1.Feasibility and safety of a new portable endoscopic system for the diagnosis and treatment of abdominal trauma in animal models

Objective:To compare the feasibility and safety of a new portable endoscopic system and the conventional endoscopic system for the detection and emergency treatment of abdominal trauma in animal models.Methods:Three healthy Bama pigs, which were fasted and water deprivation for 8 h before surgery and then underwent induction anesthesia. A layer-by-layer incision was made into the abdominal cavity of Bama pigs. An artificial pneumoperitoneum was established using a laparoscopic pneumoperitoneum machine. A bullet model was inserted into the abdominal cavity to build the bullet wound model. After the bullet model was removed, a shrapnel model was inserted into the mid-abdomen to build the shrapnel wound model. The two types of endoscopic system were used to detect, remove bullet model or shrapnel model of the three Bama pigs respectively. The procedure order of the two systems was assigned according to the random number table method. The surgical success, operation time, endoscopy pipeline patency, endoscopic operation satisfaction, adverse events and equipment defects were recorded.Results:Three surgeries were performed using the new portable endoscopic system and three other surgeries using the conventional endoscopic system, all of which were successful. The time of the new portable endoscopic system to find and remove the bullet model, and the shrapnel model were 232.33±11.68 s, 300.33±57.70 s, 170.00±44.44 s and 52.67±2.52 s, respectively. The corresponding time of the conventional endoscopic system were 232.67±21.20 s ( t=-0.054, P=0.962), 256.67±67.00 s ( t=0.880, P=0.472), 176.00±52.42 s ( t=-0.111, P=0.922), 58.67±14.84 s ( t=-0.832, P=0.493), respectively. There was no significant difference between the two systems ( P>0.05). The endoscopy tubes of the two endoscopic systems were both smooth. The operator was satisfied with the endoscopic procedures of both endoscopic systems, and no adverse event or device defect occurred. Conclusion:The portable endoscopic system proves to be safe and feasible for the diagnosis and treatment of abdominal trauma in animal models.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
3.Spatial epidemiological analysis of severe hand, foot and mouth disease in Guangxi, 2014-2018

Abstract: Objective To explore the spatial epidemiological characteristics of severe cases hand, foot and mouth disease (HFMD) in Guangxi, China, from 2014 to 2018, and to provide a basis for identifying the high-risk regions as well as the prevention and control of severe cases of HFMD in Guangxi. Methods Spatial-temporal scanning analysis, global and local spatial autocorrelation analysis were used to analyze the spatial clustering of HFMD. The trend surface analysis was used to evaluate the spatial distribution trend of HFMD. Results From 2014 to 2018, the incidence and severe case fatality rates of HFMD were 3.89/100 000 and 4.23%, respectively. Monte Carlo scanning analysis showed that the first cluster region was Cenxi City, the second cluster was mainly concentrated in northwest of Guangxi, and the aggregation time was mainly concentrated in April to May and August to October. The global spatial autocorrelation analysis showed that the severe HFMD was significant clustering distribution, and the Moran's I coefficients of the sever cases, severe morbidity and severe case fatality rate were 0.088, 0.118, 0.197, respectively (P<0.05). Local spatial autocorrelation analysis showed that hotspots of severe HFMD cases were concentrated in the southern Guangxi, mainly in Lingshan County. Anselin local Moran's I clustering and outlier analysis indicated that 5 high-high (H-H) clustering regions for fatality were Lingshan, Pubei, Zhongshan, Zhaoping and Pinggui County. There were 6 high-high (H-H) clustering regions for severe incidence rate, namely Lingshan, Qinnan, Lingyun, Youjiang, Bama Yao Autonomous and Pinggui County, and 1 high-low (H-L) clustering region, Cenxi County. The trend surface analysis showed that the overall number of severe cases of death decreased from east or west to the middle, and increased from north to middle, and then decreased to south. Conclusions Severe HFMD cases in Guangxi have obvious spatial-temporal clustering, and the hop spots are mainly concentrated in southern Guangxi. The prevention and control of HFMD in areas with high incidence of severe cases should be strengthened to reduce the burden of HFMD cases.
4.Analysis of constipation status and influencing factors in patients with lung cancer during postoperative hospitalization

