1.Design and Application of Hospital 5G Ward Round Robot Platform

Purpose/Significance To apply the 5G ward round robot platform in primary hospitals,so as to effectively improve the treatment capability and management service level of patients with critical and severe diseases.Method/Process Based on technologies such as 5 G,internet of things(IoT)and robotics,the researchers build and deploy a 5 G ward round robot platform and collaborate with domestic experts to provide mobile ward round,remote consultation,intensive care,health education and other services for patients.Re-sult/Conclusion Real time sharing of patient clinical data and efficient collaboration in the diagnosis and treatment process can be a-chieved,multi-mode,accurate and standardized remote ward round and treatment guidance can be carried out according to patients'conditions.
2.Risk factors and development of a risk assessment model for postoperative venous thromboembolism in Cushing′s disease

Objective:To investigate the incidence and prothrombotic risk factors of postoperative venous thromboembolism(VTE) in Cushing′s disease and to further develop an assessment model to identify those at high risk of postoperative VTE events.Methods:A retrospective study was performed in 82 patients who were admitted to Huashan Hospital, Fudan University during January 2019 and January 2020 and diagnosed with Cushing′s disease. These patients underwent the evaluation about their clinical, hormonal, and coagulation parameters, as well as ultrasonography and pulmonary angio-CT when necessary. The least absolute shrinkage and selection operator(LASSO) regression analysis was used to screen independent risk factors, and a nomogram model for postsurgical VTE risk assessment in Cushing′s disease was initially established, and Bootstrap method was used for internal verification. Finally, the predictive model was evaluated for calibration and clinical applicability in the study cohort.Results:Nineteen patients(23.17%) developed VTE events, with 14 cases occurring after endoscopic transsphenoidal surgery. Compared to patients without VTE, those in the VTE group were older( P<0.001), had longer postoperative bed rest, higher rates of current infection, higher HbA 1C levels, and more severe glucose tolerance impairment(all P<0.05). Through LASSO regression analysis, two independent risk factors for postoperative VTE were identified: Age and current infection. Then a VTE risk assessment nomogram model was established to predict the patients at high risk of VTE. In the nomogram model for VTE risk assessment, the area under the receiver operating characteristic curve was 0.868(95% CI 0.787-0.949), with the calibration curve closely aligning with the ideal diagonal line and the clinical decision curve exceeding the two extreme curves. Conclusions:Advanced perioperative assessment needs to be taken to screen those with high VTE risks in patients diagnosed with Cushing′s disease. Additionally, during the perioperative period, patients with Cushing′s disease should undergo mandatory physical activity or prophylactic anticoagulant therapy.
3.Numerical Analysis on Micro-Element Streaming Potential of Articular Cartilage

Objective To investigate the distribution of streaming potential generated by interstitial fluid flow in articular cartilage and obtain electrical characteristics of articular cartilage. Methods The governing equation of fluid and electrostatic theory were combined to establish a two-dimensional (2D) micro-element model of cartilage, and the steady streaming potential generated in microelement under certain pressure was calculated by finite element method. Results The streaming potential in micro-pore model of articular cartilage with the length of 5 μm was about 38.4 μV. The effect of external pressure and Zeta potential on streaming potential of articular cartilage model was significant and showed a linear increase relationship. The streaming potential decreased with the increase of ion number concentration, but the concentration had different effects on streaming potential of articular cartilage. When the ion number concentration was low, streaming potential was more dependent on ion number concentration. When ion number concentration was high, the effect of ion number concentration on streaming potential was very small. Conclusions The results of this study provide important theoretical basis for differentiation and proliferation of chondrocytes, prevention and treatment of articular cartilage diseases, development of tissue-engineered cartilage and repair of articular cartilage injury by means of electric current, electric field and electromagnetic field stimulation.
4.The value of Fib4 and APRI in predicting liver failure after hepatectomy in patients with hepatocellular carcinoma

