1.Effect of Atractylodis Macrocephalae Rhizoma on Short-chain Fatty Acids and Intestinal Barrier in Mice with Slow-transit Constipation

ObjectiveTo investigate the mechanism of Atractylodis Macrocephalae Rhizoma(AMR) in the treatment of slow-transmission constipation(STC) by observing the effects of AMR on short-chain fatty acids and intestinal barries in STC mice. MethodForty-eight male KM mice were randomly divided into blank group, model group, AMR low-, medium-, high-dose groups(2.5, 5, 10 g·kg-1) and mosapride group(2.5 mg·kg-1). Except for the blank group, all groups were gavaged with loperamide suspension(5 mg·kg-1) twice daily for 14 d to construct the STC mouse model. At the same time, each drug administration group was given the corresponding drug by gavage for consecutive 14 d, the blank and model groups were gavaged with equal volume of distilled water. The effects of the treatment of AMR on body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice were observed, the pathological changes of mouse colon were observed by hematoxylin-eosin(HE) staining and periodic acid-Schiff(PAS) staining, the levels of gastrin(GAS) and motilin(MTL) in serum were detected by enzyme-linked immunosorbent assay(ELISA), gas chromatography-mass spectrometry(GC-MS) was used to detect the contents of short-chain fatty acids(SCFAs) in mouse feces, real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) and Western blot were used to determine the mRNA and protein expression levels of zonula occludens-1(ZO-1), Occludin, and Claudin-1 in the colon of mice. ResultCompared with the blank group, the body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice in the model group were significantly decreased(P<0.05, P<0.01), the arrangement of colonic tissues was disordered, and the number of goblet cells was reduced, the levels of GAS and MTL in serum were significantly decreased(P<0.01), and the levels of SCFAs in the feces were on a decreasing trend, with the contents of acetic acid, propionic acid, butyric acid, isobutyric acid and valeric acid were significantly decreased(P<0.05, P<0.01), the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colonic tissues were significantly decreased(P<0.01). The above results suggested that STC mouse model was successfully constructed. Compared with the model group, the body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice in AMP administration groups all increased significantly(P<0.05, P<0.01), the mucosal layer of the colonic tissues was structurally intact without obvious damage, and the number of goblet cells increased, serum levels of GAS and MTL were significantly increased(P<0.01), the contents of SCFAs in the feces were all on a rising trend, with the contents of acetic, propionic, butyric and isobutyric acids rising significantly(P<0.05, P<0.01), the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colonic tissues were significantly increased(P<0.05, P<0.01). ConclusionAMR is able to improve the constipation symptoms in STC mice, and its mechanism may be related to increasing the contents of SCFAs in the intestine as well as promoting the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colon.

Objective To investigate the incidence rate,clinical characteristics,and prognosis of neonatal stroke in Shenzhen,China.Methods Led by Shenzhen Children's Hospital,the Shenzhen Neonatal Data Collaboration Network organized 21 institutions to collect 36 cases of neonatal stroke from January 2020 to December 2022.The incidence,clinical characteristics,treatment,and prognosis of neonatal stroke in Shenzhen were analyzed.Results The incidence rate of neonatal stroke in 21 hospitals from 2020 to 2022 was 1/15 137,1/6 060,and 1/7 704,respectively.Ischemic stroke accounted for 75%(27/36);boys accounted for 64%(23/36).Among the 36 neonates,31(86%)had disease onset within 3 days after birth,and 19(53%)had convulsion as the initial presentation.Cerebral MRI showed that 22 neonates(61%)had left cerebral infarction and 13(36%)had basal ganglia infarction.Magnetic resonance angiography was performed for 12 neonates,among whom 9(75%)had involvement of the middle cerebral artery.Electroencephalography was performed for 29 neonates,with sharp waves in 21 neonates(72%)and seizures in 10 neonates(34%).Symptomatic/supportive treatment varied across different hospitals.Neonatal Behavioral Neurological Assessment was performed for 12 neonates(33%,12/36),with a mean score of(32±4)points.The prognosis of 27 neonates was followed up to around 12 months of age,with 44%(12/27)of the neonates having a good prognosis.Conclusions Ischemic stroke is the main type of neonatal stroke,often with convulsions as the initial presentation,involvement of the middle cerebral artery,sharp waves on electroencephalography,and a relatively low neurodevelopment score.Symptomatic/supportive treatment is the main treatment method,and some neonates tend to have a poor prognosis.