Objective To investigate the current status of constipation during postoperative hospitalization and the factors associated with moderate to severe constipation at discharge in lung cancer patients. Methods Lung cancer patients who underwent surgery in 6 tertiary hospitals in Sichuan Province from November 2017 to January 2020 were enrolled. The MD Anderson Symptom Scale-Lung Cancer Module was used to collect postoperative constipation scores. Unconditional logistic stepwise regression was used to analyze the related influencing factors for moderate to severe constipation on the day of discharge. Results Finally 337 patients were collected. There were 171 males and 166 females, with an average age of 55.0±10.3 years. Constipation scores of lung cancer patients increased from postoperative day 1 to day 3, and showed a decreasing trend from day 3 to day 7. Moderate to severe constipation was present in 68 (20.2%) patients at discharge. The postoperative hospital stay (OR=0.743, P<0.001) and the dose of morphine used during postoperative hospitalization (OR=1.002, P=0.015) were influencing factors for moderate to severe constipation at discharge in lung cancer patients. Conclusion Lung cancer patients have the most severe constipation on postoperative day 3. Moderate to severe constipation at discharge is associated with the postoperative hospital stay and the dose of morphine used during postoperative hospitalization.

Objective: To investigate the incidence and treatment of perioperative anemia in patients with gastrointestinal neoplasms in Hubei Province. Methods: The clinicopathological data of 7 474 patients with gastrointestinal neoplasms in 62 hospitals in 15 cities (state) of Hubei Province in 2019 were collected in the form of network database. There were 4 749 males and 2 725 females. The median age of the patients was 62 years (range: 17 to 96 years). The hemoglobin value of the first time in hospital and the first day after operation was used as the criterion of preoperative anemia and postoperative anemia. Anemia was defined as male hemoglobin <120 g/L and female hemoglobin <110.0 g/L, mild anemia as 90 to normal, moderate anemia as 60 to <90 g/L, severe anemia as <60 g/L. The t test and χ2 test were used for inter-group comparison. Results: The overall incidence of preoperative anemia was 38.60%(2 885/7 474), and the incidences of mild anemia, moderate anemia and severe anemia were 25.09%(1 875/7 474), 11.37%(850/7 474) and 2.14%(160/7 474), respectively. The overall incidence of postoperative anemia was 61.40%(4 589/7 474). The incidence of mild anemia, moderate anemia and severe anemia were 48.73%(3 642/7 474), 12.20%(912/7 474) and 0.47%(35/7 474), respectively. The proportion of preoperative anemia patients receiving treatment was 26.86% (775/2 885), and the proportion of postoperative anemia patients receiving treatment was 14.93% (685/4 589). The proportions of preoperative anemia patients in grade ⅢA, grade ⅢB, and grade ⅡA hospitals receiving treatment were 26.12% (649/2 485), 32.32% (85/263), and 29.93% (41/137), and the proportions of postoperative anemia patients receiving treatment were 14.61% (592/4 052), 22.05% (73/331), and 9.71% (20/206). The proportion of intraoperative blood transfusion (16.74% (483/2 885) vs. 3.05% (140/4 589), χ²=434.555, P<0.01) and the incidence of postoperative complications (17.78% (513/2 885) vs. 14.08% (646/4 589), χ²=18.553, P<0.01) in the preoperative anemia group were higher than those in the non-anemia group, and the postoperative hospital stay in the preoperative anemia group was longer than that in the non-anemia group ((14.1±7.3) days vs. (13.3±6.2) days, t=5.202, P<0.01). Conclusions: The incidence of perioperative anemia in patients with gastrointestinal neoplasms is high. Preoperative anemia can increase the demand for intraoperative blood transfusion and affect the short-term prognosis of patients. At present, the concept of standardized treatment of perioperative anemia among gastrointestinal surgeons in Hubei Province needs to be improved.

