exposed YTE-17,both in vitro and in vivo,revealing a significant inhibition of the Wnt family member 5a(Wnt5a)/c-Jun N-terminal kinase(JNK)signaling pathway.Subsequently,we elucidated the influence and mechanism of YTE-17 on the tumor microenvironment(TME),specifically focusing on macrophage-mediated T helper 17(Th17)cell induction in a colitis-associated cancer(CAC)model with Wnt5a deletion.Additionally,we performed the single-cell RNA sequencing(scRNA-seq)on the colonic tissue from the Wnt5a-deleted CAC model to characterize the composition,lineage,and functional status of immune mesenchymal cells during different stages of colorectal cancer(CRC)progression.Remarkably,our findings demon-strate a significant reduction in M2 macrophage polarization and Th17 cell phenotype upon treatment with YTE-17,leading to the restoration of regulatory T(Treg)/Th17 cell balance in azoxymethane(AOM)/dextran sodium sulfate(DSS)model.Furthermore,we also confirmed that YTE-17 effectively inhibited the glycolysis of Th17 cells in both direct and indirect co-culture systems with M2 macrophages.Notably,our study shed light on potential mechanisms linking the non-canonical Wnt5a/JNK signaling pathway and well-established canonical β-catenin oncogenic pathway in vivo.Specifically,we proposed that Wnt5a/JNK signaling activity in IECs promotes the development of cancer stem cells with β-catenin activity within the TME,involving macrophages and T cells.In summary,our study undergoes the po-tential of YTE-17 as a preventive strategy against CRC development by addressing the imbalance with the immune microenvironment,thereby mitigating the risk of malignancies.
8.Effects and Mechanism of Lithocarpus litseifoliu on Improving Hyperuricemia Nephropathy in Mice

Objective To investigate the ameliorative effects and potential mechanism of Lithocarpus litseifoliu on renal function and inflammation in mice with hyperuricemic nephropathy(HN).Methods The HN model was established by the combined administration of adenine and potassium oxyzate.The mice were randomly divided into normal control group,model control group,benzbromarone group,and high,medium and low dose groups of Lithocarpus litseifoliu.Different drugs were given to the mice,and their body mass was recorded once a week.The levels of uric acid(UA),creatinine(Cr),urine protein(UP),blood urea nitrogen(BUN)and urine urea nitrogen(UUN)as well as the levels of tumor necrosis factor α(TNF-α)and interleukin 6(IL-6)in serum or urine of each group were collected and measured on the 21st day.Hematoxylin-eosin(HE)staining was performed to observe kidney tissue injury in mice;real-time PCR(RT-PCR)was performed to determine ATP-binding cassette subfamily G member 2(ABCG2),urate transporter protein(URAT1),glucose transporter protein 9(GLUT9),and cytosolic factor NF-κB p50(κB p50)in kidney tissues.Results Compared with the normal control group,the body mass of mice in the model control group was significantly lower after the second weeks of modeling(P<0.05),and the levels of UA,Cr,UP,BUN,UUN,TNF-α,IL-6 contents and GLUT9 mRNA and κB p50 mRNA expression contents of kidney tissues were significantly higher(P<0.01,P<0.05).Compared with the model control group,the levels of Cr,UP,BUN and UUN contents and renal tissue nuclear cytokine κB p50 mRNA expression were significantly lower in the high,medium and low dose groups of Lithocarpus litseifoliu(P<0.01,P<0.05).The UA levels were significantly lower in the high dose group of Lithocarpus litseifoliu(P<0.05),and renal ABCG2 mRNA expression was significantly higher in the medium dose group(P<0.01).The renal URAT1 mRNA expression was significantly decreased in the low dose group(P<0.01).Conclusion Lithocarpus litseifoliu has shown ameliorative effects on HN mice,and the mechanism may be related to the modulation of renal uric acid transporters,improvement of renal function and anti-inflammation effects.
9.Exploring the effect and mechanism of action of secukinumab on calcific aortic valve disease based on the IL-23/Th17 inflammatory pathway

Objective To observe whether the IL-23/Th17 inflammatory pathway is involved in the development of calcific aortic valve disease,and whether secukinumab can delay the progression of calcific aortic valve disease by inhibiting this pathway.Methods Forty-seven mice were divided into a blank control group,model group,and secukinumab group according to the random number table method.The blank control group was fed normal chow,while the model group and secukinumab group were fed pro-calcification chow for 16 weeks to establish a calcific aortic valve disease model.After intervention with secukinumab for 4 weeks,peak flow velocity changes in the aortic valves were detected under Doppler ultrasonography in all mice.Relevant indexes were determined by hematoxylin and eosin staining,Von Kossa staining,immunohistochemical staining,ELISA,and qPCR.Results Compared with the model group,the secukinumab group showed significantly reduced peak flow velocity(P<0.05)and serum IL-6,IL-17,and IL-23 levels(P<0.05)in the aortic valve.Compared with the secukinumab group,the model group's leaflet thickness was significantly increased,and there were more calcium deposits.Immunohistochemical result showed that macrophage infiltration(P<0.05),IL-17A(P<0.05)and IL-23(P>0.05)levels in the valve leaflets were reduced in the secukinumab group compared with the model group.PCR result suggested that the expression of STAT3,BMP-2,and α-SMA mRNA was significantly lower in the secukinumab group than the model group(P<0.05).Conclusions The IL-23/Th17 inflammatory pathway is involved in the pathogenesis of calcific aortic valve disease.The inflammation,fibrosis,osteogenic differentiation,and calcification of mouse valves were alleviated after intervention with secukinumab,which may delay disease progression by inhibiting the IL-23/Th17 inflammatory pathway.
10.Effect of low-level laser therapy combined with electromyogram-triggered neuromuscular stimulation on patients with subacute ischemic stroke

Objective To analyze the effects of low-level laser therapy(LLLT)combined with elec-tromyogram-triggered neuromuscular electrical stimulation(ETNS)on upper limb function and quality of life in patients with subacute ischemic stroke.Methods A total of 52 patients with sub-acute ischemic stroke admitted to our hospital from January 2020 to October 2023 were recruited,and then randomly divided into control group(26 cases)and observation group(26 cases).Fugl-Meyer Motor Function Score-Upper Limb(FMA-UE),Trunk Injury Scale(TIS),Wolf Motor Function Test Scale(WMFT)-Time(TS)and Functional Ability(FS),Chinese version of Stroke Quality of Life Scale(SS-QOL)were used before treatment(day of enrollment)and after treat-ment(5 months after enrollment)to evaluate these patients.Results After treatment,the obser-vation group had obviously higher FMA-UE score,TIS score,WMFT-FS score and SS-QOL score than those before treatment(P<0.05),and these scores were also notably higher than those in control group after treatment(P<0.01).The WMFT-TS score in the observation group after treatment was statistically lower than that before treatment(P<0.05),and also reduced than that of the control group after treatment(39.42±11.65 vs 46.12±11.89,P=0.045).Conclusion LLLT combined with ETNS shows good efficacy in improvement of upper limb function and quality of life in subacute ischemic stroke patients.
