		                        			Objective To investigate the predictive value of new simplified insulin resistance(IR)assessment indexes in identifying subclinical left ventricular systolic function impairment in patients with type 2 diabetes mellitus(T2DM).Methods A total of 150 T2DM patients with preserved left ventricular ejection fraction(LVEF≥50%)who were admitted to Department of Endocrinology of the First Affiliated Hospital of Air Force Medical University from June 2021 to December 2021 were retrospectively analyzed.All patients underwent two-dimensional speckle tracking echocardiography to measure left ventricular global longitudinal strain(GLS).According to GLS value,the subjects were divided into the normal group(GLS≥18%group,n=80)and the impaired group(GLS<18%group,n=70).Some new simplified IR assessment indicators were calculated and compared between the two groups,including body mass index(BMI),TG/HDL-C ratio,triglyceride-glucose(TyG)index,TyG-BMI index,TyG-WHR and metabolic score for IR(METS-IR).Correlation between the GLS and the new simplified IR assessment indexes was analyzed.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of different simplified IR assessment indexes,with the area under the curve(AUC)calculated.Furthermore,according to whether the subjects were complicated with hypertension,binary logistics regression analysis was performed to explore the independent correlation between the simplified IR assessment index and GLS<18%.Results Total 150 were included with aged(54.5±13.7)years with 96(64.0%)men and 54(36.0%)women.Compared with the GLS≥18%group,the TG/HDL-C ratio,TyG index,TyG-BMI,and METS-IR of subjects in the GLS<18%group were significantly increased(P<0.05).Pearson correlation analysis showed that TG/HDL-C ratio,TyG index,TyG-BMI,TyG-WHR,and METS-IR were negatively correlated with GLS(P<0.05).ROC analysis showed that TyG index had a certain predictive value for the evaluation of GLS<18%(AUC=0.678,95%CI 0.591-0.765,P<0.001).Stratification based on hypertension and further adjusting for confounding factors,TyG index remains significantly associated with GLS<18%(OR=3.249,95%CI 1.045-10.103,P=0.042).Conclusions The novel simplified insulin resistance evaluation indexes are closely associated with left ventricular subclinical systolic dysfunction in T2DM patients with preserved ejection fraction.TyG index is an effective index to identify left ventricular subclinical dysfunction in these populations.
		                        		
		                        		
		                        		
		                        	
3.The impact of programming optimization for atrioventricular synchrony after Micra AV leadless pacemakers implantation

		                        			Objective To analyze the atrioventricular synchronization rate after implantation of Micra AV leadless pacemaker,and the impact of postoperative programming optimization on atrioventricular synchronization rate.Methods A prospective cohort study was conducted to select patients with complete atrioventricular block who underwent Micra AV leadless pacemaker implantation at Beijing Anzhen Hospital from August 2022 to June 2023.Programming optimization were performed at 1 week,1 month,and 3 months postoperatively,and atrioventricular synchronization rate,electrical parameters,and echocardiography were recorded.Results A total of 68 patients with complete atrioventricular block implanted with Micra AV were selected,with an average age of(68.2±9.7)years,including 47 males(69.1%).All patients were successfully implanted with Micra AV,and there were no serious postoperative complications;The average threshold,sense,and impedance parameters were stable during 1 week,1 month,and 3 months after the procedure;There was no significant difference in the EF value of postoperative echocardiography(P=0.162);The average atrioventricular synchronization rates at 1 week,1 month,and 3 months postoperatively were(75.2%vs.83.8%vs.91.6%,P=0.001).Conclusions As an mechanical atrial sensing,Micra AV requires personalized adjustment of relevant parameters;Postoperative follow-up programming optimization plays an important role in the atrioventricular synchronization and comfort level in patients with complete atrioventricular block after implantation of Micra AV leadless pacemaker.
		                        		
