1.Efficacy of endoscopic retrograde cholangiopancreatography combined with electrohydraulic lithotripsy under the direct view of eyeMax biliary-pancreatic imaging system in treatment of difficult choledocholithiasis

ObjectiveTo investigate the safety and efficacy of endoscopic retrograde cholangiopancreatography (ERCP) combined with electrohydraulic lithotripsy under the direct view of eyeMax biliary-pancreatic imaging system in the treatment of difficult choledocholithiasis. MethodsA retrospective analysis was performed for the clinical data of 12 patients with difficult choledocholithiasis who underwent ERCP and electrohydraulic lithotripsy under the direct view of eyeMax biliary-pancreatic imaging system in Department of Gastroenterology, Jilin People’s Hospital, from May to November 2022. The clinical effect of lithotripsy and lithotomy was observed, and postoperative complications and time of surgical operation were assessed. ResultsAmong the 12 patients, 11 (91.67%) were successfully treated by electrohydraulic lithotripsy under direct view, 9 (75.00%) achieved first-attempt success in lithotripsy, and 11 (91.67%) had complete removal of calculi; 1 patient was found to have stenosis of the bile ducts caused by multiple biliary tract surgeries, and grade Ⅱ intrahepatic bile duct stones above the sites of stenosis were removed under direct view, but there were still residues of grade Ⅲ intrahepatic bile duct stones, which led to the fact that complete calculus removal was not achieved. The mean time of ERCP operation was 91.3±26.2 minutes, including a time of 41.8±22.2 minutes for energy lithotripsy. There were 2 cases of postoperative biliary tract infection which were improved after anti-infective therapy, 2 cases of hyperamylasemia which were not given special treatment, and 3 cases of mild pancreatitis which were improved after symptomatic medication, and there were no complications such as bleeding and perforation. ConclusionERCP combined with electrohydraulic lithotripsy under the direct view of eyeMax biliary-pancreatic imaging system is safe, effective, and feasible in the treatment of difficult choledocholithiasis.
2.Improvement effect and mechanism of sanguinarine on inflammatory pain in rats with lumbar disc herniation

OBJECTIVE To study the improvement effect and mechanism of sanguinarine (SG) on inflammatory pain in rats with lumbar disc herniation (LDH) and its mechanism. METHODS LDH model rats were established and divided into model group, SG low-dose, medium-dose and high-dose groups (1.00, 2.50, 6.25 mg/kg), high-dose of SG+Anisomycin [mitogen-activated protein kinase (MAPK) activator] group (6.25 mg/kg SG+5 mg/kg Anisomycin), with 10 rats in each group. Another 10 rats were included as the control group. Each group was given corresponding drugs intraperitoneally, while the control group and model group were given an equal volume of normal saline intraperitoneally, once a day, for 7 consecutive days. The general situation and neurological changes of rats in each group were observed, and the pain threshold [including paw withdrawal mechanical threshold (PWMT) and paw withdrawal thermal latency (PWTL)] of rats was determined; the histopathological changes of dorsal root ganglion (DRG) were observed in rats. The serum levels of inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β)] and pain factors [neuropeptide Y (NPY), 5-hydroxytryptamine (5-HT)] in rats were detected.The positive expressions of ionized-calcium binding adaptor molecule-1 (Iba-1) in spinal cord microglia and glial fibrillary acidic protein (GFAP) in astrocytes were observed. The expressions of proteins related to MAPK/extracellular signal-regulated kinase (ERK)/nuclear factor κB (NF-κB) signaling pathway, TNF-α and IL-1β proteins were detected in DRG tissue of rats. RESULTS Compared with the control group, the rats in the model group showed decreased appetite, hindlimb movement disorders, and disordered neuronal cell arrangement, the neurological score, the levels of TNF-α, IL-1β, NPY, the positive expressions of Iba-1 and GFAP, the phosphorylations of p38 MAPK, ERK1/2 and NF-κB p65, the protein expressions of TNF-α and IL-1β were significantly increased (P<0.05); PWMT, PWTL and the levels of 5-HT were significantly reduced (P<0.05). Compared with the model group, the rats of SG groups showed some relief in their mental appetite and hindlimb motor disorders, the intervertebral disc structure of DRG was restored, and the levels of the above quantitative indicators had significantly reversed (P<0.05). Anisomycin reversed the improvement effect of SG on inflammatory pain in LDH rats. CONCLUSIONS SG can improve inflammatory pain by inhibiting the activation of microglia in DRG tissue of LDH rats, reducing the release of inflammatory factors, and increasing pain threshold, and its mechanism of action may be related to the inhibition of MAPK/ERK/NF- κB signaling pathway.
3.Analysis of information labeling in instructions of antitussive and expectorant drugs in 34 children’s medical institutions

