1.Efficacy of focal radiofrequency ablation in the treatment of low-to-intermediate risk localized prostate cancer

Objective: To explore the efficacy of focal radiofrequency ablation (RFA) in the treatment of low-to-intermediate risk localized prostate cancer and its impact on postoperative urinary control and sexual function recovery,in order to explore the feasibility of minimally invasive methods for the treatment of localized prostate cancer. Methods: Clinical data of 28 patients with low-to-intermediate risk localized prostate cancer who underwent RFA in Nanjing Drum Tower Hospital,Affiliated Hospital of Medical School during Jun.2017 and Feb.2021 were retrospectively analyzed.The 5-year failure-free survival (FFS) rate,surgery related complications,postoperative urinary control and sexual function were collected.The differences between the survival curves of patients in the low-risk and intermediate-risk subgroups were assessed with log-rank test and Breslow test. Results: All surgeries were successfully completed under local anesthesia.During the median follow-up of 43 (40-49) months,the 5-year FFS rate predicted by Kaplan-Meier method was 78.57%; 25 patients (89.29%) did not experience surgery-related complications; 27 patients (96.43%) were able to control urination; 1 patient developed new-onset sexual dysfunction.There was no significant difference in the survival curves between patients in the low-risk and intermediate-risk groups (P>0.05). Conclusion: RFA for patients with low-to-intermediate risk localized prostate cancer has good clinical efficacy,little impact on urinary control and sexual function recovery,and few postoperative complications,which can be used as one of the treatment options for these patients.
2.Research progress of technology in femtosecond laser assisted cataract surgery

Cataract is the world's leading cause of blindness, and surgery is the most effective treatment for cataract. With the development of femtosecond laser technology and ophthalmic surgical equipment, the application of femtosecond laser systems in cataract surgery is becoming increasingly widespread. It can be used in cataract surgery for corneal incisions, anterior capsulotomy, lens fragmentation, arcuate incisions and other key operations. Compared to traditional surgery, femtosecond laser assisted cataract surgery(FLACS)offers significant advantages in precision, safety and postoperative visual outcomes. Its clinical benefits have garnered growing recognition among ophthalmologists. However, the key technologies and high-precision equipment for FLACS remain predominantly controlled by Western countries. In China, the research in this field began later. This article reviews the technological advancements in FLACS, with a focus on femtosecond laser technology, optical coherence tomography(OCT), artificial intelligence, and clinical application progress. The objective is to provide theoretical foundations and practical insights for the development of ophthalmic medical technology in China.
3.Induction of apoptosis in hepatocellular carcinoma cells by polyphyllin 9 through regulating the Fas/FasL sig-naling pathway and the inhibitory effect on the growth of transplanted tumor in nude mice

OBJECTIVE To investigate the induction of apoptosis in hepatocellular carcinoma cells by polyphyllin 9 (PP9) through the regulation of the Fas/Fas ligand (FasL) signaling pathway, and its inhibitory effect on the growth of transplanted tumor in nude mice. METHODS Based on the screening of cell lines and intervention conditions, HepG2 cells were selected as the experimental subject to investigate the effects of 2 μmol/L and 4 μmol/L PP9 treatment on cell colony formation activity, apoptosis rate, as well as the protein expressions of Fas, FasL, cleaved caspase-8 and cleaved caspase-3. Additionally, Fas inhibitor KR- 33493 was introduced to investigate the underlying mechanism of PP9’s anti-hepatocellular carcinoma activity. Using HepG2 cell tumor-bearing nude mice model as the object, and 5-fluorouracil (20 mg/kg) as the positive control, the effects of 10 mg/kg PP9 on tumor volume, tumor mass, and the protein expressions of the nuclear proliferation-associated antigen Ki-67 and cleaved caspase-3 in tumor-bearing nude mice were investigated. RESULTS Compared with the control group, 2, 4 μmol/L PP9 significantly decreased the number of clones and the clone formation rate of cells, but significantly increased the apoptosis rate, the protein expressions of Fas, FasL, cleaved caspase-8 and cleaved caspase-3 (P<0.05 or P<0.01). However, the combination of Fas inhibitor KR-33493 could significantly reverse the effect of PP9 on the up-regulation of proteins related to the Fas/FasL signaling pathway (P<0.01). Compared with the control group, the tumor volume (on day 27), mass and protein expression of Ki- 67 in nude mice of the PP9 group were significantly decreased, while the protein expression of cleaved caspase-3 was significantly increased (P<0.01). CONCLUSIONS PP9 can induce apoptosis of HepG2 cells by activating the Fas/FasL signaling pathway. Meanwhile, PP9 can also effectively inhibit the growth of transplanted tumors in nude mice.
4.Predictive model for extubation delay undergoing non-emergency major surgery based on random forest algorithm