		                        		
		                        		
		                        	
4.Inhibitory effect of Heidihuang Pills on renal fibrosis in a rat model of chronic renal failure via regulation of IGF-1 expression

		                        			AIM To explore the effect of Heidihuang Pills on renal fibrosis in a rat model of chronic renal failure(CRF)and its mechanism.METHODS Wistar rats were randomly divided into the blank group for normal feeding and the model group for the establishment of CRF rat models by 5/6 nephrectomy.Subsequently,the successfully established rat models were randomly divided into the model group,the Heidihuang Pills group(10.43 g/kg),and the Heidihuang Pills+IGF-1R blocker(JB1)group for a regimen of 7-day subcutaneous injection of 18 μg/kg JB1 followed by gavage of 10.43 g/kg Heidihuang Pills.Eight weeks after the administration,the rats had their serum levels of Scr and BUN detected;their pathological changes of renal tissue observed by HE and Masson staining;their renal protein expressions of TGF-β,HIF-1α and α-SMA detected by immunohistochemistry;their renal protein expressions of IGF-1R and TGF-β detected by Western blot;and their renal mRNA expressions of IGF-1R and TGF-β detected by RT-qPCR.RESULTS Compared with the blank group,the model group displayed increased serum levels of Scr and BUN(P<0.05);increased,degree of renal fibrosis,and renal fibrosis area(P<0.05);increased renal expressions of TGF-β,HIF-1α,α-SMA proteins and TGF-β mRNA(P<0.05);and decreased expressions of IGF-1R mRNA and protein(P<0.05).Compared with the model group,the Heidihuang Pills group displayed decreased serum Scr and BUN levels(P<0.05);decreased inflammatory cells in renal interstitium and the fibrosis degree(P<0.05);decreased renal expressions of TGF-β,HIF-1α,α-SMA proteins and TGF-β mRNA(P<0.05);and increased expressions of IGF-1R mRNA and protein(P<0.05).However,the administration of JB1 could weaken the improvement effect of Heidihuang Pills on renal fibrosis in CRF rats(P<0.05).CONCLUSION Heidihuang Pills can inhibit the renal fibrosis in CRF rats,and the inhibition process is related to up-regulated IGF-1 expression and promoted combination of IGF-1 and IGF-1R.
		                        		
		                        		
		                        		
		                        	
5.Comparison of clinical effects of different doses of meropenem in the treatment of septic shock

		                        			OBJECTIVE To compare the clinical effects of different doses of meropenem in the treatment of septic shock. METHODS One hundred and six patients with septic shock were randomly divided into standard-dose group and high-dose group, with 53 cases in each group. Patients in the standard-dose group were given standard dose of meropenem (initial intravenous injection of 1 g meropenem more than 30 minutes, followed by 1 g meropenem intravenously every 8 hours, each time for more than 3 hours); patients in the high-dose group were given high dose of meropenem (initial intravenous injection of 2 g meropenem more than 30 minutes, followed by 2 g meropenem intravenously every 8 hours, each time for more than 3 hours); other treatment measures were determined based on the specific conditions of the patients. The main observation indicators were the increments of sequential organ failure assessment (SOFA) scores and simplified acute physiology score Ⅱ (SAPS Ⅱ) after 3, 5 and 7 days of treatment in both groups. Secondary observation indicators included in-hospital mortality, 90-day all-cause mortality, 7-day microbial cure rate, 7-day clinical cure rate, serum procalcitonin (PCT) and C-reactive protein (CRP) levels after 3, 5 and 7 days of treatment, hospitalization days in the intensive care unit, ventilator treatment days, the highest dose of norepinephrine. The occurrence of adverse drug reaction in the two groups was observed. RESULTS The increments of SOFA scores and SAPS Ⅱ after 7 days of treatment, the levels of PCT and CRP after 5 and 7 days of treatment as well as the 90-day all-cause mortality in the high- dose group were significantly lower than the standard-dose group (P<0.05). There were no statistically significant differences in other indicators between the two groups (P>0.05). CONCLUSIONS High-dose meropenem treatment for septic shock has better clinical effects and is safer than standard-dose meropenem.

		                        			Objective:To screen interleukin(IL)-1β secretion-related membrane transporters by macrophage experiment in vitro and conventional knockout mice.Methods:THP-1 cell line was differentiated to obtain human THP-1-derived macrophages,and the primary macrophages were obtained from human peripheral blood.FVB wild-type mice with the same sex and age were used as the controls of MRP1 knockout mice.The macrophages in abdominal cavity and bone marrow of mice were cultivated.The cells were treated with ABCC1/MRP1,ABCG2/BCRP,ABCB1/P-gp,OATP1B1,and MATE transporter inhibitors,then stimulated by lipopolysaccharide and adenosine triphosphate.The secretion level of IL-iβ was detected by ELISA,Western blot,and immunofluorescence.Results:After inhibiting ABCC1/MRP1 transporter,the secretion of IL-1β decreased significantly,while inhibition of the other 4 transporters had no effect.In animal experiment,the level of IL-1 β secreted by macrophages in bone marrow of MRP1 knockout mice was significantly lower than control group(P<0.05).Conclusion:ABCC1/MRP1 transporter is a newly discovered IL-1β secretion pathway,which is expected to become a new target for solving clinical problems such as cytokine release syndrome.

