1.Banxia Xiexintang Regulates Autophagy of Interstitial Cells of Cajal in Gastric Antrum of Rats with Gastric Electric Dysrhythmia

ObjectiveTo observe the effect of Banxia Xiexintang on the autophagy of interstitial cells of Cajal (ICCs) in the gastric antrum of rats with gastric electric dysrhythmia, and explore the protective effect and regulatory mechanism. MethodThirty-two SD rats were randomly assigned into a normal group, a model group, a Banxia Xiexintang (24.68 g·kg-1) group, and a positive drug (2.7 mg·kg-1) group. The rat model of gastric electric dysrhythmia was established by the method of dieting every other day and drinking dilute hydrochloric acid, and Banxia Xiexintang and the positive drug were administrated for intervention. The body weight of each rat was recorded weekly. The gastric electric activity was recorded by the biological function experimental system. The ultrastructural changes of the gastric antrum tissue were observed by a transmission electron microscope. The co-expression of receptor tyrosine kinase (c-kit)/mammalian homolog of yeast Atg6 (Beclin1) in the gastric antrum tissue was detected by double immunofluorescence labeling method. The expression of microtubule-associated protein 1 light chain 3B (LC3B) and p62 protein in the gastric antrum tissue was determined by Western blot, and the LC3BⅡ/Ⅰ ratio was calculated. ResultCompared with the normal group, the modeling reduced the body weight (P<0.01) and decreased the dominant frequency and dominant power of gastric electricity (P<0.01). In addition, the modeling caused ultrastructural damage of ICCs in gastric antrum, degeneration and necrosis of organelles, and appearance of a small number of autophagic vesicles. The results of double immunofluorescence labeling showed that the modeling inhibited the positive expression of c-kit and promoted the positive expression of Beclin1 in gastric antrum tissue. Western blot results showed that the modeling increased the ratio of LC3BⅡ/Ⅰ (P<0.01) and down-regulated the expression of p62 protein (P<0.01) in the gastric antrum tissue. Compared with the model group, Banxia Xiexintang and the positive drug increased the body weight (P<0.01) and the dominant frequency and dominant power of gastric electricity (P<0.01), repaired the ultrastructural damage of ICCs in gastric antrum tissue, promoted the positive expression of c-kit and inhibited the positive expression of Beclin1 in the gastric antrum tissue. Furthermore, Banxia Xiexintang up-regulated the expression of p62 (P<0.05) and inhibited the transformation of LC3BⅠ into LC3BⅡ in gastric antrum tissue (P<0.05). ConclusionBy regulating the expression of autophagy-related proteins, Banxia Xiexintang can reduce the autophagy and regulate the number and structure of ICCs and thus improve the gastric electric rhythm of rats, which preliminarily explains the mechanism of Banxia Xiexintang in the treatment of epigastric stuffiness.
2.Study on the status and influencing factors of central catheter-related bloodstream infection in ICU nurses

Objective:In order to investigate the status quo of central line-associated bloodstream infections (CLABSI) prevention among ICU nurses and analyze its influencing factors.Methods:A total of 245 ICU nurses from Harbin Medical University Affiliated First, Second, and Cancer Hospitals from December 2021 to January 2022 were selected by convenience sampling method. To evaluate the status quo of CLABSI prevention among ICU nurses by using Central Line-associated Bloodstream Infections Prevention Knowledge and Practice Scale, a cross-sectional survey was conducted among ICU nurses by using General Data Questionnaire and ICU Nurse Alarm Fatigue Scale, and the influencing factors of CLABSI prevention were evaluated.Results:The knowledge, attitude, behavior dimensions and alarm fatigue scores of ICU nurses on CLABSI prevention were (13.74 ± 2.87), (50.92 ± 4.10), (85.44 ± 8.52), and (31.35 ± 5.06), respectively. The score of ICU nurses′ alarm fatigue was negatively correlated with the score of knowledge, attitude and behavior dimensions in CLABSI prevention knowledge and practice ( r=-0.360, -0.378, -0.408, all P<0.01). The results of multiple regression analysis showed that alarm fatigue, CLABSI training and first education degree were the main influencing factors of ICU nurses′ knowledge and attitude to CLABSI prevention ( P<0.01). Alarm fatigue, CLABSI training and the highest education degree were the main influencing factors of ICU nurses ′ CLABSI prevention ( P<0.01). Conclusions:ICU nurses′ knowledge and practice of CLABSI prevention were at the middle level. Alarm fatigue, participation in CLABSI training, and educational background (first and highest) are the influencing factors for the prevention of CLABSI among ICU nurses in terms of knowledge, belief, and behavior. The administrators should pay more attention to CLABSI prevention, provide more opportunities and ways to train and examine ICU nurses, strengthen the weak links of CLABSI prevention, and make ICU nurses master the Knowledge System of CLABSI prevention as soon as possible.
3.Molecular mechanism underlying the inhibitory effect of propofol on lipopolysaccharide-induced pyroptosis of mouse bone marrow-derived macrophages.

