1.Construction of iPSC-derived Inhibitory Neural Network Tissue with Synaptic Transmission Potentials

		                        			ObjectiveDirected differentiation of human induced pluripotent stem cells (hiPSCs) into spinal cord γ-aminobutyric acid (GABA)-ergic progenitor cells were implanted into an decellularized optical nerve (DON) bioscaffold to construct a hiPSC-derived inhibitory neural network tissue with synaptic activities. This study aimed to provide a novel stem cell-based tissue engineering product for the study and the repair of central nervous system injury. MethodsThe combination of stepwise directional induction and tissue engineering technology was applied in this study. After hiPSCs were directionally induced into human neural progenitor cells (hNPCs) in vitro, they were seeded into a DON for three-dimensional culture, allowing further differentiation into inhibitory GABAergic neurons under the specific neuronal induction environment. Transmission electron microscopy and whole cell patch clamp technique were used to detect whether the hiPSCs differentiated neurons could form synapse-like structures and whether these neurons had spontaneous inhibitory postsynaptic currents, respectively, in order to validate that the hiPSC-derived neurons would form neural networks with synaptic transmission potentials from a structural and functional perspective. ResultsThe inhibitory neurons of GABAergic phenotype were successfully induced from hiPSCs in vitro, and maintained good viability after 28 days of culture. With the transmission electron microscopy, it was observed that many cell junctions were formed between hiPSC-derived neural cells in the three-dimensional materials, some of which presented a synapse- like structure, manifested as the slight thickness of cell membrane and a small number of vesicles within one side of the cell junctions, the typical structure of a presynatic component, and focal thickness of the membrane of the other side of the cell junctions, a typical structure of a postsynaptic component. According to whole-cell patch-clamp recording, the hiPSC-derived neurons had the capability to generate action potentials and spontaneous inhibitory postsynaptic currents were recorded in this biotissue. ConclusionsThe results of this study indicated that hiPSCs can be induced to differentiate into GABAergic progenitor cells in vitro and can successfully construct iPSC-derived inhibitory neural network tissue with synaptic transmission after implanted into a DON for three-dimensional culture. 3.Effect of Guizhi Shaoyao Zhimutang on Cartilage Destruction and JAK2/STAT3 Signaling Pathway in Mice with Collagen-induced Arthritis

		                        			ObjectiveTo study the effect of Guizhi Shaoyao Zhimutang (GSZMD) on cartilage destruction in mice with collagen-induced arthritis (CIA) and its mechanism. MethodThirty-six DBA/1 mice in SPF grades were randomly divided into 6 groups, namely, the normal group, the model group, the methotrexate (MTX) group, the low-dose GSZMD group, the medium-dose GSZMD group, and the high-dose GSZMD group. Except the normal group, mice in the other 5 groups were used to establish the model of CIA by secondary immunization. The mice were given normal saline, MTX (1.5 mg·kg-1, 2 times a week), and low, medium, and high-doses GSZMD (6.3, 12.6, 25.2 g·kg-1·d-1) by intragastric administration on the day of the onset of hind limb swelling for 4 weeks. The changes in the degree of foot swelling of mice in each group were observed and recorded. The content of matrix metalloproteinase (MMP)-1, MMP-3, MMP-9, and MMP-13 in serum was determined by enzyme-linked immunosorbent assay (ELISA). The pathological changes in the ankle joint were observed by hematoxylin-eosin (HE) staining, and the cartilage destruction was observed by red fast green staining. The protein expression of the Janus kinase 2/signal transducer and activator of transcription 3 (JAK2/STAT3) signaling pathway in ankle joints were detected by Western blot. ResultAs compared with the normal group, the degree of foot swelling, the content of MMP-1, MMP-3, MMP-9, and MMP-13 in serum and the expression levels of phosphorylation (p)-JAK2/JAK2 and p-STAT3/STAT3 in ankle joints of the model group were increased (P<0.01), and the joint damage was aggravated. As compared with the model group, the degrees of foot swelling of the mice in the MTX group and the low, medium, and high-dose GSZMD group were reduced (P<0.05, P<0.01), and the content of MMP-1, MMP-3, MMP-9, and MMP-13 in serum was decreased (P<0.05, P<0.01). The pathological joint damage was alleviated, and the expression levels of p-JAK2/JAK2 and p-STAT3/STAT3 in ankle joints were decreased in the MTX group and GSZMD groups (P<0.05, P<0.01). ConclusionGSZMD can reduce the degree of joint swelling in mice with CIA, inhibit the expressions of MMP-1, MMP-3, MMP-9, and MMP-13, and alleviate the destruction of articular cartilage. 4.Clinical characteristics and guideline compliance analysis of chronic obstructive pulmonary disease patients with initial triple therapy in real-life world

