1.Measles, rubella, and mumps antibody seroprevalence among the children aged 18 years and younger in Karamay City, Xinjiang Uygur Autonomous Region

ObjectiveTo understand the measles, rubella, and mumps antibody seroprevalence among the children aged 18 years and younger in Karamay City, and to evaluate the effectiveness of vaccination. MethodsA stratified whole cluster random sampling method was used to investigate the antibody seroprevalence of measles, rubella, and mumps among the healthy children aged 18 years and younger in Karamay City, and to further analyze the positive antibody rates and the geometric mean concentration (GMC) of antibodies. ResultsA total of 620 people were investigated, and the positive rates of IgG to measles, rubella, and mumps were 72.74%,62.26%, and 86.45%, respectively, with a GMC of308.94 mIU·mL-1, 21.81 mIU·mL-1, and 249.10 U·mL-1. There were statistically significant differences in the positive rates of antibodies to measles, rubella, and mumps among different age groups (χ2measles=76.707, P<0.001; χ2rubella=60.804, P<0.001; χ2mumps=35.407, P<0.001). The differences in positive rates were statistically significant among individuals with different intervals from the time of their last dose vaccination (χ2measles=60.533, P<0.001; χ2rubella=46.331, P<0.001; χ2mumps=22.825, P<0.001). ConclusionThe antibody levels of measles, rubella and mumps among the people aged 18 years and younger in Karamay City are found to be low. Two doses of measles-mumps-rubella (MMR) vaccine should be given to children born before 2020, and if necessary, supplementary immunization with MMR vaccine should be carried out before they are enrolled in nursery and kindergarten. Additionally, regular population-based antibody surveillance should be conducted to promptly identify the people with weak immunity, which is conducive to effectively reducing and controlling the epidemic situation of measles, rubella and mumps in schools.
2.Significance and successful construction of stable transfection of MH7A cells with lower TRAF2 expression using lentiviral vector

Objective To construct a stable synovial cell line MH7A from rheumatoid arthritis(RA)patients using lentiviral vectors that interfere with the expression of tumor necrosis factor receptor associated factor 2(TRAF2),and to study the role of TNF-α-TRAF2 signaling in MH7A abnormal proliferation.Methods Based on the design principles of human TRAF2 gene sequence and shRNA sequence,three pairs of TRAF2 shRNA interference se-quences were designed and synthesized.The primers were annealed by PCR,and a linear vector was obtained by double enzyme digestion PLKO.1-puro.The linearized vector was connected to the annealed primers through Solu-tion I,and the connected products were introduced into receptive cells.The plates were coated,and positive colo-nies were selected for sequencing.Three different recombinant plasmids of PLKO.1-TRAF2-shRNA lentivirus were constructed,and lentivirus packaging plasmids was used to package logarithmic growth phase HEK 293T cells.Vi-rus solution was collected to infect MH7A cells.At the same time,puromycin was used to screen MH7A stable transgenic strains with low TRAF2 expression.CCK-8 method,Western blot,and qPCR were used to detect the proliferation function of MH7A induced by TNF-α and low expression of TRAF2,as well as downstream signal TRAF2,P65 protein expression and mRNA levels.Results PLKO.1-TRAF2-shRNA(1),PLKO.1-TRAF2-shR-NA(2),and PLKO.1-TRAF2-shRNA(3)lentivirus vector plasmids and control group lentivirus vector plasmids PLKO.1-puro were successfully constructed.The three TRAF2-shRNA lentivirus vector plasmids and control group lentivirus vector plasmids PLKO.1-puro were respectively introduced into the lentivirus packaging plasmid of HEK 293T to obtain virus solution.After infecting MH7A cells with the virus solution,they were treated with puromycin(2.00 μ G/mL)screening and obtaining MH7A stable transgenic plants after 2 days.Through qPCR and Western blot results,it was found that the expression of TRAF2 mRNA and protein in PLKO.1-TRAF2-shRNA(1)MH7A stably transfected cells was significantly reduced compared to the negative control group.The results of CCK-8 and Western blot showed that after knocking down TRAF2 in MH7A,the proliferation of MH7A cells with low TRAF2 expression induced by TNF-α and the phosphorylation level of P65 were significantly reduced.Conclusion A sta-ble transgenic strain of PLKO.1-TRAF2-shRNA(1)MH7A cells was successfully constructed to investigate the role of TNF-α-TRAF2 signal activation in mediating abnormal proliferation of RA synovial cells.
3.Epidemiological characteristics of varicella in Karamay City of Xinjiang from 2015 to 2022