OBJECTIVE To understand the information labeling of children’s medication in the instructions of antitussive and expectorant drugs commonly used in children’s medical institutions, analyze the existing problems, and propose relevant suggestions. METHODS The instructions for 100 antitussive and expectorant drugs commonly used in 20 tertiary children’s hospitals (centers) and 14 maternal and child health hospitals (centers) with regional representativeness in China were collected, and the information labeling of children’s medication in the instructions was investigated and analyzed. RESULTS There were only 7 kinds of antitussive and expectorant drugs for children, and the others were non-specific drugs for children. Among antitussive and expectorant drugs, tablets accounted for 18.00%, injections for 18.00%, and capsules for 4.00%. Among 100 antitussive and expectorant drugs, 72 (72.00%) labeled the usage and dosage for children, 63 (63.00%) labeled the medication items for children, 59 (59.00%) indicated the information of children in the precautions, and pharmacokinetic parameters for children were absent. Compared with imported antitussive and expectorant drugs or the drugs manufactured by joint ventures, there were many missing labeling of medication information for children with Chinese drugs. Among 63 kinds of drug instructions labeled with medication items for children, various instructions information had little guiding significance. CONCLUSIONS There is a lack of labeling in the instructions of antitussive and expectorant drugs for children, and the proportion of special drugs for children remains low. Pediatric drug information in the instructions has little guiding significance for pediatric medication. Relevant departments should further promote the completeness of pediatric medication information in the instructions of antitussive and expectorant drugs to ensure the rational use of children’s medication.

Tuberculosis (TB) and human immunodeficiency virus infection / acquired immune deficiency syndrome (HIV/AIDS) are both serious global public health threats. Early detection of infected persons and/or patients through TB/HIV bi-directional screening is crucial for prevention and control strategy in China and globally. In recent years, with the promotion and application of new TB and HIV detection technologies worldwide, TB/HIV bi-directional screening technologies and strategies have made remarkable changes. This expert consensus introduces the significance and challenges of TB/HIV bi-directional screening, summarizes important progress of research and applications, and makes recommendations on screening measures and procedures to further strengthen TB/HIV bi-directional screening in China.
5.Clinical effects of percutaneous elastic intramedullary nail assisted by arthrography for the treatment of radial neck fractures in children

Objective To explore clinical effect of closed reduction percutaneous elastic intramedullary nail assisted by arthrography in the treatment of radial neck fracture in children.Methods A retrospective analysis was performed on 23 chil-dren with radial neck fracture treated with arthrography assisted closed reduction and percutaneous elastic intramedullary nail internal fixation(arthrography with elastic nail group)from January 2019 to December 2022,including 12 males and 11 fe-males,aged from 2 to 12 years old with an average of(7.36±1.89)years old;According to Judet fracture types,14 children were type Ⅲ and 9 children were type Ⅳ.In addition,23 children with radial neck fracture were selected from January 2015 to December 2018 who were treated with closed reduction and percutaneous elastic intramedullary nail fixation(elastic nail group),including 11 males and 12 females,aged from 2 to 14 years old with an average of(7.50±1.91)years old;Judet classi-fication included 15 children were type Ⅲ and 8 children were type Ⅳ.Operative time and intraoperative fluoroscopy times were compared between two groups.Metaizeau evaluation criteria was used to evaluate fracture reduction,and Tibone-Stoltz evaluation criteria was used to evaluate functional recovery of elbow between two groups.Results Both groups were followed up for 12 to 24 months with an average of(16.56±6.34)months.Operative time and intraoperative fluoroscopy times of elastic nail group were(56.64±19.27)min and(21.13±7.87)times,while those of joint angiography with elastic nail group were(40.33±1 1.50)min and(12.10±3.52)times;there were difference between two groups(P<0.05).According to Metaizeau evaluation,11 patients got excellent result,9 good and 3 fair in joint angiography with elastic nail group,while in elastic nail group,5 ex-cellent,13 good,4 acceptable,and 1 poor;the difference between two groups was statistically significant(P<0.05).According to Tibone-Stoltz criteria,14 patients got excellent result,8 good,and 1 fair in joint arthrography with elastic nail group;while in elastic nail group,12 patients got excellent result,9 good,1 fair and 1 poor;there was no significant difference between two groups(P>0.05).Conclusion Compared to percutaneous elastic intramedullary nail fixation,closed reduction assisted by arthrography has advantages of reduced operation time,decreased intraoperative fluoroscopy frequency,and improved fracture reduction.Arthrography enables clear visualization of the anatomical structures of radius,head,neck,bone,and cartilage in children,facilitating comprehensive display of fracture reduction and brachioradial joint alignment.This technique more pre-cisely guides the depth of elastic intramedullary nail implantation in radius neck,thereby enhancing surgical efficiency and success rate.
6.Metabolomics of nasal lavage fluid in patients with allergic rhinitis treated by Xiaoqinglong Decoction.

