1.Platelet bacterial contamination in China: a meta-analysis

		                        			Objective: To investigate the status and influencing factors of platelet bacterial contamination in China, and to provide theoretical support for relevant policies in blood collection and transfusion institutions. Methods: A meta-analysis by systematically searching studies on platelet bacterial contamination in China published between 1998 and 2023 was conducted. Data analysis was performed using R4.4 software to combine studies that met the inclusion criteria. Results: Twenty-three studies were included after screening. The combined analysis showed that the overall contamination rate of platelets in China was 0.18% (95% CI: 0.12%-0.24%). The contamination rate of manually condensed platelets was significantly higher than that of apheresis platelet concentrates (0.28% vs 0.17%, P<0.01). No significant difference in platelet contamination rates was found between eastern and central regions (0.21% vs 0.15%, P>0.01). The contamination rate of aerobic bacteria was higher than that of anaerobic bacteria (0.11% vs 0.06%, P<0.01). Publication bias analysis indicated robust results, and sensitivity analysis showed minimal impact of excluding individual studies on the overall conclusion. Conclusion: Although the platelet contamination rate in China is generally low, significant differences exist across collection methods and regions.
		                        		
		                        		
		                        		
		                        	
2.Effect of Ultrasound-guided Superficial Parasternal Intercostal Plane Block on The Quality of Recovery in Patients Undergoing Sternotomy Cardiac Surgery

		                        			ObjectiveThis study aims to explore the effect of ultrasound-guided superficial parasternal intercostal plane block on the quality of recovery and postoperative analgesia in patients undergoing sternotomy cardiac surgery. MethodsA total of 64 patients undergoing sternotomy cardiac surgery were selected for this study. They were randomly divided into two groups: one group received a superficial parasternal intercostal plane block with ropivacaine (the ropivacaine group), while the other was given normal saline (the normal saline group). The primary outcome was the Quality of Recovery-15 (QoR-15) score on postoperative day 1 in both groups, accompanied by a comparative analysis of the pain score and opioid usage. ResultsCompared with the normal saline group, the ropivacaine group exhibited a significantly higher QoR-15 score on postoperative day 1[(89.60±13.24) vs (81.18±12.78), P=0.012]. The numerical rating scale at rest was significantly lower[(3.03±0.72) vs (4.26±0.93), P<0.001], and the numerical rating scale during coughing was also significantly reduced [(4.40±0.89) vs (5.44±1.05), P<0.001]. Concurrently, the cumulative morphine equivalent consumption during the initial 24 h postoperatively was significantly lower in patients who were administered the ropivacaine [14.15 (4.95~30.00) mg vs 40.50 (19.25~68.18) mg, P=0.002], and there was also a notable decrease in the rescue analgesia [0.00 (0.00~0.00) mg vs 0.00 (0.00~100.00) mg, P=0.007]. ConclusionUltrasound-guided superficial parasternal intercostal plane block can significantly enhance the overall quality of recovery in patients undergoing sternotomy cardiac surgery on postoperative day 1. The technique contributes to improved postoperative analgesic effects and a reduction in opioid usage, thereby facilitating early postoperative recovery. 
		                        		
		                        		
		                        		
		                        	
3.Development of self-care scale for patients with lymphedema after breast cancer surgery and verification of its reliability and validity