ObjectiveTo analyze the epidemiological characteristics of varicella in Karamay City, to inform the prevention and control of the local varicella epidemic, and to guide adjustments to the varicella vaccine immunization strategy. MethodsThe data on varicella cases from 2015 to 2022 in Karamay were collected and the epidemiological characteristics were analyzed. ResultsA total of 3 380 varicella cases were reported in Karamay from 2015 to 2022, with an average annual incidence rate of 94.91/105. The incidence of varicella showed a year-on-year downward trend from 2015 to 2022 (AAPC=-16.29%, P<0.05). Varicella cases demonstrated a seasonal variation, with peak incidence occurring from October to January of the following year and a slight increase from April to June. There were regional differences in the distribution of incidence rate, with 109.68/105 in Karamay District, followed by 89.06/105 in Baijiantan District, and 53.80/105 in Dushanzi District, respectively. The incidence rates in the 10‒14 and 5‒9 years age groups were higher than that in other age groups, with an incidence rate of 447.01/105 and 422.33/105 in each group, respectively. Most of the cases were students, accounting for 59.17% of the total cases, followed by preschool children (17.40%). There were 645 breakthrough cases of varicella, accounting for 19.08% of the total cases. The average interval between the onset of breakthrough cases and the most recent vaccination was (5.77±3.43) years. ConclusionVaricella cases mainly occurred in schools and childcare institutions in Karamay City, highlighting the need for enhanced prevention and control efforts in these settings. A second dose of varicella vaccine is recommended for children before primary school enrollment.
4. Reflections on the teaching of infectious diseases based on the One Health concept

Infectious disease is a major public health problem threatening human health and social development, and infectious disease teaching is an important part of public health education. However, traditional infectious disease teaching faces challenges such as overly theoretical and fragmented content, overly traditional and monotonous methods, and a narrow and isolated vision, which makes it difficult to adapt to the current complex and changing situation of infectious disease prevention and control. The “One Health” concept emphasizes interdisciplinary, cross-sectoral, and cross-regional communication and cooperation to achieve the harmonious unity of health for humans, animals, and the environment, which is significant in preventing and controlling infectious diseases. This paper proposes some ideas for reforming infectious disease teaching based on the One Health concept, which aims to improve the quality and effectiveness of infectious disease teaching by recognizing the close connection between human health, animal health, and environmental health. Specifically, it is suggested to reform the teaching in four aspects: introducing the basic concepts, principles, and practices of One Health, increasing the content of infectious diseases related to One Health, adopting diversified and interactive teaching methods, and establishing cross-disciplinary teaching cooperation. Concrete recommendations are provided for each aspect. This paper argues that reforming infectious disease teaching based on the One Health concept is conducive to cultivating public health talents with global vision, systems thinking, and cross-disciplinary collaboration capabilities, providing robust talent support for coping with emerging, re-emerging, and endemic infectious disease threats.
5.Analysis of nosocomial infection risk factors in neurosurgical ICU patients and its prediction model construction