syndrome coronavirus 2(SARS⁃CoV⁃2)by using single B cell,and determine its neutralizing activity. Methods Venous
blood with high antibody level was collected from people immunized with inactivated SARS⁃CoV⁃2 vaccine(Vero cells)
twice,of which peripheral blood mononuclear cells(PBMCs)were isolated by lymphocyte stratified fluid and used to isolate
single B cell expressing S protein antibody by magnetic beads coupled with S1 protein. Variable region genes of IgG heavy
chain and light chain were amplified by nested PCR after reverse transcription of single B cell,which were connected with
CMV promoter,IgG leader sequence,IgG constant region and polyA sequence by overlapping PCR to construct antibody linear
expression cassette. Linear expression cassette of the heavy chain and light chain from the same B cell was transfected to
HEK293T cells to express human monoclonal antibody of SARS⁃CoV⁃2 S protein. Immunoreactivity was detected by immuno⁃
fluorescence while neutralizing activity by pseudovirus neutralization test. Results A total of 26 monoclonal antibodies

9.Holocarboxylase synthetase deficiency induced by HLCS gene mutations: a rare disease study.

		                        			A boy, aged 16 months, attended the hospital due to head and facial erythema for 15 months and vulva erythema for 10 months with aggravation for 5 days. The boy developed perioral and periocular erythema in the neonatal period and had erythema and papules with desquamation and erosion in the neck, armpit, and trigone of vulva in infancy. Blood gas analysis showed metabolic acidosis; the analysis of amino acid and acylcarnitine profiles for inherited metabolic diseases and the analysis of organic acid in urine suggested multiple carboxylase deficiency; genetic testing showed a homozygous mutation of c.1522C>T(p.R508W) in the HLCS gene. Finally the boy was diagnosed with holocarboxylase synthetase deficiency and achieved a good clinical outcome after oral biotin treatment. This article analyzes the clinical data of a child with holocarboxylase synthetase deficiency and summarizes the etiology, diagnosis, and treatment of this child, so as to provide ideas for clinicians to diagnose this rare disease.

10.Effect of Danggui Shaoyaosan on Mitochondrial Homeostasis and AMPK/SIRT1/PGC-1α Signaling Pathway in Rats Model of Alzheimer's Disease

		                        			ObjectiveTo investigate the effect of Danggui Shaoyaosan (DSS) on the morphology and function of mitochondria in rats model of Alzheimer's disease (AD) and its possible mechanism. MethodRats model of AD was established by injection of streptozocin (STZ) into bilateral ventricles of SD rats. The 40 rats were randomly divided into sham group, model group, low, medium and high dosages of Danggui Shaoyaosan (12,24,36 g·kg-1·d-1) groups,observed the morphological changes of mitochondria in hippocampus of rats by electron microscopy after 14 days of continuous gavage. In situ end labeling(TUNEL) staining used to detect apoptosis and immunofluorescencereactive used to observe the expression of reactive oxygen species (ROS) and peroxisome proliferators-activated receptor γ coactivator lalpha (PGC-1α),quantitative Real-time polymerase chain reaction(Real-time PCR)detected the mRNA expression of dynamin-related protein 1 (Drp1),mitochondrial fusion protein 2 (MFN2) ,cytochrome C oxidase subunit Ⅳ (COX Ⅳ) and PGC-1α. Western blot detected the protein expression of adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK),phosphorylation(p)-AMPK,recombinant Sirtuin 1 (SIRT1) and PGC-1α. ResultCompared with the sham group,the results of model group showed that the damage of mitochondria in hippocampus was more obvious,accelerated the ROS production and apoptosis rate (P<0.01),decreased the mRNA level of MFN2,COX Ⅳ,PGC-1α,increased the mRNA level of Drp1,and descended the protein of p-AMPK/AMPK,SIRT1,PGC-1α (P<0.05,P<0.01).Compared with the model group,the medium and high dose of DSS group notably improved the damage of mitochondria,reduced the production of ROS and apoptosis rate (P<0.01),promoted the mRNA expression of MFN2,COX Ⅳ,PGC-1α,inhibited the mRNA expression of Drp1,and up-regulated the protein of p-AMPK/AMPK,SIRT1,PGC-1α (P<0.01).