		                        			Objective To develop a self-care scale for patients with lymphedema after breast cancer surgery and verify its reliability and validity.Methods Based on the model of knowledge,belief and practice,a questionnaire item pool was constructed after literature reviews and qualitative interviews.A questionnaire-based scale was drafted based on the established item pool by carrying out two rounds of consultation with 15 clinical nursing specialists,nursing administrators and nursing educators from 8 provinces or cities in China.Reliability and validity of the scale were tested using convenience sampling,involving 444 patients with breast cancer surgery related lymphedema from 7 general hospitals in Hubei and Henan provinces,China,between May and July 2023.Results The response rates for the two rounds of expert consultation were 93.75%and 93.33%,respectively.The authority coefficients of the two rounds were 0.86 and 0.89,respectively,and the coordination coefficients for the 2 rounds were 0.130 and 0.379,respectively.In the first round,the average importance rating was from 4.33 to 4.93 with the coefficient of variation from 0.05 to 0.19,and the full score ratio from 53.33%to 93.33%.In the second round,the average importance rating ranged from 2.86 to 4.93 with the coefficient of variation from 0.05 to 0.36,and the full score ratios from 7.14%to 92.86%.A total of 421 patients completed the survey.The overall Cronbach's α coefficient of the scale was 0.943,the overall split-half reliability was 0.824,the scale-level content validity index(S-CVI)was 0.912,and the item-level content validity index(I-CVI)of the total scale ranged from 0.857 to 1.000.The KMO value of exploratory factor analysis was 0.919,the Bartrett spherical test value was 4671.724(P<0.001),and the cumulative variance contribution rate was 64.155%.Confirmatory factor analysis showed a good model fit.After the reliability and validity tests,the scale was finalised and determined to consist of three dimensions with 33 items:knowledge(9 items),attitude(6 items)and behaviour(18 items).Conclusion The self-care scale for the patients with lymphedema after breast cancer surgery has demonstrated good reliability and validity,and makes it an effective assessment tool for the patients with lymphedema after breast cancer surgery.

		                        			Hyperkalemia is one of the common ion metabolism disorders in clinical practice. Hyperkalemia is defined as serum potassium higher than 5.0 mmol/L according to the guidelines at home and abroad. Acute severe hyperkalemia can cause serious consequences, such as flaccid paralysis, fatal arrhythmia, and even cardiac arrest. The use of renin-angiotensin- aldosterone system inhibitors, β-blockers and diuretics, low-sodium and high-potassium diets, and the presence of related comorbidities increase the occurrence of hyperkalemia. Hyperkalemia risk exist in all clinical departments, but there is a lack of a standardization in the management of multi- department cooperation in hospital. Therefore, a number of domestic nephrology and cardiology department experts have discussed a management model for multi-department cooperation in hyperkalemia, formulating the management standard on hospital evaluation, early warning, diagnosis and treatment, and process. This can promote each department to more effectively participate in nosocomial hyperkalemia diagnosis and treatment, as well as the long-term management of chronic hyperkalemia, improving the quality of hyperkalemia management in hospital.
		                        		
		                        		
		                        		
		                        	
5.Research Status and Trends of Cohort Studies on Efficacy Evaluation of Traditional Chinese Medicine:A Bibliometrix-based Visual Analysis on Literature from 2017 to 2022

		                        			ObjectiveTo explore the research status and trends of cohort studies on traditional Chinese medicine (TCM) efficacy evaluation from 2017 to 2022 and provide ideas and references for research in this field. MethodsSix databases including Pubmed, Web of Science, Embase, Scopus, Cochrane Library and CNKI were searched from January 1st, 2017 to December 31st, 2022. The total number of annual publications, journals, highly cited literatures, and keywords were quantitatively and visually analyzed by Bibliometrix. ResultsA total of 328 articles were included, which were published in 141 journals. The number of articles published in this field showed an overall upward trend, and retrospective cohort studies (282 papers, 85.98%) accounted for the largest proportion. A total of 151 cohort studies (46.04%) were conducted based on the database and showed an overall upward trend. The subjects were mainly patients with tumors (77 papers, 23.48%), and cardiovascular and cerebrovascular diseases (64 papers, 19.51%). The top 3 highly cited literatures mainly explore the association between TCM and survival outcome and quality of life in patients with malignant tumors. Fourteen and twenty-five high-frequency keywords were included in Chinese and English literature respectively, which formed 3 clusters such as research methods, statistical analysis and diseases. ConclusionIt was the current status to focus on retrospective cohort studies and focus on patients with tumors or cardiovascular and cerebrovascular diseases. 6.An integrated curriculum for epidemiology and medical statistics teaching in undergraduate students majoring in clinical medicine: lesson learned from teaching reform

		                        			Epidemiology and medical statistics are essential courses for undergraduate students majoring in clinical medicine. By studying the two courses, they can obtain the core skills for their future clinical practice. High-level medical schools both at home and abroad have accumulated successful experiences in curriculum, teaching methods and teaching models of the two disciplines. These colleges have also carried out the exploration of the curriculum reform centering on "organ systems integration". This paper summarizes the current status of epidemiology and medical statistics teaching and curriculum integration in representative medical schools both at home and abroad, and puts forward suggestions for deepening teaching reform and optimizing the curriculum system to provide reference for the integration of epidemiology and medical statistics curriculums for undergraduate students majoring in clinical medicine in China.
		                        		