Objective To analyze the risk factors of nosocomial infection among the patients in neuro-surgical ICU,and to construct the risk prediction model to provide reference for the prediction of nosocomial infection in neurosurgical ICU patients.Methods The clinical data of 280 patients admitted and treated in the neurosurgery ICU of this hospital from January 2021 to December 2022 were retrospectively analyzed.The pa-tients were divided into the infection group and non-infection group based on whether or not nosocomial infec-tion occurring,140 cases in each group.A total of 196 patients were extracted as the training set by a ratio of 7︰3 for constructing the model,while the remaining 84 patients served as the validation set for conducting the internal verification.The logistic regression was used to analyze the risk factors of nosocomial infection in the neurosurgery ICU patients,and a predictive model was established.The receiver operating characteristic (ROC) curve was drawn to evaluate the predictive effect of the model.Results The multivariate logistic re-gression analysis indicated that old age,long surgery time,catheter use and glucocorticoids use were screened as the main risk factors of nosocomial infection occurrence in neurosurgery ICU patients.The nomogram mod-el was constructed based on the results of multivariate analysis,the area under the curve of training set and validation set were 0.796 and 0.875,respectively.The correcting model reflected good consistency between actual diagnosis and predictive diagnosis.Conclusion The model constructed in this study has the high predic-tive value for the nosocomial infection occurrence risk in the patients of the neurosurgery ICU.
6.CT radiomics for differentiating spinal bone island and osteoblastic bone metastases

Objective To observe the value of CT radiomics for differentiating spinal bone islands(BI)and osteoblastic metastases(OBM).Methods Data of 109 BI lesions in 98 patients and 282 OBM lesions in 158 patients(including 103 OBM in 48 lung cancer cases,86 OBM in 52 breast cancer cases and 93 OBM in 58 prostate cancer cases)from 3 medical institutions were retrospectively analyzed.Data obtained from institution 1 were used as the internal dataset and divided into internal training set and internal validation set at a ratio of 7∶3,from institution 2 and 3 were used as external dataset.All datasets were divided into female data subset(including OBM of female lung cancer and breast cancer)and male data subset(including OBM of male lung cancer and prostate cancer).Radiomics features were extracted and screened to construct 3 different support vector machine(SVM)models,including model1 for distinguishing BI and OBM,model2 for differentiating OBM of female lung cancer and breast cancer,and model3 for differentiating OBM of male lung cancer and prostate cancer.Diagnostic efficacy of model1,CT value alone and 3 physicians(A,B,C)for distinguishing BI and OBM were assessed,as well as differentiating efficacy for different OBM of model2 and model3.Receiver operating characteristic(ROC)curves were drawn,and area under the curves(AUC)were calculated and compared.The differential diagnostic efficacy of model2 and model3 were also assessed with ROC analysis and AUC.Results AUC of model1 for distinguishing spinal OBM from BI in internal training set,internal validation set and external dataset was 0.99,0.98 and 0.86,respectively.In internal training set,model1 had higher AUC for distinguishing BI and OBM than that of physician A(AUC=0.78),B(AUC=0.87)and C(AUC=0.93)as well as that of mean CT value(AUC=0.78,all P<0.05).AUC in internal training set,internal validation set and external dataset of model2 for identifying female lung cancer and breast cancer OBM was 0.79,0.75 and 0.73,respectively,of model3 for discriminating male lung cancer from prostate cancer OBM was 0.77,0.74 and 0.77,respectively.Conclusion CT radiomics SVM model might reliablely distinguish OBM and BI.
7.Clinical characteristics and risk factors of MDRRR-TB patients with decreased peripheral blood CD4+T lymphocytes

