1.Neuroprotective Mechanisms of Ciliary Neurotrophic Factor in Retinal Ganglion Cells: Insights from Microarray Analysis

Purpose:
This study investigated the changes in gene expression in retinal ganglion cells (RGCs) following ciliary neurotrophic factor (CNTF) treatment to elucidate the underlying mechanisms contributing to its neuroprotective effects.
Methods:
RGCs isolated from Sprague-Dawley rat pups were treated with recombinant CNTF. Gene expression was analyzed via microarray. Differentially expressed genes (DEGs) were defined as those with a fold change greater than 2 or less than –2. The DEGs were further explored using Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analyses.
Results:
Our analysis identified 71 upregulated and 58 downregulated genes. A2m exhibited the highest increase, with a fold change of 4.97, whereas Rho displayed the most significant decrease in expression, with a fold change of –6.38. GO and KEGG pathway analyses revealed substantial involvement in sensory organ development and the phototransduction pathway.
Conclusions
This study provides new insights into the impact of CNTF on gene expression in RGCs, suggesting broader neuroprotective mechanisms that could inform future therapeutic strategies for retinal degenerative diseases. Our findings emphasize the importance of further investigation into the complex gene network responses to CNTF treatment.
2.Causes and Clinical Characteristics of Compressive Optic Neuropathy in South Korean Patients

Purpose:
We aimed to investigate the causes and clinical features of compressive optic neuropathy in Koreans.
Methods:
We analyzed the medical records of patients diagnosed with compressive optic neuropathy from March 2014 to December 2023 to determine the cause of optic nerve compression, symptoms and symptom patterns at first visit, accompanying symptoms, types of visual field defects, and visual prognosis after surgery. In addition, the results of visual field tests and optical coherence tomography were analyzed to determine the positivity rate of each test and factors affecting test results.
Results:
A total of 73 patients were diagnosed with compressive optic neuropathy, and the most common cause was pituitary tumor (37 patients, 50.7%) followed by meningioma (16 patients, 21.9%), hemangioma (4 patients, 5.5%), thyroid ophthalmopathy (4 patients, 5.5%), and paranal sinus tumor tumor (2 patients, 2.7%). In terms of symptom pattern, half of the patients had vision and visual field defects that appeared gradually (41 patients, 56.2%), but there were also quite a few patients whose symptoms appeared suddenly (17 patients, 23.3%). The positivity rate of the diagnostic test was highest for visual field testing (89.0%). In pituitary tumors, tumor size affected diagnostic test results (p = 0.01).
Conclusions
The most common cause of compressive optic neuropathy in South Koreans was a pituitary tumor. Deterioration of visual function can also occur acutely, and visual field testing was useful for diagnosis.
3.Clinical Outcomes of XEN45 Gel Stent Implantation (Ab Externo, Open Conjunctival Approach) versus Trabeculectomy: A Real-World Study

Purpose:
To compare the clinical outcomes of XEN45 gel stent implantation surgery (ab externo, open conjunctiva) with those of trabeculectomy.
Methods:
We retrospectively reviewed electronic medical record of 57 patients (62 eyes) treated with XEN implantation surgery (ab externo, open conjunctival approach) between April 1, 2021, and July 31, 2023, by the same surgeon. Preoperative clinical data including intraocular pressure (IOP), the number of glaucoma medications, and visual acuity were collected from 1 day to 12 months postoperatively. These data were compared and analyzed with those of 67 patients (78 eyes) received trabeculectomy between February 1, 2017, and April 30, 2022, by the same surgeon. Statistical analyses were performed with p < 0.05 as significant.
Results:
Complete surgical success rate was 33.9% and 57.7% of the XEN and trabeculectomy groups, respectively (p = 0.005). The qualified success rate was 79.0% and 93.6%, respectively (p = 0.011). Postoperatively, the XEN group used more glaucoma medications than the trabeculectomy group (1.21 ± 1.05 vs. 0.69 ± 0.90, p = 0.003 at postoperative month 12). After postoperative month 1, the XEN group had a higher IOP (15.77 ± 5.07 mmHg vs. 13.17 ± 3.81 mmHg; at postoperative month 12, p = 0.001) and lower corneal astigmatism than the trabeculectomy group (1.32 ± 0.79 diopters vs. 1.88 ± 1.45 diopters, p = 0.020 at postoperative month 6). There was no significant difference in preoperative and postoperative best-corrected visual acuity (logMAR) between the groups at any of the follow-up period (favorable visual acuity subgroup; logMAR < 0.7). Postoperative complications were 0 cases of XEN group and 13 cases of trabeculectomy group (0% vs. 16.7%, p = 0.001). Also, XEN surgery (24 minutes 40 seconds ± 6 minutes 26 seconds) had a shorter operation time than the trabeculectomy (40 minutes 18 seconds ± 8 minutes 27 seconds, p < 0.001).
Conclusions
Compared to trabeculectomy, XEN surgery (ab externo, open conjunctiva) showed relatively lower effectiveness (surgical success rate, IOP reduction). However, it demonstrated advantages as a minimally invasive glaucoma surgery, including a surgical success rate approaching about 80%, stability in inducing corneal astigmatism, fewer postoperative complications, and shorter operation times.
4.From the ‘Essential Healthcare Policy Package’ to the ‘Healthcare Reform 1st Implementation Plan’