4.Salvianolic acid A contributes to cartilage endplate cell restoration by regulating miR-940 and miR-576-5p.

OBJECTIVE:
To investigate whether Salvianolic acid A (SAA) can restore cartilage endplate cell degeneration of intervertebral discs and to identify the mechanism via regulation of micro-RNA.
METHODS:
Cartilage endplate cells were isolated from lumbar intervertebral disc surgical samples and were treated with serum containing a series of concentrations of SAA (2, 5, and 10 ?M) for 24, 48, and 72 h to identify a proper dose and treatment time of SAA. The effect SAA on interlenkin-1β (IL-1β)-induced extracellular matrix degradation of cartilage endplate cells were analyzed by Alcian blue staining and assessment of the expression levels of ADAMTS-5, MMP3 and Col2a1. Further, the potential target miRNAs were preliminarily screened by micro-RNA sequencing combining qRT-PCR and Western blot, and then, the miRNAs mimics and inhibitors were used to verify the regulatory effect of SAA on potential target miRNAs.
RESULTS:
The 10 μM SAA treatment for 48 h significantly enhanced the viability of cartilage endplate cells, and increased Col2a1 expression and glycosaminoglycan accumulation that were repressed by IL-1β, and reduced the effect of IL-1β on ADAMTS-5, and MMP3. Screening analysis based on micro-RNA sequencing and Venny analysis identified the downstream micro-RNAs, including miR-940 and miR-576-5p. Then, the miR-940-mimic or miR-576-5p-mimic were transfected into CEPCs. Compared with the SAA group, the expression of ADAMTS-5 and MMP3 increased significantly and the expression of COL2A1 obviously decreased after overexpression of miR-940 or miR-576-5p in CEPCs.
CONCLUSION
Salvianolic acid A attenuated the IL-1β-induced extracellular matrix degradation of cartilage endplate cells by targeting regulate the miR-940 and the miR-576-5p.

5.GC-MS method for the determination of the genotoxic impurity chlorocyclohexane in trihexyphenidyl hydrochloride bulk drug

Gas chromatography-mass spectrometry (GC-MS) method was established for trace analysis of the potential genotoxic impurity chlorocyclohexane in trihexyphenidyl hydrochloride bulk drug, utilizing an RXI-5SIL MS column at isothermal temperature of 60 °C for the entire 6-minute run time.The inlet temperature was 180 °C and a split ratio of 10∶1 was used with the injection volume of 1.0 μL.The selective ion monitoring mode was set at m/z 82 for chlorocyclohexane with a detector voltage of 0.3 kV and an ion source temperature of 240 °C.The method was verified with respect to specificity, limit of detection (LOD), limit of quantitation (LOQ), accuracy, precision and robustness.Good linear correlation was achieved with coefficient r of 0.999 9 in the concentration range of 59.72-493 ng/mL.The intra- and inter-day precision was satisfactory (RSD ≤ 5.0%) and robust (RSD ≤ 1.65%).The proposed method in this study can be adequately adopted as a tool for quality assurance of trihexyphenidyl hydrochloride in routine test of potential genotoxic impurity.