		                        			Objective:To observe the clinical characteristics and guideline compliance of chronic obstructive pulmonary disease (COPD) patients with initial triple therapy in real-life world.Methods:This study is a cross-sectional study. The subjects of the study were COPD patients admitted to 13 hospitals in Hunan Province and Guangxi Zhuang Autonomous Region from December 2016 to December 2021. The initial treatment was triple inhaled drugs. The data collected included gender, age, diagnosis, body mass index (BMI), history of acute exacerbation (AE) in the past year, pulmonary function, COPD Assessment Test (CAT) score, modified British Medical Research Council Dyspnea Questionnaire (mMRC), inhaled drugs and other indicators. The characteristics and differences of COPD patients before and after 2020 were analyzed.Results:7 184 patients with COPD were enrolled in this study, including 2 409 COPD patients treated with initial triple therapy, accounting for 33.5%(2 409/7 184). Taking January 1st, 2020 as the cut-off point, 1 825 COPD patients (75.8%) received initial treatment with triple inhaled drugs before 2020 and 584 patients (24.2%) after 2020 were included in this study. Compared with COPD patients before 2020, the COPD patients after 2020 had higher FEV 1% [(40.9±15.5 )% vs (39.3±15.5)%, P=0.040], lower CAT [(15.8±6.5)point vs (17.5±6.2)point, P<0.001], less AE in the past year [1(0, 2)times vs 1(0, 2)times, P=0.001] and higher rate of non-AE [255(43.7%) vs 581(37.1%), P=0.006]. In addition, before 2020, patients with COPD were mainly treated with open triple drugs (1 825/1 825, 100%); after 2020, 306 patients (52.4%) received open triple inhaled drugs, and 278 patients (47.6%) received closed triple inhaled drugs. Conclusions:In real-life world, most of patients with COPD treated with triple therapy have severe lung function, obvious symptoms and high risk of acute exacerbation. The real-world prescribing of triple therapy in patients with COPD does not always reflect recommendations in guidelines and strategies, and overtreatment is common. 		                        			For preterm infants with a birth weight less than 1 500 g, the higher intensity of resuscitation in the delivery room is related to lower rate of antenatal corticosteroid therapy, lower gestational age, and lower birth weight. The infants undergoing tracheal intubation or ECRP in the delivery room have an increased incidence rate of adverse clinical outcomes. 7.Effect of bloodletting at tip of ear with different amounts on pain in patients with early-stage stye: a randomized controlled trial.

		                        			OBJECTIVE:
		                        			To compare the curative effect between bloodletting at tip of ear with different amounts combined with western medication and western medication alone for early-stage stye.
		                        		
		                        			METHODS:
		                        			A total of 108 patients with early-stage stye were randomly divided into a 50 μL group (36 cases, 4 cases dropped off), a 100 μL group (36 cases, 2 cases dropped off) and a western medication group (36 cases, 5 cases dropped off). The patients in the western medication group were treated with levofloxacin eye drops and levofloxacin hydrochloride eye gel. Based on the treatment of the western medication group, the patients in the 50 μL group were treated with 50 μL bloodletting (about 3 drops) at tip of ear while the patients in the 100 μL group were treated with 100 μL bloodletting (about 6 drops) at tip of ear; the bloodletting was given once a day for 3 days. After treatment, the changes of visual analogue scale (VAS) and clinical effect were observed, and the patients were followed up by telephone on the 8th day.
		                        		
		                        			RESULTS:
		                        			After treatment, the VAS score in each group was reduced (<0.01), and the score in the 50 μL group and 100 μL group was lower than that in the western medication group (<0.05, <0.01), and the score in the 100 μL group was lower than that in the 50 μL group (<0.05). After treatment, the cured rate was 76.5% (26/34) in the 100 μL group, 71.9% (23/32) in the 50 μL group and 51.6% (16/31) in the western medication group, and there were no significant differences among three groups (>0.05). One week after the onset of the disease, all the patients in the 50 μL group and 100 μL group were cured, and one patient in the western medication group was not cured, and treated with routine surgery.
		                        		
		                        			CONCLUSION
		                        			Based on the conventional western medication treatment, bloodletting at tip of ear can significantly reduce the pain of stye, and the effect of 100 μL bleeding is better than 50 μL.
		                        		