		                        		
		                        		
		                        	
7.A XGBoost model for risk prediction of lower extremity deep vein thrombosis after internal fixation surgery for thoracolumbar fractures

		                        			Objective To construct a predictive model for the occurrence of lower extremity deep vein thrombosis(DVT)after internal fixation surgery for thoracolumbar fractures by using extreme gradient boosting(XGBoost).Methods Data of 220 patients who underwent internal fixation surgery for thoracolumbar fractures in the First Affiliated Hospital of Wenzhou Medical University from January 2019 to December 2022 was collected.The dataset was divided into a training set(154 cases)and a testing set(66 cases).The training set was processed by using the synthetic minority over-sampling technique and the predictive model was build based on XGBoost.The performance was compared on the testing set by using area under receiver operating characteristic curve,accuracy,F1 score,sensitivity and specificity.The interpretability analysis base on SHAP was conducted to quantify the degree of contribution of influencing factors.Results The XGBoost model outperformed logistic regression,support vector machine and random forest models on multiple metrics,with an area under the curve of 0.761 on the original testing set.The decision curve indicated that the XGBoost model has clinical application value.Conclusion The XGBoost model based on factors such as age,body mass index,and postoperative albumin,D-dimer,total protein,erythrocyte sedimentation rate,prothrombin time can effectively predict the occurrence of lower extremity DVT after internal fixation surgery for thoracolumbar fractures,which has good potential for clinical application.
		                        		
		                        		
		                        		
		                        	
8.Clinical efficacy of distal radius dome osteotomy combined with Vickers ligament release in the treatment of Madelung′s deformity

		                        			Objective:To analyze the clinical efficacy of the distal radius dome osteotomy combined with Vickers ligament release (DRO+ VR) in the treatment of Madelung′s deformity.Methods:A retrospective case series analysis.The clinical data of 15 children with Madelung′s deformity treated in the Children′s Hospital of Nanjing Medical University from January 2012 to August 2023 were collected, and the children were divided into the DRO+ VR group and the other operation group according to the surgical method.The follow-up time was recorded, and the ulnar tilt (UT), lunate subsidence (LS), palmar carpal displacement (PCD), lunate fossa angle (LFA), wrist flexion (WF), wrist extension (WE), and visual analogue scale (VAS) were measured before surgery and at the last follow-up.The independent samples t-test was used for measurement data comparison, and the Fisher′s exact test was used for categorical variable comparison. Results:The follow-up time was 10-96 months, with the time of (27.17±15.51) months in the DRO+ VR group and (48.00±24.06) months in the other operation group.There was no significant difference in preoperative general data, imaging indexes and functional evaluation between the 2 groups (all P>0.05).The comparison of imaging indexes and functional evaluation of children in the DRO+ VR group before surgery and at the last follow-up showed that the differences in UT [(49.00±3.10) ° vs.(31.83±2.40) °], LFA [(34.50±3.78) ° vs.(49.83±4.02) °], LS [(7.29±3.61) mm vs.(2.29±1.48) mm], PCD [(12.06±3.39) mm vs.(4.35±2.37) mm], WF [(61.17±1.47) ° vs.(67.50±3.33) °], WE [(48.67±1.86) ° vs.(60.50±4.42) °], and VAS [(7.33±1.03) points vs.(2.67±0.52) points] were statistically significant (all P<0.05).At the last follow-up, the results of imaging indexes and functional evaluation of the 2 groups showed that the improvement of UT was (17.17±2.32) ° in the DRO+ VR group and (51.78±7.66) ° in the other operation group, which had statistically difference ( t=-2.241, P=0.043). Conclusions:DRO+ VR is an effective approach for treating Madelung′s deformity in children.It can significantly improve children′s imaging indexes and wrist joint function and alleviate pain symptoms.Surgical intervention should be actively considered for patients with severe deformity and pain symptoms related to Madelung′s deformity.
		                        		