OBJECTIVE:
To explore clinical effect of the first metatarsophalangeal joint fusion combined with lateral toe rotation Weil osteotomy in treating hallux valgus with severe metatarsal adduction.
METHODS:
From March 2017 to August 2021, 37 patients ( 69 feet ) with severe plantar adductor hallux valgus were treated with the first metatarsophalangeal joint fusion combined with rotational Weil osteotomy were retrospectively analyzed, including 8 males(11 feet) and 29 females (58 feet), aged from 67 to 83 years old with an average of (70.03±2.87) years old;3 cases on the left side, 2 cases on the right side and 32 cases on both sides. Visual analogue scale(VAS) was used to evaluate degree of pain relief before operation, 6 weeks after operation and at the final follow-up. American Orthopaedic Foot and Ankle Surgery (AOFAS) forefoot score was used to evaluate function of the affected foot before operation and final follow-up. Hallux valgus angle(HVA) and intermetatarsal angle(IMA) were measured before operation and at the final follow-up.
RESULTS:
Thirty-seven patients(69 feet) were followed up from 12 to 48 months with an average of(22.8±0.6) months. Bone healing was achieved at the first metatarsophalangeal joint from 7 to 10 weeks with an average of (8.00±1.21) weeks after operation, without delay and nonunion. HVA was increased from (44.30±2.84)° before operation to (15.20±2.13) °at the final follow-up, and had statistical difference(t=65.781, P<0.05);while no difference in IMA before and after operation(P>0.05). VAS was decreased from (6.73±1.48) points to (2.78±0.71) points at 6 months after operation(t=3.279, P<0.05), and had difference compared with the latest follow-up(1.16±1.12)(t=4.859, P<0.05). AOFAS forefoot score increased from (52.14±5.78) preoperatively to (86.70±4.86) at the fonal follow-up, and 25 feet got excellent results, 40 feet good and 4 feet fair.
CONCLUSION
The first metatarsophalangeal joint fusion combined with lateral toe rotation Weil osteotomy in treating severe plantar adduction hallux valgus could significantly relieve pain and appearance of forefoot, stabilize the first sequence, and significantly improve walking function.

7.Antioxidant and Anti-inflammatory Capacity of Ferulic Acid Released from Wheat Bran by Solid-state Fermentation of Aspergillus niger.

OBJECTIVE:
A strain of Aspergillus niger (A. niger), capable of releasing bound phenolic acids from wheat bran, was isolated. This strain was identified by gene sequence identification. The antioxidant and anti-inflammatory capacity of ferulic acid released from wheat bran by this A. niger strain (FA-WB) were evaluated.
METHODS:
Molecular identification techniques based on PCR analysis of specific genomic sequences were conducted; antioxidant ability was examined using oxygen radical absorbance capacity (ORAC), cellular antioxidant activity (CAA) assays, and erythrocyte hemolysis assays. RAW264.7 cells were used as a model to detect anti-inflammatory activity.
RESULTS:
The filamentous fungal isolate was identified to be A. niger. ORAC and CAA assay showed that FA-WB had better antioxidant activity than that of the ferulic acid standard. The erythrocyte hemolysis assay results suggested that FA-WB could attenuate AAPH-induced oxidative stress through inhibition of reactive oxy gen species (ROS) generation. FA-WB could significantly restore the AAPH-induced increase in intracellular antioxidant enzyme activities to normal levels as well as inhibit the intracellular malondialdehyde formation. TNF-a, IL-6, and NO levels indicated that FA-WB can inhibit the inflammation induced by lipopolysaccharide (LPS).
CONCLUSION
Ferulic acid released from wheat bran by a new strain of A. niger had good anti-inflammatory activity and better antioxidant ability than standard ferulic acid.

8.Epidemiological survey and study of hepatitis E virus among HIV-infected population in Hubei Province

Objective To determinate the seroprevalence of hepatitis E virus (HEV) exposition among HIV-infected people in Hubei Province so as to provide basic data for effective prevention and control of HIV and hepatitis E virus. Methods A total of 335 serum samples of HIV-infected people were collected from January to June 2017.Serum samples were subjected to anti-HEV IgG andanti-HEV IgM screening. Data were statistically analyzed. Results In 335 HIV-infected people,0.89% (3/335) of serum samples were anti-HEV IgM positive and 41.49% (139/335) of them were anti-HEV IgG positive.The anti-HEV IgG positive rate was not linearly correlated with age ( 2linear=0.756,P=0.385), and there was no significant difference between the age groups (P>0.05). There was no significant difference in anti-HEV IgG positive rate between different gender( 2=0.085,P=0.771).Anti-HEV IgG positive rate for the crowd with CD4 value ≤ 200 cell/μl was higher than that with CD4 >200 cell/μl,but showed no significant difference( 2=1.016,P=0.314),and CD4 indexes of three positive anti-HEV IgM patients were 222, 446 and 198 in cell/μl.The anti-HEV IgG positive rate in HIV-infected population in eastern Hubei (30.77%, 32/1 041) was statistically different from that in central Hubei (47.92%, 46/96) ( 2=6.169,P=0.013) , and it was also different from that in southeastern Hubei (45.79%, 49/107) ( 2=5.034,P=0.025). Conclusions There were higher HEV infection rate among HIV-infected people in Hubei Province; thus, it is essential to positively carry out double-way screening for these two diseases at the same time, and effective prevention and control of these viruses are required.