		                        		
		                        		
		                        	
8.Key technologies in digital breast tomosynthesis system:theory, design, and optimization.

		                        			OBJECTIVE:
		                        			To develop a digital breast tomosynthesis (DBT) imaging system with optimizes imaging chain.
		                        		
		                        			METHODS:
		                        			Based on 3D tomography and DBT imaging scanning, we analyzed the methods for projection data correction, geometric correction, projection enhancement, filter modulation, and image reconstruction, and established a hardware testing platform. In the experiment, the standard ACR phantom and high-resolution phantom were used to evaluate the system stability and noise level. The patient projection data of commercial equipment was used to test the effect of the imaging algorithm.
		                        		
		                        			RESULTS:
		                        			In the high-resolution phantom study, the line pairs were clear without confusing artifacts in the images reconstructed with the geometric correction parameters. In ACR phantom study, the calcified foci, cysts, and fibrous structures were more clearly defined in the reconstructed images after filtering and modulation. The patient data study showed a high contrast between tissues, and the lesions were more clearly displayed in the reconstructed image.
		                        		
		                        			CONCLUSIONS

9.Application of video education combined with Teach-back in patients with rheumatoid arthritis

		                        			Objective? To explore the effect of video education combined with Teach-back on the compliance of patients with rheumatoid arthritis(RA) to exercise at home and the condition of disease activity. Methods? By convenience sampling, 80 patients with RA who were hospitalized in the Rheumatology and Immunology Department of Sichuan Provincial People's Hospital from March to November 2018 were included in the study. By random number table, the patients were divided into the intervention group and control group, the control group was given routine discharge instruction and the patients were taught about the functional exercise orally; on the basis of that, the intervention group was given the video education combined with Teach-back and assisted to download videos, Family Exercise Compliance Scale and Disease Activity Score in 28 Joints(DAS28) were used to evaluate the patients' compliance in functional exercise at home and disease activity condition before intervention, 1 month and 3 months after intervention. Results? Finally, 73 cases(37 cases in the control group, 36 cases in the intervention group) in two groups completed the research plan. One and 3 months after intervention, the compliance with family functional exercise of the intervention group was better than the control group from the same period with statistical significance(P<0.05); the scores of Tender Joint Coun(t TJC28), Swollen Joint Coun(t SJC28), Patient General Health(GH) of the intervention group were all lower than the control group from the same period with statistical significance (P<0.05), however, the scores of Erythrocyte Sedimentation Rate(ESR) and DAS28 didn't demonstrate obvious intervention effects (P>0.05). Conclusions? Video education combined with Teach-back can effectively improve the compliance of RA patients with home functional exercise, help to cultivate healthy behavior, and hence reduce the number of tender joint, swelling joints, reduce the degree of pain, improve the prognosis of patients.
		                        		
		                        		
		                        		
		                        	
10.23-Gauge pars plana vitrectomy and scleral buckle for the treatment of proliferative rhegmatogenous retinal detachment with inferior breaks

		                        			Objective To compare the safety and efficacy of 23-Gauge pars plana vitrectomy (PPV) and PPV combined with scleral buckling-PPV (SB-PPV) in proliferative rhegmatogenous retinal detachments with inferior breaks.Methods Retrospectively nonrandomized clinical case study was conducted in 70 patients with proliferative rhegmatogenous retinal detachment associated with inferior breaks between January 2013 and December 2016,including 39 eyes receiving SB-PPV procedures as the SB-PPV group and 31 eyes undergoing PPV procedures as the PPV group.And anatomical success rate for one procedure,lens trauma rate,retinotomy rate,postoperative best corrected visual acuity (BCVA) outcome and intraocular pressure (IOP) were recorded and analyzed.Results The anatomical success rate for one procedure was 92.3% (36/ 39) in the SB-PPV group and 74.2% (23/31) in the PPV group,and the difference was statistically significant (P < 0.035).The lens trauma rate was 2.6% (1/39) in the SB-PPV group and 19.4% (6/31) in PPV group,with the difference being statistically significant (P <0.05).The retinotomy rate was 5.1% (2/39) in the SB-PPV group and 32.3% (10/31) in the PPV group,and the difference approached statistically significance (P <0.05).There was no significant difference in the postoperative BCVA and IOP between the two groups (both P > 0.05).Conclusion SB-PPV can increase the anatomical success rate for one procedure in patients with rhegmatogenous retinal detachment associated with inferior breaks,and reduce retinotomy rate and lens trauma rate.