This study used nasal lavage fluid for metabolomics to explore its feasibility, and applied it to the clinical metabolomics study of Xiaoqinglong Decoction in the treatment of allergic rhinitis(AR), aiming to investigate the molecular mechanism of Xiaoqing-long Decoction in the treatment of AR through differential changes in local nasal metabolism. AR patients were selected as the research subjects, and nasal lavage fluid was collected as the sample. Metabolomics analysis using liquid chromatography-mass spectrometry was performed on normal group, AR group, and Xiaoqinglong Decoction group. The differences in metabolic profiles among the groups were compared using principal component analysis and partial least squares discriminant analysis, and differential metabolites were identified and subjected to corresponding metabolic pathway analysis. The results showed that Xiaoqinglong Decoction significantly improved the symptoms of AR patients. The metabolomics analysis revealed 20 differential metabolites between AR group and Xiaoqinglong Decoction group. The core metabolite with a trending return in comparison to normal group was trimethyladipic acid. The metabolites were involved in multiple pathways, including β-alanine metabolism, glutathione metabolism, and phenylalanine, tyrosine, and tryptophan biosynthesis. The feasibility of applying nasal lavage fluid in nasal metabolomics was preliminarily demonstrated. Differential metabolites and enriched pathways in the treatment of AR patients with Xiaoqinglong Decoction were identified, indicating that it may improve rhinitis symptoms through the regulation of various metabolites, including antioxidant effects and correction of Th1/Th2 imbalance.

7.Preparation and Application of Monoclonal Antibody Against Human von Willebrand Factor Propeptide.

OBJECTIVE:
To develop monoclonal antibodies that can specifically recognize human von Willebrand factor (VWF) propeptide (VWFpp) in plasma, and establish a rapid and reliable method for the detection of VWFpp antigen in plasma by using the double-antibody sandwich ELISA with the obtained anti-VWFpp monoclonal antibody.
METHODS:
The recombinant human VWFpp (D1 and D2 regions) protein expressed in eukaryotic cells was used as immunogen to immunize BALB/c mice with routine method, so as to obtain clones of fusion cells. After screening and identification, hybridoma cell lines secreting monoclonal antibodies against VWFpp were selected, and then double-antibody sandwich ELISA assay was used to construct VWFpp antigen detection kit for the determination of VWFpp in human plasma. The levels of VWFpp antigen in plasma of 12 leukemia patients who underwent bone marrow transplantation were dynamically detected.
RESULTS:
Two hybridoma cell lines that can be subcultured continuously and secrete monoclonal antibodies against VWFpp were obtained and named SZ175 and SZ176 respectively. Identified by ELISA and Western blot, the antibodies could both specifically recognize VWFpp but couldn't recognize mature VWF (without propeptide). Based on the principle of double-antibody sandwich ELISA, monoclonal antibodies SZ175 and SZ176 were successfully made into a kit for detecting VWFpp antigen. The plasma VWFpp levels of leukemia patients before and after bone marrow transplantation were dynamically detected. The results showed that the plasma VWFpp levels of the patients after transplantation were significantly higher than those before transplantation.
CONCLUSION
Two monoclonal antibodies against VWFpp were successfully prepared, and a double-antibody sandwich ELISA detection kit for VWFpp antigen was constructed, which provides a powerful tool for further study on the biological function of VWFpp, the clinical diagnosis and classification of von Willebrand disease (VWD), and the prognostic monitoring of endothelial injury-related diseases.

