OBJECTIVE To establish the precise management model for the centralized preparation of cytotoxic drugs in pharmacy intravenous admixture services (PIVAS), and evaluate the effects of its application. METHODS Pharmacists in PIVAS established the precise management model by soliciting clinical opinions and consulting literature on the centralized preparation of cytotoxic drugs and continuously refining every step of the preparation of cytotoxic drugs, based on data feedback from the information closed-loop management system and the limit of stability time of finished solutions. The indicators such as the preparation time, delivery time, the storage time of finished infusion solutions after preparation, and the completion rate of infusion within the stability time limit were analyzed before the implementation (January to December 2023) and after the implementation (January to December 2024) of this model, to evaluate its application effectiveness. RESULTS The overall framework for the precise management model included upgrading the functions of the prescription review system, improving the prescription review database, providing specialized training for PIVAS pharmacists, managing dynamic batch decision for drug preparation, managing special drugs, managing finished infusion distribution, and establishing a continuous improvement mechanism. Compared with before implementation, the average preparation time of the second and third batches of cytotoxic drugs with more concentrated morning preparation tasks in this model was significantly shorter than before implementation (P<0.05); the delivery time of finished infusion after implementation ([ 11.49±2.92) min] was significantly shorter than the delivery time before implementation ([ 22.11±5.03) min] (P<0.001); the storage time of some drugs with shorter stable time limit and carboplatin in combination regimens (paclitaxel or docetaxel+carboplatin) was significantly shortened compared to before implementation (P<0.05), and the completion rate of infusion within the stability time limit was significantly improved compared to before implementation (P<0.05). CONCLUSIONS Our hospital has successfully established a precise management model for the centralized preparation of cytotoxic drugs in PIVAS. This mode can significantly shorten the preparation time of each batch of PIVAS in the morning, make batch decisions more reasonable and improve the infusion completion rate within the stable time limit of the finished product.

ObjectiveTo decipher the mechanism of Wenxiao powder in alleviating corticosterone-induced depression-like behaviors in mice. MethodMale ICR mice were randomized into normal, model, paroxetine (20 mg·kg-1), and low- and high-dose (3.27, 6.54 g·kg-1, respectively) Wenxiao powder groups. The mice in normal and model groups received equal volume of saline. Other groups except the normal group were injected with corticosterone subcutaneously 0.5 h after gavage to induce depression. Mice were tested for depression-like behaviors after drug administration. Enzyme-linked immunosorbent assay (ELISA) was performed to measure the corticosterone content in the serum. Nissl staining was performed to observe the damage of hippocampal neurons. Immunofluorescence staining was employed to observe the expression of double cortin (DCX) in the dentate gyrus (DG) of the hippocampus. Western blot was employed to determine the expression of proteins in the brain-derived neurotrophic factor (BDNF)/tyrosine kinase receptor B (TrkB)/extracellular signal-regulated kinase (ERK)/cAMP-response element-binding protein (CREB) pathway in the hippocampus. ResultCompared with the normal group, the model group showed decreased sucrose preference rate, increased immobility time in the tail suspension test (P<0.01), and reduced residence time in the central area of the open field and the total movement distance (P<0.05, P<0.01). In addition, the modeling elevated the corticosterone level in the serum (P<0.01), decreased the volume and intensified the nuclear staining of hippocampal neurons in the DG area, reduced the expression of DCX in the DG area, and down-regulated the protein levels of BDNF, phosphorylated (p)-TrkB, p-ERK, and p-CREB in the hippocampus (P<0.05, P<0.01). Compared with the model group, low-dose Wenxiao powder improved the mouse behavivors in the sucrose preference, open field, and tail suspension tests (P<0.05, P<0.01), and high-dose Wenxiao powder improved the behaviors in the sucrose preference and open field tests (P<0.05, P<0.01). In addition, Wenxiao powder lowered the serum corticosterone level (P<0.01) and recovered the structure and morphology of neurons with obvious nuclei and presence of Nissl bodies in the DG area of the hippocampus. Moreover, Wenxiao powder at both doses promoted the expression of DCX in the DG area, and high-dose Wenxiao powder up-regulated the protein levels of BDNF, p-TrkB, p-ERK, and p-CREB in the hippocampus (P<0.05, P<0.01). ConclusionWenxiao powder can alleviate corticosterone-induced depression-like behaviors and promote neurogenesis in mice possibly by activating the BDNF/TrkB/ERK/CREB signaling pathway.

Chinese Circulation Journal 2024;39(3):267-272
Objectives:To finely divide the aortic sinus into sections and accurately localize the coronary ostium through CARTO three-dimensional mapping, and to assess the clinical effects of treating aortic sinus cusps premature ventricular contraction(ASC-PVC)and the ablation risk in the corresponding area with zero X-ray radiofrequency ablation. Methods:A total of 66 patients who underwent radiofrequency ablation for ASC-PVC from January 2020 to January 2023 were included in this analysis, patients were divided into experimental group(n=34)and conventional group(n=32).In the conventional group, the CARTO 3 system was used to create an aortic sinus model through the conventional method. The earliest stimulating target was identified by using electrical stimulation mapping(ESM).Radiofrequency ablation treatment was performed after the distance between the target and the coronary ostium was precisely measured by coronary angiography through the hollow tube of the ablation catheter or coronary angiography tube.In the experimental group, the CARTO 3 system was used to build a model of the aortic sinus and the coronary ostium and aortic sinus were divided into sections. The earliest stimulating target was identified by ESM.After localizing coronary ostium through the impedance changing pattern on the ablation catheter tips, catheter ablation was performed with zero X-ray.The data regarding the impedance of the ablation catheter in the aortic sinus were collected. The total operative time, the operative time in the aortic sinus, contrast dosage,X-ray exposure time, immediate and short-term success rates of the operation and complication rates were compared between the two groups.Besides, the distribution of successfully ablated targets and their relationship with the risk of ablation were analyzed in both groups. Results:There was no significant statistical difference in the immediate and short-term success rates between the two groups(93.8%vs.94.1%;90.6%vs.88.2%,both P>0.05).The experimental group did not receive contrast agents during the operation, and the total operation time and intra-aortic sinus operation time in the experimental group were significantly shorter than those in the conventional group([58.76±4.94]min vs.[66.91±5.94]min,P<0.001;[43.12±4.49]min vs.[50.31±5.18]min,P<0.001).During the process of moving the ablation catheter from the intra-aortic sinus to the coronary artery opening and into the coronary artery, the impedance suddenly increased, which was significantly different from the impedance in other parts of the intra-aortic sinus(all P<0.001). Conclusions:Radiofrequency ablation of ASC-PVC with zero X-ray can simplify the procedures and shorten the operative time. The steep increase in impedance at the tip of the ablation catheter can be used as a basis for localizing the coronary ostium.Dividing the aortic sinus into sections allows a detailed assessment of the risk for ablation treatment at the targets.
