1.Self?management of blood pressure among elderly hypertensive patients in new rural communities of Shanghai

Objective To investigate the status of self‐management of blood pressure (BP) among elderly hypertensive patients living in new rural communities of Shanghai. Methods We enrolled a total of 2 241 hypertensive patients over 60 years old who participated in the National Key Research and Development Project "Cohort study on hypertension" of the 13th Five‐Year Plan in Langxia, Shanghai from May to July 2018. Questionnaires, physical examinations, and laboratory tests were conducted. We collected participants' demographic information, cardiovascular history, and self‐management behaviors related to hypertension. The status of self‐management of BP among participants with hypertension was analyzed. Pearson's chi‐squared test was used to explore the differences between males and females. Results Participants'mean age was (70.0±6.8) years, and males accounted for 38.8% of all participants. The systolic and diastolic BP levels of participants were (147.8±15.4) mmHg (1 mmHg=0.133 kPa) and (82.8±9.8) mmHg, respectively. The systolic blood pressure level of women was higher than that of men, and the diastolic blood pressure level of men was higher; the difference was statistically significant (all P<0.05). The awareness, treatment, and control rate of hypertension were 79.0%, 78.0%, and 25.6%, respectively. Males had a significantly higher control rate than females (P<0.05). The status of self‐management of blood pressure was as follows: (1) As to lifestyle, 84.4% of participants lacked exercise, and 69.2% were overweight or obese. The prevalence of smoking and drinking was higher in males (40.7% and 46.2%, respectively), and the proportion of females (12.4%) with anxiety and/or depression was higher than that of males (P all<0.001). (2) Concerning BP‐monitoring, only 10.1% of participants measured BP every week, 22.9% had BP measurement instruments at home, and 4.6% recorded their BP levels. There was no significant difference in self‐blood pressure monitoring behavior between men and women. (3) Regarding adherence to medication treatment, 97.6% took antihypertensive drugs regularly, and 21.6% were on combination therapy. The proportion of men taking combined drugs was higher than that of women, but there was no significant difference. Conclusions The rates of awareness, treatment, and control of hypertension in elderly hypertensive patients in new rural communities of Shanghai were above the national average. However, the status of self‐management of BP was still not satisfactory. More attention should be paid to healthy lifestyle, home BP monitoring, and effective pharmaceutical treatment strategies for hypertension.
2.Effects of treatment on ultrastructure of olfactory bulb and GFAP expression in mice with Parkinson's disease.

OBJECTIVE:
To observe the effects of treatment on the ultrastructure of olfactory bulb and the expression of substantia nigra glial fibrillary acidic protein (GFAP) in mice with Parkinson's disease (PD) induced by lipopolysaccharide (LPS), and to provide methods and evidence for early prevention and treatment of PD.
METHODS:
Forty C57BL/6 male mice were randomly divided into a blank group, a model group, an electroacupuncture (EA) group and a medication group, 10 mice in each one. The mice in the model group, EA group and medication group were treated with 30-day nasal perfusion of LPS to establish PD model. From the first day of model establishment, the mice in the EA group were treated with electroacupuncture at bilateral "Yingxiang" (LI 20) and "Yintang" (GV 29) for 20 min, once a day; 5-day treatment was taken as one session, and 4 sessions were given with an interval of 2 days between sessions. The mice in the medication group were treated with intraperitoneal injection of L-DOPA, 10 mg/mL, once a day; 5-day treatment was taken as one session, and 4 sessions were given with an interval of 2 days between sessions. After treatment, the behavioristics changes were observed by using footprint analysis and swimming test score; the ultrastructure of olfactory bulb was observed by using transmission electron microscopy; the expression of GFAP in substantia nigra was measured by using western blot method.
RESULTS:
① After model establishment, the mice in the model group, the EA group and medication group showed significant symptoms of quiver and fear of chill, and the BMI was significantly lower than that in the blank group (all <0.05). The results of behavioristics test indicated footprint length and swimming time in the model group, the EA group and medication group was significantly lower than those in the blank group (all <0.01), indicating the successful establishment of PD model. ② After one-session treatment, the symptoms of quiver and fear of chill were not observed in the EA group. After 4-session treatment, the BMI in the EA group was significantly higher than that in the model group (<0.05); the symptoms of quiver and fear of chill were not observed in the medication group, but the BMI was similar with the model group (>0.05). ③ After treatment, the footprint and swimming time in the model group were significantly lower than that in the blank group (both <0.01); the footprint and swimming time in the EA group and medication group were significantly higher than those in the model group (all <0.01).④ After treatment, compared with the blank group, the organelles and ultrastructure of olfactory bulb in the model group were significantly improved; the ultrastructure of olfactory bulb in the EA group was improved compared with that in the model group. ⑤ After treatment, the expression of substantia nigra GFAP in the model group was significantly higher than that in the blank group (<0.01); the expression of substantia nigra GFAP in the EA group and medication group was significantly lower than that in the model group (both <0.05).
CONCLUSION
The early treatment of can improve behavioral disorders in LPS-induced early PD mice, and the mechanism may be related to the regulation of olfactory disorders and the expression of GFAP in substantia nigra.

3.Effects of Xueshuantong injection on levels of inflammatory factors and homocysteine in patients with acute cerebral infarction

Objective To investigate the effects of Xueshuantong injection on levels of inflammatory factors and homocysteine (Hcy) in patients with acute cerebral infarction.Methods 128 patients of acute cerebral infarction who received therapy in the hospital from January 2014 to May 2015 were collected as research objects to retrospectively analyze their clinical materials.According to therapeutic schemes, those patients were divided into control group (n=62) and observation group (n=66).The control group were given conventional treatments, such as anti-platelet aggregation, lipid regulation, maintaining balance of water-electrolyte and acid-base, controlling blood glucose, controlling blood pressure and so on.Besides that, the observation group were given Xueshuantong injection.After two weeks treatment, the curative efficacy, levels of inflammatory factors as high-sensitivity C reactive protein (hs-CRP), tumor necrosis factor α(TNF-α), interleukin 8 (IL-8), Hcy and adverse reactions were compared.Results The total efficacy of observation group was 93.9%, which was statistically higher than that of 80.6%in control group (χ2 =5.166,P=0.023).After treatment, hs-CRP, TNF-α, IL-8 levels in observation group were significantly lower than those in control group (all P<0.05).After one and two weeks treatment, The Hcy level in observation group was statistically lower than that in control group ( all P<0.05 ).The adverse reactions in observation group was 8 cases ( 12.1%), 6 cases ( 9.7%) in control group, with no significant difference between two groups (χ2 =0.196, P =0.658 ). Conclusion Xueshuantong injection is effective in treatment of acute cerebral infarction, which could significantly increase curative efficacy, alleviate inflammatory reaction and reduce level of Hcy with less adverse reactions.