OBJECTIVE:
To investigate the molecular mechanism underlying the inhibitory effect of propofol on pyroptosis of macrophages.
METHODS:
Macrophages derived from bone marrow were extracted and divided into three groups: control group, LPS+ATP group and propofol+LPS+ATP group. The control group was not given any treatment; LPS+ATP group was given LPS 1 μg/mL stimulation for 4 h, then ATP 4 mM stimulation for 1 h; Propofol+LPS+ATP group was given propofol+LPS 1 μg/mL stimulation for 4 h, then ATP stimulation for 1 h. After treatment, the supernatant and cells of cell culture were collected. the cell activity was detected by CCK8 and flow cytometry. The inflammatory cytokines IL-1βand IL-18 were detected by Elisa. Western blot was used to detect the expression of caspase-1 protein and TLR4 on cell membran Immunohistochemical fluorescence was used to detect apoptosis of cells.
RESULTS:
LPS+ATP significantly decreased the viability of the macrophages and increased the cellular production of IL-1β and IL-18, activation of caspase-1 protein and the expression of TLR-4 on the cell membrane ( < 0.05). Treatment with propofol obviously reversed the changes induced by LPS+ATP.
CONCLUSIONS
LPS+ATP can induce pyroptosis of mouse bone marrow-derived macrophages, and propofol effectively inhibits such cell death, suggesting that propofol anesthesia is beneficial during operation and helps to regulate the immune function of in patients with sepsis.

4.Clinical research of acupuncture combined with metformin or the treatment of diabetes at the early stage

Objective To investigate the clinical application of acupuncture combined with metformin or the treatment of diabetes at the early stage.Methods According to the random table method,123 patients with diabetes at the early stage were divided into control group (n=61) and the treatment group (n=62).The patients in the control group were treated with metformin,while the patients in the treatment group were treated with acupuncture combined with metformin.Two groups of patients were treated for 12 weeks.The clinical total effective rate of two groups of patients after treatment was counted.The FPG,HbA1c,FINS,FCP,TG,TC,LDL-C,HDL-C,LEP,GLP-1,RETN,HOMA-IR and ISI of the two groups before and after treatment were compared.Results The total effective rate of treatment group was 91.9% (57/62),while the control group was 65.6% (40/61),and the difference between two groups was statistically significant (x2=12.818,P<0.01).After treatment,the FPG,HbA1c,FINS.FCP,HOMA-IR,ISI,TG,TC,LDL-C,LEP,RETN of treatment group were significantly lower than those of the control group (t were 14.149,14.301,4.254,7.552,9.470,11.032,9.993,12.000,11.802,5.385,9.121,P<0.01),while the HDL-C,GLP-I of treatment group were significantly higher than those of the control group (t were 3.402,14.680,P<0.01).Conclusions Acupuncture combined with metformin or the treatment of diabetes at the early stage has a good efficacy,can correct the disorder of glucose and lipid,improve the insulin resistance.