Abstract: Objective To explore clinical characteristics and the related factors of multidrug-resistant tuberculosis and rifampicin-resistant tuberculosis (MDR/RR-TB) patients with decreased peripheral blood CD4+T lymphocytes, providing a basis for the prevention and treatment of multidrug-resistant tuberculosis. Methods A retrospective analysis was conducted on 311 MDR/RR-TB patients hospitalized at Chengdu Public Health Clinical Center from January 2018 to December 2020. according to whether accompanied by peripheral blood CD4+T lymphocyte reduction, patients were divided into two groups, the decreased group (n=115) with CD4+T lymphocytes count <414 cell/μL and the normal group (n=196) with CD4+T lymphocytes count ≥ 414 cell/μL. Clinic data, including demographics, types of tuberculosis, complications, clinical symptoms, chest imaging, and treatment outcomes were collected. The binary logistic regression equation was used to analyze the risk factors of MDR/RR-TB with peripheral blood CD4+T lymphocyte reduction. Results In total, 311 cases with MDR/RR-TB were enrolled, with a male-to-female ratio of 1.68∶1. The median age [M(P25, P75)] was 32 (24, 45) years. The median CD4+T lymphocyte count was 492.0 (328.0, 661.0) cells/μL, and 115 patients had CD4+T lymphocyte counts <414 cells/μL. Compared with the normal group, the proportion of older patients, male, baseline anemia, baseline leukopenia, baseline hypoalbuminemia, combined with other chronic respiratory diseases, hematogenous disseminated pulmonary tuberculosis, tuberculous pleurisy, and tuberculous meningitis were higher (P<0.05), while the success rate of treatment was lower in the decreased group (P=0.024). Being male (OR=2.045, 95%CI : 1.147-3.648), older age (OR=1.032, 95%CI : 1.012-1.052), baseline anemia (OR=2.246, 5%CI : 1.457-3.426), and baseline leukopenia (OR=2.398, 95%CI : 1.387-4.148) were risk factors for decreased CD4+T lymphocyte count in MDR/RR-TB patients. Conclusions MDR/RR-TB patients with decreased CD4+T lymphocytes are more likely to suffer from severe tuberculosis (hematogenic disseminated tuberculosis, tuberculous meningitis) and tuberculous pleurisy, meanwhile the success rate of treatment was lower. Male, elder age, baseline anemia, and baseline leukopenia are associated with decreased CD4+T lymphocyte count in MDR/RR-TB patients, and the immune status of these patients needs to be paid attention to in clinical work.
8.Cloning, expression and purification of fructose-2, 6-bisphosphatase gene CpF2KP in papaya.

Papaya, which is mainly cultivated in the southeastern region of China, is one of the four famous fruits in Lingnan. It is favored by people because of its edible and medicinal value. Fructose-6-phosphate, 2-kinase/fructose-2, 6-bisphosphatase (F2KP) is a unique bifunctional enzyme with a kinase domain and an esterase domain that catalyzes the synthesis and degradation of fructose-2, 6-bisphosphate (Fru-2, 6-P2), an important regulator of glucose metabolism in organisms. In order to study the function of the gene CpF2KP encoding the enzyme in papaya, it is particularly important to obtain the target protein. In this study, the coding sequence (CDS) of CpF2KP, with a full-length of 2 274 bp, was got from the papaya genome. The amplified sequence of full-length CDS was cloned into the vector PGEX-4T-1 which was double digested with EcoR I and BamH I. The amplified sequence was constructed into a prokaryotic expression vector by genetic recombination. After exploring the induction conditions, the results of SDS-PAGE showed that the size of the recombinant GST-CpF2KP protein was about 110 kDa. The optimum IPTG concentration and temperature for CpF2KP induction were 0.5 mmol/L and 28 ℃, respectively. The purified sin[A1] gle target protein was obtained after purifying the induced CpF2KP protein. In addition, the expression level of this gene was detected in different tissues, and showed that the gene was expressed at the highest level in seeds and the lowest in pulp. This study provides an important basis for further revealing the function of CpF2KP protein and studying the involved biological processes of this gene in papaya.

Model informed precision dosing for warfarin is to provide individualized dosing by integrating information related to patient characteristics, disease status and pharmacokinetics /pharmacodynamics of warfarin, through mathematical modeling and simulation techniques based on the quantitative pharmacology. Compared with empirical dosing, it can improve the safety, effectiveness, economy, and adherence of pharmacotherapy of warfarin. This consensus report describes the commonly used modeling and simulation techniques for warfarin, their application in developing and adjusting dosing regimens, medication adherence and economy. Moreover, this consensus also elaborates the detailed procedures for the implementation in the warfarin pharmacy service pathway to facilitate the development and application of model informed precision dosing for warfarin.