Background:Clinical outcomes of undifferentiated arthritis (UA) are diverse,and only 40 % of patients with UA develop rheumatoid arthritis (RA) after 3 years.Discovering predictive markers at disease onset for further intervention is critical.Therefore,our objective was to analyze the clinical outcomes of UA and ascertain the predictors for RA development.Methods:We performed a prospective,multi-center study from January 2013 to October 2016 among Chinese patients diagnosed with UA in 22 tertiary-care hospitals.Clinical and serological parameters were obtained at recruitment.Follow-up was undertaken in all patients every 12 weeks for 2 years.Predictive factors of disease progression were identified using multivariate Cox proportional hazards regression.Results:A total of 234 patients were recruited in this study,and 17 (7.3%) patients failed to follow up during the study.Among the 217 patients who completed the study,83 (38.2%) patients went into remission.UA patients who developed RA had a higher rheumatoid factor (RF)-positivity (42.9% vs.16.8%,x2=8.228,P=0.008),anti-cyclic citrullinated peptide (CCP) antibodypositivity (66.7% vs.10.7%,x2 =43.897,P < 0.001),and double-positivity rate of RF and anti-CCP antibody (38.1% vs.4.1%,x2 =32.131,P < 0.001) than those who did not.Anti-CCP antibody but not RF was an independent predictor for RA development (hazard ratio 18.017,95% confidence interval:5.803-55.938;P < 0.001).Conclusion:As an independent predictor of RA,anti-CCP antibody should be tested at disease onset in all patients with UA.
10.Comparison of Long-term Outcomes in Patients with Premature Triple-vessel Coronary Disease Undergoing Three Different Treatment Strategies: A Prospective Cohort Study.

BACKGROUNDPatients with premature triple-vessel disease (PTVD) have a higher risk of recurrent coronary events and repeat revascularization; however, the long-term outcome of coronary artery bypass grafting (CABG), percutaneous coronary intervention (PCI), and medical therapy (MT) alone for PTVD patients is controversial. The aim of this study is to evaluate the long-term outcome of PTVD patients among these three treatment strategies, to find out the most appropriate treatment methods for these patients.
METHODSOne thousand seven hundred and ninety-two patients with PTVD (age: men ≤50 years and women ≤60 years) were enrolled between 2004 and 2011. The primary end point was all-cause death. The secondary end points were cardiac death, myocardial infarction, stroke, or repeat revascularization.
RESULTSPCI, CABG, and MT alone were performed in 933 (52.1%), 459 (25.6%), and 400 (22.3%) patients. Both PCI and CABG were associated with lower all-cause death (4.6% vs. 4.1% vs. 15.5%, respectively, P < 0.01) and cardiac death (2.8% vs. 2.0% vs. 9.8%, respectively, P < 0.01) versus MT alone. The rate of repeat revascularization in the CABG group was significantly lower than those in the PCI and MT groups. After adjusting for baseline factors, PCI and CABG were still associated with similar lower risk of all-cause death and cardiac death versus MT alone (all-cause death: hazard ratio [HR]: 0.35, 95% confidence interval [CI]: 0.23-0.53, P < 0.01 and HR: 0.35, 95% CI: 0.18-0.70, P = 0.003, respectively, and cardiac death: HR: 0.32, 95% CI: 0.19-0.54, P < 0.01 and HR: 0.36, 95% CI: 0.14-0.93, P = 0.03, respectively).
CONCLUSIONSPCI and CABG provided equal long-term benefits for all-cause death and cardiac death for PTVD patients. Patients undergoing MT alone had the worst long-term clinical outcomes.