Objective:To evaluate the value of preoperative aspartate aminotransferaseto platelet ratio index (APRI) and fibrosis index 4 (Fib4) in predicting posthepatectomy liver failure (PHLF) of primary hepatocellular carcinoma.Methods:The data of 587 patients with hepatocellular carcinoma admitted to the First Affiliated Hospital of Xinjiang Medical University from January 2014 to January 2020 were retrospectively collected and analyzed, including 412 males and 175 females, aged (56.8±11.2) years. Univariate and multivariate logistic regression were used to analyze the influencing factors of PHLF. The ability of Child-Pugh score, model for end-stage liver diseas (MELD) score, APRI and Fib4 to predict PHLF was evaluated through the receiver operating characteristic (ROC) curve of subjects.Results:Among 587 patients, 186 (31.7%) had liver failure after hepatectomy. In multivariate logistic regression analysis, APRI ( OR=2.660, 95% CI: 1.314-5.384, P=0.007) and Fib4 ( OR=1.322, 95% CI: 1.157-1.511, P<0.001) were risk factors for PHLF in patients with hepatocellular carcinoma. The higher the number, the greater the risk of PHLF. The predicted area under the ROC curve of PHLF in patients with hepatocellular carcinoma was Fib4(0.719)>APRI(0.686)>MELD score(0.618)>Child-Pugh score(0.565). Conclusion:APRI and Fib4 were risk factors of PHLF in patients with hepatocellular carcinoma. They predict the occurrence of PHLF better than Child-Pugh score and MELD score.
5.Correlation analysis of third lumbar skeletal muscle index and inflammatory factors and other factors in peripheral blood in gastric cancer patients

Objective:To study the correlation between third lumbar skeletal muscle index(LSMI) and inflammatory factors and other factors in peripheral blood in gastric cancer patients.Methods:From October 2017 to December 2019, patients with gastric cancer admitted to West China Hospital Sichuan University were included. The LSMI of patients was obtained by dividing the area of skeletal muscle at the third lumbar vertebra level by the square of the height based on preoperative abdominal imaging data. The correlation between preoperative LSMI and inflammatory factors and other factors in peripheral blood were analyzed by person correlation analysis.Results:This study included 132 patients with gastric cancer. Among them, 39 were classified as stage Ⅰ, 36 were stage Ⅱ, and 57 were stage Ⅲ, respectively. Pearson correlation analysis suggested that the LSMI of gastric cancer patients was positively correlated with peripheral red blood cell count( P<0.01), hemoglobin( P<0.01), and prealbumin( P<0.01), and negatively correlated with interleukin-6(IL-6, P=0.027) and C-reactive protein(CRP, P= 0.014). Conclusion:Our study suggested that LSMI can be used as a nutritional index in gastric cancer patients and IL-6 and CRP played an important in the occurrence and development of sarcopenia in gastric cancer patients.
6.Working memory span training improves working memory capacity

Objective:To explore whether working memory span training can expand working memory capacity.Methods:A randomized controlled trial design was adopted and a total of 60 healthy college students were recruited and randomly divided into training group ( n=30, receiving adaptive training of spatial breadth task) and control group ( n=30, receiving non-adaptive training of low difficulty spatial breadth task). The cognitive behavior and event-related potential (ERP) data of all subjects when completing the change awareness task were collected before and after training.The SPSS 22.0 statistical software was used for data analysis. The differences between the training group and the control group before and after training were compared by repeated measurement analysis of variance. Results:Repeated measurement ANOVA showed that there were significant time and group interactions at the levels of cognitive behavior(K score, F=5.352, P=0.025) and ERP (CDA, F=4.644, P=0.037) levels. Further post test found that compared with pre-training (pre-test), the K-score ((0.51±0.93), (1.61±1.07), F=26.81, P<0.001) and CDA ((-1.49±1.07)μV, (-2.03±0.94)μV, F=4.731, P=0.041) of the training group increased significantly after training (post-test), and there was no significant difference in K-score and CDA of the control group before and after training (boh P>0.05). Conclusion:Working memory span task can be used as an effective training paradigm to improve working memory capacity.
7.Effects of Collagen Fiber Bundle on Mechanical Properties of Articular Cartilage

Objective To study the effects of collagen fiber bundle on mechanical properties of articular cartilage, so as to provide references for clinicians to guide the rehabilitation of patients with early cartilage injury. Methods The two-dimensional (2D) numerical model of fiber-reinforced porous viscoelasticity was established, with consideration of the relationship of fiber distribution, elastic modulus, porosity and permeability with cartilage depth. The influences from local fracture of the fiber bundle, the progressive fracture from the surface and the fiber bundle size on mechanical properties of the cartilage were studied, and the maximum principle strain of cartilage matrix was obtained. Results The maximum principal strain of the matrix occurred at a position in middle layer of the cartilage, about upper 1/3 of the cartilage, which was not affected by fiber breakage mode and fiber bundle size. The strain of the cartilage with thicker fiber bundles decreased. Conclusions The middle layer of the cartilage was prone to mechanical damage. The thicker fiber bundle could reduce the maximum principal strain of the matrix. Once the fiber bundle broke, the maximum principal strain of the cartilage matrix with thicker fiber bundle became larger, leading to an easier evolution of the cartilage damage.
8.The Nomogram model in predicting prognosis of hepatocellular carcinoma patients based on four inflammatory markers