Objective To construct and validate a clinical prediction model for delayed extubation undergoing non-emergency major surgery based on the random forest algorithm.Methods Clinical data of 7 528 patients undergoing non-emergency major surgery under general anesthesia from January 2018 to De-cember 2022 were retrospectively collected.The patients were divided into two groups according to whether extubation was performed within 2 hours after surgery:non-delayed extubation group(≤2 hours)and de-layed extubation group(>2 hours).All the patients were randomly divided into a training set and a valida-tion set in a ratio of 7 ∶ 3.The predictive factors for delayed extubation after surgery were screened through LASSO regression and Logistic regression.The random forest model was established and verified by random forest algorithm.Results There were 123 patients(1.6%)experienced delayed extubation after surgery.ASA physical status,department,intraoperative use of flurbiprofen ester,dexmedetomidine,glucocorticoid,hypocalcemia,severe anemia,intraoperative blood transfusion,and airway spasm were identified as inde-pendent predictive factors for delayed extubation.The area under curve(AUC)value of the random forest prediction model in the validation set was0.751(95%CI0.742-0.778),and the sensitivity was98.1%,and the specificity was 41.9%.Conclusion The predictive model of delayed extubation undergoing non-e-mergency major surgery based on random forest algorithm has a good predictive value,which may be helpful to prevent delayed extubation undergoing non-emergency major surgery.
5.Quantitative value of energy spectrum CT material separation technology in nonalcoholic fatty liver

Objective To investigate the feasibility of energy spectrum CT material separation technology for quantitative evaluation of nonalcoholic fatty liver patients,to compare the accuracy of this method with the conventional liver/spleen CT ratio for grading liver fat content.Methods Sixty patients diagnosed with nonalcoholic fatty liver and 20 healthy volunteers were chosen to undergo liver MR multi-echo(ME)Dixon and energy spectrum CT scans.The proton density fat fraction(PDFF),fat concentration(FC),and liver/spleen CT ratio were then measured for each participant.According to PDFF,nonalcoholic fatty liver patients were divided into mild fatty liver group,moderate fatty liver group,and severe fatty liver group.Results With the increase in PDFF,FC increased and the liver/spleen CT ratio decreased.The difference between FC groups in normal,mild,moderate and severe fatty liver groups was statistically significant(P<0.05),while the difference between the liver/spleen CT ratio of normal group and mild fatty liver group was not statistically significant(P>0.05).The receiver operating characteristic(ROC)curve analysis showed that when the critical value of FC was 351.19 mg/mL,the sensitivity,specificity and area under the curve for normal group and fatty liver group were 0.95,0.1 and 0.99,respectively.Conclusion The energy spectrum CT material separation technology has a good correlation between the fat content measured by the MR ME Dixon,which is superior to the fat content measured by the liver/spleen CT ratio.For patients with nonalcoholic fatty liver,FC in energy spectrum CT has high accuracy in differentiating normal and mild fatty liver.
6.Chronic Fatigue Syndrome Treated with Massage Combined with Aromatic Therapy in 34 Cases: A Randomized Controlled Study