6.Herbal Textual Research on Mori in Famous Classical Formulas

Through consulting the ancient herbs, medical books and modern literature, this paper made textual research on the name, origin, producing area, quality evaluation, collection and processing of medicinal materials of Sang (Mori Folium, Mori Cortex, Mori Ramulus, Mori Fructus) in famous classical formulas, in order to provide a basis for the development of famous classical formulas containing medicinal materials of Sang. According to the research, Mori Folium and Mori Cortex were first used as medicines in Shengnong Bencaojing , Mori Ramulus was first used as medicine in Jinxiaofang, and Mori Fructus was first used as medicine in Xinxiu Bencao. Before the Tang dynasty, there were Nyusang and Shansang. Since Tang dynasty, there were many sources of medicinal materials of Sang, including Baisang (Morus alba), Jisang (M. australis), Shansang (M. mongolica), etc. According to textual research, the mainstream varieties were M. australis, M. alba and their cultivated varieties. In modern times, according to the relevant information and the Chinese Pharmacopoeia, M. alba is the original base. In ancient times, the origin of mulberry changed with the development of sericulture, mulberry has been widely planted since the Song dynasty. In the Ming and Qing dynasties, mulberry has been planted most in Jiangsu and Zhejiang. In modern times, they are mainly produced in Jiangsu, Zhejiang, Anhui, Hunan and other places. In recent years, due to the related policies and strategies such as "moving silkworms from east to west", the center of silkworm breeding has gradually transferred to the west. As for the quality evaluation and harvesting and processing of mulberry medicinal materials, Most of the ancient and modern records of Mori Folium are the same. They are harvested after frost, and dried after removing impurities. The quality is better when the leaves are large and thick, yellowish green, holding prickly hands and undergoing frost. The harvesting period of Mori Cortex is slightly different in ancient and modern records. Ancient books record that it can be harvested all the year round, but in modern times, it is mostly harvested from late autumn to the next spring. The processing methods include removing soil and fibrous roots, scraping off yellow-brown rough skin, peeling off white skin and drying in the sun. The quality is better when they are white, thick, flexible, free of rough skin and full of powder. There are few records about the collection, processing and quality evaluation of Mori Ramulus and Mori Fructus in ancient Chinese herbal books. According to modern literature, Mori Ramulus is usually collected in late spring and early summer, with leaves removed, slightly dried, sliced while fresh, and dried in the sun. The best quality of Mori Ramulus is fine and tender with the yellow and white section. Mori Fructus is harvested from April to June when the fruit turns red, and dried in the sun, or slightly steamed and dried in the sun, and it is better to be big, dark purple, oily and thick. There are many processing methods of mulberry medicinal materials. Ancient books record stir frying, baking, burning and steaming of Mori Folium, in modern times, there is honey-roasted method, but most of them are used as raw products. In ancient materia medica, Mori Cortex has firing method, baking method, stir-frying method, honey-fried method, etc. In modern times, there are stir-fried and honey-fried methods, and most of them are used as raw products. Ancient books record that Mori Ramulus has cutting and frying methods, while modern ones have cutting, frying, wine-processed and bran-processed methods. Processing methods of Mori Fructus are consistent in ancient and modern times, and they are mostly dried after being cleaned or steamed. Based on the research results, it is suggested that M. alba should be selected as mulberry medicinal materials in the famous classical formulas, and appropriate medicinal parts and processing methods can be selected according to the indications of the famous classical formulas.
7.Herbal Textual Research on Menthae Haplocalycis Herba in Famous Classical Formulas