		                        		
		                        		
		                        	
9.Effect of Tongsheng Prescription on Trimethylamine N-Oxide and Related Metabolites Substrate in Patients with Acute Cerebral Infarction After Intervention and Its Efficacy Evaluation

		                        			Objective To investigate the effect of Tongsheng Prescription on trimethylamine N-oxide(TMAO)and related metabolites substrate in patients with acute cerebral infarction after intervention and to evaluate its clinical efficacy.Methods A prospective randomized controlled study was conducted in 48 patients with acute ischemic stroke with large vessel occlusion(AIS-LVO)who had received endovascular treatment(EVT).The patients were divided into an observation group(25 cases)and a control group(23 cases).After emergency EVT,the control group was treated with routine western medicine,and the observation group was treated with nasogastric feeding or oral use of Tongsheng Prescription on the basis of treatment for the control group.The course of treatment lasted for seven days,and then the patients were followed up for three months.Before operation,7 days and 90 days after operation,National Institutes of Health Stroke Scale(NIHSS)scores in the two groups were observed.The functional prognosis evaluated with Modified Rankin Scale(MRS)scores was compared between the two groups,and the effects of Tongsheng Prescription on serum TM AO and related metabolites choline,betaine and L-carnitine were investigated.Results(1)On the postoperative day 7 and 90,the NIHSS scores of the two groups were decreased compared with those before operation(P<0.01),and the decrease of NIHSS scores on the postoperative day 7 and 90 in the observation group was significantly superior to that in the control group(P<0.01).(2)On the postoperative day 90,the rate of good functional outcome in the observation group was 96.00%(24/25),which was significantly higher than 69.57%(16/23)in the control group,and the difference was statistically significant(P<0.05).(3)On the postoperative day 7 and 90,the levels of TMAO,L-carnitine and betaine in the observation group tended to decrease compared with those before operation,but the difference was not statistically significant(P>0.05).However,the levels of TMAO,L-carnitine and betaine in the control group were significantly increased compared with those before operation(P<0.01).On the postoperative day 7,the levels of TMAO,L-carnitine and betaine in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).(4)Spearman correlation analysis showed that TMAO was positively correlated with NIHSS score,L-carnitine and choline(P<0.05 or P<0.01).Conclusion Tongsheng Prescription has a significant effect on AIS-LVO patients seven days after EVT.The Prescription is beneficial to obtain a good functional outcome,and its mechanism may be related to the decrease of TMAO level through L-carnitine and betaine pathways,so as to improve the neurological deficit symptoms of stroke patients.
		                        		
		                        		
		                        		
		                        	
10.Application of different transbronchoscopic biopsies in the diagnosis of senile central lung cancer

		                        			Objective To study the application value of different transbronchial biopsies in the diagnosis of central lung cancer in elderly patients.Methods The clinical data of 97 elderly patients with central lung cancer diagnosed by pathology from June 2020 to June 2023 in the 923rd Hospital of Chinese People's Liberation Army Joint Logistic Support Force were retrospectively analyzed.According to the different initial transbronchial biopsy methods,the patients were divided into the endobronchial biopsy(EBB)group(n=51)and the conventional transbronchial needle aspiration(cTBNA)group(n=46).The histopathological results were statistically analyzed,and the first biopsy positive rates of EBB and cTBNA in the diagnosis of senile central lung cancer were calculated and compared.At the same time,the differences of biopsy tolerance and complications between the two groups were evaluated.Results The squamous cell carcinoma proportions in both groups were over 50%.There was no significant difference in the first biopsy positive rate between the two groups(P>0.05).The incidence of temporary retreat of the scope due to subjective tolerance in the EBB group was higher than that in the cTBNA group,and the difference was statistically significant(P<0.05).There was a statistically significant difference in the incidence of intraoperative complications of different grades between the two groups(P<0.001).Among them,the incidence of grade 2 and above complications during surgery in the EBB group was significantly higher than that in the cTBNA group(P<0.001).Conclusion For elderly patients with central lung cancer,the success rate of the first biopsy of EBB and cTBNA is roughly equivalent,but the incidence of postoperative complications of the latter is significantly lower than that of the former.cTBNA can be used as the first biopsy method for this population.