Obejective To investigate and compare the clinical efficacy of aroma massage with traditional massage in the treatment of chronic fatigue syndrome (CFS). MethodsSeventy-two CFS patients were randomized into the aroma massage group and the traditional massage group, with 36 patients in each group. Both groups were treated with essential oil aromatherapy and massage were performed on the head, face, and abdomen stimultaneously. The aroma massage group used antifatigue oil, while the traditional massage group used placebo essential oil. The two groups were treated for 30 minutes each time, once every other day, three times a week, for a total of 4 weeks. The fatigue scale (FS-14) score, traditional Chinese medicine (TCM) syndrome score and patient satisfaction evaluation scale score before and after the treatment of the two groups were assessed to study the improvement of fatigue and other clinical symptoms of the patients as well as the changes of the TCM symptom scores, and to evaluate the clinical efficacy of the two groups. ResultsWith three drop-outs in the traditional massage group and two drop-outs in the aroma massage group, 33 cases in the traditional massage group and 34 cases in the aroma massage group were included in the outcome analysis. The FS-14 scores and TCM syndrome scores significantly decreased after treatment in both groups (P<0.01); compared to those in the traditional massage group (FS-14: 4.42±2.44; TCM syndrome score: 34.12±7.67), the FS-14 score (3.06±2.37) and TCM syndrome score (28.79±5.62) in the aroma massage group were significantly lower (P<0.05 or P<0.01). There was no significant difference in the satisfaction scores of the patients in the two groups after the treatment (P>0.05). The total effective rate of the aroma massage group was 94.12% (32/34), which was significantly higher than 84.85% (28/33) of the traditional massage group (P<0.05). ConclusionBoth aromatherapy and traditional massage can improve the clinical symptoms of fatigue in CFS patients, and aromatherapy is more effective, which is easy to operate with high patient acceptance.

Tuberous sclerosis complex(TSC)is a rare genetic disease that can lead to benign dysplasia in multiple organs such as the skin, brain, eyes, oral cavity, heart, lungs, kidneys, liver, and bones. Its main symptoms include epilepsy, intellectual disabilities, skin depigmentation, and facial angiofibromas, whilst incidence is approximately 1 in 10 000 to 1 in 6000 newborns. This case presents a middle-aged woman who initially manifested with epilepsy and nodular depigmentation. Later, she developed a lower abdominal mass, elevated creatinine, and severe anemia. Based on clinical features and whole exome sequencing, the primary diagnosis was confirmed as TSC. Laboratory and imaging examinations revealed that the lower abdominal mass originated from the uterus. CT-guided biopsy pathology and surgical pathology suggested a combination of leiomyoma and abscess. With the involvement of multiple organs and various complications beyond the main diagnosis, the diagnostic and therapeutic process for this patient highlights the importance of rigorous clinical thinking and multidisciplinary collaboration in the diagnosis and treatment of rare and challenging diseases.
8.Clinical study of precise prostate puncture by perineal color ultrasound combined with magnetic resonance in the diagnosis of prostate cancer

Objective To explore the clinical effect of perineal color ultrasound combined with magnetic resonance imaging(MRI)in the diagnosis of prostate cancer.Methods A retrospective analysis was performed on 80 patients with prostate biopsy in our hospital from July 2020 to September 2022.According to different puncture methods,they were divided into transrectal prostate biopsy(TRPB)group(40 cases)and transperineal prostate biopsy(TPPB)group(40 cases).With pathological examination results as the gold standard,puncture time,puncture needle number,puncture positive needle number,pathological examination results,Gleason score and complication rate of the two methods were analyzed,and the diagnostic value of TRPB and TPPB in prostate cancer was also analyzed.Results There were no significant differences in the number of puncture needles,the number of puncture positive needles,the malignant rate of pathological examination and Gleason score between the two groups(P>0.05).The puncture time in TRPB group was longer than that in TPPB group(P<0.05).The incidence of postoperative fever and puncture point bleeding in TRPB group was higher than that in TPPB group,and the incidence of pain was lower than that in TPPB group,with statistical differences(P<0.05).There was no significant difference in the incidence of hematuria and lower urinary tract symptoms between the two groups(P>0.05).The accuracy of TPPB method in the diagnosis of prostate cancer was 92.5%higher than that of TRPB method(75.0%),with statistical difference(P<0.05).There was no significant difference in sensitivity and specificity between the two methods for diagnosis of prostate cancer(P>0.05).Conclusion The application of perineal color ultrasound combined with MRI precision prostate puncture has high application value in the diagnosis of prostate cancer,which can effectively reduce the risk of complications such as fever and puncture point bleeding,shorten the operation time,and have high safety,which is worthy of promotion and application.
9.Pathogenic spectrum and molecular characteristics of infectious diarrhea among children in Putuo District, Shanghai

