1.Risk factors and predictive model of complicating urosepsis after percutaneous nephrolithotripsy

Objective To explore the risk factors of complicating urogenic sepsis after percutaneous nephrolithotripsy(PCNL)and construct a nomogram prediction model.Methods The data of 291 patients with stage 1 PCNL in 940 Hospital of Joint Logistics Support Force from October 2016 to October 2021 were retrospectively analyzed.The patients were divided into the sepsis group and non-sepsis group according to whether complicating urogenic sepsis after operation.The general data,stone-related data,operation-related data and laboratory detection related data were included.The independent risk factors were screened by the univariate and multivariate logistic regression analysis,and the nomogram prediction model was constructed.Results The results of univariate and multivariate logistic regression analysis showed that age≥60 years old(OR=6.438,95%CI:1.548-26.769),urinary leukocyte 3+(OR=5.651,95%CI:1.614-31.766),urinary nitrite positive(OR=7.117,95%CI:1.190-42.561),operation time≥90 min(OR=4.626,95%CI:1.137-18.817)and perfusion volume 30 L(OR=3.312,95%CI:1.090-10.061)were the independent risk factors of postoperative complicating urogenic sepsis.C-index of the constructed nomogram prediction model in the modeling samples was 0.937,the calibrated C-index was 0.914,and the model predictive efficien-cy was good.Conclusion Age ≥60 years old,urinary leukocyte 3+,urinary nitrite positive,operation time 90 min and perfusion volume ≥30 L are the independent risk factors for complicating urogenic sepsis after PCNL;the constructed nomogram prediction model has a good predictive efficiency for the occurrence of post-operative urogenic sepsis.
2.Alleviatory effect of curcumin on cardiomyocyte toxicity induced by doxorubicin by regulating SIRT3/SOD2 signaling pathway

Objective:To discuss the effect of curcumin on ameliorating doxorubicin(DOX)-induced cytotoxicity in the H9c2 cardiomyocytes,and to clarify its mechanism.Methods:The H9c2 cardiomyocytes were treated with DOX to establish the cardiotoxicity model.The H9c2 cells were divided into normal group,DOX group,DOX+curcumin group,and DOX+curcumin+silent information regulator 3(SIRT3)inhibitor-3(3-TYP)group.After 24 h,the morphology of the cells in various groups were observed;CCK-8 method was used to detect the viabilities of the cells in various groups;TUNEL staining was used to detect the apoptotic rates of the cells in various groups;enzyme-linked immunosorbent assay(ELISA)method was used to detect the activities of catalase(CAT)and superoxide dismutase(SOD)and the levels of malondialdehyde(MDA)in the cells in various groups;2,7-dichlorofluorescein diacetate(DCFH-DA)staining was used to detect the levels of reactive oxygen species(ROS)in the cells in various groups;Western blotting method was used to detect the expression levels of B-cell lymphoma 2(Bcl-2),Bcl-2-associated X protein(Bax),nicotinamide adenine dinucleotide phosphate(NADPH)oxidase 2(NOX2),NADPH oxidase 4(NOX4),SIRT3,acetylated superoxide dismutase 2(Ac-SOD2),and superoxide dismutase 2(SOD2)proteins in the cells in various groups.Results:Compared with normal group,the H9c2 cells in DOX group exhibited swelling,the activity of the cells was decreased(P<0.05),the CAT and SOD activities were decreased(P<0.05),the MDA and ROS levels were increased(P<0.05),the expression levels of NOX2,and NOX4 proteins were increased(P<0.05),the ratio of Bcl-2/Bax and the expression level of SIRT3 protein were decreased(P<0.05),and the expression levels of SOD2 and Ac-SOD2 proteins were increased(P<0.05).Compared with DOX group,the H9c2 cells in DOX+curcumin group showed improved morphology,the activity of the cells was increased(P<0.05),the CAT and SOD activities were increased(P<0.05),the MDA and ROS levels were decreased(P<0.05),the expression levels of NOX2,and NOX4 proteins were decreased(P<0.05),the ratio of Bcl-2/Bax and the expression level of SIRT3 protein were increased(P<0.05),and the expression levels of SOD2 and Ac-SOD2 proteins were decreased(P<0.05).Compared with DOX+curcumin group,the cells in DOX+curcumin+3-TYP group exhibited swelling,the activity of the cells was decreased(P<0.05),the apoptotic rate of the cells was significantly increased(P<0.05),the CAT and SOD activities were decreased(P<0.05),the MDA and ROS levels were significantly increased(P<0.05),the expression levels of NOX2 and NOX4 proteins were increased(P<0.05),the ratio of Bcl-2/Bax and the expression level of SIRT3 protein were decreased(P<0.05),and the expression levels of SOD2 and Ac-SOD2 proteins were increased(P<0.05).Conclusion:Curcumin suppresses the oxidative stress and apoptosis by activating the SIRT3/SOD2 signaling pathway,improving the cell activity and alleviating the DOX-induced cytotoxicity in the H9c2 cells.
3.Two-phase hemodynamic analysis of intracranial arteries based on high-resolution magnetic resonance tube wall imaging