8.Effect of calcium on biological properties of the ameloblast ALC

Objective:
To observe the effect of calcium on biological characteristics (proliferation, apoptosis and cell cycle) of ALC ameloblasts. .
Methods:
ALC cell lines were cultured in vitro in DMEM medium with high glucose at different concentrations (0, 2.0, 2.5, 3.0 and 3.5 mmol/L CaCl2 aqueous solution) for 24 h and 48 h, respectively. Changes of ALC cells under two kinds of incubation time were observed with an inverted microscope. CCK-8 method was used to analyze the effect of calcium ion on ALC cell proliferation. Hoechst staining was used to observe the effect of calcium ion on ALC cell apoptosis. PI staining and FCM method were used to analyze the effect of calcium ions on the growth cycle of ALC cells. Western blot was used to detect the effect of calcium ions on the expression of Cyclin A, Cyclin B and Cyclin D in ALC cells
Results:
In the 0 mmol/L CaCl2 group, ALC cells were oval or polygonal in shape, and the cells were closely connected and grew like paving stones. In other concentration groups, the morphology of ALC cells did not change significantly after calcium intervention for 24 h and 48 h. Results of CCK-8 method showed that the survival rate of ALC cells slightly decreased with increasing calcium ions concentration after calcium intervention for 24 h and 48 h. However, there was no significant differences in this trend. Results of Hoechst staining showed that the number of ALC cell apoptosis did not increase significantly after different concentrations of calcium intervention for 24 h and 48 h. With the increase of calcium ion concentration, results of PI staining and FCM method showed that the cell cycle of ALC cells gradually increased in S phase and decreased in G1 and G2 phase gradually. Western blot results showed that the expression of Cyclin A and Cyclin B in ALC cells decreased and the expression of Cyclin D increased after different concentrations of calcium intervention for 24 h and 48 h.
Conclusion
In this study, calcium has no significant effect on the proliferation and apoptosis of ALC cells. Calcium, however, has an effect on the ALC cell cycle. Results of this study show that calcium ions has no obvious toxic or side effects on the ameloblasts, which could be used to explore the possible mechanism and effect of calcium on dental fluorosis.
9.The Effect of VWF Propeptide on VWF Mutant in D1 Domain.

OBJECTIVE:
To investigate whether co-transfection of wild-type VWFpp with VWF mutant in D1 region is able to correct VWF defects in biosynthesis and secretion.
METHODS:
Four VWF mutant plasmids were single transfected into HEK 293 cells, or co-transfected into HEK 293 cells with the wild type VWFpp plasmids. The VWF in supernatant and lysate of transfected cells were analyzed by ELISA, vertical VWF multimer electrophoresis. The retention of VWF in endoplasmic reticulum of transfected cells were detected by immunofluorescence confocal microscope.
RESULTS:
In the vertical VWF multimer analysis, with co-expressing VWF mutant and VWFpp, the VWF multimer bands disappeared, and the VWF antigen in both supernatant and lysate of cells decreased, compared with the single expression of VWF mutant. Although the intracellular levels of VWF antigens decreased after co-expression, the retention rate of VWF mutant decreased in endoplasmic reticulum.
CONCLUSION
VWFpp can reduce the retention of VWF in endoplasmic reticulum, assists the transport of VWF between subcellular organelles. However, VWFpp inhibits the biosynthesis and secretion of VWF about the mutant in D1 domain.

10.Effect of modified hyaluronic acid hydrogel controlled release of brain derived neurotrophic factor on growth, differentiation and apoptosis of embryonic neural stem cells in rats

Objective:
To investigate the effect of modified hyaluronic acid(HA) hydrogel-controlled release brain-derived neurotrophic factor(BDNF) on the growth, differentiation, and apoptosis of rat embryonic neural stem cells(rFNSCs).
Methods:
The experiment took rat embryonic stem cells as the research object. The experiments were divided into three groups, blank control group [rFNSCs co-cultured with poly(lactic-co-glycolic acid)(PLGA)and HA],conditional control group(rFNSCs co-cultured with BDNF-PLGA and HA), and experimental group(rFNSCs co-cultured with BDNF-PLGA and modified hyaluronic acid hydrogel that is Dex-HA). ELISA was used for detecting the release curve of BDNF in the controlled-release scaffold. The Live/Dead assay detected the apoptosis of rFNSCs in each group, CCK-8 experiment was used to test the cell proliferation activity of rFNSCs after coculture with hydrogel. The differentiation ratios of rFNSCs in different groups were detected by immunocytochemical staining.
Results:
ELISA result showed that the controlled-release scaffold could release BDNF for 14 days.Live/Dead result showed the number of rFNSCs apoptotic in experimental group was less than blank control group and conditional control group. CCK-8 experiment result showed that the cell proliferation activity of rFNSCs in the experimental group was showed stronger survivability than blank control group and conditional control group. The difference was statistically significant(P<0. 05). Moreover, immunocytochemical staining showed that the experimental group hydrogel could promote the neuronal differentiation of rFNSCs and reduce their differentiation into astrocytes in vitro compared with the blank control group and the conditional control group. The difference was statistically significant(P<0. 05).
Conclusion
modified hyaluronic acid hydrogel controlled release brain-derived neurotrophic factor can promote the growth and differentiation of rFNSCs and reduce their apoptosis.