Essential medical care is a concept referring to indispensable medical care. World Health Organization has defined essential healthcare services as the evidence-based technologies needed to cost-effectively solve health problems. However, most countries have no such term that describes essential healthcare services. Rather, it has been used to efficiently allocate medical resources in countries with limited resources. Therefore, a clear definition of essential healthcare services must be accompanied by a specific purpose and a clear direction for the need. The target to whom medical services are to be provided, the region and institution to be provided, the content of medical services, and the purpose of provision should be distinguished. Recently, as public interest in essential healthcare services has increased, the government announced “the Essential Healthcare Policy Package” and “the 1st Healthcare Reform Implementation Plan” to specifically promote it. But concerns of feasibility and financial estimates are being raised. So, here, I analyze the government policy package and its 1st implantation plan, and suggest policy proposals for them.
6.Prevalence and Factors Influencing Behavioral Addictions among School Adolescents: A Study in the Gwangju-Jeonnam Region

Background:
The aim of this study is to evaluate the prevalence and associated psychosocial factors of behavioral addictions among school adolescents living in the Gwangju and Jeonnam regions in Korea.
Methods:
A self-reported survey was conducted from December 4, 2023, to January 31, 2024, including 855 middle and high school students residing in the Gwangju-Jeonnam regions. Aside from the information on demographic characteristics, data on depression, anxiety, Internet gaming addiction, gambling problems, and resilience was obtained.
Results:
The prevalence of Internet gaming addiction among adolescents was 5.4%, while the prevalence of gambling problems was 3.3%. The male adolescents had a significantly higher risk of behavioral addiction compared with the female adolescents. The logistic regression analysis revealed that male and depression were significant risk factors for Internet gaming addiction. For gambling problems, male was identified as a significant risk factor.
Conclusion
The findings of this study suggested that the prevalence of behavioral addiction among school adolescents has been relatively higher than that of previous studies, emphasizing the need for community-based prevention and intervention strategies tailored to the sex difference and psychological factors associated with adolescent behavioral addictions.
8.Diagnostic Ability and Correlation of Digital 11C-Methionine PET/CT in Primary Hyperparathyroidism with Inconclusive Standard Imaging

Purpose:
11C-Methionine PET/CT is a promising method for detecting parathyroid lesions in patients with primary hyperparathyroidism (PHPT). We aimed to determine the diagnostic ability and correlation of digital 11C-Methionine PET/CT for parathyroid lesions in patients with PHPT, particularly in cases where standard imaging methods yielded inconclusive results.
Methods:
This retrospective analysis was conducted on patients diagnosed with PHPT who underwent digital 11C-Methionine PET/CT imaging because of ambiguous results on standard imaging work-up ( 99m Tc-MIBI parathyroid scan and/or neck ultrasonography). Quantitative 11C-Methionine PET/CT parameters, including maximum standardized uptake value (SUVmax), mean SUV (SUVmean), peak SUV (SUVpeak), parathyroid methionine volume (PMV), and whole methionine uptake(WMU: PMV multiplied by SUVmean) were calculated with various thresholds, and their correlations with biochemical andpathologic parameters were investigated.
Results:
This study included 22 consecutive patients (10 men and 12 women) with a median age of 64.0 years. The lesion detection rate and sensitivity of digital 11C-Methionine PET/CT were 81.8% (18/22) and 100.0% (18/18), respectively.Quantitative analysis revealed that serum PTH (r = 0.490, P = 0.039) and serum calcium (r = 0.583, P = 0.011) were signifi-cantly correlated with PMV50%.
Conclusion
Digital 11C-Methionine PET/CT offers good performance in the detection of parathyroid lesions in PHPT patients with inconclusive standard imaging work-up. The volume parameter of PMV50% significantly correlated biochemi-cal parameters and can serve as a complementary diagnostic tool.

10.Resveratrol attenuates aging-induced mitochondrial dysfunction and mitochondria-mediated apoptosis in the rat heart

RESULTS:
Resveratrol significantly reduced cardiac hypertrophy and remodeling in aging hearts. In addition, resveratrol significantly ameliorated aging-induced mitochondrial dysfunction (e.g., decreased oxygen respiration and increased hydrogen peroxide emission) and mitochondria-dependent apoptotic signaling (the Bax/Bcl-2 ratio, mitochondrial permeability transition pore opening sensitivity, and cleaved caspase-3 protein levels).Resveratrol also significantly attenuated aging-induced apoptosis (determined via cleaved caspase-3 staining and TUNEL-positive myonuclei) in cardiac muscles.
CONCLUSION
This study demonstrates that resveratrol treatment has a beneficial effect on aging-induced cardiac remodeling by ameliorating mitochondrial dysfunction and inhibiting mitochondria-mediated apoptosis in the heart.