ObjectiveTo investigate the pathogenic spectrum and molecular characteristics of infectious diarrhea among children in Putuo District of Shanghai from 2018 to 2023, and to provide scientific basis for the prevention and control of infectious diarrhea in children. MethodsFecal samples from the cases visited sentinel hospitals for children’s diarrheal disease in Putuo District, Shanghai, were collected from January 2018 to December 2023. A total of 11 species of bacteria were isolated and cultured, and 5 species of viruses were detected by real-time fluorescent polymerase chain reaction (PCR). The molecular typing of some positive strains was analyzed by the standard pulsed-field gel electrophoresis (PFGE) method. The polymerase-capsid protein linkage region of some norovirus-positive samples was amplified by reverse transcription PCR, and was sequenced and analyzed by bioinformatics software. The Chi-square test and Fisher’s exact probability test were used to compare the pathogen-positive rates in 2018‒2019 and 2020‒2023. ResultsOf the 707 cases of children with infectious diarrhea, the total positive rate was 47.67%, with a single bacterial positivity rate of 16.27%, a single viral positivity rate of 22.63%, and a mixed positivity rate of 8.77%, respectively. The dominant pathogens were rotavirus (10.75%), norovirus (10.33%), enteropathogenic Escherichia coli (8.06%), Salmonella (6.36%), enteroaggregative Escherichia coli (5.52%), and Campylobacter (5.23%). Bacterial infections were predominant in summer and fall, and viral infections were predominant in winter and spring. The total positive rate decreased in 2020‒2023 compared with that of 2018‒2019 (χ2=5.753,P<0.05). Thirty-seven strains of Salmonella, 81 strains of diarrheagenic Escherichia coli, and 19 strains of Campylobacter were completed for the molecular typing analysis by PFGE, which were classified into 28, 80 and 18 banding types, respectively, with a wide range of banding similarity. Nineteen copies of norovirus GⅡ group gene sequences were analyzed and classified into 5 genotypes, which were mainly GⅡ.Pe-GⅡ.4 and GⅡ.P16-GⅡ.2 types. ConclusionRotavirus, norovirus, diarrheagenic Escherichia coli, Salmonella, and Campylobacter are the dominant pathogens of infectious diarrhea in children in Putuo District of Shanghai. The pathogen spectrum shows a trend of seasonal epidemic characteristics, with a diversity of molecular characteristics of some pathogens. Surveillance and monitoring on molecular characteristics of the pathogens of infectious diarrhea in children should be strengthened in different seasons, so as to provide a laboratory basis for the prevention and control of infectious diarrhea.
10.Etiological characteristics of Brucella melitensis in Henan Province, 2013-2022

Objective:To analyze the genus, drug resistance/virulence and phylogenetic characteristics of Brucella strains isolated from brucellosis surveillance sentinels in Henan Province from 2013 to 2022, and provide baseline data for the surveillance, early warning and outbreak tracing of brucellosis. Methods:Blood samples were collected from patients with Brucella infection for strain isolation, culture and species identification, drug susceptibility test, whole genome sequencing, splicing and assembly, functional/virulence/resistance gene prediction analysis and phylogenetic tree drawing based on single nucleotide polymorphism (SNP). Results:In 36 brucellosis patients, the majority were men (86.11%, 31/36), young adults aged 18-50 (88.89%, 32/36) and farmers/herdsmen (72.22%, 26/36). A total of 36 strains of Brucella melitensis were isolated, and average 1 305 functional proteins of 21 categories were predicted by strain genome; all the strains carried four main virulence factors (pmm, VirB group, BtpA/BtpB, BvrS/BvrR). The drug sensitivity rate was 100.00% to six types of antibiotics including levofloxacin, rifampicin, doxycycline, streptomycin, tetracycline and gentamicin, they showed different resistances to three antibiotics including compound trimethoprim-sulfamethoxazole, ciprofloxacin and ampicillin. The strains carried four types of resistance genes and two clusters of resistance genes, with four combinations of genotypes, the resistance mechanisms included antibiotic degradation/modification enzymes, resistant nodular cell differentiation (RND) efflux pumps, 16S/23S ribosomal rRNA binding site mutations, etc. The number of SNP differed in the genomes of 36 Brucellamelitensis strains ranged from 0 to 454 and phylogenetic tree was divided into three major branches, with relative branch distances between 0.000 0 and 0.498 6 for each strain. Conclusions:Human Brucellamelitensis strains isolated from surveillance sentinels in Henan from 2013 to 2022 carried multiple virulence and antibiotic resistance genes and had different drug resistance phenotypes. Single nucleotide polymorphism analysis and phylogenetic tree analysis showed significant differences in phylogenetic relationships among different strains.