In this paper, the name, origin, producing area and other aspects of Menthae Haplocalycis Herba in the famous classical formulas were carried out by consulting herbal literature, medical books, prescription books in the past dynasties and related modern documents. Through the textual research, it can be seen that the name of Bohe was used as the correct name in the mainstream of the past dynasties, and there were still multiple synonyms, most of which originated from the false transmission of dialectal accent, producing area and efficacy. There are many varieties recorded in the literature of the past dynasties such as Bohe, Longnao Bohe, Hubohe and Shibohe. According to the textual research, Bohe, Longnao Bohe and Yebohe are consistent with Mentha haplocalyx, whcih is the mainstream variety. Longnao Bohe is named for its form of producing area, Shibohe is Mosla chinensis, Daye Bohe is Agastache rugosa, and Nanbohe is M. crispata. Menthae Haplocalycis Herba has been widely planted since Tang dynasty. It was mainly grown in Jiangsu, Zhejiang, Jiangxi and Sichuan in Ming and Qing dynasties, and Jiangsu is the genuine production area. Its quality is best if it has dry body, no roots, many leaves, green color and strong smell. In ancient times, the stems and leaves of Menthae Haplocalycis Herba were often picked and dried in summer and autumn, which is basically the same as the records of modern times when the stems and leaves are luxuriant in summer and autumn, or when the flowers bloom to three rounds, they are picked in sunny days and cut in different times, and then dried in the sun or in the shade, and the raw products was often used as medicine in ancient and modern times. Before the Song dynasty, Menthae Haplocalycis Herba was recorded as pungent and warm. Until the Song dynasty, it was written as “extremely cool” in Lyuchanyan Bencao. It may have been thought in the early stage that it was similar to several warm herbs, such as Perilla frutescens, Stachys japonica, Elsholtzia ciliata and M. chinensis in appearance, all of which have the function of Xinsan, so it was recorded as warm. Since the Qing dynasty, Menthae Haplocalycis Herba has been recorded as cool property in the mainstream materia medica, Menthae Haplocalycis Herba recorded as pungent and cool in the 2020 edition of Chinese Pharmacopoeia, and its effect is to dissipate wind heat, clear the head, relieve the pharynx and so on, the records of efficacy in ancient and modern times are basically the same. Based on the research results, it is suggested that raw products of M. haplocalyx should be selected when developing the famous classical formulas containing Menthae Haplocalycis Herba.
8.Evolution of CT characteristics of reversed halo sign in pulmonary tuberculosis

Objective:To explore the evolution of CT characteristics of the "reversed halo sign" of pulmonary tuberculosis, and to further improve the recognition of its CT signs.Methods:Clinical and CT data of 12 patients with pulmonary tuberculosis who were clinically and pathologically confirmed and accompanied with CT manifestation of "reversed halo sign" in First Affiliated Hospital of Henan University of Science and Technology from August 2013 to April 2020 were analyzed retrospectively. Pathological and imaging contrastl analysis was performed on 1 patient undergoing surgical treatment.Results:Among 12 cases with "reversed halo sign", there were 2 cases with single lesion in unilateral lung, 2 cases with multiple lesions in unilateral lung, and 8 cases with multiple lesions in bilateral lungs. Three cases showed only "reversed halo sign", 9 cases showed both halo-like sign and uniform fireworks sign. "Tree-in-bud "sign was found in all 12 patients in the outer ring of the "reversed halo sign". Eight patients received three or more CT examinations, and six of them showed reduction of density and volume of the "reversed halo sign" after standardized anti-tuberculosis treatment. Under the natural course of the disease in two cases, the overall enlargement of the lesion was observed in 1 case, and the overall density of the lesion was reduced and the outer ring wall of the "reversed halo sign" was thinned in 1 case. The pathology of one case after surgical lobectomy showed granulomatous inflammatory nodules of varying sizes containing Langerhans nodule giant cells in the lung parenchyma. The typical caseous necrotic granulomatous nodules were rare here. The "reversed halo sign" showed dense Langerhans nodules in the outer ring, sparse central areas with fibrous hyperplasia and alveolar wall thickening.Conclusions:The outer ring of "reversed halo sign" of pulmonary tuberculosis shows as "tree-in-bud" sign, and its center shows as the fine reticulation pattern. After effective anti-tuberculosis treatment, both the overall density of "reversed halo sign" and the lesion size reduced. Finally, the lesions mostly present as as fine grid shadows for a long time.
9.Real World Study on Compliance of Subcutaneous Specific Immunotherapy in Patients with Allergic Rhinitis and/or Bronchial Asthma in the Pearl River Delta Region