Based on high-resolution magnetic resonance tube wall imaging,reverse medical engineering modeling techniques are used to obtain individualized intracranial artery models of high prevalence in ischemic stroke.Three types of intracranial artery models,including Newtonian fluid model,non-Newtonian fluid model and non-Newtonian fluid with two-phase flow model,are established for transient numerical simulations using computational fluid dynamics method.The hemodynamic parameters such as blood flow field,wall shear stress distribution and erythrocyte volume distribution are analyzed to investigate the possible mechanisms involved in the formation and progression of intracranial atherosclerosis.It is found that blood flow velocity increased significantly at the end of the internal carotid artery and the middle cerebral artery.Low-speed vortex flow and disturbed flow appear in the local vessels.The difference in blood flow velocity between the center and the edge of the wall is large,with an obvious low-flow velocity area on the outer side.A clear central core area is formed in the stenotic wall under the action of high-speed blood flow,resulting in thinner edge layer and lower erythrocyte volume fraction.The combination of low erythrocyte distribution in the edge layer and low flow velocity on the outer side of the wall exacerbates endothelial cell necrosis,hypoxia,endothelial dysfunction,and leads to atherosclerosis.Compared with Newtonian and non-Newtonian fluid models,non-Newtonian fluid with two-phase flow model has greater variability for hemodynamic parameters and shows higher fidelity in simulating blood flow,which provides a reference for clinical diagnosis and treatment of cerebrovascular diseases.
4.Integrated top-down and bottom-up proteomics mass spectrometry for the characterization of endogenous ribosomal protein heterogeneity

Ribosomes are abundant,large RNA-protein complexes that are the sites of all protein synthesis in cells.Defects in ribosomal proteins(RPs),including proteoforms arising from genetic variations,alternative splicing of RNA transcripts,post-translational modifications and alterations of protein expression level,have been linked to a diverse range of diseases,including cancer and aging.Comprehensive character-ization of ribosomal proteoforms is challenging but important for the discovery of potential disease biomarkers or protein targets.In the present work,using E.coli 70S RPs as an example,we first developed a top-down proteomics approach on a Waters Synapt G2 Si mass spectrometry(MS)system,and then applied it to the HeLa 80S ribosome.The results were complemented by a bottom-up approach.In total,50 out of 55 RPs were identified using the top-down approach.Among these,more than 30 RPs were found to have their N-terminal methionine removed.Additional modifications such as methylation,acetylation,and hydroxylation were also observed,and the modification sites were identified by bottom-up MS.In a HeLa 80S ribosomal sample,we identified 98 ribosomal proteoforms,among which multiple truncated 80S ribosomal proteoforms were observed,the type of information which is often overlooked by bottom-up experiments.Although their relevance to diseases is not yet known,the integration of top-down and bottom-up proteomics approaches paves the way for the discovery of proteoform-specific disease biomarkers or targets.

The oral cavity of each person is home to hundreds of bacterial species.While taxa for oral diseases have been studied using culture-based characterization as well as amplicon sequencing,metagenomic and genomic information remains scarce compared to the fecal microbiome.Here,using metagenomic shotgun data for 3346 oral metagenomic samples together with 808 published samples,we obtain 56,213 metagenome-assembled genomes(MAGs),and more than 64%of the 3589 species-level genome bins(SGBs)contain no publicly available genomes.The resulting genome collection is representative of samples around the world and contains many genomes from candi-date phyla radiation(CPR)that lack monoculture.Also,it enables the discovery of new taxa such as a genus Candidatus Bgiplasma within the family Acholeplasmataceae.Large-scale metagenomic data from massive samples also allow the assembly of strains from important oral taxa such as Por-phyromonas and Neisseria.The oral microbes encode genes that could potentially metabolize drugs.Apart from these findings,a strongly male-enriched Campylobacter species was identified.Oral sam-ples would be more user-friendly collected than fecal samples and have the potential for disease diagnosis.Thus,these data lay down a genomic framework for future inquiries of the human oral microbiome.
6.Efficacy and safety of vagus nerve stimulation in the treatment of refractory epilepsy.