Objective:To construct a Nomogram model in predicting recurrence-free survival (RFS) and overall survival (OS) at six months, one year and two years after hepatocellular carcinoma (HCC) resection by using inflammatory markers combined with other routine clinical indicators.Methods:The data of 314 patients with HCC who underwent first time hepatectomy at Beijing Chaoyang Emergency Rescue Center and Air Force Characteristic Medical Center from January 2013 to January 2018 were analyzed. HCC patients who underwent hepatectomy at the First Medical Center of PLA General Hospital from January 2011 to January 2016 ( n=106) were used as the external validation group. Univariate and multivariate Cox proportional risk model was used to analyze independent risk factors of recurrence and death in HCC patients. A Nomogram model was constructed based on independent risk factors. Validation of the efficacy of the Nomogram model was done based on external data. Results:In the experimental group, 174 patients relapsed. The median RFS was 26 months. The 6 months, 1 year and 2 years RFS were 26.8%, 43.9%, and 68.8%, respectively. A total of 142 patients had died. The median survival time was 30 months. The 6 months, 1 year and 2 years OS were 5.9%, 23.6% and 63.1%, respectively. In the external validation group, 63 patients had developed recurrence, with a median RFS time of 28 months. The 6 months, 1 year and 2 years RFS were 26.4%, 45.3%, 54.7%, respectively. The median survival time was 31 months. The 6 months, 1 year and 2 years OS were 7.5%, 25.5%, 46.6%, respectively. Tumor size (>6.0 cm, HR: 1.447), vascular invasion ( HR: 1.408), TBil (>0.94 mg/dl, HR: 1.949), NLR (>2.54, HR: 2.843), AGR (≤0.88, HR: 2.447) were independent risk factors of HCC recurrence ( P<0.05). Tumor size (>6.0 cm, HR: 2.207), vascular invasion ( HR: 1.529), and NLR (>2.54, HR: 2.708) were independent risk factors of death for HCC patients ( P<0.05). The C-indexes of half-year, one-year and two-year RFS were 0.764 (95% CI: 0.677-0.854), 0.710 (95% CI: 0.615-0.824) and 0.673 (95% CI: 0.601-0.786), respectively. The C-indexes of half-year OS, one-year OS and two-year OS were 0.729 (95% CI: 0.648-0.841), 0.708 (95% CI: 0.608-0.813) and 0.664 (95% CI: 0.618-0.771), respectively. Conclusion:In this study, the construction of a Nomogram model in predicting prognosis of HCC patients was helpful to guide clinicians in improving preoperative treatment plans and in providing ideas for individualized treatment of patients.
9.Application of psychological intervention in breast cancer patients

Objective To explore the effects of psychological intervention in breast cancer patients. Methods From March 2016 to July 2017,this study selected 141 breast cancer patients with surgery at Jiangsu Taizhou People's Hospital as subjects by convenience sampling. All of patients were divided into control group(n=69)and observation group(n=72)based on admission order. Control group carried out routine nursing. On the basis of routine nursing,observation group implemented psychological nursing intervention. This study compared the treatment compliance,anxiety,depression and nursing satisfaction of patients between two groups. Results After intervention,the treatment compliance of patients in observation group was better than that in control group with a statistical difference(P<0.05);the scores of Self-rating Anxiety Scale(SAS) and Self-rating Depression Scale(SDS)of patients in observation group were lower than those in control group with statistical differences(P<0.05);patient satisfaction of observation group was higher than that of control group with a statistical difference(P< 0.05). Conclusions Positively and effectively psychological nursing intervention for breast cancer with surgery can improve their compliance,reduce negative emotions and promote harmonious nurse-patient relationship which is worthy of clinical generalization and application.
10.Effect of cluster intervention in reducing ectopic rate of PICC catheterization in department of neurosurgery

Objective To study effect of cluster intervention in reducing ectopic rate of peripherally inserted central venous catheter(PICC) in department of neurosurgery.Methods A total of 100 patients in our hospital were selected as research objects,and were randomly divided into two groups by random number table method,with 50 cases in each group.The control group was given routine nursing intervention,and the observation group was given cluster intervention,ectopic catheter,catheter indwelling time,hospitalization time,comfort score,life quality score,nursing satisfaction of two groups were compared.Results The observation group had lower catheter ectopic rate (P < 0.05),longer catheter indwelling time (P < 0.05),and shorter hospitalization time than that in the control group (P < 0.05).Comfort scores,life quality scores were higher than that in the control group (P < 0.05).Nursing satisfaction of observation group was significantly higher than that in the control group (96% vs.82%,P < 0.05).Conclusion Cluster nursing intervention can effectively reduce ectopic catheter,shorten hospital stay,improve comfort and quality of life,and enhance nursing more satisfaction.