ObjectiveTo investigate the causes of discontinuation of subcutaneous allergen immunotherapy (SCIT) in patients with allergic rhinitis (AR) and / or bronchial asthma (BA) in the Pearl River Delta region, and to improve the compliance of these patients. MethodsIt was a retrospective, multi-center real world study. Patients with AR and / or BA receiving SCIT in six hospitals in the Pearl River Delta region from January 2015 to December 2020 were recruited. The clinical data of all patients were collected, and the patients with abscission were followed up by telephone. ResultsIn this study, 1 244 patients who received SCIT were included. A total of 427 patients stopped SCIT before 3 years (34.3%). Children, married patients and patients with positive family history of allergy and total immunoglobulin E (tIgE) > 200 U / mL showed relatively good compliance (P<0.05), while more patients of AR alone stopped SCIT before 3 years than patients of AR complicated with BA (P<0.05). Patients with high symptom score and drug score before treatment had better compliance (P<0.001). A total of 279 cases (65.3%) of the non-adherence group were followed by telephone. The first three reasons for discontinuing treatment were the treatment did not achieve the expected effect (115 cases, 41.2%), factors pertaining to learning and daily life (74 cases, 26.5%) and adverse reactions (29 cases, 10.4%). The number of patients stopping SCIT decreased gradually with time; the proportion of patients who discontinued treatment due to adverse reactions in the first year of treatment was higher than that in the second and third years (P<0.05). ConclusionFailure to achieve the expected effect was the primary reason for poor compliance of patients receiving SCIT and discontinuation mainly occurred on the early stage of the treatment. The supervision from family members is helpful to improve the compliance, suggesting that individualized measures need to be taken to reduce the loss of SCIT.
10.Dihydromyricetin reduces lipid accumulation in LO2 cells via AMPK/mTOR-mediated lipophagy pathway and inhibits HepG2 cell proliferation in vitro.

OBJECTIVE:
To explore the mechanism underlying the hepatoprotective effect of dihydromyricetin (DMY) against lipid accumulation in light of the lipophagy pathway and the inhibitory effect of DMY on HepG2 cell proliferation.
METHODS:
LO2 cells were cultured in the presence of 10% FBS for 24 h and treated with 100 μg/mL DMY, or exposed to 50% FBS for 24 h followed by treatment with 50, 100, or 200 μg/mL DMY; the cells in recovery group were cultured in 50% FBS for 24 h and then in 10% FBS for another 24 h. Oil red O staining was used to observe the accumulation of lipid droplets in the cells, and the levels of TC, TG, and LDL and activities of AST, ALT and LDH were measured. The expression of LC3 protein was detected using Western blotting. AO staining and transmission electron microscopy were used to determine the numbers of autophagolysosomes and autophagosomes, respectively. The formation of autophagosomes was observed with MDC staining, and the mRNA expression levels of LC3, ATG7, AMPK, mTOR, p62 and Beclin1 were determined with q-PCR. Flow cytometry was performed to analyze the effect of 50, 100, and 200 μg/mL DMY on cell cycle and apoptosis of HepG2 cells; DNA integrity in the treated cells was examined with cell DNA fragmentation test.
RESULTS:
DMY treatment and pretreatment obviously inhibited lipid accumulation and reduced the levels of TC, TG, LDL and enzyme activities of AST, ALT and LDH in LO2 cells (P < 0.05). In routinely cultured LO2 cells, DMY significantly promoted the formation of autophagosomes and autophagolysosomes and upregulated the expression of LC3 protein. DMY obviously attenuated high FBS-induced inhibition of autophagosome formation in LO2 cells, up- regulated the mRNA levels of LC3, ATG7, Beclin1 and AMPK, and downregulated p62 and mTOR mRNA levels (P < 0.05 or 0.01). In HepG2 cells, DMY caused obvious cell cycle arrest, inhibited cell proliferation, and induced late apoptosis and DNA fragmentation.
CONCLUSION
DMY reduces lipid accumulation in LO2 cells by regulating the AMPK/ mTOR-mediated lipophagy pathway and inhibits the proliferation of HepG2 by causing cell cycle arrest and promoting apoptosis.