OBJECTIVES:
Vagus nerve stimulation (VNS) is a neuromodulative therapeutic technique for patients with drug-resistant epilepsy who are not suitable for resection or who have experienced a failed resection. This study aims to explore the efficacy and safety of VNS in patients with refractory epilepsy, and to analyze the influential factors for the efficacy.
METHODS:
A retrospective review of clinical data were conducted for 35 patients, who were treated for refractory epilepsy through VNS surgery in the Department of Neurosurgery, Xiangya Hospital, Central South University from April 2016 to August 2019. All patients were analyzed in terms of the clinical and follow-up data.
RESULTS:
After a mean follow-up of 26 months (6-47 months), outcome was as follows: 7 patients were MuHugh class I, 13 patients were MuHugh class II, 8 patients were MuHugh class III, and 7 patients were MuHugh class IV-V. The total efficacy rate in the short duration group was significantly higher than that in the long duration group (77.8% vs 50.0%,
CONCLUSIONS
VNS is a safe and effective option in treating patients with refractory epilepsy, especially for those with short duration.

7.Applied value of vascular compression grading in prognosis evaluation of patients with trigeminal neuralgia

Objective:To investigate the predictive value of trigeminal nerve vascular compression grading evaluated by preoperative magnetic resonance imaging (MRI) in patients with trigeminal neuralgia (TN) after microvascular decompression (MVD).Methods:Two hundred and seventy patients with TN accepted MVD in our hospital from January 2015 to December 2017 were chosen in our study; their clinical and MRI data were retrospectively analyzed. By referring to Sindou's method and Jannetta standard, these patients were divided into patients with mild vascular compression ( n=71) and patients with severe vascular compression ( n=199) according to preoperative MRI data; these patients were divided into typical TN patients ( n=219) and atypical TN patients ( n=51) according to their symptomatic characteristics; the differences in prognoses after one year of follow-up were compared among patients from different categories. Results:The degree of intraoperative vascular compression was basically consistent with the preoperative imaging evaluation results, and the intraoperative vascular compression in 256 patients was consistent with preoperative imaging evaluation grading, with a coincidence rate of 94.8% (256/270). The difference in prognosis between patients with mild vascular compression and patients with severe vascular compression was statistically significant ( Z=-3.420, P=0.001), and the mean rank indicated that the prognosis of patients with severe vascular compression was better than that of patients with mild vascular compression (142.01 vs. 117.25). The prognosis difference between typical TN patients and atypical TN patients was statistically significant ( Z=-5.810, P=0.000), and the mean rank indicated that the prognosis of typical TN patients was better than that of atypical TN patients (144.45 vs. 97.08). Conclusions:Preoperative MR imaging evaluation is a reliable method to assess the degree of vascular compression. Patients with severe vascular compression and typical TN have high postoperative pain relief rate after MVD.

In May 2018, 718 HCWs from the department related to the diagnosis and treatment of influenza in 17 hospitals (6 tertiary hospitals, 5 secondary hospitals and 6 primary hospitals) from Guangzhou, Jiangmen, Zhuhai and Dongguan were selected by using stratified sampling method. Questionnaire survey and face-to-face interview were used to collect the information of influenza vaccination, the intention of the quadrivalent influenza vaccine, the acceptance of free and required vaccination policies, and recommendations for increasing influenza vaccination intentions from 2015 to 2017. The multivariate logistic regression was used to analyze factors associated with the vaccination intention.
Results:

Conclusion HCWs in the Pearl River Delta region had weak inclination of getting quadrivalent influenza vaccine. HCWs who were age (≥50 years old), worked in department of prevention and health care, learned about the quadrivalent influenza vaccine, knowed that HCWs are priority, and had a history of trivalent influenza vaccination from 2015 to 2017 were factors positively associated with the vaccination intention.
